ABSTRACT

Objective:To observe the clinical features of cytomegalovirus (CMV) retinitis (CMVR)- related uveitis after hematopoietic stem cell transplantation (HSCT).Methods:A retrospective clinical study. From October 2015 to May 2020, 14 cases of 21 eyes of CMVR patients with CMVR after HSCT confirmed by the ophthalmological examination of The First Affiliated Hospital of Soochow University were included in the study. Among them, there were 5 males with 8 eyes and 9 females with 13 eyes. The average age was 35.12± 12.24 years old. All the affected eyes were examined by slit lamp microscope combined with front lens and fundus color photography. At the same time, fluorescein fundus angiography (FFA) was performed to examine 10 eyes of 5 cases; 3 cases of 3 eyes were examined for inflammatory cytokines in aqueous humor. All eyes received intravitreal injection of ganciclovir; patients with a history of systemic CMV infection received intravenous infusion of ganciclovir/foscarnet. The retinal lesions in the eye were completely resolved or the aqueous CMV-DNA was negative as a cure for CMVR. The uveitis symptoms, signs, FFA manifestations and the test results of inflammatory factors in aqueous humor before and after the CMVR cure was observed. The follow-up time after CMVR was cured was 3-42 months, and the average follow-up time was 14.28±13.12 months.Results:All eyes with CMVR were diagnosed with retrocorneal dust and/or stellate keratic precipitates (KP), anterior chamber flare and cells, and varying degrees of vitreous flocculent opacity; the retina was typical of a mixture of hemorrhage and yellow-white necrosis like "scrambled eggs with tomatoes". After CMVR was cured, there were 16 eyes (71.4%, 10/14) in 10 cases with KP, anterior chamber flare, cell and vitreous opacity. FFA examination revealed that the majority of retinal leakage during the active period of CMVR was necrotic foci and surrounding tissues; after CMVR was cured, the majority of retinal leakage was the retina and blood vessels in the non-necrotic area. The test results of inflammatory factors in aqueous humor showed that interleukin (IL)-6, IL-8, and vascular endothelial cell adhesion molecules were significantly increased in the active phase of CMVR; after 3 months of CMVR cured, inflammatory factors did not increase significantly.Conclusion:CMVR-associated uveitis after HSCT show as chronic panuveitis, with no obvious eye congestion, KP, anterior chamber flare, cell and vitreous opacity, and retinal vessel leakage which could exist for a long time (>3 months).

Pal.Beauv, Rosa canina L., Carumcarvi L. and Majorana hortensis Moench. and other compatible applications. They have certain effect on jaundice, hepatitis, chronic cholecystitis, and gallbladder pain.

This article introduces the research and application methods of the Russian medicinal and edible fungus inonotus obliquus in the treatment of influenza, which has shown that Inonotus obliquushas certain inhibiting effect on respiratory viruses and can be used to treat influenza .Besides, it could be combined with herbal plants like Echinacea, Acorus calamus L. root, Anisum, Cerasus vulgaris Mill. gum, Syzygium aromaticum, Geranium flower, Chamomilla recutita, Myristica, Origanum vulgare L., which have effect on treatinginfluenza andrehabilitation.

The prescription is simple, practical, safe and easy for patients, but it should be tested in our country. Considering the different constitution of patients and the source of medicinal materials, the attention should be paid to adjust the dosage.

ABSTRACT

In this paper, we reviewed the findings from previous studies on the anti-tumor efficacy of inonotus obliquus, one of the most famous edible and medical fungi, in Russia and the former Soviet Union since the 1950s. To be specific, the findings from clinical researches regarding the medicinal benefits of inonotus obliquus for treating gastric cancer, esophageal cancer, lung cancer, laryngeal cancer, breast cancer, and leukemia have been summarized, indicating inonotus obliquus may relieve symptoms, improve immune and neural function, improve life quality, and may prolong survival in the cancer patients.

To be specific, we summarized morphological characteristics, distribution, growing conditions, history of application, active ingredients, and mechanism of actions of inonotus obliquus in this review, and illustrated specific notes for its potential medicinal value and health benefits. Our manuscript has provided new insight for further research and application of inonotus obliquus in China.

ABSTRACT

According to the literatures pertaining inonotus obliquus (commonly known as chaga) documented in Russia and the former Soviet Union, the research progress of inonotus obliquus since the 1950s in Russia and the former Soviet Union and latest research progress made over the last two decades in China, , the authors further elaborated potential effects and benefits of inonotus obliquus from the perspective of traditional Chinese medicine (TCM) based on prior understanding of Flavor Effect of Inonotus obliquus. It is believed that Inonotus obliquus can benefit the five internal organs, invigorating the circulation of qi, blood, and nutriment, and thus may improve the body's adaptability and self-protection when exposing to pathogenic factors. The mechanisms of the multiple effects of Inonotus obliquus have the characteristics of multi-component, multi-target, multi-system, and multi-pathway. Inonotus obliquus can be considered to be used as both TCM medication and TCM dietary supplement.

