ABSTRACT

Objective@#To observe the bone marrow signals of acetabulum and proximal femur of asymptomatic non-professional marathoners by 3.0 T magnetic resonance imaging (MRI) T1WI, and evaluate the bone marrow transformation, so as to obtain the effect of Marathon exercise on bone marrow composition and function.@*Methods@#The study group was randomly selected to participate in and complete the whole marathon at least once a year in the past two years. The training mileage of long-distance running was not less than 1 600 kilometers per year. There were no symptoms such as hip pain. There were no abnormalities in hip joint physical examination. The age of 22-53 years old. A total of 31 and 62 hips were evaluated. The control group was randomly selected 29 healthy persons (58 hip joints), aged 23-53 years, without hip pain and regular exercise. All subjects underwent hip joint MRI scan, and the hip joint MRI showed normal. At least 12 hours before MR scan, he did not engage in long-distance running or other sports. The bone marrow signal intensity of acetabulum and proximal femur in T1WI was compared with that of surrounding muscles and fat. The signal intensity was graded from low to high and evaluated by grade. The research group was divided into two groups according to the training years of marathon (running age). The running age of group A was more than 4 years and group B was less than 4 years. The distribution of bone marrow signal in proximal femur was also evaluated by a more intuitive 3-4 classification method. Mann-Whitney U test was used for statistical analysis.@*Results@#Bone marrow signal grading evaluation showed that there were significant differences in bone marrow signal grade distribution between the two groups (Z=-6.828, -4.779, -3.046, -5.266, -3.490, - 5.053, P<0.05). In the study group, there were 14, 28 hips and 168 parts in group A, 17, 34 hips and 204 parts in group B, bone marrow signals were graded. There were significant differences in acetabulum, femoral neck and upper femoral shaft bone (Z=-2.202, -2.214, -2.730, P<0.05), but no significant differences in femoral head, trochanter and trochanter bone (Z=-0.886, -1.642, -0.711, P>0.05). To evaluate the classification of bone marrow signals in proximal femur, 62 cases of bone marrow signals in the study group were classified as follows: 10 cases with type 1a, 24 cases with type 1b, 17 cases with type 2 and 11 cases with type 3. In the control group, 58 cases of bone marrow signals in proximal femur were classified as follows: 2 cases with type 1a, 13 cases with type 1b, 26 cases with type 2 and 17 cases with type 3. There were significant differences between the two groups (Z=-4.003, P<0.05).@*Conclusion@#The T1WI signal intensity of asymptomatic non-professional marathoners′ acetabulum and proximal femur bone marrow is lower than that of non-marathoners; the T1WI signal intensity of acetabulum, femoral neck and upper femoral shaft bone marrow of the elderly runners is lower; that is, marathon exercise has certain influence on the bone marrow signal of acetabulum and proximal femur, that is, the change of bone marrow signal in weight-bearing area has a certain phase with the amount of exercise.

ABSTRACT

Objective To evaluate the application value of inhibitor enhanced modified carbapenemase inactivation method (imCIM) in the detection of class B carbapenemase.The differences between imCIM and EDTA disc potentiation test (EDPT) were comparatively analyzed.Methods A total of 181 strains of carbapenem insensitive strains were collected,among which there were 44 strains of Klebsiella pneumoniae,44 strains of Escherichia coli,43 strains of Acinetobacter baumannii and 50 strains of Pseudomonas aeruginosa.The 83 strains of carbapenem-sensitive strains were composed of 25 strains of Klebsiella pneumoniae,16 strains of Escherichia coli,25 strains of Acinetobacter baumannii and 17 strains of Pseudomonas aeruginosa.The class B carbapenemase in the 264 strains of pathogenic bacteria was screened by imCIM and EDPT,and PCR results were used as gold standard.The statistical analysis wasperformed with consistency check,related-sample Wilcoxon signed rank sum test,independent samples Kruskal-Wallis H test and ROC curve.Results Among the 181 strains of carbapenem insensitive strains,PCR results of 144 strains were positive for drug resistance gene.The samples of class A,B and D of carbapenemase were 39,77 and 28 strains respectively.The results of imCIM showed that 70 strains were positive,and the other 111 strains were negative.The imCIM results of 166 strains were consistent with those of PCR.The results of EDPT showed that 72 strains were positive,and the other 109 strains were negative.The EDPT results of 134 strains were consistent with those of PCR.The results of PCR,EDPT and imCIM of 83 carbapenem sensitive strains were negative.The sensitivity and specificity of imCIM were 85.71% (66/77) and 97.86% (183/187),and the value of Kappa was 0.859.The sensitivity and specificity of EDPT were 66.23 % (51/77) and 88.77 % (166/187),and the value of Kappa was 0.561.The difference of inhibition zone of imCIM (AdimCIM) was different from EDPT(AdEDPr) and the difference was statistically significant (Z =-6.941,P < 0.05).In the imCIM detection,the AdimciM level of class B carbapenemase showed different population distribution position from class A and D carbapenemase with the statistically significant difference (x2 =108.887,P < 0.05).The areas under the ROC curve of imCIM and EDPTwere 0.988 (95%CI:0.977 to0.999) and0.936 (95%CI:0.909 to0.963),respectively.Conclusion imCIM should be accurate,efficient and convenient for screening of carbapenem phenotype for its high sensitivity and specificity,and suitable for epidemiological monitoring.

