ABSTRACT

Hemoglobin-based oxygen carriers(HBOCs) is a category of artificial blood substitute with efficient oxygen carrying and delivery functions. Due to the outstanding oxygen-carrying properties of HBOCs, the studies of expanding its clinical applications are undergoing rapidly. Studies have shown the efficacy of HBOCs in organ transplantation as machine perfusion solution in maintaining the organ metabolism or meliorating the margin donor organ quality with promising perspectives. This article aims to review the application status and prospect of HBOCs in organ transplantation in recent years.

ABSTRACT

【Objective】 To observe the effect of glutaraldehyde polymerized bovine hemoglobin injection (code: HSRP1) oxygen-carrying fluid on early perfusion of severe hemorrhagic anemia in rabbits. 【Methods】 The rabbit model of controlled severe hemorrhagic anemia was established. Twelve modeled rabbits were divided into glutaraldehyde polymerized bovine hemoglobin injection (code: HSRP1) group and sodium lactate ringer′s injection (LR) group, with 6 rabbits in each group(half male and half female). HSPR1 group and LR group were treated with HSRP1 and LR, respectively. The survival rate of experimental rabbits was observed, and the indexes of hemodynamics, venous blood gas, plasma hemoglobin, base surplus, lactic acid and bicarbonate were measured before and after blood loss, as well as each point within 24 h after infusion. 【Results】 The survival rate of HSRP1 group was significantly different from that of LR group (P<0.05); After exsanguination, the mean arterial pressure of each group was significantly different from that before exsanguination (P<0.05), but there was no significant difference between HSPR1 group and LR group after infusion; In the second stage of perfusion, the blood lactate concentration and base excess in the HSRP1 group were significantly different from those in the LR group at each time point (P<0.05), at 2 h after perfusion, the respiratory rate started to differ significantly from that of the LR group (P<0.05), heart rate was significantly different from LR group at 4 h after perfusion(P<0.05); There were no significant differences between HSRP1 group and LR group in plasma venous oxygen partial pressure, venous oxygen saturation and plasma hemoglobin at all time points. 【Conclusion】 HSPR1 is used for severe traumatic hemorrhagic shock in rabbits, and can improve the survival rate of experimental rabbits by providing oxygen to hypoxic tissues and correcting anaerobic metabolism. As a new oxygen-carrying fluid, HSPR1 can correct the oxygen supply balance of patients with severe hemorrhagicanemia in early stage.

Based on ASP.NET framework, The Intelligent Estimated System for Rational Deployment of Medical Equipment (MERDIS) is designed and developed with SQL Server 2012 database and C# language. The system is used to realize the rational deployment suggestions and evaluation of medical equipment in hospitals. The system input the data of hospital medical equipment and clinical pathway into the database, and then feedback the deployment information to users which are calculated by big data information, so as to achieve the purpose of giving rational deployment of hospital medical equipment.

ABSTRACT

Objective@#To observe the expression level of microRNA-181 (miR-181) and importin-α3 in oxidized low density lipoprotein (ox-LDL) induced vascular endothelial cell injury models, and explore the effect and mechanism of miR-181 on endothelial cell injury.@*Methods@#Human vein endothelial cell line CRL-1730 were cultured and vascular endothelial cell injury model was established by intervention with ox-LDL. The cells were divided into control group (intervened by double distilled water), low-dose group (intervened by 10 μg/ml ox-LDL) and high-dose group (intervened by 20 μg/ml ox-LDL). In addition, cells of low-dose group were divided into miR-181 mimic group (miR-181 mimic was transfected) and mimic control group (miR-181 mimic control was transfected). Cell viabilities, mRNA and protein expression level of interleukin-6 (IL-6), miR-181, importin-α3, and nuclear transcription factor-κB (NF-κB) were measured by methyl thiazolyl tetrazolium (MTT), real-time PCR and Western blot, respectively.@*Results@#(1) The cell viabilities in low-dose group and high-dose group were lower than control group (0.207±0.012 and 0.204±0.007 vs. 0.323±0.018, all P<0.01). The relative IL-6 mRNA expression in low-dose group and high-dose group were higher than control group (1.24±0.16 and 1.36±0.23 vs. 0.22±0.03, all P<0.01). The relative miR-181 mRNA expression in low-dose group and high-dose group were lower than control group (0.91±0.11 and 0.88±0.07 vs. 2.20±0.13, all P<0.01). The relative importin-α3 mRNA expression in low-dose group and high-dose group were higher than control group (1.23±0.22 and 0.55±0.03 vs. 0.44±0.06, all P<0.01). The relative NF-κB mRNA expression in low-dose group and high-dose group were higher than control group (1.67±0.34 and 0.41±0.11 vs. 0.11±0.04, all P<0.01). The relative importin-α3 protein expression in low-dose group and high-dose group were higher than control group (1.44±0.23 and 1.31±0.22 vs. 0.29±0.08, all P<0.01). The relative NF-κB protein expression in low-dose group and high-dose group were higher than control group (0.43±0.05 and 0.37±0.04 vs. 0.16±0.03, all P<0.01). (2)The cell viabilities in miR-181 mimic group was higher than in mimic control group (0.262±0.008 vs. 0.211±0.021, P<0.01). The relative miR-181 mRNA expression level in miR-181 mimic group was higher than in mimic control group (4.23±0.34 vs. 0.88±0.16, P<0.01). The relative importin-α3 mRNA expression level in miR-181 mimic group was lower than in mimic control group (0.24±0.03 vs. 1.08±0.13, P<0.01). The relative NF-κB mRNA expression level was lower in miR-181 mimic group than in mimic control group (0.13±0.03 vs. 0.51±0.06, P<0.01). The relative importin-α3 protein expression level was lower in miR-181 mimic group than in mimic control group (0.34±0.06 vs. 1.67±0.26, P<0.01). The relative NF-κB protein expression level was lower in miR-181 mimic group than in mimic control group (0.43±0.02 vs. 1.53±0.36, P<0.01).@*Conclusions@#Lower miR-181 expression but higher importin-α3 and its downstream NF-κB signaling are associated with ox-LDL induced vascular endothelial cell injury and up-regulation of miR-181 could alleviate ox-LDL induced vascular endothelial cell injury possibly via importin-α3/NF-κB pathway.

