ABSTRACT

Meckel's diverticulum is one of the most common congenital anomalies, and its incidence is about 2% in the population. Most of the cases are asymptomatic and only 5% of cases are symptomatic with complications, including bleeding, intestinal obstruction, inflammation and perforation. Bleeding from a Meckel's diverticulum is usually painless and is sometimes massive. Recently, the use of double balloon enteroscopy has allowed improved access in patients with obscure gastrointestinal bleeding; however, a case of bleeding from a Meckel's diverticulum treated with double balloon enteroscopy hs rarely been reported. Double balloon enteroscopy can diagnose a Meckel's diverticulum without difficulty as the lesion is usually located at the distal ileum, and the method provides endoscopic hemostasis for the bleeding. Therefore, double balloon enteroscopy might be a useful diagnostic and therapeutic modality for bleeding from a Meckel's diverticulum. We experienced a case of bleeding from a Meckel's diverticulum that was detected and was treated by the use of double-balloon enteroscopy in a 47-year-old man with recurrent episodes of melena.

ABSTRACT

Pulmonary alveolar proteinosis is a rare disorder in which lipoproteinaceous material accumulates within the alveoli. We report a case of pulmonary alveolar proteinosis in a 41 year old female patient. She complained of a dry cough in the preceding 6 months. She presented symptoms of mild hypoxemia and diffuse infiltration at both lower lung fields. A milky fluid was obtained by bronchoalveolar lavage. We confirmed by light microscopic examinations of the lung tissues obtained by transbronchial lung biopsy. Through several follow-ups, the patients symptoms were mild.

ABSTRACT

Foreign bodies in the stomach usually are accidentally swallowed and rarely produce symptoms. For diagnosis, suspicion and report of related history with abdominal symptoms are important. Simple radiographs and endoscopy easily identify foreign objects. Abdominal computed tomography may be useful in some cases, although it is not essential. We experienced a case of a gastric foreign body incidentally diagnosed by a follow-up abdominal computed tomography (CT) for hepatoma. A 53-year-old male with B viral liver cirrhosis and hepatoma, visited our hospital for a routine follow-up check of hepatoma. He had a symptom of epigastric discomfort for two weeks. An abdominal CT showed a foreign body penetrating the stomach wall, and it was successfully removed endoscopically.

ABSTRACT

Pneumocystis carinii pneumonia (PCP) is an infectious disease of immune-compromised host. Sometimes it is difficult to differentiate PCP with diffuse pulmonary hemorrhage. Association between PCP and diffuse pulmonary hemorrhage has been reported in 30% of PCP with HIV positive patients. But association between PCP and diffuse pulmonary hemorrhage has not been reported in non-HIV positive patients without any known underlying causes of diffuse pulmonary hemorrhage. We report a case of PCP with diffuse pulmonary hemorrhage in 66 years old male patient. We confirmed PCP and diffuse pulmonary hemorrhage with bronchoalveolar lavage. We can exclude the possible other causes of diffuse pulmonary hemorrhage except PCP. PCP may be one of possible cause of diffuse pulmonary hemorrhage in non-HIV immune compromised patient.
