ABSTRACT

Objective:To monitor the cerebral vascular blood flow parameters in the early stage of simulated acute exposure to high altitude hypoxia by transcranial color Doppler (TCCD), and to evaluate the change trend of cerebral hemodynamics and cerebrovascular reactivity.Methods:Sixty-four healthy volunteers were selected to observe the changes of peak systolic flow velocity(Vs), end diastolic flow velocity(Vd), mean flow velocity(Vm), resistance index (RI) and pulsatility index (PI) of middle cerebral artery (MCA) 30 minutes after they quickly entered the simulated altitude of 4 500 meters. Combined with breath holding test, breath holding index (BHI) was used to evaluate cerebrovascular reactivity (CVR), and subjects were divided into ≤30 years old group and >30 years old group, and the changes of CVR after hypoxia of the two groups were compared. Results:In the early stage of hypoxic environment, compared with baseline, SpO 2 decreased, heart rate increased, and blood flow velocity of middle cerebral artery(Vs, Vd, Vm) increased significantly, BHI showed a decreasing trend (all P<0.01). After hypoxia, the BHI rate of change in >30 years old was lower than that of the subjects ≤30 years old ( P<0.05). After breath holding, cerebral blood flow velocity increased significantly, PI and RI decreased significantly (all P<0.01). Conclusions:Cerebral blood flow is very sensitive to hypoxia. The application of TCCD technology can evaluate the trend of cerebral blood flow dynamics and cerebrovascular reserve capacity in real time and accurately, which is helpful to provide objective basis and research basis for the prevention and treatment of high altitude hypoxia.

ABSTRACT

Objective To explore the clinical characteristics of tuberous sclerosis complex (TSC). Methods The clinical data of one child with TSC were collected. The clinical features and gene mutation were analyzed. Results A 36-day-old girl had abnormal nodules found by echocardiography, which was considered multiple cardiac rhabdomyomas. There were multiple hypomelanotic macules distributed over the skin surface of the trunk and legs. Cranial MRI showed cortical nodules, subependymal nodules and cerebral white matter radial migration line. A mutation in the TSC2 gene (c.4541-4544delCAAA) was found by second generation high-throughput sequencing technology and tuberous sclerosis complex was confirmed. Conclusion Gene detection is helpful in the early diagnosis of tuberous sclerosis complex.

ABSTRACT

Chemotherapy induced phlebitis is a kind of acute and local inflammation caused by chemotherapeutics, which has complicated factors. There are many prevention measures. One of them is phytotherapy, which is widely used as a part of natural therapy. This article summarized the phytotherapy used for chemotherapy induced phlebitis, with the purpose to provide the references for enhancing the clinical intravenous administration and ensuring the smooth progress of chemotherapy.

ABSTRACT

Primary hepatic malignant fibrous histiocytoma(PHMFH)remains extremely rare with less than 60 cases reported in literature. From October 1999 to May 2008,5 patients with PHMFH had been admitted to Yantai Cancer Hospital,Yantai Yuhuangding Hospital and the First Affiliated Hospital of Wenzhou Medical College, and the results of spiral CT were analyzed.Six tumors were detected, and the maximum diameter of the tumors was more than 8.5 cm.CT plain scanning revealed that all tumors were hypodense,4 patients with tumor necrosis and cystic degeneration, and the tumor in 1 patient wag with homogen density.Enhanced CT scanning showed marked enhancement of the solid component of tumor in 4 patients, slight enhancement in 1 patient, and a"fast in and fast out"sign in 5 patients.Hepatic bile ducts and portal vein were not involved. One patient was with portal lymph node metastasis and 1 with ioferior vena cava involvement. The characteristics of spiral CT imaging of PHMFH include tumor necrosis, cystis and invasion,as well as"fast in and fast out"sign, which could help to diagnose, although the ultimate diagnosis depends on histopathological examination.
