ABSTRACT

Objective:To observe the therapeutic timing and dosage of intravenous immunoglobulin (IVIG) in children with severe adenovirus pneumonia.Methods:Clinical data of children with severe adenovirus pneumonia treated with IVIG at the Department of Respiratory, Guangzhou Women and Children′s Medical Center, Guangzhou Medical University from January 2019 to January 2020 were retrospectively analyzed.Participants were classified as early presenters (5-10 days of illness course) and later presenters (11-15 days of illness course) according to the timing of IVIG treatment.They were further subdivided into plan 1 group[1 g/(kg·d) IVIG for 2 days] and plan 2 group [0.4-0.5 g/(kg·d) IVIG for 3-5 days]. Continuous variables and categorical variables between groups were analyzed by the nonparametric Mann- Whitney U test and the Fisher′ s exact test, respectively. Results:A total of 202 patients with the median age of 12 (12, 36) months were recruited, involving 128 early presenters (63.37%) and 74 later presen-ters (36.63%). Later presenters had a longer duration of fever [18.00(14.00, 23.25) days vs.11.00(9.00, 14.00) days], more demands for mechanical ventilation (33.78% vs.20.31%), and higher incidence of bronchiectasis (9.46% vs.1.56%) than those of early presenters (all P<0.05). For early presenters, no significant differences were detected in the demand for advanced life support, outcomes and sequelae between plan 1 group and plan 2 group (all P>0.05). For later presenters, a shorter duration of fever [18.00(14.00, 21.00) days vs.21.00(15.50, 30.75) days] and lower usage of extracorporeal membrane oxygenation (ECMO, 2.13% vs.18.52%) were observed in the plan 1 group than that of plan 2 group (all P<0.05). The incidence of post-infectious bronchiolitis obliterans and bronchiectasis as pulmonary sequelae was comparable between plan 1 group and plan 2 group ( P>0.05). The incidence of adverse events was 5.77% during IVIG infusion, showing no significant difference between plan 1 group and plan 2 group ( P>0.05). Conclusions:Early treatment of IVIG are very important to improve the prognosis of children with severe adenovirus pneumonia.For later presenters, a high dosage of IVIG is effective in reducing the ECMO use and shortening the duration of fever, thus providing clinical benefits.

ABSTRACT

【Objective】 To study the relationship between quality of life and clinical symptoms of patients with the first-episode depression. 【Methods】 The untreated patients (n=36) with the first-episode depression and the depression patients (n=71) on drug treatment were included according to the strict screening and exclusion criteria. Subjects in control group (n=59), who matched in age, gender and education, were included according to the corresponding enrollment criteria. World Health Organization Quality of Life—Brief (WHOQOL-BREF) and The Social Adaptation Self-evaluation Scale (SASS) were used to evaluate all the subjects. The Hamilton Depression Rating Scale (HAMD), The Hamilton Anxiety Scale (HAMA) and Montgomery-Asberg Depression Rating Scale (MADRS) were used to evaluate the depressive patients. 【Results】 There was no significant difference in age or education level between all the groups. The quality of life was significantly lower in the first group and the treatment group than in the control group; no significant difference was found between patients in the two depression groups. Correlation analysis showed that the total scores of physiology, environment and quality of life in the first group and the treatment group were negatively correlated with HAMD, HAMA and MADRS. SASS was positively correlated with the total scores of physiology, psychology, environment and quality of life. 【Conclusion】 The quality of life and social adaptation of patients with the first-episode depression and those with depression after treatment were significantly reduced. More serious depression symptoms are linked to the worse quality of life, while better social adaptation is linked to the better quality of life.

