ABSTRACT

【Objective】 To investigate the variation of hematological parameters in male plateletpheresis donors. 【Methods】 A total of 194 male plateletpheresis donors from Fujian Blood Center were divided into two groups according to the frequency of blood donation: Group 1 (n=107), with the number of plateletpheresis donation less than or equal to 12 per year; Group 2 (n=87), with the number of plateletpheresis donation more than 12 per year. Serum ferritin (SF) and related iron metabolism indexes, red blood cell count (RBC), hemoglobin (Hb), hematocrit(Hct), platelet count (Plt) and other blood routine indexes, as well as percentage of reticulocyte counts (RET%), immature reticulocyte fraction(IRF) and other reticulocyte indexes were measured before blood donation and analyzed by statistical methods. 【Results】 Compared with Group 2, the RBC, Hb, Hct, SF in Group 1 were significantly higher, while Plt, RET%and IRF were significantly lower(P<0.05), and the probability of ferritin decrease in Group 1 was lower, with significant difference(P<0.05). 【Conclusion】 As the number of donation increased, male plateletpheresis donors were prone to iron deficiency, and the bone marrow hematopoiesis were obviously enhanced. We should be more concerned about male plateletpheresis donors who donated more than 12 times per year, further more, SF monitoring should be conducted and the blood donation interval should be appropriately extended.

ABSTRACT

Objective:The study was planned to evaluate the clinical utility of microcytic anemia factor (MAF) and low hemoglobin density (LHD%) in the screening of iron deficiency in blood donors. Methods:A total of 400 blood donors, 200 male and 200 female, were randomly admitted to Fujian Blood Center from January1, 2022 to February 28, 2022 by the way of stratified sampling. According to the fourth edition of Diagnostic and therapeutic criteria for hematological diseases, the patients were divided into three groups: normal group (N=299), iron depletion group (ID, n=54) and iron deficient erythropoiesis group (IDE, n=47), Blood routine indexes including hemoglobin (HGB), mean corpuscular volume hemoglobin (MCV), mean corpuscular hemoglobin content (MCH), mean corpuscular hemoglobin concentration (MCHC) and iron metabolism indexes including serum ferritin (SF), serum iron (SI), total iron binding capacity (TIBC), transferrin saturation (TS) and unsaturated iron binding capacity (UIBC) were measured, MAF and LHD% were calculated by formula.One-way ANOVA or Kruskal-Wallis H tests were used to analyze the differences among three groups. Spearman correlation analysis was used to analyze the correlation between MAF and LHD% and iron metabolism indexes. The Receiver Operating Characteristic (ROC) curve was used to evaluated the diagnostic value of MCH, MCHC, MAF and LHD% for iron deficiency in blood donors. Results:MAF in ID group which was 11.81±0.81 were higher than the IDE group which was 10.69±0.95 and lower than the healthy group which was 13.17±1.24, the total difference among the three groups was statistically significant ( F=110.784, P<0.001), the difference between two groups was statistically significant ( P<0.01); LHD% in ID group which was 2.61 (1.87, 3.91)% were lower than the IDE group which was5.60(2.99, 8.02)% and higher than the healthy group which was1.74 (1.22, 2.73)%, the total difference among the three groups was statistically significant ( H=62.166, P<0.001), the difference between two groups was statistically significant ( P<0.01).In 101 iron deficiency blood donors, Spearman correlation analysis showed that MAF was positively correlated with SF, SI and TS ( r=0.426, P<0.01; r=0.547, P<0.01; r=0.566, P<0.01);contrarily, LHD% was negatively correlated with SF, SI and TS ( r=-0.397, P<0.01; r=-0.400, P<0.01; r=-0.479, P<0.01).The areas under the ROC curve of MCH, MCHC, MAF and LHD% diagnostic ID were 0.745, 0.646, 0.819 and 0.646, respectively;the cut-off value of MAF was 12.56, with a sensitivity of 67.90% and a specificity of 83.30%.While the areas under the ROC curve of MCH, MCHC, MAF and LHD% diagnostic IDE were 0.901, 0.834, 0.941 and 0.834, respectively; the cut-off value of MAF was 11.73, with a sensitivity of 87.60% and a specificity of 87.20%. Conclusions:MAF performed a high diagnostic value of iron deficiency, especially IDE, and can be used as a marker in the diagnosis of iron deficiency in blood donors.

ABSTRACT

Objective:To evaluate the application effect of virtual reality in labor pain management by Meta analysis. Methods:The randomized controlled trials on the impact of virtual reality on labor pain was searched by PubMed (Medline), Web of Science, The Cochrane Library, Embase, CINAHL, China National Knowledge Infrastructure (CNKI), Wanfang database, Chinese biomedical database, and VIP database from their inception until July 2021. Two reviewers independently screened literature, extracted data and assessed the risk of bais of included studies. Meta-analysis was performed using the Comprehensive Meta-Analysis (CMA) software Version 3.Results:Totally, 658 pregnant women were identified in the search, which were narrowed down to 8 studies. The results of Meta-analysis showed that the virtual reality group decreased the labor pain compared with the normal nursing group ( MD=-1.13, 95% CI-1.59 to -0.67, P<0.01), especially during the active phase of labor ( MD=-1.20, 95% CI-1.96 to -0.45, P<0.01). Besides, compared with the normal nursing group, the virtual reality group had a shorter first stage duration of labor ( MD=-0.93, 95% CI-1.51 to -0.34, P<0.01) and a lower level of anxiety ( SMD=-1.54, 95% CI-2.22 to -0.85, P<0.01). However, there was no significant difference between two groups in the pain scores during the latent phase of labor ( MD=-0.41, 95% CI-1.30 to 0.47, P>0.05) and the second stage duration of labor ( MD=-0.37, 95% CI-3.27 to 2.53, P>0.05). Conclusions:The findings of this Meta-analysis showed that the virtual reality significantly reduced the labor pain, shorted the first stage labor duration and lowered anxiety level. However, due to the limited quantity and quality of the included studies, its conclusions need to be verified by more large sample multicenter randomized controlled trials with rigorous design in the future.

