ABSTRACT

OBJECTIVE: To explore related factors of changes in gouty patients by ultrasonography after initiation of uric acid lowering therapy. METHODS: There were 72 gout patients enrolled who admitted to First hospital of Peking University from December 2012 to June 2017. All the patients had clinical and ultrasound examination at both knees, ankles and feet joints at the baseline. Regular uric acid lowering therapy started for one year. The endpoint was the last time who repeat the ultrasound examination during the follow up.According with uric acid level and its measuring time, the area under the curve was calculated to reflect the uric acid burden. RESULTS: In the 216 jonts of 72 patients, double contour sign were detectable in 112/216(51.85%), tophi were detectable in71/216(32.87%).Among MTP Joints, DCS was detected in 32/144(22.22%), and tophi in 47/144(32.64%). Among Knees joints, DCS was detected in 33/144(22.92%), tophi in 4/144(2.78%). Among ankle joints, DCS was detected in 47/144(32.64%), tophi in 20/144(13.89%).DCS disappearance occurred in 38/55(69.09%) joints positive for DCS at baselineTime-to-disappearance was 148 days(985~133 days). Tophi disappeared completely in 18/45 joints(40%). Time-to-disappearance was 382.5 days(686~397.25 days). There was no significant difference in age, BMI, duration of disease and complications between DCS persisted group and DCS disappearance group.SUA in the DCS disappeared group at baseline, second, third, fourth and ninth months of follow-up. was significantly lower than that of the non-disappeared group. SUA load in the DCS group was significantly lower than the non-disappeared group. Post hoc tests showed that SUA levels fell significantly from baseline through fourth month of follow-up in DCS disappearance group, and SUA of the sixth months was the lowest. CONCLUSION: To screen for specific features of gout such as DCS or tophi by US at initiation of ULT and during follow-up is a useful, and effective way to detect the lowering and often disappearance of burden of urate load in gouty joints.

ABSTRACT

Rapid mass spectrometry analysis of protein in the solution sample was carried out by using a charged bubble extraction ionization device. In this work, experimental parameters including gases ( N2 and CO2 ) , bubble path length, voltage, and gas pressure on the charged bubble extraction ionization of lysozyme were investigated. Under the optimum experimental parameters including CO2 as extraction gas, bubble path length of 32 cm, solution voltage of 2 kV and gas pressure of 0. 05 MPa, this method was successfully used for the protein detection with the limits of detection of 1 ×10-8 mol/L in water solution and 1 ×10-7 mol/L in diluted urine (200 times in ultrapure water) . In addition, the limit of detection of 1 ×10-5 mol/L in undiluted urine was obtained with a urine volume of 6 mL at 2 kV voltage and 0. 06 MPa gas pressure. By comparing the desalting effects in charged bubble extractive ionization mass spectrometry and ESI-MS, it was found that the bubble charged extraction ionization method could obtain a wider and lower charge state distribution of protein ions, and had higher tolerance ability when facing non-volatile inorganic salts. Off-line study of the collected catalase after charged bubble extraction showed that 53. 9% enzyme activity was remained, which indicated that the proposed method was a soft ionization method. The method had merits including no sample pretreatment, no chemical reagent contamination and high speed, showing potential application to mass spectrometry analysis of protein components in solution. Therefore, this study provided a new method for mass spectrometry analysis of biological protein samples.

ABSTRACT

AIM: To compare the efficacy and safety of bevacizumab with mitomycin ( MMC ) in augmenting trabeculectomy for glaucoma. METHODS: Databases including PubMed, CBM, CNKI, VIP and WanFang Data were electronically searched for all randomized controlled trials ( RCTs) about comparing the efficacy and safety between bevacizumab and MMC in augmenting trabeculectomy for glaucoma before the date of Jun. 2016. Two reviewers independently screened literatures according to the inclusion and exclusion criteria, and evaluated the included studies. Then, Meta-analysis was performed. RESULTS: A total 4 RCT involving 286 eyes ( 143 for bevacizumab group, 143 for MMC group) were included. The results of Meta-analysis showed that there was no significant difference between bevacizumab and MMC in the last follow-up after surgery in IOP (WMD=2. 21, 95%CI: -0.17 to 4.58, P=0.07), complete success rate (OR=0. 69, 95%CI:0. 26 to 1. 81, P=0. 45) and the numbers of anti-glaucoma medicine ( OR= 0. 12, 95%CI: -0. 15 to 0.39,P=0. 39). And there was no significant difference between bevacizumab and MMC in postoperative complications:hypotony (OR=0.7, 95%CI:0.12 to 4.05, P=0.69), bleb leak (OR=1, 95%CI: 0. 21 to 4. 74,P=1), encapsulated bleb (OR=1. 15, 95%CI: 0. 38 to 3. 44, P=0.81), choroidal detachment (OR=1. 22, 95%CI: 0. 29 to 5.22, P=0. 78) and cataract (OR=1. 15, 95%CI: 0. 38 to 3.44, P=0. 81). CONCLUSION: Bevacizumab and MMC in augmenting trabeculectomy for glaucoma have similar efficacy and safety. Bevacizumab can't result in better outcome in term of IOP reduction. Clinicians should choose suitable solution according to disease characteristics.

ABSTRACT

In this paper, microscopic identification method was adopted to observe the microscopic characters of ten batches of Medicago sativa seeds. And M. sativa seeds were identificated by TLC method in contrast to trigonelline and stachydrine hydrochloride. The impurities, moisture, ash, sour insoluble ash were detected based on Chinese Pharmacopoeia 2010 version (Vol I ). An HPLC method was also established for determination of trigonelline in the M. sativa seeds. The contents of impurities, moisture, ash, sour insoluble ash should not exceed 5%, 10%, 6%, and 2%, respectively. The content of trigonelline should be not less than 0.795 6 mg x g(-1). The experimental methods were accurate and reliable, and can be used as the quality control of the seeds of M. sativa.