ABSTRACT

Objective:To investigate the normal range of exhaled nitric oxide (FeNO) in 6-18-year-old children in China, so as to provide a data base for the establishment of FeNO standards for Chinese children.Methods:A multi-center study was conducted on 5 949 children aged 6-18 (3 101 males and 2 848 females) in 16 pro-vinces of 7 administrative districts in China.According to the technical standard recommended by American Thoracic Society/European Respiratory Association, FeNO was measured, and the relationship of FeNO with the sex, age, height, weight, body mass index and region was discussed.Results:The geometric mean FeNO value of Chinese children aged 6-18 was 14.1 ppb, and its 95% confidence interval (skewness distribution) was 1.0-38.2 ppb.The geometric mean FeNO values of children aged 6-11 and 12-18 were 13.1 ppb and 15.7 ppb, respectively, and their 95% confidence intervals (skewness distribution) were 1.0-38.1 ppb and 2.0-38.2 ppb.For children at and under 11 years old, FeNO decreased with age, with a mean decline of 1 ppb per year.The multiple linear regression results suggested that there was a significant correlation between FeNO and age for children aged 6-11, and FeNO of children aged 12-18 was significantly correlated with the gender, height, and region(all P<0.01). Conclusions:FeNO values of Chinese children and adolescents in this study are higher than those obtained by the previous study conducted from 2010 to 2012.For children aged 12-18, 16 ppb is recommended as the clinical cut-off point.For children at or under 11 years old, the influence of age on FeNO should be considered, and the cut-off point of FeNO decreases by 1 ppb as the age is reduced by one year.

ABSTRACT

Apelin is a specific ligand for the G protein-coupled receptor APJ, which is expressed in many tissues and has a variety of biological functions, playing different roles in different tissues. High doses of Apelin in the ventricle of hypothalamus can promote hepatic gluconeogenesis and glycogenolysis, and participate in the development of diabetes. In the peripheral target tissues of insulin, Apelin can improve insulin resistance by regulating tissue metabolism. In gut systems, Apelin can affect the maturation of digestive organs in young rats, and has no significant effect on the feeding and body weight of mice. Apelin can promote glucose into muscle tissues and improve the glucose tolerance. In addition, Apelin also has protective effects on cardiomyocytes and nerve cells. The in-depth study of the Apelin regulating mechanisms on metabolic of type 2 diabetes can help to fully evaluate the value and risks of Apelin in the treatment of diabetes.

ABSTRACT

Objective To investigate the effects of exogenous Apelin-13 on the expression of glucose and fatty acid metabolism related genes in the liver and skeletal muscle of diabetic rats.Methods Forty male Wister rats were randomly divided into a control group (n=8) and an experimental group (n=32).In the experimental group,a rat model of type 2 diabetes mellitus was established by a high glucose and high-fat diet and intraperitoneal injection of streptozotocin (STZ).The diabetic rats were randomized into a diabetic model group and an Apelin-13 treatment group with 14 rats in each group.Rats in the Apelin-13 treatment group were intraperitoneally injected with 0.1 μmol· kg· d-1 Apelin-13 for 10 weeks,while the control group and the diabetic model group were injected with an equal volume of 0.9% NaCl solution for 10 weeks.At the end of the 10 week treatment,fasting blood glucose values in each group were measured.Levels of mRNA expression of glucose-6-phosphate (G-6-P),phosphoenolpyruvate carboxykinase (PEPCK),peroxisome proliferatoractivated receptor alpha (PPAR-α),acy1 CoA synthetase long-chain family member1 (ACSL1),and carnitine palmitoyltransferase1 (CPT1) in the liver and levels of mRNA expression of PPAR-α and glucose transporter type 4 (GLUT4) in skeletal muscle were detected by real-time fluorescence quantitative polymerase chain reaction (PCR).Results Levels of mRNA expression of liver PPAR-α,liver ACSL1,liver CPT1,and GLUT4 in skeletal muscle were lower in the diabetic model group (0.309±0.073,0.508±0.056,0.389±0.118 and 0.289±0.066,respectively) than in the control group (0.971±0.028,0.990±0.015,0.987±0.015 and 0.994±0.009,respectively) (all P<0.05);In the Apelin 13 treatment group,their mRNA expression levels (0.663±0.085,0.802±0.079,0.752 ±0.097 and 0.509±0.119,respectively) were higher than in the diabetic model group,but lower than in the control group (all P<0.05).Liver G-6 P and PEPCK mRNA levels in the diabetic model group (1.727±0.05 and 1.309±0.130) were higher than in the control group (1.002±0.005 and 0.993± 0.010) (both P<0.05),but lower than in the Apelin13 treatment group (2.586±0.208 and 1.842± 0.234) (both P<0.05).Skeletal muscle PPAR-α mRNA levels in the diabetic model group (0.477± 0.118) and the Apelin-13 treatment group (0.566±0.0780) were lower than in the control group (0.993±0.013) (both P<0.05),but showed no significant difference between the two experimental groups (P > 0.05).Conclusions Apelin-13 increases the expression of the PPAR,ACSL1,and CPT1 genes in the liver and,to a certain extent,improves fatty acid oxidation metabolism in the liver in type 2 diabetic rats.It also increases the expression of the G-6-P and PEPCK genes,promotes gluconeogenesis in the liver,and may be related to the development of type 2 diabetes.In skeletal muscle,Apelin 13 increases GLUT4 gene expression,moderately improves skeletal muscle metabolism and may play a role in the regulation of oxidative stress.

