ABSTRACT

ABSTRACT Introduction: Prednisolone causes pro-inflammatory impulses to be inhibited and anti-inflammatory signals to be promoted. As a result, it alters how the body's immune system reacts to certain diseases. The World Anti-Doping Agency, however, has banned SNP and other glucocorticosteroids. An electrochemical sensor can be developed using a gold nanocomposite, polypyrrole nanoparticles and synthesized carbon nanotubes (Au-PPy NPs@CNTs). Objective: Develop an electrochemical sensor to detect prednisolone. Method: Au-PPy NPs@CNTs nanocomposite was chemically synthesized with a modified glassy carbon electrode (GCE) surface. Results: According to SEM data, the nanocomposite was composed of amorphous Au NPs, and PPy NPs deposited in tubes strongly entangled in a CNTs network. The wide linear range and low detection limit of the Au-PPy NPs@CNTs/GCE as prednisolone sensors were attributed to the combined catalytic performance of the Au and PPy NPs@CNTs nanostructures. Conclusion: The results of prednisolone detection in each specimen using the amperometric method indicated good accuracy. The accuracy and precision of Au-PPy NPs@CNTs/GCE for prednisolone detection were explored in blood samples from 5 young athletes aged 20-24 years who used prednisolone tablets (RSD less than 4.25%). In addition to monitoring prednisolone concentrations in athletes' serum, Au-PPy NPs@CNTs/GCE can be used as a reliable prednisolone sensor. RESUMO Introdução: A prednisolona faz com que os impulsos pró-inflamatórios sejam inibidos e os sinais anti-inflamatórios sejam promovidos. Como resultado, ela altera a forma como o sistema imunológico do corpo reage a certas doenças. A Agência Mundial Antidoping, no entanto, proibiu o SNP e outros glucocorticoesteroides. Usando um nanocomposto de ouro, nanopartículas de polipirrol e nanotubos de carbono sintetizados (Au-PPy NPs@CNTs), um sensor eletroquímico pode ser desenvolvido. Objetivo: Desenvolver um sensor eletroquímico para detectar a prednisolona. Método: O nanocompósito Au-PPy NPs@CNTs foi sintetizado quimicamente com uma superfície de eletrodo de carbono vítreo modificado (GCE). Resultados: De acordo com dados da SEM, o nanocomposto foi descoberto como sendo composto de Au NPs e NPs de PPy amorfo, depositados em tubos fortemente emaranhados em uma rede de CNTs. O amplo alcance linear e o baixo limite de detecção do Au-PPy NPs@CNTs/GCE como sensores de prednisolona foram atribuídos ao desempenho catalítico combinado das nanoestruturas de Au e PPy NPs@CNTs. Conclusão: Os resultados da detecção de prednisolona em cada espécime usando o método de amperometria indicaram boa precisão. A precisão e a acurácia de Au-PPy NPs@CNTs/GCE para a detecção de prednisolona foram exploradas em amostras de sangue preparadas de 5 atletas jovens de 20 a 24 anos que usaram comprimidos de prednisolona (RSD inferior a 4,25%). Além de monitorar as concentrações de prednisolona no soro dos atletas, Au-PPy NPs@CNTs/GCE pode ser usado como um sensor confiável de prednisolona. Nível de evidência II; Estudos terapêuticos - investigação dos resultados do tratamento.


RESUMEN Introducción: La prednisolona hace que se inhiban los impulsos proinflamatorios y se promuevan las señales antiinflamatorias. Como resultado, altera la forma en que el sistema inmunológico del cuerpo reacciona a ciertas enfermedades. Sin embargo, la Agencia Mundial Antidopaje ha prohibido el SNP y otros glucocorticosteroides. Utilizando un nanocompuesto de oro, nanopartículas de polipirrol y nanotubos de carbono sintetizados (Au-PPy NPs@CNTs), se puede desarrollar un sensor electroquímico. Objetivo: Desarrollar un sensor electroquímico para detectar la prednisolona. Método: Se sintetizó químicamente el nanocompuesto Au-PPy NPs@CNTs con una superficie de electrodo de carbono vítreo (GCE) modificada. Resultados: Según los datos del SEM, se comprobó que el nanocompuesto estaba compuesto de Au NPs y NPs de PP amorfo y depositados en tubos fuertemente enredados en una red de CNTs. El amplio rango lineal y el bajo límite de detección de Au-PPy NPs@CNTs/GCE como sensores de prednisolona se atribuyeron al rendimiento catalítico combinado de las nanoestructuras de Au y PPy NPs@CNTs. Conclusión: Los resultados de la detección de prednisolona en cada muestra mediante el método amperométrico indicaron una buena precisión. Se exploró la exactitud y precisión de las Au-PPy NPs@CNTs/GCE para la detección de prednisolona en muestras de sangre preparadas a partir de 5 jóvenes atletas de entre 20 y 24 años de edad que utilizaban tabletas de prednisolona (RSD inferior al 4,25%). Además de controlar las concentraciones de prednisolona en el suero de los deportistas, Au-PPy NPs@CNTs/GCE puede utilizarse como un sensor fiable de prednisolona. Background/Aims@#Endoscopic wound suturing is an important factor that affects the ability to remove large and full-thickness lesions during endoscopic resection. We aimed to evaluate the effect of a traction metal clip with a fishhook-like device on wound sutures after endoscopic resection. @*Methods@#From July 2020 to April 2021, patients who met the enrollment criteria were treated with a fishhook-like device during the operation to suture the postoperative wound (group A). Patients with similar conditions and similar size wounds who were treated with a “purse-string suture” to suture the wounds were retrospectively analyzed as the control group (group B). Difference in the suture rate, adverse events, time required for suturing, and number of metal clips were compared between the two groups. @*Results@#The time required for suturing was 7.72±0.51 minutes in group A and 11.50±0.91 minutes in group B. This difference was statistically significant (F=13.071, p=0.001). The number of metal clamps used in group A averaged 8.1 pieces/case, and the number of metal clamps used in group B averaged 7.3 pieces/case. This difference was not statistically significant (F=0.971, p>0.05). @*Conclusions@#The traction metal clip with the fishhook-like device is ingeniously designed and easy to operate. It has a good suture effect on the wound after endoscopic submucosal dissection and effectively prevents postoperative adverse events.

