ABSTRACT

Objective:To construct a scientific and complete competency evaluation index system for postgraduate tutors of clinical medicine.Methods:Based on literature research, group discussion and expert interviews, the research used the competency model theory, and conducted twice expert consultation by Delphi method, then the index weight was determined by mathematical analysis method and precedence diagram method. SPSS 22.0 was used for data statistical analysis.Results:According to the characteristics of the competency model and expert opinions, 4 first-level indicators, 12 second-level indicators, and 40 third-level indicators were finally determined, and the tutor evaluation index system was constructed.Conclusion:The system is highly scientific and reliable, which will provide a reference for the comprehensive evaluation of postgraduate tutors at medical colleges and will be of great significance for strengthening the construction of tutors and ensuring the cultivation quality of postgraduate.

ABSTRACT

Objective@#To describe the MICM (morphology, immunology, cytogenetics and molecular biology) characteristics of a case of acute myelomonocytic leukemia M 4C . @*Methods@#The medical history data of the case of M 4C admitted to our hospital was reviewed. The results of bone marrow cell morphology, cytochemical stains, bone marrow biopsy, immunophenotype, cytogenetics, molecular test and NGS (next-generation sequencing) of the case were analyzed. @*Results@#The bone marrow smear showed markedly active proliferation of bone marrow cells in which the myelomonocytic cells accounted for 85.6%. Cytochemical stains showed peroxidase (POX) stain partially and weakly positive; specific esterase AS-DCE partially positive; non-specific esterase α-NBE partially positive and smothered by sodium fluoride; non-specific esterase AS-DAE partially positive and smothered by sodium fluoride. Bone marrow biopsy showed hyperproliferative cells and diffused hyperplasia of blasts. Immunophenotype analysis showed that the abnormal cell population was positive for CD11B, CD64, CD56, cMPO, CD33, CD41, CD61, CD38 and CD58, but negative for CD13, CD34, CD117, CD7, CD123, HLA-DR, CD10, CD19, CD20, CD2, CD14, CD235, CD15, CD303, CD304, CD25, cCD79a, cCD3, cCD22, CD1a and TDT. Cytogenetic analysis showed 47, XY, t(9;11) (p22;q23),+mar. The molecular test for leukemia showed MLLT3/KMT2A gene rearrangement. NGS showed NRAS and TET2 mutation. The case was finally diagnosed as AML (acute myelomonocytic leukemia) M 4C with t(9;11)(p22;q23), MLLT3-KMT2A. @*Conclusion@#Leukemia M 4C may show the characteristics of both granulocytes and monocytes with complex morphological features. The combined examination of MICM should be necessary for the diagnosis of M 4C with great significance.

ABSTRACT

Bacillus subtilis can be widely used as an important microorganism for metabolic engineering and recombinant proteins expression in industrial biotechnology and synthetic biology. However, it is difficult to make accurate regulation of exogenous gene by biological tools in B. subtilis, which limits the application of B. subtilis in synthetic biology. The purpose of this study is to develop regulatory tools for precise control of gene expression by using non-coding RNAs, by which the activation of heterologous gene could be achieved without the auxiliary protein factors. We constructed the synthetic riboswitch E and aptazyme AZ using the theophylline aptamer. Six different native promoters from B. subtilis were functionally adapted with the E and AZ to fabricate an array of novel regulatory elements activated by theophylline. Then, we determined the performance of these elements using green fluorescence protein as reporter, and then further verified using red fluorescence protein and pullulanase as cargo proteins. Results showed that the same kind of RNA elements with different promoters showed different levels of efficiency. Promoter PsigW and E combination (sigWE) had the highest induction rate in B. subtilis. Compared with the control group, it can produce the induction rate of 16.8. Promoter PrpoB and AZ combination (rpoBAZ) showed the highest induction rate of 6.2. SigWE mediated mCherry induction rate was 9.2, and P43E mediated pullulanase induction rate was 32.8, in which enzyme activity reached 81 U/mL. This study confirmed that GFP, mCherry and pullulan can all be regulated by riboswitch and aptazyme, but there were differences between different combinations of promoters with RNA regulators.

