ABSTRACT

Adhering to and strengthening the Party′s overall leadership is the primary guarantee for promoting the high-quality development of public hospitals. How to comprehensively strengthen the internal audit led by the Party and improve the efficiency of internal audit governance in public hospitals, so as to play the role of internal audit in effectively promoting the high-quality development of public hospitals, is an urgent problem to be solved. The author analyzed the current situation and practical reasons of the internal audit under the Party′s control in public hospital in China, constructed the mechanism framework of the internal audit under the Party′s control, and further analyzed and processed the implementation path of the internal audit under the Party′s control from three aspects: establishing the Party Committee Audit Committee to strengthen the top-level design of the internal audit, linking the three major audits to give full play to the supervision force of the internal audit, and implementing the eight major restructuring to achieve the closed-loop governance of the internal audit under the Party′s control.

ABSTRACT

Retinal blood vessels are the only circulatory system that can be observed under non-invasive conditions. By observing the morphological changes of retinal blood vessels, the changes of blood circulation can be indirectly reflected. The occurrence, development and evolution of different diseases can be discovered. With the development of new detection technologies, especially the wide application of fundus photography and optical coherence tomography, a more intuitive and non-invasive quantitative index is provided for retinal vascular measurement. It is important for the diagnosis, guiding treatment and follow-up of related vascular diseases. This article reviews the development of retinal vessel diameter measurement methods and related applications in clinical diagnosis and treatment.

ABSTRACT

To investigate the effect and mechanism of psoralen on calvarial osteoblasts injuries caused by tricalcium phosphate (TCP) wear particles in vitro. Primary osteoblasts were obtained from the calvaria of neonatal SD rat by the series of digestion and were identified with ALP staining. Calvarial osteoblasts were treated with TCP wear particles for 48 h to establish the in vitro model of osteoblasts injuries. The rat osteoblasts were randomly divided into control group, TCP wear particles (0.1 mg/ml) group, psoralen treated (at the concentrations of 10, 10, 10 mol/L) groups. WST assay and the flow cytometry were used to detect the cell viability of osteoblasts and apoptosis, respectively. Chemical colorimetry was performed to examine ALP activity of osteobalsts. When the osteoblasts were treated for 14 day, mineral nodules formation was observed with alizarin red S staining. Western blot was applied to examine protein expressions of glucose regulated protein78/94(GRP78/94), inositol dependent enzyme 1 alpha (IREα), spliced X-box binding protein 1 (XBP1s) and phosphorylated c-Jun N-terminal kinase (p-JNK) in calvarial osteoblasts. Compared with control group, the cell viability of osteoblasts, ALP activity and mineral nodules formation in TCP group were decreased significantly (P<0.05), while the percentage of apoptosis and protein expressions of GRP78/94, IRE1α, XBP1 and p-JNK were obviously increased in calvarial osteoblasts (P<0.05). Compared with TCP group, the injuries of calvarial osteoblasts and cell apoptosis in psoralen treated groups were obviously decreased (P<0.05), and the expression levels of GRP78/94, IRE1α, XBP1 and p-JNK were down-regulated remarkably (P<0.05). Psoralen prevents osteoblasts injuries caused by TCP wear particles through IRE1α-XBP1s-JNK signaling pathway activation.