ABSTRACT

Objective To investigate the feasibility of modified rapid Carba NP test for the detection of carbapenemase,and analyze the differences between the modified method and Carba NP test.Methods A total of 264 strains of gram-negative bacillus,including 164 carbapenem-resistant strains and 100 sensitive strains,were collected,and their carbapenemase were detected by Carba NP test and the modified rapid Carba NP test,respectively.The differences between the two tests were evaluated based on PCR as a reference.Results Among 164 carbapenem-resistant strains,carbapenemase gene was detected in 144 strains by PCR.The carbapenemase gene was negative in 100 sensitive strains.Among 164 carbapenem-resistant strains,135 were positive for the Carba NP test,while 130 for the modified rapid Carba NP test.One hundred of sensitive strains were negative for the two Carba NP tests.Compared with the results of PCR,the sensitivity,specificity and Kappa value of the Carba NP test were 91.7% (132/144),97.5% (117/120) and 0.886,respectively,while those of the modified rapid Carba NP test were 89.6% (129/144),99.2% (119/120) and 0.879,respectively.There was no significant difference in the positive rates between Carba NP test and the modified rapid Carba NP test (x2 =1.45,P > 0.05).Conclusion The modified rapid Carba NP test which has high consistency with the PCR method,is faster and cheaper than the Carba NP test,and may be applied to epidemiologic survey and the early monitoring of nosocomial infections.

ABSTRACT

Objective To evaluate the usefulness of the submucosal tunnel ureteroileostomy using ileovalvular segment as afferent loop in preventing ureteral refluxes and upper urinary tract infection in detenia ceacal-colon continent urinary diversion. Methods Ten patients underwent submucosal tunnel ureteroileostomy in detenia ceacal-colon continent reservior and detenia coloneobladder and the follow up for ureteral reflux and upper urinary tract infection was carried out from 1998 to 2001. Results The patients were followed up for 6 to 36 months.No evidence of ureteral reflux and upper urinary tract infection was observed.Unilateral hydronephrosis was found in one patient caused by the ureteroileal anastomotic stricture. Conclusions Submucosal tunnel ureteroileostomy is an effective fashion for detenia ceacal-colon continent urinary diversion in preventing the postoperative complications of ureteral reflux and upper urinary tract infection.

ABSTRACT

The relative content of DNA-RNA and the nuclear area/nucleolar area ratio of the hyperplastic tissue,glottic polypus,dysplastic tissue of grade I to I laryngeal squamous cell carcinoma were measured in 33 cases with image analysis technique.It was found that the changes of DNA-RNA content and the ratio of nuclear/nucleolar area were parallel to those of the severity of the carcinoma,and RNA was markedly increased in grade 11 and I dysplastic tissue.On the basis of the current grading of laryngeal pathology,the combined determination of DNA-RNA can provide more sensitive indicators to the stages of malignant degeneration of the laryngeal squamous cell carcinoma.

ABSTRACT

Porcine transfer factor(TF-P) was prepared from porcine spleen by means of hom-ogenization and vaccum dialysis. Fractionized by Sephadex G-10 gel chromatography,TF-P could be separated into seven fractions but five fractions were obtained from human transfer factor(TF-h) .These fractions of TF-P and TF-h and different dilutionsof TF-P were tested for biological activity in vitro. By the use of ~3H-TdR uptake assay we found that, both of whole TF-P and TF-hpossessed suppresivs activity to PHA-induced human periperal blood lymphocyte tran-sformation. Whereas, Fraction Ⅲ of TF-P and Fraction Ⅲ and Ⅳ of TF-h containedcomponents responsible for augmentation of PHA-induced responses. When TF-P was diluted 10~(-2) to 10~(-5) and cultured with normal human peripheralblood lymphocytes, we found that the original suppression effects of whole TF-P toPHA-induced lymphocyte transformation were converted to normal. The optimum dil-ution of TF-P was 10~(-3), which were shown to augment responses. It shoud be notedthat the responsiveness of normal human lymphocytes to TF-P was dose dependent.