ABSTRACT

Objective To investigate the characteristics of self perceived burden (SPB) in patients with chronic obstructive pulmonary disease (COPD) and explore the relationship between the perceived burden and self-efficacy,coping style.Methods A total of 96 cases with COPD in our hospital from January 2015 to June 2017 were enrolled in the study.The clinical data of patients were retrospectively analyzed.The patients were surveyed by self perceived burden scale,self-efficacy scale and coping style scale.The relationship between self perceived burden and self efficacy,coping style were investigated.Results The self perceived burden score of COPD patients was (33.76 ± 7.65) points with moderate level.The symptom management self-efficacy,common disease management self-efficacy score of COPD patients were (8.13 ± 1.09) and (8.22 ± 1.13) respectively with moderate self-efficacy.The total score of coping style in COPD patients was (35.09 ± 10.83) points with positive coping style.The each dimension and the total score of self perceived burden were negatively correlated with positive coping style in COPD patients (P < 0.05),and positively correlated with negative coping styles (P < 0.05).The total score of coping style had no significant correlation with perceived burden scores (P > 0.05).The each dimension and the total score of self perceived burden were negatively correlated with the each dimension scores and total scores of self-efficacy in COPD patients (P < 0.05).Conclusions There are significant correlation between SPB and self-efficacy and coping style in COPD patients.Medical staff should strengthen communication with patients,and guide them with positive coping style in the face of disease,which can effectively improve their self-efficacy and reduce the self perceived burden and promote physical and mental health.

ABSTRACT

Objective To explore the roles and functions of the community nurses in community health service centers and their main work content in Xi′an. Methods A total of 320 community nurses in Xi′an were investigated by General Information Questionnaire and revision of Community Nurses′Roles and Functions Questionnaire. Results The work often undertaken by community nurses were: health education, community prevention, providing direct care and chronic disease management, however, the hospice care, care for people with mental disorders, family planning guidance and rehabilitation nursing were seldom involved. The scores of each role from high to low were as follows:the organizers and managers, coordinators and partners, health educators, caregivers, observer and researcher and spokesman with(2.56±0.99),(2.43±0.97), (2.38±0.84),(2.34±0.87),(2.29±0.10),(2.07±0.99) points. Conclusions The job of community nurses in Xi′an includes the medical service, disease prevention, healthcare, rehabilitation and health education etc. However, the well done jobs only are limited to some of simple functions, such as health education and health guidance are done well. The work of hospice care and mental care which need more professional abilities and interpersonal cooperation need to be promoted.

