ABSTRACT

PURPOSE: As a treatment of giant cell tumors (GCT) of the long bone, anhydrous alcohol was used in our institution due to the potential complications associated with other previous adjuvants. Therefore, this study evaluated the feasibility and effectiveness of anhydrous alcohol as a new adjuvant in the treatment of a GCT. MATERIALS AND METHODS: One hundred and three GCT patients were treated and followed up for an average of 6 years (range, 1-29.3 years) with a mean age of 33 years (range, 16-67 years). Curettage, additional burring, and reconstruction with a bone graft or cementing were performed in 69 patients. Among them, anhydrous alcohol was used as an adjuvant in 38 patients. RESULTS: Four (10.5%) patients given the anhydrous alcohol treatment had a local recurrence, whereas 15 recurrences (48.4%) developed in 31 patients treated without the anhydrous alcohol (p=0.001). There were no anhydrous alcohol-related complications. According to Kaplan-Meier's analysis and the log rank test, the anhydrous alcohol adjuvant showed a significant effect (p=0.014) in preventing the recurrence of GCT. CONCLUSION: The data suggests that anhydrous alcohol can be used as an effective adjuvant without potential risks for the treatment of a GCT of the long bone.

ABSTRACT

Chronic granulomatous disease is characterized by recurrent life-threatening infections and the formation of granulomatous lesions caused by the failure of phagocytic oxidative microcidal activity that has resulted from a defect of the oxidase system. The fungal osteomyelitis of the spine in this disease is almost always caused by Aspergillus, and its treatment is not yet established although surgical management, antifungal agents, granulocyte transfusions or combination methods have been reported. We report a patient with aspergillosis who had a long-standing pulmonary lesion which subsequently spread to the adjacent thoracic wall and spine. After decompression and drainage by costotransversectomy, the severe pain was relieved but the kyphosis increased and a paraplegia developed slowly because of resistance to the antifungal therapy and the inability to perform a bone graft. The patient died 2 months after the operation because of pulmonary complications.

ABSTRACT

PURPOSE: One-stage corrective osteotomy of a symptomatic malunion of a distal radius fracture, is not easy because of the contracture of surrounding soft tissues. We have treated these malunions by a one-stage corrective osteotomy procedure that uses an intraoperative gradual distraction method of the osteotomy site. We have analysed the clinical and radiological results of our procedure, retrospectively. MATERIALS AND METHODS: We have operated on 7 wrists of 7 patients who had malunion of a distal radius fracture who complained of both wrist pain and functional impairment. The distal radius was osteotomized and distracted gradually for 20 to 30 minutes using a lamina spreader. Then the gap was filled with a pre-designed iliac bone graft. The results were analysed functionally using the Cooney's wrist score, cosmetically using Ladd's method, and radiologically using Kreder's method. RESULTS: The Cooney's wrist score improved from an average 44 preoperatively to 91 at the latest follow-up. The clinical results were excellent in 4 patients, good in 2 patients and fair in one patient. CONCLUSION: A one-stage corrective osteotomy with iliac bone graft using a laminar spreader is followed by creep phenomenon is an useful method for the treatment of the malunion of distal radius fracture.
