ABSTRACT

ObjectiveTo evaluate the efficacy and safety of a perioperative rehabilitation clinical pathway of acetabular fracture in light of orthopedics rehabilitation team approach. MethodsA prospective randomized control trial was conducted in 82 patients with acetabular fractures who had been admitted from the Emergency Department of Orthopaedic Trauma, Beijing Jishuitan Hospital from June, 2019 to January, 2021. The patients were randomly divided into control group (n = 41) and intervention group (n = 41). The control group was managed routinely, while the intervention group received the rehabilitation clinical pathway, for 24 weeks. The Visual Analogue Score (VAS) of pain, the Barthel Index (BI) and Majeed Pelvic Score were compared. ResultsFinally, 76 patients completed the trial. There was no statistical difference in VAS score between two groups in all periods (|Z| < 1.926, P > 0.05). The BI score was higher in the intervention group than in the control group at discharge, two weeks, six weeks and twelve weeks after operation (|Z| > 2.121, P < 0.05); and no significant difference was found before operation and 24 weeks after operation (|Z| < 1.862, P > 0.05). Majeed Pelvic Score was higher in the intervention group than in the control group two weeks, six weeks, twelve weeks and 24 weeks after operation (|Z| > 2.428, P < 0.05). Six, twelve and 24 weeks after operation, the excellent rate of Majeed Pelvic Score was higher in the intervention group than in the control group (χ2 > 6.136, P < 0.05). ConclusionIn comparison with traditional protocol in acetabular fracture, the perioperative rehabilitation clinical pathway was proved effective and of great safety in the light of the integration of orthopedics and rehabilitation mode for improving the function and activities of daily living of patients.

ABSTRACT

Objective:To evaluate the efficacy and safety of perioperative rehabilitation approaches based on the concept of Enhanced Recovery After Surgery (ERAS) for pelvic fractures.Methods:A prospective randomized control trial was conducted to include 114 emergency patients who had been admitted to Department of Orthopaedic Trauma, Beijing Jishuitan Hospital for surgical treatment of pelvic fractures from June 2019 to December 2020. Of them, 57 were assigned into an intervention group according to a random digits table. They were 42 males and 15 females, aged from 18 to 77 years and subjected to management of pelvic fractures with tentative perioperative ERAS approaches which were adjusted at different stages. The other random 57 patients were assigned into a control group. They were 40 males and 17 females, aged from 17 to 70 years and subjected to management of pelvic fractures with conventional rehabilitation approaches which included postoperative in-hospital consultation and guidance by rehabilitation physicians. The 2 groups were compared in terms of Majeed pelvis scores and Barthel indexes at postoperative 2, 6, 12 and 24 weeks, and visual analogue scale (VAS) pain scores and SF36 scores at postoperative 12 and 24 weeks.Results:A total of 105 patients (55 in the intervention group and 50 in the control group) were completely followed up for 151 to 254 d (mean, 177 d). The 2 groups were comparable due to no significant difference between them in the preoperative general data ( P>0.05). The Majeed scores (44±13, 67±16, 86±14 and 98±7) and Barthel indexes (57±13, 79±16, 95±8 and 100±2) at postoperative 2, 6, 12 and 24 weeks in the intervention group were significantly higher than those in the control group [(35±16, 51±16, 73±14 and 91±12) and (45±19, 67±18, 86±12 and 98±4)] (all P<0.05). At postoperative 12 and 24 weeks, the SF-36 scores (129±15 and 141±6) in the intervention group were significantly higher than those in the control group (114±15 and 131±12) ( P<0.05). There was no significant difference in the pain degree between the 2 groups ( P>0.05). Conclusion:In management of pelvic fractures, compared with conventional perioperative rehabilitation approaches, the perioperative ERAS rehabilitation approaches may improve early functional outcomes and thus help the patients restore their activities of daily living earlier.