OBJECTIVE:To observe the effects of different doses of dexmedetomidine on tracheal intubation stress response in elderly patients with hypertension. METHODS:A total of 105 elderly hypertensive patients who underwent selective general anes-thesia with tracheal intubation were selected from our hospital during Jun. 2014-May 2016,and then divided into group A,B,C according to random number table,with 35 cases in each group.Three groups received general anesthesia,Dexmedetomidine injec-tion 0.3,0.6,1.0 μg/kg by intravenous infusion and Remifentanil injection 0.5 μg/kg 15 min before intubation.After 10 min of ad-ministration,3 groups stopped drug infusion and received tracheal intubation fiberoptic bronchoscope. Mean arterial pressure (MAP),heart rate(HR),blood pulse oxygen saturation(SpO2),respiratory rate(RR)were observed in 3 groups at 5 min after entering surgery room(T1),immediately after infusion(T2),immediately after intubation(T3),1 min after intubation(T4),5 min after intubation(T5). Ramsay sedation scores,cough scores and intubation tolerance scores were also recorded in 3 groups at T3, and the occurrence of ADR was compared among 3 groups. RESULTS:At T1,there was no statistical significance in MAP,HR, SpO2or RR among 3 groups(P>0.05).At T2,above indexes of 3 groups were decreased significantly than at T1,with statistical significance(P<0.05);there was no statistical significance among 3 groups(P>0.05).At T3,T4 and T5,MAP,HR,SpO2and RR of 3 groups were increased significantly than at T2,and MAP and HR of group A were significantly higher than those of group B and C,with statistical significance(P<0.05). There was no statistical significance in SpO2or RR among 3 groups(P>0.05). At T3,Ramsay sedation scores,cough scores,intubation tolerance scores of group B and C were significantly higher than those of group A,with statistical significance(P<0.05). The incidence of hypertension,tachycardia,cough and restlessness in group A were significantly higher than group B and C(P<0.05),the incidence of bradycardia and respiratory depression in group C were significantly higher than group A and B,with statistical significance(P<0.05). CONCLUSIONS:Dexmedetomidine 0.6 μg/kg and 1.0 μg/kg can effectively inhibit stress response and maintain hemodynamics stable in elderly hypertensive patients during tracheal in-tubation,and dexmedetomidine 0.6 μg/kg has relatively lower incidence of ADR.

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the reconstruction of thumb II-III degree defect by big toe wrap-around flap combined with second toe proximal interphalangeal joint.</p><p><b>METHODS</b>Between May 2004 to July 2012, 25 cases with thumbs II-III degree defects were treated. The ipsilateral big toe wrap-around flap combined with second toe proximal interphalangeal joint was designed to reconstruct the thumb defects based on the thumb size on the healthy side. The distal end of hallux was covered by flap at tibial side. The wound at the fibula side was covered by skin graft and flap. The bone defect at the second toe was filled with remnant hallux or iliac bone.</p><p><b>RESULTS</b>All the 25 reconstructed thumbs survived completely. The patients were followed up for 6-26 months without any joint degeneration. According to the finger reconstruction functional criterion publicized by China Medical Association, the finger function was assessed as excellent in 10 case, good in 11 cases, poor in 4 cases. Joint necrosis happened in 2 cases and bone malunion in 2 cases. The 4 cases received dead bone debridement and iliac bone graft. No malfunction occurred in the donor site.</p><p><b>CONCLUSIONS</b>Our surgical method is a currently effective way to reconstruct II-III degree thumb defects. Satisfactory functional and cosmetic results can be achieved both in donor site and recipient site.</p>

ABSTRACT

OBJECTIVE:To establish the Second-order Derivative UV-spectrophotometry for the determination of transdermal osmolality of metoprolol tartrate (MP)in vitro.METHODS:The quantitation of the vibration amplitude (?A)between the characteristic valley and the characteristic peak of MP appeared at 224nm and 234nm in UV-scanning.RESULTS:The linear concentration range of metoprolol tartrate was 5~60?g/ml,the average recovery was 98.89%with average RSD at 2.05%.CONCLUSION:This method can accurately determine the transdermal osmolality of MP in vitro yet without the interference of skin exudates,its operation is simple and fast and the results are accurate and reliable.
