ABSTRACT

Objective To study the risk factors of mortality in severe sepsis and septic shock patients. Methods 142 patients with severe sepsis and septic shock in ICU were observed and divided into alive group (98 patients)and dead group (44 patients)by using hospital mortality. The risk factors of mortality in severe sepsis and septic shock patients were assessed by binary logistic regression. Results Independent mortality risk factors were inotropic agents (OR =4.329,95%CI:1.045 -17.937,P =0.043),blood glucose >10 mmol/L (OR =3.771,95%CI:1.214 -11.710,P =0.022)and APACHE Ⅱ score (OR =3.098,95%CI:2.012 -4.760,P =0.000),while PaO2 /FiO2 after early goal -directed therapy (EGDT)was protective factor (OR =0.682,95%CI:0.500 -0.930, P =0.016).Conclusion Severe sepsis and septic shock patients with inotropic agents, blood glucose >10mmol/L, high APACHE Ⅱ score and decreased PaO2 /FiO2 after EGDT indicate poorly prognosis.

ABSTRACT

To investigate the effect of berberine on glycemia regulation in rats with diabetes and the related mechanisms.Diabetic-like rat model was successfully induced by intraperitoneal injection of streptozotocin in 50 out of 60 male SD rats, which were then randomly divided into 5 groups with 10 rats in each:control group (received vehicle only), positive drug control group (sitagliptin 10 mg·kg·d), low-dose berberine group (30 mg·kg·d), moderate-dose berberine group (60 mg·kg·d), and high-dose berberine group (120 mg·kg·d). All animals were fed for 3 d, and fasting blood sampling was performed on day 3 of administration. Rats were given glucose (2 g/kg) by gavage 30 min after the last dose. Blood and intestinal samples were obtained 2 h after glucose loading. Fasting blood glucose (FBG) and 2-h postprandial plasma glucose (2h-PPG) were detected by using biochemical analyzer, and insulin, glucagon-like peptide-1 (GLP-1) and dipeptidyl peptidase-Ⅳ(DPP-Ⅳ) were measured by using ELISA kit.No significant difference in FBG and serum DPP-Ⅳ level were found between berberine groups and control group (all>0.05). Compared with control group, serum levels of GLP-1 and insulin were increased in high-and moderate-dose berberine groups, while 2h-PPG was decreased (all<0.05); GLP-1 levels in the intestinal samples were increased, while DPP-Ⅳ levels were decreased in all berberine groups (all<0.05).Short-term berberine administration can decrease 2h-PPG level in streptozotocin-induced diabetic rat model through local inhibition of intestinal DPP-Ⅳ. The efficacy of DPP-Ⅳ inhibitor may be associated with its intestinal pharmacokinetics.

ABSTRACT

Objective To evaluate the effect of resistance training on glucolipid metabolism in a population with pre-diabetic metabolism (PDM).Methods Sixty persons with PDM were randomly divided into a resistance training group,an aerobic training group and a control group, each of 20 members. The exercise intervention groups exercised 3 times a week for 12 weeks in accordance with the exercise prescription, while the control group was without any regular aerobic exercise or resistance training. Before and after the 3 months of exercise training, fasting blood glucose (FBG),2 hours postpradial blood glucose (PBG),HbAlc,and lipid profile were tested.Body mass index (BMI),waistline, and blood pressure were also measured. Results Before the intervention, there were no significant differences in any of the average values among the 3 groups. In the resistance group, the average FBS (5.52 ± 0.52 mmol/L),HbA1 c (5.92 ± 0.36%) and TG (1.65 ± 0.92 mmol/L) had all decreased significantly after the training. In the aerobic group the average waistline, dilated blood pressure,FBG and HbAlc had decreased significantly. In the control group the average 2hrs PBG and LDL-C had both increased significantly compared to 3 months earlier.Compared with the resistance group, the average 2hrs PBGs were significantly higher in both the aerobic and control groups after the training. Moreover, compared with the aerobic group, the value in the control group was also significantly higher. Conclusion Both resistance training and aerobic exercise can lower fasting blood glucose and HbA1 c in PDM patients without obvious effect on BMI or low density lipoprotein level.Compared with aerobic exercises, resistance training had significant advantages in decreasing 2-hour postprandial blood glucose.

ABSTRACT

Adult stem cells are drawing more and more attention due to the potential application in degenerative medicine without posing any moral problem. There is growing evidence showing that the human amnion contains various types of adult stem cell. Since amniotic tissue is readily available, it has the potential to be an important source of regenerative medicine material. In this study we tried to find multipotent adult stem cells in human amnion. We isolated stem cells from amniotic mesenchymal cells by limiting dilution assay. Similar to bone marrow derived mesenchymal stem cells, these cells displayed a fibroblast like appearance. They were positive for CD105, CD29, CD44, negative for haematopoietic (GlyA, CD31, CD34, CD45) and epithelial cell (pan-CK) markers. These stem cells had the potential to differentiate not only into osteogenic, adipogenic and endothelial lineages, but also hepatocyte-like cells and neural cells at the single-cell level depending on the culture conditions. They had the capacity for self-renewal and multilineage differentiation even after being expanded for more than 30 population doublings in vitro. So they may be an ideal stem cell source for inherited or degenerative diseases treatment.