ABSTRACT

OBJECTIVE@#To investigate the clinical results of locking compression plate combined with autologous iliac bone graft in the treatment of aseptic ulnar nonunion.@*METHODS@#From March 2009 to July 2017, 22 patients with aseptic ulnar diaphyseal nonunion with complete follow-up data were treated with surgery, including 12 males and 10 females, aged from 16 to 58 (39.7±9.9) years old and ranging in course of disease from 10 to 192 (39.4±55.7) months. There were 15 atrophic nonunions, 5 hypertrophic nonunions and 2 synovial pseudo-articular nonunions. After debridement of the nonunion, locking compression plate was used to fix the nonunion and autogenous iliac bone graft was given. Bone healing rate, surgical complications and clinical results were evaluated.@*RESULTS@#All the patients were followed up, and the duration ranged from 13 to 42 months, with a mean of (22.5±8.2) months, and 1 patient did not heal. Visual analogue pain scores ranged from 0 to 3 (0.9±0.9). Pronation of forearm was 47 to 86 (69.0±14.7) degrees, supination was 35 to 85 (63.0±9.4) degrees, wrist flexion was 20 to 80 (51.0±10.2) degrees, wrist flexion was 32 to 88 (71.0±11.7) degrees, elbow flexion contracture was 0 to 25 (9.0±5.6) degrees, further flexion was 105 to 150 (134.0±13.9) degrees, and grip strength was 87% on the opposite side. According to the Anderson scoring system, 8 cases were excellent, 11 were satisfied, 2 were not satisfied, and 1 was failed.@*CONCLUSIONS@#LCP combined with autologous iliac bone graft can effectively treat aseptic ulna diaphyseal nonunion.

ABSTRACT

Objective To explore the impact of family single room practices in NICU on maternal anxiety and breastfeeding status and neuropsychological development in preterm infants. Methods One-hundred NICU-born preterm infants were divided into the experimental group (n= 48) and the control group (n=52) according to random number table. Neonates in the experimental group were guaranteed parental care in the family ward for at least 4 hours a day during hospitalization in NICU, while the control group used a traditional video-based approach for daily scheduled visits. Finally, the results of exclusive breastfeeding, nosocomial infection, maternal anxiety, parental satisfaction, and full-term NBNA score after correction of gestational age were collected and recorded. Results The rates of exclusive breastfeeding were 82.50%(33/40),42.50%(17/40)during hospitalization and six months old in the experimental group, and 17.07%(7/41),12.20%(5/41)in the control group. The difference between the two groups was statistically significant during hospitalization and at 6 months of age (χ2=32.25, 9.40,P <0.01). The score of maternal anxiety was 33.00(31.00, 41.75)in the experimental group, and 43.00(34.50, 46.00)in the control group. The difference between the two groups was statistically significant (Z=-2.97, P =0.003).The NBNA score in the experimental group was 38.50(38.00, 40.00), and 37.00(36.00, 39.00)in the control group. The difference between the two groups was statistically significant(Z=-2.86, P<0.01). Conclusions The development of family ward is conducive to raising the rate of exclusive breastfeeding, reducing the incidence of nosocomial infection, relieving the anxiety of mother-infant separation and enhancing the satisfaction of parents, which can promote the behavioral and neuropsychological development of preterm infants.

Magnesium sulfate is a supplementary therapy strategy in pediatric asthma,its mechanisms includes inhibiting calcium ion from inflowing into respiratory muscle,reducing the release of the neurotransmitter in nerve-muscle junction and the depolarization of acetylcholine in the motor endplates,etc.Nebulised magnesium sulfate not only can improve the pulmonary function of the children with asthma,but also be safe and effetive.With the application of magnesium sulfate in adults,especially acute severe ones,its mechanisms,dosage,safety and efficiency of magnesium sulfate in pediatric asthma draw more attention.

ABSTRACT

The 24 h normal developing zebrafish embryos were used to evaluate the acute toxicity and the compounds of respective fractions were analyzed by UFLC-Q-TOF-MS simultaneously. Nine concentration groups with respective concentration and a blank control group were designed for each fraction to investigate their effect on survival rates of zebrafish embryos 96 h after drug administration, and calculate the median lethal concentration (LC₅₀) of different fractions to zebrafish embryos. The results showed that all of the fractions had acute toxicity to zebrafish embryos except VEKD, and the order was as follows: VEKB, VEKC, VEKA and VEKD. According to the results of UFLC-Q-TOF-MS, the chemical ingredients contained in VEKB and VEKC were mainly composed of ingenane-type and japhane-type diterpenoids, respectively. It could be speculated that japhane-type diterpenoids might be the active compounds with lower toxicity associated with the results of toxicity study, providing some references for the further research on effective material basis of Kansui stir-baked with vinegar according to the principle of "drastic medicine, no death risks".