ABSTRACT

Objective To investigate the relationship between the characteristics of psychological defense style and clinical symptoms of eating attitude in patients with anorexia nervosa patients. Methods Totally 92 ca?ses ( 46 cases of restricting type,46 cases of binge?eating/purging type,48 cases of early onset before 16 years old, 44 cases of late onset after 16 years old) meeting the Diagnostic and Statistical Manual of Mental Disorders,Forth Edition ( DSM?4) for anorexia nervosa (AN) were collected.92 gender?marriage?and education?matched normal cases in a psychiatric hospital were selected as the healthy control group(HC) .All were assessed with defensive style questionnaire(DSQ) and eating attitude test?26(EAT?26).DSQ and EAT?26 were compared between AN and HC,between early onset of AN( before 16 old years) and late onset of AN( after 16 old years) ,and between AN?R and AN?P.Then correlation study was done for the AN. Results Compared with HC,AN showed higher scores in the immature defense,neurotic defense ((4.49±1.13)vs (3.55±0.93),(4.33±0.76)vs (3.88±0.69), P0.05) . Conclusion AN patients tend to use more immature defense and neurotic de?fense compared with HC,and AN?P patients use more immature defense compared with AN?R. Immature defense and neurotic defense are related to the eating attitude,and the immature defense is related the duration.

ABSTRACT

<p><b>OBJECTIVE</b>To examine the expression of nucleotide-binding oligomerization domain 1 (NOD1), nuclear factor-kappa B (NF-κB) and human beta-defensins in candidal albicans leukoplakia and to investigate the effect of candida albicans infection on key proteins in NOD1 signaling pathway and the expression of human beta-defensin.</p><p><b>METHODS</b>Forty cases of oral leukoplakia samples were collected and stained by hematoxylin-eosin staining, periodic acid-Schiff staining, silver staining and immunohistochemical methods. Nineteen samples were positive with these four methods and judged as candidal albicans leukoplakia, and the other twenty- one samples judged as leukoplakia without candidal albicans infection. Western blotting was used to detect the expressions of NOD1 and NF-κB in these forty samples. In addition, the immunohistochemical method was adopted to investigate the relationship between NOD1, NF-κB, human beta-defensin 1, 2, 3 expressions and candida albicans.</p><p><b>RESULTS</b>The positive rate of candida albicans in oral leukoplakia was 48% (19/40). The expressions of NOD1 and NF-κB in the candida albicans leukoplakia were lower than that in leukoplakia without candida albicans infection. The mean optical density value of NOD1, NF-κB, human beta-defensin 1, 2, 3 in candidal albicans leukoplakia were 0.25 ± 0.01, 0.30 ± 0.02, 0.35 ± 0.02, 0.42 ± 0.03, 0.36 ± 0.02 respectively, which were significantly lower than that in leukoplakia without candida albicans infection (0.31 ± 0.02, 0.47 ± 0.03, 0.42 ± 0.02, 0.53 ± 0.04, 0.47 ± 0.03) (P < 0.05).</p><p><b>CONCLUSIONS</b>By inhibiting the NOD1 signaling pathway, candida albicans infection may reduce the expression level of human beta-defensin 1, 2, 3 in oral leukoplakia.</p>

ABSTRACT

Objective To study the relationship between personality and defense mechanism of obsessivecompulsive disorder (OCD) patients.Methods Thirty-four OCD patients were tested with Swedish University Scales of Personality(SSP) and defensive style questionnaire( DSQ), and the result was compared with that of thirty-one normal controls.Results The SSP scores of STA,PsTA,SS,E,TI and M of OCD patients were significantly higher than that of normal controls (t = 2.06 ~ 5.10, P < 0.05 ).Comparing with normal controls, OCD patients tended to use immature and intermediate mechanism such as projection, passive aggression, unconscious emerge, phantasy, withdrawl and reaction formation, but less used mature mechanism such as sublimation, suppression and humor (t = -2.27 ~ 6.73, P< 0.05 ).All subscales of SSP in OCD patients, excluding SD and D, had positive correlation with total score of immature mechanism (r = 0.375 ~ 0.719, P < 0.05 ).The OCD patients' SSP scores of STA had positive correlation with mature mechanism (r = 0.378, P < 0.05 ).The OCD patients' STA, PsTA, I, AS, E, M,VTA and PhTA score of SSP had positive correlation with intermediate mechanism (r= 0.381 ~0.548, P< 0.05 ).All subscales of SSP in OCD patients, excluding SD, had positive correlation with total score of concealment (r =0.342 ~ 0.530, P < 0.05 ).Conclusion OCD patients have the neurotic and aggressive features of personality;and the personalities of OCD patients have significant correlation with defense mechanism.

