ABSTRACT

Objective To explore the factors that contribute to the anxiety and depression in obstructive sleep apnea hypopnea syndrome (OSAHS) patients in terms of excessive daytime sleepiness (EDS) and sleep quality.Methods A total of 196 OSAHS patients,including 103 severe patients and 93 mild-moderate patients,were enrolled.Polysomnography was carried on at the sleep center of the First Hospital of China Medical University between May 2013 and November 2015.According to the Epworth sleepiness scale (ESS) and the subject daytime sleepiness symptom,all patients were divided into EDS group and non-EDS group.The patients' general information and subjective symptoms were recorded.Emotional states were assessed with self-rating anxiety scale (SAS) and self-rating depression scale (SDS).Sleep quality was evaluated with Pittsburgh sleep quality index (PSQI).The anxiety and depression related factors were studied by regression analysis.Results (1) In severe OSAHS group,the patients with EDS showed higher PSQI(6.22± 2.57 vs.4.05± 3.72,P<0.01) and oxygen desaturation index(ODI)[(57.70±17.53) events/h vs.(48.23 ± 22.01)events/h,P<0.05] when compared with those without EDS.(2) In both severe and mild-moderate OSAHS groups,the patients with EDS presented higher SAS scores (severe:33.86±7.60 vs.28.95 ± 4.71,mild-moderate:37.46± 10.68 vs.33.40± 11.07,P<0.05)and SDS scores(severe:32.81 ± 8.36 vs.28.90±4.53,mild-moderate:36.98± 12.77 vs.31.70±10.94,P<0.05)when compared with those without EDS.(3) The multiple regression analysis showed that the SAS scores were related to ESS,PSQI,insomnia and nasal obstruction (R2=0.356,P<0.05),and the SDS scores were related to ESS,PSQI and insomnia(R2=0.344,P<0.05).Conclusions The anxiety and depression of OSAHS patients are closely related to the severity of EDS and sleep quality.Both severe and mild-moderate OSAHS patients with EDS have worse anxiety and depression scores.

ABSTRACT

To explore the clinical characters, treatment and prognosis for pediatric optic pathway gliomas (OPGs).
 Methods: A total of 26 patients with OPGs, who were diagnosed and treated at Neurosurgery of Xiangya Hospital of Central South University between June 2010 and November 2017, were retrospectively reviewed, and their average age was 5.5 years old. The influential factors for patients' progression-free survival (PFS) and overall survival (OS) were analyzed.
 Results: All patients were classified into Type II and Type III based on Dodge classification and received surgery treatment. Vision was improved after surgery in 20 patients. Twenty-four patients (92.3%) were continually followed up, and 14 patients (58.3%) received post-radiation treatment. Twenty-one patients were still alive and 15 patients' symptoms were not progressed. The PFS and OS in patients received radiation therapy were better than those without radiation therapy (PFS: P<0.01; OS: P<0.05). The postoperative visual prognosis might be related to the choice of surgical approach.
 Conclusion: Treatment of children with OPGs should include surgery and postoperative radiotherapy. The eyesight protection in surgery is as important as tumor resection.

ABSTRACT

Objective To investigate the effects of chronic intermittent hypoxia on somatotropic axis hormone levels in rats.Methods Mature male Wistar rats were exposed to air or intermittent hypoxia randomly.The serum levels of growth hormone-releasing hormone (GHRH),growth hormone (GH) and somatostatin (SS) were measured before exposure,at the 4th,8th,and 12th week after exposure.Different hormone levels in two groups were compared and analyzed.Results Compared with the control group,GHRH levels in chronic intermittent hypoxic group showed a significant decline at the 4th week [(732.77± 46.99)pg/ml vs.(893.59±40.00) pg/ml,P<0.05],while SS levels at the 8th week [(30.71 ±2.27) pg/ml vs.(44.69±3.36) pg/ml,P<0.05] and GH levels at the 12th week [(1.20±0.29) ng/ml vs.(2.06±0.13) ng/ml,P<0.05]were similarly reduced.As the duration of intermittent hypoxia was prolonged,the GHRH levels did not decrease further [4th week (732.77±46.99) pg/ml vs.8th week (607.54± 131.61) pg/ml vs.12th week (730.05±40.63) pg/ml,P>0.05].However,the serum SS levels decreased further from the 8th week to the 12th week [(30.71±2.27) pg/ml vs.(24.41±4.06) pg/ml,P<0.05].Conclusion Chronic intermittent hypoxia might inhibit the function of somatotropic axis.Hypothalamic hormones are the earlyonesto be influenced,thereafter the entire axis.

ABSTRACT

Intracranial Rosai-Dorfman disease (RDD) is a rare clinical histiocytosis proliferative disease. A 12-year-old boy with dizziness and headache for 1 month was admitted into Pediatric Neurosurgery of Xiangya Hospital, Central South University. The patient underwent total tumor resection and postoperative application of hormones and chemotherapy. During follow-up of 8 months, patient's condition was stable and no tumor recurrence was observed. For patient with a trend of tumor progression, stereotactic biopsy can help to confirm the diagnosis and determine the surgical strategy such as disposal of bone flaps. The treatment is mainly based on surgical intervention, supplemented by radiotherapy, chemotherapy and hormone therapy. Without affecting the nerve function, the surgeon should try to completely resect the tumor.

ABSTRACT

This study was aimed to investigate the effect of baicalin on the proportion of Th22 cells and the concentration of IL-22 bothin vivo andin vitro, in order to explore the immune mechanism of baicalin on inflammatory bowel disease mice model. The 3.5% dextran sodium sulfate (DSS) was used on C57BL/6 mice for the establishment of colitis mice model. Mice were randomly divided into the blank control group, model group, and baicalin group. Flow cytometry and ELISA were used in the detection of the proportion of Th22 cells and concentration of IL-22 in peripheral blood serum, respectively. The spleen lymphocytes of mice were isolated and cultured by baicalin medium (0, 10, 20, 40μM) for 48 h. Flow cytometry was used in the detection of the proportion of Th22 cells. The results showed that baicalin reduced the proportion of Th22 cells and the expression of IL-22 bothin vivo andin vitro experiments. It was concluded that baicalin can inhibit Th22 cell differentiation and expression of IL-22in vitro and DSS-induced colitis mice. It indicated that baicalin had a good treatment potential in Th22 cell-mediated inflammatory diseases.

ABSTRACT

Objective To investigate the effects of statins on inflammation factors in the treatment of patients with rheumatoid arthritis(RA) .Methods Relevant randomized controlled trials (RCTs) were retrieved from the following electronic databases :PubMed(January 1966 to August 2011) ,CENTRAL(Cochrane Controlled trials Register ;issue 3 ,2011) and EMBASE .com(Janu-ary 1984 to August 2011) .Two independent reviewers systematically identified prospective RCTs to detect the effect of statins on inflammation in patients with RA .A Meta-analysis of these clinical trials was then performed .Results 7 RCTs ,enrolling a total of 279 patients .Inflammation level of RA was assessed by erythrocyte sedimentation rate (ESR) and C-reactive protein(CRP) level .St-atins had a positive effect on reducing the CRP [x-0 .58s ,95% CI:(-1 .08 ,-0 .09) ,P=0 .02] ,but did not has a positive effect on reducing the ESR[x-3 .62s ,95% CI:(-12 .15 ,4 .92) ,P=0 .41] level in patients with RA .Conclusion Statins therapy in patients with RA may have a positive effect on reducing the inflammation factors level .And more RCTs would be necessary to further con-firme the effect .