ABSTRACT

Objective To understand the delay factors of the treatment and diagnosis among pulmonary tuberculosis patients, and to provide scientific evidence for tuberculosis control.Methods A total of 128 pulmonary tuberculosis patients who was newly diagnosed and treated in Xiaoshan were enrolled in this study.The data of demography,medical history and tuberculosis knowledge were collected through questionnaires.Statistical method was used to analyze the current situation and delay factors of the treatment and diagnosis.Results The rate of treatment delay was 25.00%.Female patients without hemoptysis symptom,maintenance elderly,little knowledge of tuberculosis and low degree of education were relative to treatment delay among pulmonary tuberculosis patients (P <0.05 ).Multivariate logistic regression analysis showed that female(OR =6.39,95%CI:1.66 -24.61),was the delay factors of treatment.The rate of diagnosis delay was 50.78%.Multivariate logistic regression analysis showed that preliminary treatment in village or private clinics (OR =2.10,95%CI:1.00 -4.39),preliminary treatment without X ray (OR =3.91,95%CI:1.16 -13.19)and maintenance elderly (OR =57.43,95%CI:4.43 -744.30)were risk factors for delayed diagnosis.Conclusion The influencing factor for treatment and diagnosis delay are female,preliminary treatment in village or private clinics,preliminary treatment without X ray and maintenance elderly.

ABSTRACT

The present study was designed to determine the major chemical constituents of the leaves of Rhododendron dauricum L. Compounds were isolated and purified by various chromatographic methods, and their structures were elucidated by physicochemical properties and spectral data. The present study identified two new C-methyl flavanones, 5, 7, 3', 5'-tetrahydroxy-6, 8-di-C-methyl flavanone (1) and 5, 4'-dihydroxy-8-C-methylflavanone-7-O-β-D-glucopyranoside (2), and one new flavonoid glycoside, quercetin-3-O-β-D-(6"-O-cinnamoyl)-galactoside (3), along with seven known compounds, including syzalterin (4), poriolin (5), farrerol-7-O-β-D-glucopyranoside (6), myrciacetin (7), quercetin-3-O-β-D-(6-p-hydroxy-benzoyl)-galactoside (8), quercetin-3-O-β-D-(6-p-coumaroyl)-galactoside (9), and 5, 7, 3', 5'-tetrahydroxyl flavanone (10). Compounds 1-3 were determined to be new flavonoids; compounds 4-6 were isolated from this species for the first time; and compounds 7-10 were reported for the first time from this genus.