ABSTRACT

Objective: To identify the different protein expression profiles between human oral squamous cell carcinoma (OSCC) and normal oral mucosa tissues, and provide experimental data for further study of the development mechanism of OSCC. Methods: 10 cases of OSCC and paired normal oral mucosa tissues were collected and analyzed through two-dimensional gel electrophoresis (2-DE) and matrix-assisted laser desorption/ionization time of flight mass spectrometry (MALDI-TOF-MS). Results: (1) The average protein spots of OSCC were 2 325±390, while that of normal oral mucosa tissues were 2 487±281. (2) 29 differential protein spots were found between OSCC and normal oral mucosa. Moreover, these protein spots were all down- regulated in OSCC compared with normal oral mucosa. Among these spots, 3 were identified as fibrin beta, triosephosphate isomerase (TIM) and unknown protein through mass spectrometry and bioinformation. Conclusion: Down-regulation of fibrin beta, Triosephosphate isomerase(TIM) and unknown protein are found in the development of OSCC and the mechanism needs further study.

BACKGROUND: It is conceivable that bone marrow stem cells can differentiate into smooth muscle cells (SMCs) and contribute to neointimal formation in atherogenesis. However, the mechanism remains unknown. The "milieu-induced-differentiation" hypothesis focuses on the key role of cell-to-cell contact and cytokine on the differentiation of stem cells. Bone marrow mesenchymal stem cells (MSCs) have the potential to differentiate into SMCs.OBJECTIVE: To induce MSCs into SMCs in vitro, and investigate the influence of the differentiated SMCs or cell factors on MSCs differentiation.DESIGN: Controlled experiment in vitro with repeated observation and measurement based on cells.SETTING: Department of Cardiology, First Hospital of Xi'an Jiaotong University.MATERIALS: The experiment was accomplished in the Laboratory of Cardiology, First Hospital of Xi'an Jiaotong University between May 2003 and May 2004. SD rats of either gender were provided by the Animal Center of Xi'an Jiao Tong University, 60-80 g, 90-110 g. The following antibodies were used: Mouse anti human SM-α-actin (NeoMarkers),Mouse anti human Calponin (NeoMarkers), TRITC-coupled goat anti mouse IgG antibody (SBA). Mouse anti rat CD34 conjugated FITC (Santa Cruz), Mouse anti rat CD71 conjugated FITC (Oxford Biotechnology), Mouse anti rat anti-CD90 conjugated PE (Oxford Biotechnology). Lipofectamine 2000 (Invitrogen). PEGFP-N3 (the laboratory).METHODS: Bone marrow mesenchymal stem cells were obtained from rat bone marrow by using percoll density gradient centrifugation. SMCs were isolated by using tissue explantation method. Flow cytometer was used to detect the immunofluorescence stain. Then MSCs and SMCs were identified. MSCs were transfected with pEGFP-N3 by Lipofectamine 2000, while untransfected MSCs were taken as controls. Conditioned culture of MSCs and SMCs: ①MSCs at passage 3 were seeded on chamber slides in a 12-well culture plate. The medium was DMEM containing 0%, 5%,7.5% fetal bovine serum (FBS) and SMCs conditioned medium containing 0%, 5%, 7.5% FBS, respectively. The cells were cultured for 10-14 days and immunofluorescence analysis was performed by using monoclonal antibodies against SM-α-actin, calponin.②Indirect co-culture of MSCs with SMCs were established using a semi-permeable membrane cell culture insert. The inserts were plated into culture well. SMCs were cultured on the inside of inserts while MSCs were added to the outside of inserts, respectively. MSCs were culture alone in medium containing 3%, 7.5% FBS and immunofluorescence analysis was performed by using monoclonal antibodies against SM-α-actin, calponin.③MSCs were transfected with pEGFP-N3. After 24 hours, the MSCs were cocultivated with SMCs at an equal density for 7-14 days.As a control, MSCs were cultured alone. MSCs co-cultured were stained with antibodies against calponin, SM-α-actin. MAIN OUTCOME MEASURES: ①Identification of MSCs by floe cytometer.②cytoplasmic antigen expression of SMCs. RESULTS: ①Immunofluorescence analysis showed that MSCs expressed SM-α-actin, but did not express calponin. As a control, SMCs expressed both SM-α-actin and calponin.②Flow cytometry showed that MSCs expressed CD71 of low level, CD90 of high level and no expression of CD34. ③The MSCs transfected with green fluorescence protein continued to express for 2-3 weeks. ④MSCs grew well in SMCs conditioned medium or different concentrations of FBS. Cell growth was FBS concentration dependent in indirect co-culture system of MSCs and SMCs. Several double-positive cells in direct co-culture system were detected enhanced green fluorescence protein and antibodies against calponin, SM-α-actin. CONCLUSION: ①SMCs conditioned medium and cell factor only promote MSCs growth and cytoplasmic granules increase. But these do not induce MSCs differentiate into SMCs. ②The cell-to-cell contact is essential for MSCs differentiation to SMCs.