According to the literatures of the former Soviet Union and Russia about inonotus obliquus (commonly known as chaga),and the research progress in the past 20 years in China,and the author's 30-year experience on inonotus obliquus' clinical application,and the records of medicinal properties of birch in ancient China literatures,the potential effect of inonotus obliquus is analyzed according to traditional Chinese medicine theory.It is believed that the taste of inonotus obliquus is slightly bitter and cold.The main effect of inonotus obliquus is strengthening the body resistance,the specific benefits of which include nourishing qi,nourishing blood,nourishingyin,tonifying spleen,invigorating the spleen and stomach,nourishing liver,tonifying kidney and nourish the heart and calm the nerves.The effect of inonotus obliquus also includes eliminate evil (pathogenic factors),with clearing heat and detoxicating,soothing liver and relieving stagnation,and promoting blood circulation and resolving hard lump.

ABSTRACT

Objective To establish a sensitive,simple and accurate HPLC-MS/MS method to quantify glycyrrhetic acid(glyc?yrrhetinic acid)in mice blood,and to further study pharmacokinetic profiles of glycyrrhetic acid after oral administration of glycyrrhi?zin and Bu-Zhong-Yi-Qi-Wan(BY). Methods Rats were intragastric administered of glycyrrhizin(glycyrrhizic acid,61.5 mg/kg) and BY extract(3 g/kg,with the same mole of glycyrrhizin moiety),respectively. Plasma samples were collected after administration and extracted with liquid-liquid extraction,then by separated by liquid chromatography on a C8 reversion phase chromatographic col?umn with gradient elution. Concentration of glycyrrhetic acid was detected by the validated HPLC-MS/MS. Non-compartmental pharma?cokinetic profiles were constructed using the software of Das 2.0 software(Shanghai,China),and the pharmacokinetic parameters were compared using unpaired Student′s t-test. Results This bioanalytical method was fully validated and showed good linearity(r>0.99),wide dynamic range(5-1000 ng/ml),and favorable accuracy and precision. Compared with the glycyrrhizin pure form group, BY significantly reduced the Cmax and AUC0-t of glycyrrhetic acid by 56%and 76%,respectively. Whereas no significant differences in Tmax,T1/2 and MRT were observed between the two groups. Conclusion The constituents in the BY prescription have significantly reduced the oral bioavailability of glycyrrhetic acid in rats than those in the glycyrrhizin pure form and the results indicate that some components in the BY have an inhibition effect on the absorption process of glycyrrhizin in the gut.

ABSTRACT

Russia's post-degree medical education first appeared in the end of the 19th century and had improved along with the progress in science, technology and education. From 1990s, new waves of technology revolution and the rise of post-occupation training further pushed the post-degree medical education to move forward. Legally established post-degree education as a form of occupational education enables Russian medical professionals to constantly update knowledge and absorb new information to keep up with the new developments in medical science and technology. In this sense, post-degree medical education has become a major way to train not only medical professionals but professionals of acupuncture and moxibustion, and thus has been of great significance for TCM and acupuncture to transmit and develop in Soviet Union and Russia. This paper introduced the general situation of post-degree medical education in Russia, its influence to the transmission and development of TCM and acupuncture in Soviet Union and Russia, as well as the enlightenment to medical education.

