ABSTRACT

Melorheostosis is a rare disorder of osteopathia. The clinical characteristics of melorheostosis in children is totally different from that in adults. The radiographic features of melorheostosis include the hyperostosis in endosteal on the one side of the cavum medullare and formed streakiness. The soft tissue contracture of the limb and joint deformities are the symptoms of melorheostosis. Here, the authors reported a melorheostosis case of 6 years old girl who suffered from severe valgus deformity of the right knee with permanent patellar dislocation. A surgical stabilization was applied with lateral soft tissue release, medial soft tissue stabilization and transferred the vastus medialis laterally (kinetic stability). After 19 years follow up postoperatively, the limb developed well in satisfied alignment with good function of knee joints, even participated in some sports activities. Other authors reported a similar case of melorheostosis with surgical treatment and achieved good outcomes in limb realignment and reduction. The further suggested that the surgical treatment should be produced before epiphyseal closure.

ABSTRACT

From March 2000 to March 2003,15 cases of post internal fixation of fractures and arthroplasty infection were selected from Department of Traumatic Orthopedics,Tianjin Hospital,including 6 males and 9 females.The average age of re-operation was 56 years old.The average time from the primary surgery to diagnosis of infection was 0-19 months.Of the 15 cases,4 of arthroplasty infection underwent prosthesis extraction and temporary fixation with bone cement containing antibiotic and underwent another arthroplasty 6 months later.Of 11 cases of infection following internal fixation,the internal fixation was effective in 2 cases,and the fixation was removed after fracture healing,and ineffective in 9 cases.The fixation was removed primarily and focus of infection was cleared,lavaged with antibiotic solution,and another internal fixation was performed.The pus samples of 15 cases showed positive.Eleven cases(73.3%) had chronic infection including 9 occurring following fracture fixation and 2 following arthroplasty.Four cases(26.7%) had acute infection,including 2 occurring following arthroplasty and 2 following fracture fixation.No infection caused by blood circulation was found.The Gram-positive bacteria,staphylococcus aurens,staphylococcus epidermidis,and enterococcus occupied the most part of the infections in spite of the location and type of the infections,and Gram-positive bacteria-staphylococcus aurens was the most common.The order of the antibiotics sensitivity was Vancomycin,Rifampicin,Gentamicin,and Cephazoline.In addition,antibiotics sensitivity was varied in different infections.In samples of 15 cases,2 cases were sensitive to two bacteria at the same time following fracture fixation.The staphylococcus aurens,staphylococcus epidermidis,and enterococus are the high risk of injection following internal fixation of fractures and arthroplasty.The sensitive antibiotics and long-term medication are effective methods to treat the infection.

ABSTRACT

Objective To compare the different effects of continuous and intermittent administration of etidronate disodium in bone formation for osteoporosis.Methods Fifty 3 month old virgin female rats weighing (200?10) g were divided equally into 5 groups:pseudo ovariectomized group (group A), ovariectomized group (group B),estrogen replacement group (group C),intermittent etidronate disodium administered group (group D) and continuous etidronate disodium administered group (group E).Postoperative 30 days,group C was given nylostricl 0 1 mg?kg -1 ?d -1 for 100 d;group D was intermittently given etrdonate disodium 10 mg?kg -1 ?d -1 for 14 d and after 35 d interval the drugs were given once more;group E was continuously given etrdonate disodium 10 mg?kg -1 ?d -1 for 100 d.All rats were given drugs orally and were killed at day 100.The bone density,bone mineral content and bone histomorphometrical study were made.Results The average of BMD values of group A,B,C,D,E were [(47 6?2 7),(31 6?7 0),(38 2?4 9),(41 8?8 4),and (29 4?9 4) ( [AKx-]?s ),mg/cm 2] respectively.The value of group E was significant lower than that of group A and group D ( P

ABSTRACT

Objective To study the effect on bone formation by intermittent administration of simvastatin. Methods Seventy three month old virgin female rats, weighted (200?10) g each had been divided into two groups ovariectomized (50 rats) and pseudo ovarectomized (20 rats). Ten rats each of Group A and Group B were sacrified at the 30 th day for the confirmation of successful establishment models of the osteoporosis. Then the remaining animals were re divided into 5 groups: Group A, pseudo ovarectomized control, 10 rats; Group B, ovarectomized control, 10 rats; Group C, given Nylestriol 0.1 mg/(kg?d), 10 rats; Group D, 10 rats, given calcium 10 mg and vitamin D3 2 IU/(kg?d); Group E, 10 rats given simvastatin 5 mg/(kg?d) for 14 days; and the drug was suspended for 28 days, then simvastatin was given for another 14 days. All of the animals were given the agents through gastric tube 30 days after surgery. At 120 th day all the rats were sacrified for the measurements of bone mineral content and bone mineral density by the dual energy X ray absorptiometry for bone mophometrical study.
