ABSTRACT

Objective:To investigate the application of sentinel lymph node biopsy (SLNB) in early-staged cervical cancer by laparoscopy. Methods:It was a prospective, single-arm, single-center clinical study. Seventy-eight cases of cervical cancer patients were collected from July 2015 to December 2018 at the Fourth Hospital of Hebei Medical University. All the patients were injected with tracer into the disease-free block of cervical tissue after anesthesia by the same surgeon who learned sentinel lymph node (SLN) mapping technique in Memorial Sloan-Kettering Cancer Center, and underwent SLN mapping followed by complete pelvic lymphadenectomy. Moreover, all the dissected lymph nodes were stained with hematoxylin eosin staining (HE) pathological examination. Besides, the negative SLN on hematoxylin-eosin staining were detected by immunohistochemistry cytokeratin staining micro-metastasis. To analyze the distribution, detection rate, false negative rate the sensitivity and negative predictive value of the SLN in early-staged cervical cancer by laparoscopy, and explore the value of SLN mapping in predicting the lymph nodes metastasis in early-staged cervical cancer. Results:The overall detection rate of SLN in cervical cancer was 99% (77/78), bilateral detection rate was 87% (68/78). The average of 12.4 lymph node (LN) and 3.6 SLN were dissected for each patients each side. SLN of cervical cancer were mainly distributed in the obturator space (61.5%, 343/558), followed by external iliac (23.5%, 131/558), common iliac (7.3%, 41/558), para-uterine (3.8%, 21/558), internal iliac (2.2%, 12/558), para abdominal aorta (1.1%, 6/558), and anterior sacral lymphatic drainage area (0.7%, 4/558). Fourteen cases of LN metastasis were found among all 78 cases. There were a total of 38 positive LN, including 26 SLN metastasis and 12 none sentinel LN metastasis. Through immunohistochemical staining and pathological ultra-staging, 1 SLN was found to be isolated tumor cells (ITC), and 5 SLNs were found to be micro-metastases (MIC), accounting for 23% (6/26) of positive SLN. SLN mapping with pathological ultra-staging improved the prediction of LN metastasis in cervical cancer (2/14). Metastatic SLN mainly distributed in the obturator space (65%, 17/26), peri-uterine region (12%, 3/26), common iliac region (15%, 4/26), and external iliac region (8%, 2/26). The consistency of the diagnosis of lymph node metastasis by SLN biopsy and postoperative retroperitoneal lymph node metastasis showed that the Kappa value was 1.000 ( P<0.001), indicated that the metastasis status of SLN and retroperitoneal lymph node were completely consistent. The sensitivity, specificity, accuracy, false-negative rate, and negative predictive value of SLN biopsy in the diagnosis of lymph node metastasis were 100%, 100%, 100%, 0, and 100%, respectively. Conclusions:SLN in early-staged cervical cancer patients were mainly distributed in the obturator and external iliac space, pathalogical ultra-staging of SLN could improve the prediction of LN metastasis. Intraoperative SLN mapping is safe, feasible and could predict the state of retroperitoneal LN metastasis in early-staged cervical cancer. SLNB may replace systemic pelvic lymphadenectomy.

ABSTRACT

Objective To explore the effect of emergency pre-hospital care intervention procedures on patients with acute cerebral hemorrhage treatment. Methods One hundred and eighty patients with acute cerebral hemorrhage rescued in the ambulance of our hospital from June 2011 to June 2014 were selected as the observation group , and another 120 patients of acute cerebral hemorrhage escorted by family members to our hospital during the same period were set as a control group. The patients in the observation group were given guidance nursing, on-site emergency care, emergency care and hospital transit emergency care, and the patients in the control group were given rescue in the hospital. The two groups were compared in terms of effective transit time. Results The effective transit time for the patients in the observation group was (0.87 ± 0.19)h, significantly shorter than the control group (1.26 ± 0.35) h (P<0.05). The complication rate in the observation group was 22.22%, significantly lower than that of the control group (54.17%) (P<0.05). Conclusion Pre-hospital emergency care intervention for patients with acute cerebral hemorrhage can effectively shorten transit time, reduce complications and improve outcome and improve survival, worthy of promotion.

ABSTRACT

BACKGROUND@#To observe the effect of linear alkylbenzenesulfonate (LAS) on oxidative stress and collagen fiber in skin tissue of mice and to explore the correlation between oxidative stress and collagen metabolism.
@*METHODS@#Forty healthy Kunming mice (male) were randomly divided into 4 groups: a control group, a low-, middle- and high-dose group of LAS (LD, MD and HD groups), treated with LAS at 150, 300 and 600 mg/L respectively (n=10 per group). The skin on the back of mice was smeared with distilled water or different dosage of LAS for 60 days. The measured indexes included general condition of mice, HE and Masson staining of skin, the content of hydroxyproline (Hyp) in skin tissue, the activity of super oxidase dismutase (SOD) and the content of malondialdehyde (MDA) in skin tissue and serum, and the activity of lactate dehydrogenase (LDH) in serum.
@*RESULTS@#Compared with the control group, the changes of diet, daily activities and mental state of mice with different dose of LAS were not obvious during the experiment, but the body weight of mice in the experimental groups reduced obviously after 4 weeks of experiment (P<0.01), and their skin tissue was thinner, some of epidermis of skin contained areas with cellular necrosis and abscission. Superficial layer of dermis was infiltrated by inflammatory cells. The collagen fibers were looser and dimmer. At the same time, the content of MDA and the activity of LDH increased remarkably (P<0.01), while the activity of SOD and the content of Hyp decreased obviously (P<0.01).
@*CONCLUSIONS@#LAS can induce oxidative stress in the skin tissue of mice, which can destroy the integrity of skin structure and collagen fiber and reduce the content of collagen fiber. The oxidative damage might be the primary cause for disorders of collagen fiber.

ABSTRACT

OBJECTIVE@#To investigate the effect of linear alkylbenzenesulfonate (LAS) on the reproductive capacity and life-span of Drosophila melanogaster.@*METHODS@#Drosophila melanogaster images within 8 h after eclosion were collected with ether anesthesia. The female and male of similar size and normal shape and behavior were selected. The Drosophila melanogasters were cultured in the culture medium containing LAS of different densities. We divided the Drosophila melanogaster into 4 groups according to LAS concentrations: a low dose group with LAS 150 mg/kg, a middle dose group with LAS 300 mg/kg,a high dose group with LAS 600 mg/kg, and a control group without LAS, respectively. The changes of the reproductive capacity, median lethal time, mean life-span and max mean life-span of drosophila melanogaster with different doses of LAS were measured and compared with those of the control.@*RESULTS@#The pupa numbers of filial generation of Drosophila melanogaster in the low, middle, and high dose groups (85.07%, 84.59% and 71.88%, respectively) were lower than those in the control group (P<0.01). The median lethal time, mean life-span and max mean life-span of Drosophila melanogaster in the low, middle, and high dose groups were shorter than those in the control group (P<0.05). The change of life-span of Drosophila melanogaster in the high dose group was remarkable: the median lethal time of female and male shortened 13 days and 15 days, the mean life-span of female and male shortened 18 days and 14 days, and the max mean life-span of female and male shortened 14 days and 12 days, respectively.@*CONCLUSION@#LAS has definite toxicity to Drosophila melanogaster, which can degrade the reproductive capacity of Drosophila melanogaster and shorten the life-span of Drosophila melanogaster.