ABSTRACT

Objective To investigate the diagnostic value of contrast-enhanced CT and MR diffusion-weighted imaging (DWI) in differentiating autoimmune pancreatitis (AIP) from pancreatic carcinoma (PC).Methods Twelve patients with AIP (AIP groups) and 15 patients with PC (PC groups) were included in this study.Contrast-enhanced CT and DWI were performed in all patients.The manifestations of the pancreatic lesions,pancreatic duct and bile duct were observed.The CT attenuation values and the apparent diffusion coefficient (ADC) values of the lesions were calculated.Frequencies of the CT manifestations,and the mean CT attenuation values,and the ADC values were compared between the two groups.Results Most of the AIP showed diffuse enlargement,sausage shape,peripancreatic capsule,penetrating sign of pancreatic duct and thickness of the bile duct.The frequencies of the findings mentioned above were significantly different between AIP and PC (P<0.05).The CT attenuation values of the lesions,and the rate of lesions' CT attenuation values to livers' in 3 phases of contrast enhanced CT were significantly different between AIP and PC (P<0.05).The ADC values of AIP were significantly lower than that of PC (P<0.05).Conclusion The changes of morphology of pancreas,pancreatic and bile duct may facilitate in differentiating AIP from PC.Quantitative analysis of CT attenuation values and ADC values measured on MR images may be helpful in differentiating AIP from PC.

ABSTRACT

Enzymatic synthesis is an important way to produce β-alanine, but the biological method is expensive generally because of the high price of substrate. In this paper, a two-step enzymatic cascade system was developed, combining L-aspartase from Escherichia coli DH5α and L-aspartate α-decarboxylase from Corynebacterium glutamicum. This system catalyzes Fumarate and ammonia to β-alanine. The optimal ratio of AspA and PanD was 1:80 (W/W), and the concentration of AspA was 10 μg/mL, at 37 ℃ and pH 7.0. When the concentration of Fumarate was 100 mmol/L, the reaction reached its equilibrium after 8 h, and the yield of β-alanine was 90 mmol/L (7 g/L). The yield of β-alanine can reach 126 mmol/L (9.8 g/L) when the concentration of Fumarate increased to 200 mmol/L. Extending reaction time, the conversion rate did not change obviously. Using this two-step enzymatic cascade system, β-alanine from cheaper substrate Fumarate can be obtained.

ABSTRACT

Objective To evaluate the value of the magnetic resonance imaging (MRI)in the diagnosis and classification of plas-ma-cell mastitis (PCM).Methods The breast MRIs of eighteen pathologically confirmed PCM patients were retrospectively ob-served and analyzed.The manifestations of lesions,including shape,size,margin,and characteristic enhancement on T1 WI,T2 WI with fat suppression and contrast enhanced T1 WI were observed and documented respectively.Results PCM were divided into three types according to MRIs:the inflammation type 4 cases,the abscess type 9 cases,and the mixed type 5 cases.In the inflammation type,3 cases of lesions were wide extending while the other 1 limited in nipple and areola.All 4 cases of lesions were hypointensity on T1 WI and hyperintensity on T2 WI with fat suppression.And in the contrast enhanced scanning,2 cases showed patchy hyperin-tensity and 2 cases showed branch-like enhancement.In abscess type,6 cases of lesions were wide and 3 cases limited in one quad-rant.Of all 9 cases,there were 3 cases with single abscess and 6 with multiple abscess;and all cases showed hypointensity on T1 WI and hyperintensity on T2 WI with fat suppression.In the enhanced scanning,there were 4 cases with honeycomb-like hyperintensity and 5 cases with annular and patchy enhancement.In mixed type,inflammation was accompanied with abscess and fistula.All cases were hypointensity on T1 WI and hyperintensity on T2 WI with fat suppression.In the enhanced scanning,3 cases showed honeycomb-like enhancement,1 case showed multiple annular enhancement and 1 case showed annular and patchy enhancement.Regions of in-terest were selected and time-signal intensity curves were obtianed in every enhancement cases,all of which were influent.Of all ca-ses,5 showed high-protein deposition in the expansion ducts,exhibiting hyperintensity on T1 WI and hypointensity on T2 WI with fat suppression.Conclusion The abscess type is the most common in PCM and the second is the mixed type.Annular and honeycomb-like enhancement are characteristic appearance of these two types,which are helpful in the diagnosis and classification of PCM.