ABSTRACT

Background Intracorneal macrophages play a critical role in corneal neovascularization (CNV)by secreting relative chemokines.But macrophages are characteristic by heterogeneity which has different biologic functions under different induction or stimulation from microenvironment.Objective This study was to detect the effects of chemokine (C-X3-C motif) ligand 1 (CX3CL1) and chemokine (C-C motif) ligand 2 (CCL2) on macrophages in vitro.Methods CNV was induced by corneal alkali burn in the left eyes of 20 male BALB/c mice aged 7-8 weeks.The CNV was evaluated under the slit lamp microscope 4 days after alkali burn,and then the corneal sections were prepared after mice were sacrificed.The expressions of CCR2 and CX3CR1 in the corneal specimens were detected by histo-fluorescence staining.Human peripheral blood mononuclear cells were separated using density gradient centrifugation and incubated in RPMI-1640 medium containing 10% fetal bovine seruml(FBS) with 30 μg/L granulocyte-macrophage colony-stimulating factor (GM-CSF).The cells were divided into CD68 +CCR2 group and CD68+CX3CR1 group,and the percentage of the CX3CR1 and CCR2 expressions in the infiltrated macrophages of corneal specimens and human monocyte-derived macrophages was assayed by flow cytometry.The cultured cells were stimulated using human recombinant CX3CL1 and CCL2 proteins,and real-time PCR was used to detect the relative expressions of angiogenesis-related factors in macrophages.Results CNV was found in corneas 4 days after alkali burn and the CNV onsets from corneal limbus to central zone observed by a slit lamp.CCR2 and CX3CR1 were expressed in the F4/80-positive macrophages in alikali burned corneas.The macrophages grew for two weeks and appeared more dead cells in without GM-CSF group,but in GM-CSF induced group,the number of macrophages was increased.The percentage of CX3CR1-positive cells was 75% and that of CCR2-positive cells was 45%.Real-time PCR showed that expression level of vascular endothelial growth factor (VEGF) mRNA increased and that ADAMTS-1 mRNA or TSP-1 mRNA decreased on macrophages after CCL2 stimulation,with significant differences in the 150 mg/L CCL2 group compared with the control group (t =-5.09,P =0.03 ; t =3.01,P =0.04 ; t =4.27,P =0.02).However,the VEGF mRNA expression decreased and ADAMTS-1 mRNA and TSP-1 mRNA increased after CX3CL1 stimulation,showing significant differences between the 150 mg/L CX3CL1 group and the control group (t=6.35,P=0.O2;t=-2.92,P=0.04; t=-3.81,P=0.03).Conclusions These results suggest that the macrophages have high heterogeneity.CCL2-and CX3CL1-expressing macrophages can regulate the expressions of angiogenesis-related factors.Macrophage chemokine signal may be a good target for treatment of neovascular ocular disease.

ABSTRACT

A novel method for analysis of three active components curcumin, demethoxycurcumin and bisdemethoxycurcumin in Curcuma longa L. was developed by HPLC coupled with electrochemical detection. Three curcuminoids were well separated on a C18 column and detected with high sensitivity. A mobile phase containing acetonitrile and 10 mM Na2HPO4-H3PO4 (pH 5.0) (50:50, v/v) was used. Good linearity was obtained in the range of 0.208-41.6, 0.197-39.4, and 0.227-114μM for curcumin, demethoxycurcumin and bisdemethoxycurcumin respectively. The limit of detection reached up to 10 ? 8 M, which was lower than that by UV detection. The relative standard deviations (RSDs) ranged from 1.06%to 1.88%for intra-day precision and from 4.30%to 5.79%for inter-day precision, respectively. The proposed method has been applied in real herb sample and recoveries ranging from 86.3%to 111%were obtained.

ABSTRACT

Objective To investigate the relationship between primary TCM syndromes and heart rate variability in hypertension, evaluate the development of disease. Methods 182 hypertension patients were recruited in accordance with the standard of hypertension. According to the Chinese medicine dialectical criteria, these patients were further divided into 5 groups, namely, hyperactivity of liver-fire group, phlegm-dampness retention group, blood stasis group, hyperactivity of Yang due to yin deficiency group and Qi and blood deficiency group. Another 60 healthy people were recruited as a control group. 24h Holter was adopted to monitor heart rate variability. Results Analysis of TCM syndromes and heart rate variability, anger on inflammation group had positive relation with SDNN, RMSSD, LF, HF and TP(r were respectively 0.161,0.372, 0.223, 0.281, 0.301);Phlegmy wet resistance groups had positive relation with SDNN, HRV triangular index, VLF, LF, VLF/HF, LF/HF, LFnorm.(r were respectively 0.175, 0.211, 0.174, 0.262, 0.159, 0.264, 0.263);blood stasis group had negative relation with LF/HF, LFnorm(r were respectively-0.185、-0.018).Yang due to yin deficiency group had negative relation with SDNN, RMSSD, VLF, LF.(r were respectively -0.204, -0.161,-0.184,-0.197);Qi and blood deficiency group had negative relation with SDNN, SDANN, RMSSD and HRV triangular index, VLF, LF, LFnorm, and had positive relation with HFnorm(r were respectively-0.353,-0.348,-0.258, -0.303, -0.268, -0.361, -0.352, 0.267). Conclusion Hypertension patients with different type of TCM syndromes had different characteristics of autonomic nerve functional disorders, which both reflected the important pathological changes of TCM syndromes and had close relation with the course of hypertension, providing objective basis for clinical diagnosis of syndrome differentiation.
