ABSTRACT

Objective To analyze MRI findings and pathological data of intracranial solitary fibroma tumor(SFT)to improve the understanding of this disease and the level of imaging diagnosis. Methods MRI images and clinical pathological data of 6 patients with intracranial SFT confirmed by surgery and pathology were analyzed retrospectively. Results All tumors were located in the supratentorium,in which 4 cases were irregularly lobulated,2 cases were oval. All lesions were clear boundaries, and size 3.0-8.6 cm (an average of 6.3 cm).The tumor was located in the right fronto-perietral area in 1 case, cross of the frontal midline in 2 cases, cross of the occipital midline in 2 cases and cross of the cerebellum in 1 case.On T1WI,6 cases of tumor parenchyma showed slight low signal. On T2WI,4 cases of tumors showed mixed signal with the"Yin and Yang"sign, and peritumoral edema was showed in 4 cases, tortuous flow void blood vessels were showed in 5 cases. On FLAIR,mixed signal was showed. On DWI,slightly higher signal was showed. On MRI enhanced study, tumor parenchyma was significantly increased,in which 3 cases of T2WI low signal areas were significantly enhanced, cystic necrosis and edema were not enhanced, and 3 cases showed"dural tail sign".On pathological study, tumor cells were spindle-shaped,bundle and braided distribution, rich in collagen fibers.Immunohistochemistry showed CD34(+), CD99(+),Vim(+).1 case was malignant, and 1 case was potentially malignant. Conclusion MRI features of intracranial SFT have certain characteristics,it is difficult to be characterized qualitatively.MRI has obvious advantages in the relationship of tumor location, shape, range and its relationship with surrounding tissue. It has important clinical application value for the clinical operation plan and the residual or recurrence of tumor after operation.

ABSTRACT

OBJECTIVETo compare the effects between acupuncture with regulating mind and spleen and wes-tern medication for diarrhea irritable bowel syndrome (IBS-D).

METHODSEighty-one patients were randomly at the ratio of 2 to 1 assigned into an acupuncture group (54 cases) and a western medication group (27 cases). Acupuncture with regulating mind and spleen was applied in the acupuncture group for 6 weeks at Baihui (GV 20), Yintang (GV 29), Tianshu (ST 25), Zusanli (ST 36), Shangjuxu (ST 37), Sanyinjiao (SP 6), and Taichong (LR 3), once every other day, 3 times a week. Pinaverium bromide tablet was used orally in the western medication group for 6 weeks, 50 mg a time, 3 times a day. IBS symptom severity score (IBS-SSS) were observed before and after 1-week, 6-week treatment. Pittsburgh sleep quality index (PSQI) was applied before and after 6-week treatment. Also, clinical efficacy was evaluated in the two groups.

RESULTSThree patients dropped out in the acupuncture group, and 1 in the western medication group. Except abdominal distension score after 1-week treatment in the western medication group, single scores and total scores of IBS-SSS apparently reduced in the two groups after 1-week and 6-week treatment (<0.01,<0.05). After 1-week treatment, the abdominal pain score of the acupuncture group was obviously lower than that of the western medication group (<0.05). After 6-week treatment, with abdominal pain relief advantage, other results including the number of pain days, defecation satisfaction, life disturbance degree and total score of the acupuncture group were obviously lower than those of the wes-tern medication group (<0.01,<0.05). After 6-week treatment, the PSQI score and its change before and after treatment in the acupuncture group were superior to those in the western medication group (both<0.05). The relief rate and relief plus obvious effective rate in the acupuncture group were higher than those in the western medication group[51.0% (26/51) vs 19.2% (5/26),<0.01; 64.7% (33/51) vs 34.6% (9/26),<0.05].

CONCLUSIONSAcupuncture with regulating mind and spleen for diarrhea irritable bowel syndrome can more effectively relieve abdominal pain than pinaverium bromide tablet at the early stage. Its total effect and single effects are better at the later stage on abdominal pain, seizure frequency, defecation satisfaction, life disturbance, and sleep quality.

ABSTRACT

OBJECTIVE To survey the antibiotic usage and its rationality in clinical.METHODS From Jan 2009 to May of 2009,950 medical records were selected to conduct a retrospective investigation.RESULTS The incidence of antibiotic usage was 88.2%,71.6% cases were intended for active treatment,28.4% for preventive regimen.72% cases in internal medicine were for active treatment whereas 94% were for surgical prophylaxis. In terms of the antibiotic use, single drug accounted for 56.2%,two-drug 38.9% and three-drug 4.9%.Inspection rate of drug treatment was 69.7% and hospital infection rates was 2.1%.CONCLUSIONS To improve antibiotic administration, systematic efforts are required to keep the medical staff updated of antibiotic principles so as to improve clinical rational use of antibiotics.