ABSTRACT

Objective:We aimed to explore the clinical features, computed tomography (CT) findings, treatment, and prognosis of bronchopulmonary carcinoid. Methods:Clinical data of 31 patients with primary carcinoid tumor of the lung were retrospectively re-viewed. The prognostic factors were analyzed via Cox univariate and multivariate analyses. Results: Clinical symptoms included coughing or expectoration in 17 of the 31 cases, hemoptysis or blood-stained sputum in 7 cases, and chest pains or shortness of breath in 8 cases. Six cases were asymptomatic. The CT scans showed round or oval nodules with clear boundaries, and enhancement CT scans indicated mild, homogeneous enhancement. Immunohistochemistry results revealed the positive expression rates of synaptophy-sin (Syn), chromogranin A (CgA), and neuron-specific enolase (NSE) were 90.3%(28/31), 87.1%(27/31), and 90.3%(28/31), respec-tively. Therapy and prognosis results were as follows:28 of the total number of patients underwent surgery, among which 3 underwent postoperative adjuvant therapy, 2 received chemotherapy; and only 1 refused treatment. The 1-year overall survival rates were 100%(18/18) and 92.3%(12/13), whereas the 3-year survival rates were 94.4%(17/18) and 69.2%(9/13) in the typical and atypical carcinoid cases, respectively. Cox univariate analysis results revealed that lymphatic metastasis (P=0.02), tissue types (P=0.017), TNM stage (P=0.005), and therapies (P=0.01) were the prognostic factors. Cox multivariate analysis results showed that lymphatic metastasis (P=0.032) and tissue types (P=0.002) were the independent prognostic factors. Conclusion:Compared with other lung cancers, the bron-chopulmonary lung carcinoid has no special clinical manifestation in clinical and radiographic images. The diagnosis was mainly based on histopathology results. Surgery was the main and effective treatment, whereas chemotherapy and radiotherapy showed unsatisfactory results. The overall prognosis was satisfactory. However, the atypical carcinoid was inferior to the typical carcinoid in terms of progno-sis. Pathological typing and lymph node metastasis were significant prognostic factors.

ABSTRACT

BACKGROUND: With the rapid development of modern society, psychological problems in college students have been paid high attention by educatienal organizations of psychological health in China. OBJECTIVE: To probe into the influencing factors of psychological problems in college students. DESIGH: Cluster sampling 1∶2 paired observation, case-control study. SETTING: Medical College of Wuhan Polytechnic University. PARTICTPANTS: 1 200 sophomores with the age ranged 19-20 years were selected from Wuhan Polytecnic University by cluster sampling at June 2001, and 145 students with the scores of more than 3 factors greater than 3 points were screened by Symptom Checklist 90 (SCL-90) (including 9 factors of somatization, compulsion, interpersonal sensitivity, depression, anxiety, hostility, phobia, paranoid and schizophrenia. The higher the grade was, the severer the symptom was) to be the psychological problem group. Subjects were selected to be the normal controls at the ratio of 1∶2 with those in the psychological problem group, whose classes, specialties and ages (the difference was less than 2 years) were similar to those of the enrolled patients. 290 students normal in the indexes of SCL-90 were taken as the matched group. Of all subjects in both experiment groups, there were 173 males and 262 females with an average age of 19.2 years. METHODS: ① The subjects in the two experiment groups were investigated of the psychological health by SCL-90 as well as the personality characteristics and Eysenck Personality Questionnaire (EPQ) (For Adult). The EPQ statistic software designed by the Institute for Juvenile and Children, Tongji Medical College of Huazhong University of Science and Technology was adopted to acquire the E score (introversion, extroversion), N score (nervousness), P score (psychoticism), L score (conceal) and quantity form quadrant of E-N coordinate picture, so as to define the tendencies of characters. ② The psychological problem was considered to be the dependent variable, while all relevant factors (The influencing factors of psychological problem included the understanding from parents, relationship with family members, desire for learning, personality characteristics, over-high P score, over-high/low E score, over-high/low N score) were taken as the independent variable. Single-factor Logistic regression analysis was conducted at 1:2. ③ Based on 1:2 single-factor Logistic regression analysis, the variances were selected from factors of P < 0.01 together with the Logistic regression models of multifactor at 1:2 by stepwise regression method.MAIN OUTCOME MEASURES: ① Comparison in the SCL-90 score between the psychological problem group and the normai control group. ② Single-factor and multivariate analysis of the relevant factors of psychological problems. RESULTS: A total of 435 enrolled students were involved in the analysis of results. ① The average score of somatization, compulsion, interpersonal sensitivity, depression, anxiety, hostility, phobia, paranoid and schizophrenia in students of the psychological problem group was higher than that in the normal control group (t=-13.39, 29.64, 27.09, 26.04, 19.52, 18.13, 14.15, 18.61, 22.41, P< 0.01). ② Nine factors were selected in the single factor analysis after the Logistic regression analysis, including 6 protective factors of understanding from parents, relationship with family members, desire for learning, personality characteristics, E > 61.5, N < 38.5, while he other 3 factors (P > 61.5, E < 38.5, N > 61.5) were risk factors. The multivariate analysis showed that factors of relationship with family members, desire for learning as well as personality characteristics were protective factors, while it was a risk factor when N > 61.5.CONCLUSION: The psychological problems in college students relate to the relationship with family members, understanding from the parents, desire for learning as well as the character trait and personality characteristics.