ABSTRACT

Objective To investigate the effect of different parts leukoencephalopathy on cognitive function in patients with acute cerebral infarction,and analyze the relationship between injury site of cholinergic pathways and cognitive function.Methods Ninety-seven patients with acute cerebral infarction were divided into leukoencephalopathy group (59 cases) and non-leukoencephalopathy group (38 cases)according to the cranial MRI T2 scanning.The all patients of 2 groups were evaluated by cholinergic pathways hyperintensities scale (CHIPS) and reforming Scheltens scale.The cognitive function of all patients were evaluated by Montreal cognitive assessment (MoCA).The difference of the cognitive function between 2 groups was observed,and the relationship between CHIPS score,reforming Scheltens scale score and MoCA score was investigated.Results There were statistical differences in MoCA total score and visuospatial/execution,memory,attention score between leukoencephalopathy group and non-leukoencephalopathy group [(20.86 ± 4.52) scores vs.(23.47 ± 4.49) scores,(3.80 ± 1.68) scores vs.(3.11 ± 1.47) scores,(2.78 ±1.57) scores vs.(1.95 ± 1.80) scores,(4.00 ± 2.08) scores vs.(3.87 ± 2.04) scores] (P < 0.01 or < 0.05).There were statistical differences in CHIPS score and reforming Scheltens scale score between cognitive dysfunction group (35 cases)and non-cognitive dysfunction group (24 cases)[(47.77 ± 12.36) scores vs.(39.83 ±7.98) scores,(5.14 ± 1.73) scores vs.(2.58 ±2.10) scores] (P <0.01).There was negative correlation between MoCA total score and frontal periventricular score,occipital periventricular score,parietal lobe score,periventricular total score,deep alba total score and reforming Scheltens scale total score (P <0.01).There was negative correlation between visuospatial/execution score,attention score,fixing score,MoCA total score and CHIPS score (P < 0.01).There was negative correlation between attention score,fixing score,MoCA total score and reforming Scheltens scale score (P < 0.01).Conclusions In acute cerebral infarction patients leukoencephalopathy is probably related to cognitive function,and the severity of leukoencephalopathy correlates with the degree of cognitive function impairment.Different parts leukoencephalopathy can induce different influence on cognitive function.The cognitive function impairment caused by leukoencephalopathy correlates with the impairment of cholinergic pathways,with main effects of visuospatial/execution function,and the severity correlates with the impairment of cholinergic pathways.

<p><b>OBJECTIVE</b>To study in situ intestinal absorption kinetics of baicalin contained in Tiangou Jiangya capsules, and the effect of different intestinal segments, pH value, drug concentration and P-gp inhibitor on the absorption.</p><p><b>METHOD</b>The in situ intestinal perfusion test was adopted, and HPLC method was used to determine the content of baicalin in samples at different time points. Ultra-violet (UV) spectrophotometry was used to determine the content of phenol red in samples at different time points.</p><p><b>RESULT</b>When pH value was at 5. 0, 6. 5, 7. 4, the absorption of baicalin was not impacted. P-gp inhibitor verapamil could enhance the absorption of baicalin. When the quality concentration of the test solution ranged between 5-20 g L -1 , the linearity of the absorption amount of baicalin increased. The absorption kinetic equation of baicalin was Y = -0. 073 7X +0. 118 7 (r = 0. 994 8) , K. 0. 073 7 h -1 , t1/2 9. 40 h.</p><p><b>CONCLUSION</b>Baicalin is mainly absorbed in colon. The absorption of baicalin shows the first-order kinetics process, with the absorption mechanism of passive diffusion. Baicalin is a substrate for P-gp.</p>

ABSTRACT

OBJECTIVE@#To evaluate the efficacy and safety of a short course of nebulized budesonide via transnasal inhalation in chronic rhinosinusitis with nasal polyps.@*METHOD@#Thirty patients with severe eosinophilic nasal polyps were randomized into experimental group (n=15) and control group (n=15). The experimental group received nebulised budesonide suspension (1 mg twice daily) via transnasal inhalation for one week and control group-received budesonide nasal spray (256 microg twice daily). Visual analogue scales (VAS)of nasal symptoms (including nasal obstruction, nasal discharge, loss of smell, and headache/facial pain) and endoscopic polyp scores (Kennedy scores) and morning serum cortisol concentration were performed to both groups before and after the treatment.@*RESULT@#Nebulized budesonide inhalation caused a significant improvement in all nasal symptoms especially nasal obstruction (baseline: 8.4 +/- 0.7; after treatment: 4.0 +/- 0.8, P<0.01) and reduced polyp size compared with before treatment. Additionally, the patients treated with nebulized budesonide showed more obvious improvement in nasal symptoms and polyp size than control group. The morning serum cortisol concentration was mild decreased after one week treatment in nebulized steroid group [baseline: (17.6 +/- 2.4) microg/dl, after treatment: (14.8 +/- 2.6) microg/dl, P<0.01], but all values still were located in normal range (normal range: 5-25 microg/dl).@*CONCLUSION@#A short course of nebulized budesonide transnasal inhalation can rapidly improve nasal symptoms, reduce polyp size, and does not cause obvious HPA axis suppression. Based on these, it is recommend that transnasal inhalation with nebulized budesonide suspension should be performed as a pre-operative routine in patients with nasal polyp.

