ABSTRACT

Objective To analyze the clinical and laboratory results and epidemiological characteristics of brucellosis.Methods The clinical and laboratory characteristics brucellosis cases (confirmed by Brucellosis Guidelines 2012) at the Department of Infectious Disease of the First Hospital of Jilin University between 2000 and 2016,were retrospectively evaluated.The patients were divided into two groups according to acute and chronic stages.According to the range of lesions,the lesions were divided into two groups,and comparative analysis of data between groups was done.Results The ages of the 607 patients (489 males,118 females) were ranged from 2 to 80 years.Among the 607 patients,570 (93.9%) were diagnosed with acute brucellosis,37 (6.1%) with chronic brucellosis.Focal involvement was found in 418 (68.9%) patients.Among them,382 patients were in acute stage,and the other in chronic phase (36).Acute patients' alanine aminotransferase (ALT) and aspartate transaminase (AST) were higher than those of chronic brucellosis (U/L:47.2 vs 26.7,44.5 vs 24.0,Z =-3.684,-3.754,all P < 0.05).Although the white blood cell count,platelet values,C-reactive protein,and blood sedimentation were higher in patients with focal involvement (x 103/mm3:7.3 vs 5.4,211 vs 176;mg/L:56.0 vs 30.5;mm/h:37 vs 28,Z =-4.407,-2.709,-2.334,-2.137,all P < 0.05),patients without focal involvement had higher ALT and AST (U/L:33.3 vs 53.9,30.2 vs 48.1,Z =-4.102,-3.730,all P < 0.05).Peripheral arthritis was the most common type of osteoarticular involvement in 317 patients with focal involvement,and peripheral arthritis was found to be the most frequently identified in the knee joint,112 patients (35.3%).Totally 80 patients were followed up,the relapse rate was 20.0% (16/80),2 patients treatment failure,only 1 patient died after 1 month of treatment who was diagnosed neuropathic brucellosis.Conclusions Focal involvement should be investigated in the presence of leucocytosis,and pain of a certain part.

ABSTRACT

Objective To investigate the diagnostic value of T-SPOT.TB in patients with fever.Methods T-SPOT.TB was carried out in 456 febrile patients who had no determined reason for fever or who could not be excluded from infection with Mycobacterium tuberculosis.These patients were followed up for their final clinical diagnosis and the diagnostic value of T-SPOT.TB was evaluated using the receiver operating characteristic (ROC) curve.Results In the present study,85 patients were clinically diagnosed with tuberculosis (TB) (including 34 cases of pulmonary TB,24 cases of TB infection defined as T-SPOT.TB positive without TB lesion,but effective with anti-TB treatment,10 cases of tuberculous pleurisy and 17 cases of other extrapulmonary TB) and 371 patients had no definite evidence of Mycobacterium tuberculosis infection (including 42 cases of suspected TB and 329 cases of other diseases).T-SPOT.TB had a high sensitivity of 84.3% in TB patients and a high negative predictive value of 94.2 %,but the specificity was 68.1% and the positive predictive value was 41.4 %.The Kappa value was 0.380 and the area under ROC curve was 0.762 (95%CI:0.707-0.817).T-SPOT.TB had a higher sensitivity in extrapulmonary TB than in pulmonary TB (88.2 % vs 78.1 %),with the negative predictive values of 95.3 % and 92.6 %,Kappa values of 0.438 and 0.299,and the areas under ROC of 0.791 (95 % CI:0.724-0.857) and 0.719 (95%CI:0.623-0.815),respectively.Ninety-nine cases were T-SPOT.TB positive out of 329 patients diagnosed with other diseases.Conclusions T-SPOT.TB test can be used as a method for TB diagnosis in patients without obvious TB lesions.Due to its relatively high negative predictive value,T-SPOT.TB can be used to rule out infection with Mycobacterium tuberculosis.

ABSTRACT

Objective To investigate the effect of radix salviae milliorhize on hemodynamics of portal hypertensive rats.Methods 60 Wistar rat models with portal hypertension were induced by CCl_4.Among them 56 rats with portal hypertension were divided at random into 4 groups: radix salviae 3 d group,radix salviae 10 d group,radix salviae 15 d group,and control group.After administration,the blood flow of vena portae,superior mesenteric artery,superior mesenteric artery and pressure of vena portae,average arterial pressure were measured.(Results Compared) with 0.9% Natrii Chloridi control group,the blood flow of vena portae,superior mesenteric artery,superior mesenteric artery and pressure of vena portae,average artery pressure in radix salviae milliorrhize 3,10 and 15 d groups were decreased significantly(P0.05).Conclusion Radix salviae milliorhize can be employed for treating portal hypertension.Radix salviae milliorrhize can decrease the blood flow and portal pressure,moreover,the effect can be enhanced with the elongation of treatment time.
