ABSTRACT

Huacao Community Health Service Center has established an integrated chronic kidney disease (CKD) management system since 2015,which greatly improved CKD management in the community.From September 2016 to August 2017,4084 residents were screened by the family doctor team,975 were suspected to have CKD (23.87%),among whom 202 (20.72%) were referred to district CKD treatment center,and 197 (97.52%) were confirmed as CKD.The detection rate of suspected CKD among patients with hypertension and diabetes mellitus was the highest,which was much higher than that among outpatients and those with routine health check up.It is suggested that the integrated management of CKD should be widely implemented to cope with the prevalence of CKD,particularly for those with hypertension and diabetes.

ABSTRACT

Objective:To analyze survival in patients with advanced oral cancer from prospective clinical trials. Methods:From 2008 to 2010, 256 patients with oral cancer at clinical stage III/IVA were randomly categorized into two groups. Patients in the experi-mental group received neo-adjuvant chemotherapy, surgery, and post-operative radiation, and patients in the control group underwent surgery and post-operative radiation. All patients were routinely followed-up after treatments. Survival was analyzed using Kaplan–Meier method and log-rank test, and differences were considered statistically significant at P value lower than 0.05. Results: Each group was composed of 128 patients. With the median follow-up period of 60 months, the 5-year overall survival rate was 61.7%and the disease-free survival rate was 53.9%. The overall survival rate (P=0.350) and the disease-free survival rate (P=0.160) were not sig-nificantly different between the experimental and control groups. Patients with positive pathological response to neo-adjuvant chemo-therapy exhibited significantly improved overall survival (P<0.05). Conclusion:Radical surgery should be emphasized to improve the prognosis of oral cancer. Functional reconstruction could also improve the quality of life and survival of patients. Despite that neo-adju-vant chemotherapy could not improve the survival of patients with advanced oral cancer in entirety, it could benefit patients exhibiting positive treatment responses.

ABSTRACT

<p><b>OBJECTIVE</b>The purpose of this study was to establish an ideal operation method to deal with head and neck carcinomas of late stage and involving the carotid.</p><p><b>METHODS</b>A total of four metastasized head neck carcinomas involving the carotid arteries were included in this clinical trial. The primary tumors of the four cases tumors were carcinomas of tongue, esophagus, laryngeal and parotid glands. All four cases were examined by means of TCD, DSA, SPECT to confirm good compatibility of brain blood circulation before operations. The bony petrous sections of carotid arteries were drawn off after the exposure of the bone tissue for exterior skull. A by-pass was set up by inserting a shunt tube between the common carotid and the infernal carotid artery. The extended tumor resection was carried out involving the resection of the carotid artery. The artery was reconstructed with saphena magna vein transplantation in one case, and replaced by artificial vessels in two cases. Tissue defects were repaired by pedicle PMMF.</p><p><b>RESULTS</b>The B-typed ultrasound and TCD examination two weeks to six months post-operatively showed the four cases revealed good condition of transplanted carotid arteries, except one case occurred Honer's syndrome, but there was no central nerve's symptom or complication in the other three cases.</p><p><b>CONCLUSION</b>This operation method could provide a good chance of survival by curettage of later staged head neck tumors. The authors recommend reconstruction of carotid artery in the operation treatment of malignant head neck carcinomas during late stages.</p>

ABSTRACT

Objective: To study the feasibility of closed and three-dimensional reconstruction of maxillary defects with titanium mesh and free forearm flap. Methods: Maxillary defects, 3 of type Ⅱ and 7 of type Ⅲ, due to tumor or trauma were closely reconstructed with titanium mesh to restore the profile of maxilla and with radial flap to close the oral and the nasal wound surfaces following total maxillectomy (in 3 cases) and subtotal maxillectomy(in 7 cases). Results: 10 cases were followed up for 3~18 months. All the flaps were alive. The maxilla nasal and oral cavity were restored in all the cases and denture was applied in 3 cases. No neoplasm were fond by CT. Mouth opening was 2.5~4.0cm. Epithelium on the surface of the titanium mesh was found by nasopharyngoscope in 2 cases. Conclusion: The closed and three-dimensional reconstruction of maxillary defect with titanium mesh and free forearm flap can restore the shape and the function of maxilla.