OBJECTIVE@#To observe the therapeutic effect of tympanoplasty with soft-wall reconstruction of ear canal for chronic otitis media with cholesteatoma.@*METHOD@#Seventy-three patients (76 ears) suffering from chronic otitis media with cholesteatoma were treated with canal wall down mastoidectomy with tympanoplasty. Postauricular myo-periosteal flap was used to the soft-wall reconstruction of ear canal, and the cavityplasty of auricular concha was not performed. The auricular bone prosthesis was made of the autogeneic mastoid cortical bone or residual incus. The postoperative modality and the function of external auditory canal and the postoperative hearing and the postoperative complications were observed.@*RESULT@#The mean dry ear time was (21.1 +/- 3. 1) days after surgery in this study. The postoperative modality of external auditory canal was normal on the whole. The patients were followed up between 6 months and 24 months after surgery. The postoperative average air conduction hearing was improved by (14.5 +/- 6.1) dB HL.@*CONCLUSION@#Tympanoplasty with soft-wall reconstruction of ear canal using the postauricular myo-periosteal flap can recover the modality and function of external auditory canal on the whole, and the cavityplasty of auricular concha is not needed. The postoperative hearing can be improved by this technique satisfactorily.

OBJECTIVE@#To investigate the anti-tumor effects and the mechanism of the recombinant fowlpox virus expressing Apoptin gene on human laryngeal carcinoma Hep-2.@*METHOD@#Hep-2 cells cultured in vitro were infected with vFVApoptin. The anti-tumor effects on Hep-2 cells were measured through MTT staining and, the mitochondrial trans-membrane potential (delta psi m) and reactive oxygen species (ROS) were analyzed by flow cytometry. Western blot was used to detect the release of cytochrome c (Cyto c). Caspase-3/9 activities were measured by colorimetric assay.@*RESULT@#vFVApoptin could restrain Hep-2 cells significantly and, had the function of down-regulating delta psi m, up-regulating ROS, promoting Cyto c release and activating Caspase-3/9.@*CONCLUSION@#Cyto c were released from mitochondria by the function of up-regulating ROS of vFVApoptin. Cyto c triggered Caspase-9 and, after the activation of Caspase-9, downstream apoptotic factors, such as caspase-3, were activated. Eventually, Hep-2 cells were suppressed by mitochondrial pathway apoptosis induced by vFVApoptin.

ABSTRACT

Objective To investigate the prevalence and pattern of androgenetic alopecia (AGA) in Shanghai through a community-based survey. Methods A cluster sampling survey was done among the residents in Beixinjing Community, Changning District, Shanghai. All the subjects were asked to fill a questionnaire to provide their general information, including sex, age, native place, physical status, life habit, family history, etc. The diagnosis of AGA was made by dermatologists. To determine the pattern of hair loss,Norwood-Hamilton classification system and Ludwig classification system were used for male AGA and female AGA, respectively. All the data were statistically analyzed by EpiData and SPSS11.5 software. Results Totally, 7056 subjects completed the questionnaire, including 3519 males and 3537 females, and the response rate was 72.5%. AGA was diagnosed in 809 patients, consisting of 701 males aging from 19 to 91 years (mean 64.16±11.9 years) and 108 females aging from 35 to 91 years (mean 70.46±18.89 years). The standardized prevalence (SP) was 9.47% in total, 15.73% in males and 2.73% in females; the difference was significant between males and females (χ2=356.00, P<0.001). A family history of AGA was observed in 52.7% of all subjects including 391 (55.78%) males and 35 (32.41%) females. Type Ⅲ vertex involvement was the most common type in men aging from 20 to 70 years old, and type Ⅵ in those over 70 years old. Grade Ⅰ and Ⅱ predominated in female AGA. Conclusions The results of this survey indicate that the prevalence of AGA is remarkably higher in men than that in women. Furthermore, the prevalence is steadily increased with advancing age in Shanghai.