ABSTRACT

Objective:To find biomarkers for oral lichen planus by comparing differential expressing proteins. Methods:10 cases of oral lichen planus and normal oral mucosa tissues were collected.Total protein was extracted; differential proteome profiles were established and analyzed by two-dimensional polyacrylamide gel electrophoresis(2D-PAGE) and matrix-assisted laser desorption/ionization time of flight mass spectrometry (MALDI-TOF-MS). Results:(1)The well-resolved,reproducible 2-DE patterns of oral lichen planus and normal oral mucosa were obtained. The results showed that average protein spots were 1 576?67 and 1 608?73 in oral lichen planus and normal oral mucosa respectively, (2) The 13 differential protein spots were identified by Imaging Master 2D image analysis software between oral lichen planus and normal oral mucosa. There were 7 protein spots in oral lichen planus were higher than those in normal oral mucosa, 6 protein spots in oral lichen planus were lower than those in normal oral mucosa. 10 differential expressing proteins were analyzed by mass spectrometry and bioinformation. 4 of them were well characterized including manganese superoxide dismutase (Mn-SOD), Annexin I, vimentin and unknown proteins. Conclusion:Differential expression proteins might be candidate biomarkers for diagnosis of oral lichen planus;and proteomic technique is valuable for screening the diagnostic biomarkers.

Object To select the cell line of Ginkgo biloba L that may produce high yield of ginkgolide B (GB) in suspension culture and to study the stability of GB in subculture Methods Calculus was induced with stem, root and leaf of selected elite species and high yield suspension cell lines chosen by hypoxia stress Results Seven suspension cell lines with improved yield of GB were obtained Among which cell line MH 3 gave a 3 71 fold increase of cellular bioproduct with an increased content up to 302 ?g/g DW after culturing for 18 d, a leading record country wide In shaking bottle culture, it showed a consistent yield of GB with a content of 291 ?g/g DW in 6 successive subcultures, coefficient of variation=0 131 Conclusion The suspension cell line selected by hypoxia stress gave a higher yield and stability in successive transfer culture

ABSTRACT

Objective To understand morphological characteristics of sinoatrial node cells(SNCs) by taking purified isolation and culture of neonatal rat's SNCs in vitro and to understand the expression characteristics of surface antigens hyperpolarization activated cyclic nucleotide gated cation channel 4(HCN4) and connexin 45(Cx45) on SNCs.Methods The SNCs were isolated from sinus node tissues removed from neonatal rats by trypsin and purified cultured with differential attachment and 5'-bromodeoxyuridine(BrdU) treatment.Results Three different types of cells were observed among the purified cultured SNCs: spindle,triangular and irregular cells.The spindle cells took up the greatest proportion,had the fastest beats,and had less developed organelle and scarce muscular fibrils.Immunofluorescence staining was positive against HCN4 and Cx45 on the spindle cells.Small amounts of HCN4 was observed on triangular cells.Conclusion ① Isolation by trypsin combined with differential attachment and BrdU treatment is a reliable approach to culturing sinoatrial node cells.② The spindle cells with the fastest beats and positive expressions of HCN4 and Cx45 may be the sinus node pacemaker cells.
