ABSTRACT

Objective To study whether portal vein thrombosis can be prevented by partial splenectomy combined with portoazygos devascularization,by preserving the integrity of the blood flow of the inferior splenic vessels.Methods 156 patients with portal hypertension and megalosplenia were divided into two groups.62 patients in group A who underwent partial splenectomy and 94 patients in group B who underwent total splenectomy.The postoperative complications,levels of WBC and PLT,volumes of residual spleen,hepatic vein pressure gradient (HVPG),and prevalence of portal vein thrombosis between the two groups were compared.Results The surgery time of group A was slightly higher than group B [(189.0± 38.5) vs.(128.0±36.3) min,P<0.05].The blood loss of group A was slightly higher than that of group B [(328.0±68.9) vs.(294.0±49.1) ml,P<0.05].The postoperative infection rate,change in HVPG and rehemorrhage rate between the two groups showed no significant differences (P>0.05).The elevation in Leucocyte after operation of the two groups were the same,peaking on day 3.The platelet count of group A increased more gently than group B (2 weeks vs 1 week),and its peak was closer to the normal threshold (P<0.05).The thrombosis rate of the portal vein system in group A was significantly lower than group B within 1 year (P<0.01).The residual spleen did not enlarge after operation (P>0.05).Conclusion Subtotal splenectomy combined with pericardial devascularization in patients with portal hypertension and megalosplenia preserved the blood flow of the subsplenic polar vessels and reduced the incidence of portal vein thrombosis.

ABSTRACT

Objective To study the impact of splenectomy and pericardial devascularization on the occurrence and development of portal vein thrombosis and the liver function in patients with cirrhosis complicated by portal hypertension.Methods 29 patients with cirrhosis and portal hypertension who underwent splenectomy and pericardial devascularization in the 302 Hospital of PLA from December 2012 to June 2013 were retrospectively studied.The incidences of PVT before and after operation were monitored.The liver function was assessed using the Child-Pugh classification.Results 29 patients with cirrhosis and portal hypertension underwent splenectomy and pericardial devascularization.The incidences of PVT in the preoperative period,12 days,3 months,6 months after operation were 10.3%,89.7%,51.7%,24.1%,respectively.The Child-Pugh scores in the preoperative period,12 days,3 months,6 months after operation were (5.2 ± 0.4),(5.6 ± 0.7),(5.2 ± 0.7),(5.3 ± 0.7),respectively.Conclusions The incidences of postoperative PVT increased after operation,but it decreased on long-term follow-up after operation.The liver function did not change.

ABSTRACT

Objective To explore the expression differences of long noncoding RNA (LncRNA) LOC100288637 in liver canc‐er ,the effect of LOC100288637 on liver cancer cell proliferation and the relevant mechanisms .Methods Based on the analysis of the GEO database data sets GSE58043 and GSE10694 ,we found that both LOC100288637 and hsa‐miR‐101‐3p has obvious expression differences in liver cancer tissues .RNA hybrid revealed the possibility of combination between LOC 100288637 and hsa‐miR‐101‐3p;RT‐PCR was performed to measure the expression level of LOC100288637 in tissues and cells ;fluorescence in situ hybridization was used to observe LOC100288637 localization in cells ;cell proliferation was determined by CCK8 experiment after LOC100288637 siRNA knock down .The expression of LOC100288637 in cells were measured after treated with hsa‐miR‐101‐3p mimics .Results Relative quantitative expression of LOC100288637 in liver cancer tissues group was significantly higher than that in no tumor tis‐sues group (P<0 .05);relative quantitative expression of LOC100288637 in liver cancer cell lines were significantly higher than that in normal liver cell line(P<0 .05) .LOC100288637 was located in both cytoplasm and nucleus of HepG2 cells ,and mainly in cy‐toplasm .Cell proliferation vitality of HepG2 reduced after treated with LOC100288637‐siRNA(P<0 .01) .Relative quantitative ex‐pression of LOC100288637 in HepG2 reduced after treated with hsa‐miR‐101‐3p mimics ( P< 0 .05 ) .Conclusion LncRNA LOC100288637 may play an important role in liver cancer development ,it can be down regulated by hsa‐miR‐101‐3p and affect the proliferation of liver cancer cells .

ABSTRACT

Objective:To analyze the features and clinical significance of damages to non pulmonary organs like the heart,kidney and peripheral blood in SARS patients.Methods:SPSS 11.0 and SDAS software packages were used to retrospectively analyze the damages to the heart,kidney and peripheral blood in 330 clinically confirmed SARS cases.Results:(1)The heart damage occurred within 1 week after SARS onset in mild cases; the rates of LDH,HBDH,CK and CK MB returning to normal were 74.2%,80.6%,100% and 100%,respectively.In severe cases,the heart damage usually occurred 1 week after SARS onset; the rates of LDH,HBDH,CK and CK MB returning to normal were 32.2%,29.5%,55.2% and 44.4%,respectively.Ten patients whose CK MB didnot return to normal died.(2) The changes of RBC count appeared after an average of 1 week (1 10 d) in 54.2% of the patients.It returned to normal in all the mild SARS cases after an average of 12.5(4 21) d but didnot in 18.4% of the severe cases ( P

ABSTRACT

Objective To investigate the relationship between motor abnormality of sphincter of Oddi(SO) and hepatolithiasis.Methods The hepatolithiasis rabbit models were established in adult rabbits(n=64) which were randomly divded into 2 groups:control group(n=16)and experimental group(n=48),including 2 wk,4 wk and 8 wk group(n=16 per group).During the formation process of hepatolithiasis,bile character and ratio of stone formation were both observed,then the myoelectric waves of SO were recorded.Finally,The SO segment was sliced and observed by transmissional electron microscope.Results The rate of true stone formation in group 8 wk(68.75%,11/16)was higher than that in group 4wk(12.50%,2/16)and in group 2wk(0.00%,0/16).Compared with the control group,there was significant decrease of both SO myoelectric peak amplitude and SO myoelectric frequency in the experimental group.Electron microscopy showed that SO cells in control group and 2wk group had no change,but the 4wk and 8wk groups underwent changes including fibrolysis of myofilament,increase of collagen fibers,fusion and thickening of microfilament,decrease and array disorder of macula densa,swelling of chondrosome and endoplasmic reticulum,widening of perinuclear space and deviation and pyknosis of nucleus,and formation of inclusion body in cytoplasm;and the degeneration degree in 8wk group was obvious than that in group 4wk.Conclusions The functional abnormality of SO may contribute important effects on the formation of hepatolithiasis.Fibrous degeneration of SO maybe the pathogenesis of functional abnormality of SO.