Objective To investigate the association of HLA-DRB31*03,*04 and *11 alleles with alopecia areata(AA)in Han Nationality in East China.Methods Polymerase chain reaction-sequence specific primer(PCR-SSP)method was conducted in 158 Chinese Han patients with AA as well as in 172 healthy human controls in East China.The relationships of HLA-DRB1 polymorphism to age of onset,episode frequency,clinical course,family history,and severity of AA were evaluated.Results No significant differences were observed for the frequency of HLA DRB1*03,*11 alleles between the patients and human controls,while increased frequency of HLA-DRB1*04 was observed in patients(OR=1.99,Pc=0.01).Multiple logistic regression analysis revealed that HLA-DRB1*04 was more prevalent in patients with an onset after 16 years of age(OR=1.94,Pc=0.02),those without family history(OR=1.97,Pc=0.02),those with recurrent AA(OR=2.49,Pc=0.02),those with a clinical course of more than 1 year(OR=2.94,Pc=0.01),those with severe AA(OR=3.53,Pc=0.00)and tbose with single episode of AA(OR=1.83,Pc=0.04)in comparison with the normal human controls.Conclusion This study demonstrates that HLA-DRB1*04 allele is associated with the occurrence and clinical types of AA in Han Nationality in East China.

ABSTRACT

<p><b>OBJECTIVE</b>To determine the role of recipient lymphopenia state in the expansion and function of leukemia specific cytotoxic T lymphocytes (CTLs).</p><p><b>METHODS</b>C57BL/6 mice were induced to lymphopenia with 6 Gy total body irradiation. Spleen T cells or leukemia specific T cells from EGFP+ transgenic C57BL/6-EGFP mice were adoptively transferred by intravenous injection. The mice were challenged subcutaneously with 1 x 10(6) FBL3 leukemic cells at day 2 after irradiation. The peripheral WBC count, percentage of EGFP+ cells, subsets of T cells and tumor sizes were monitored.</p><p><b>RESULT</b>Both of the spleen T cell and leukemia specific CTL proliferated efficiently with the percentage of EGFP+ cells of 28. 81% and 42.24%, respectively, after infused into lymphopenic recipients. However, spleen T cells had no anti-leukemia effect regardless of its expansion. In contrast, leukemia specific CTLs showed a more rapid and extensive expansion under the condition of lymphopenia and a enhanced anti-leukemia immunity.</p><p><b>CONCLUSION</b>Transfusion of leukemia specific CTLs under lymphopenia state could be a feasible strategy to expand leukemia specific CTLs and generate favorable anti-leukemia effect in vivo.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To study the synergistic effect of STI571, an inhibitor of tyrosine kinase in combination with arsenic trioxide (As(2)O(3)) on a multidrug-resistant leukemia cell line expressing bcr-abl.</p><p><b>METHODS</b>The cytotoxic effect of STI571 alone or in combination with different concentrations of As(2)O(3) on both bcr-abl and mdr1 positive leukemia cell line K562-n/VCR was detected by MTT method.</p><p><b>RESULTS</b>The cytotoxic effect of STI571 (1 micromol/L) combined with As(2)O(3) at concentrations 10(-5), 10(-6), 10(-7), 10(-8) mol/L (IC(50) 0.155 micromol/L) on K562-n/VCR cells was significantly higher than that of As(2)O(3) alone (IC(50) 1.879 micromol/L). The synergistic interaction on K562-n/VCR cells increased the cytotoxic effect by 12.1-fold.</p><p><b>CONCLUSION</b>Combination of STI571 with As(2)O(3) has a synergistic inhibiting effect on leukemia cells expressing bcr-abl and mdr1.</p>