ABSTRACT

@#Objective To present the initial clinical experience of robot-assisted thoracoscopic esophagectomy for patients with esophageal cancer and to analyze the short-term outcomes of these patients. Methods Between February 2016 and December 2017, 148 patients with esophageal carcinoma underwent robotic esophagectomy and two-fields lymph node dissection. There were 126 males and 22 females at average age of 62.0±8.0 years. Demographic data, intraoperative characteristics and short-term surgical outcomes were collected and analyzed. Results 106 patients underwent McKeown esophagectomy and 42 patients underwent Ivor-Lewis esophagectomy. The mean operation time was 336.0±76.0 min, the mean intraoperative blood loss was 130.0±89.0 ml, the mean number of lymph nodes removed was 21.0±8.0 and the mean length of postoperative hospital-stay was 12.0±7.2 days. Postoperative complications included anastomotic fistula (n=8, 5.4%), pulmonary infection (n=13, 8.7%), hoarseness (n=23, 15.5%), tracheoesophageal fistula (n=1, 0.7%), chylothorax (n=4, 2.7%) and incision infection (n=2, 1.4%). There was no intra-operational massive hemorrhage or in-hospital mortality. Conclusion Both robot-assisted McKeown and Ivor-Lewis esophagectomy are safe and feasible with good early outcomes.

@#Objective To evaluate the advantages about video-assisted thoracoscopic surgery (VATS) lobectomy with optimized management of surgical instruments package. Methods A total of 200 patients with lung cancer were enrolled, which included 78 males and 122 females, aged 24-83 years at median age of 56.8 years. All of them were divided into 2 groups including a routine group (n=100) and an optimized management of surgical instruments group (n=100). The total operation time, bleeding, instrument weights, utilization rate of instruments, counted and cleaning time in 2 groups were recorded and analyzed. Results The average operation time and average lost blood of the routine group was 117.62±42.52 min and 53.14±50.69 ml, respectively, and the one of the optimized instruments group was 120.48±40.62 min, 56.10±49.87 ml, respectively, with no significant difference between the two groups (P=0.112, P=0.231, respectively). The utilization rate of instruments in the routine group (58.02%±2.39%) was significantly lower than that of the optimized instruments group (94.00%±1.48%, P=0.014). The counted time, the loading and unloading time and the cleaning time of instruments in the routine group was 112.00±26.00 s, 70.00±15.00 s, 1 010.00±130.00 s, respectively, much longer than the time of the optimized instruments group, which was 65.00±23.00 s, 20.00±4.00 s, 665.00±69.00 s, respectively. There was a statistical difference between the two groups (P=0.028, P=0.011, P=0.039, respectively). The value of instruments in the routine group (177 574.00±14 438.00 yuan) was apparently higher than that of the optimized instruments group(132 027.00±10 311.00 yuan), with a statistical difference (P=0.032). Conclusion It is demonstrated that optimized management of surgical instruments package in VATS lobectomy can greatly improve the utilization rate of instruments and work efficiency, with no effects on the operation time and amount of bleeding in lobectomy.

ABSTRACT

@#In this review, development and application of the minimally invasive esophagectomy(MIE) for esophageal cancer are discussed including the types of MIE procedures, short- and long- term outcome after MIE; as well the future of MIE is forecasted. Main procedures of MIE performed currently include esophagectomy via thoracoscopy and laparoscopy and cervical esophagogastrosty, Ivor-Lewis MIE via thoracoscopy and laparoscopy, and hiatal MIE. Ivor-Lewis MIE gradually becomes a standard surgical option for the cancer of distal esophagus or esophagogastric junction while the solution of intrathoracic anastomosis via thoracoscopy has achieved. Several methods of intrathoracic anastomosis are reported such as hand-sewn, circular stapler, side-to-side and triangular anastomosis. MIE could decrease operative blood loss, shorten hospital stay and ICU stay, reduce postoperative especially pulmonary complications, and harvest more lymph nodes compared to open esophagectomy. The long-term survival has been proved similar with that after open esophagectomy for esophageal cancer. MIE has developed rapidly in recent years with some aspects in future prospectively: individual MIE treatment and quality of life, fast track after surgery, and robot-assisted MIE, as well the endoscopic submucosal dissection for esophageal cancer is mentioned.

ABSTRACT

Objective To assess which of topical tacrolimus and topical highly potent steroids,is more effective and safer in the treatment of pediatric vitiligo.Methods The PubMed,Cochrane library,Scopus and CINAHL plus databases were retrieved.The search was confined to English language articles.The randomized controlled trial(RCT) articles were included in our study.The quality of the identified articles was examined by using the CASP Randomised Controlled Trials Checklist.Results As a result,there were only a few studies related to the comparison.However,there were only two RCTs regarding a comparison of topical tacrolimus 0.1% and clobetasol propionate 0.05 % in childhood vitiligo.Conclusion When the body surface area (BSA) involved in the child is <20 %,and the disease is not rapidly spreading,topical therapy is the first choice.Topical tacrolimus may be considered as an alternative therapy for childhood vitiligo,especially for acrofacial and segmental types,before considering other modalities,but still need to observe long-term side effects.
