ABSTRACT

The arteriovenous fistula and cirsoid aneurysm, characterised by abnormal arterial and venous connections with grossly dilated and expansile mass of vessels, may predispose to dramatic complications. Cirsoid aneurysms of the scalp have been reported to be difficult to remove. Multiple treatment schemes have been described and, as yet, no standard form of therapy exists. Three cases of cirsoid aneurysms of the scalp are presented. Each of them was treated with embolization and total excision after ligation of the feeding arteries. All of them were well cured.

ABSTRACT

The 302 patients with head-injured military personnel who had admitted to the Department of neurosurgery, Capital Armed Forces General Hospital were clinicostatistically analysed and discussed with the review of the literatures. The results were as follows: The most common cause of head injury was traffic accident(38.4% of the total cases) and the next were, in order of incidence, simple fall(9.5%), exercise(3.9%), assault(13.5%) and gun shot wound or explosive injury(7.2%). 2) Skull fracture was identified in 214 cases;98 cases of linear, 87 cases of depressed, 4 cases of basal, 2 cases mixed skull fracture. 3) The 85.7% of epidural hematomas were associated with skull fracture and the most common site of hematoma was frontal area. 4) Of all cases of subdural hematoma, acute type was prevalent(67.9%) and its main involved wite was frontotemporoparietal areas. 5) The associated injuries were found in 35.7% of patients. 6) The minor head injury of which GCS is from 13 to 15 was 148 cases, the moderate head injury of which GCS is fromn 9 to 12 was 49 cases, the severe head injury of which GCS is form 3 to 8 was 105 cases. 7) Of 105 cases of severe head injured patients the mortality was 61.9% and good recovery was 7.6%. But outcome was different according not only to GCS but also the type of lesion. 8) The operative morality was 24.1% and the good recovery was achieved 55.2% of patients. The remaining 8.9% resulted in disabled or vegetative state. 9) In the accidents of gun shot wound or explosive injury, operation mortality rate was 31.8%, but 8 cases(36.3%) were seen good recovery.

ABSTRACT

The complexily in development of the dural sinus may result in many variations. Studies are being done on the variation of the straight sinus for clinical application of neurological diagnosis and operation. However, no data is available on Koreans, which made this study necessary. The present study was made on 50 Korean adult cadavers (average age of 67). The results were as followlng. 1. A bulbous formation of the rostral aspects of the straight sinus was present in 22 cases(44%). 2. Complete double straight sinus was present in 3 cases(6%). 3. Partial septum was observed in 8 cases (16%) with varied position and length. 4. The straight sinus terminated into both transverse sinuses(52%), the left transverse sinus(24%), the confluence of sinuses (18%), and the right transverse sinus(6%). 5. Average length and width of the straight sinus were 48.5mm and 5.9mm, respectively. 6. Nineteen cases(38%) showed no tentorial sinus drainage into the straight sinus on both sides. ABSTRACT

A series of 24 cases of gun shot and explosive injuried patients of the brain is analyzed according to causes, types, operability, the relation between Glasgow coma scale(GCS) and operative mortality, associated injuries, complications and sequelae. The authors notice that gun shot and explosive injury of the brain are more worse than blunt head trauma because of injury mechanism by itself.

ABSTRACT

We experienced a case of cavernous hemangioma suspected to cerebral cysticecosis. The 23-year-old male patient was admitted to our hospital due to sudden severe headache and generalized tonic-clonic seizure. The computerized tomography showed multiple calcification of bilateral cerebral hemisphere, especially right, and cystic mass on right frontal area with mass effect. The mass was diagnosed cavernous hemangioma under the microscopic examination.