ABSTRACT

Objective:To investigate the clinical features and prognosis of patients with corona virus disease 2019 (COVID-19).Methods:The clinical data and chest computed tomography (CT) results of 496 patients with COVID-19 admitted to Wuhan No.7 Hospital from January 22 to February 24, 2020 were retrospectively analyzed. The nucleic acid of 2019 novel coronavirus (2019-nCoV) was detected by real-time fluorescence reverse transcription polymerase chain reaction.Results:There were 246(49.6%) males and 250(50.4%) females. The patients ranged from 10 to 91 years old. All of the patients had a history of living in Wuhan City or close contact with diagnosed patients. The detection of nucleic acid of 2019-nCoV in 496 patients were all positive, and one patient was recuperation positive 14 days after cured and discharged. There were 13 mild cases, 101 ordinary cases, 337 severe cases and 45 critical cases. Twelve (2.4%) patients were asymptomatic, 417(84.1%) patients had fever, and 67(13.5%) had normal body temperature. Other major symptoms included dry cough (229 cases (46.2%)), fatigue (129 cases (26.0%)), short breath (77 cases (15.5%)), expectoration (86 cases (17.3%)), dyspnea (43 cases (8.7%)), chest pain (11 cases (2.2%)) and diarrhea (86 cases (17.3%)). Seventy-five cases (15.1%) showed decreased peripheral blood white blood cell counts and 305 cases (61.5%) showed decreased lymphocyte proportions. Serum alanine aminotransferase and aspartate aminotransferase elevations were presented in 91 cases (18.3%) and 176 cases (35.5%), respectively. Infiltrates on chest CT were seen in 483 cases (97.4%), with 68.7%(332/483) in both lungs, 20.3%(98/483) in right lung, 11.0%(53/483) in left lung. No infiltrates on chest CT were seen in 13(2.6%) patients. As of February 24, 2020, 120(24.2%) patients were cured and discharged, 102(20.6%) patients improved, 52(10.5%) patients died, and the remaining patients were still under treatment. Among the dead patients, 16 cases (30.8%) aged from 61 to 70 years old, 32 cases (61.5%) aged≥71 years old, and 38 cases (73.1%) had underlying diseases.Conclusions:The COVID-19 lesions mainly involve both lungs. The main clinical features are fever and dry cough. The CT imaging findings of the lungs are mostly frosted hyaline. Peripheral white blood cell and leukomonocyte counts are decreased or normal. Older individuals with underlying diseases have increased risk of death.

Objective@#To assess the longitudinal mitral annular plane systolic excursion (MAPSE) of different directions in normal fetuses during mid-late pregnancy based on two-dimensional speckle tracking imaging (STI).@*Methods@#Seventy-six normal fetuses during middle and late pregnancy were selected at 26-32 weeks of gestation. The peak MAPSE was measured by free angle M-mode echocardiography (FAM) perpendicular to the lateral annulus in the mitral annular plane. The time-displacement curves of interventricular septal mitral annulus in three different directions including points A, B and C through transverse level of apex were recorded by STI. The peak MAPSE of interventricular septal mitral annulus (SEPT-MAPSE-A, SEPT-MAPSE-B, SEPT-MAPSE-C) in three different directions including points A, B and C and the time to peak (TTP: SEPT-TTP-A, SEPT-TTP-B, SEPT-TTP-C) were recorded respectively. The time-displacement curves of lateral mitral annulus in three different directions including points A, B and C through transverse level of apex were recorded by STI. The peak MAPSE of lateral mitral annulus (LAT-MAPSE-A, LAT-MAPSE-B, LAT-MAPSE-C) in three different directions including points A, B and C, the time to peak(LAT-TTP-A, LAT-TTP-B, LAT-TTP-C) were recorded respectively. Finally, the data were analyzed statistically.@*Results@#The peak MAPSE of the lateral mitral annulus in 3 different directions including points A, B and C[LAT-MAPSE-A (3.62±1.01)mm, LAT-MAPSE-B (3.95±1.04)mm, LAT-MAPSE-C (4.45±1.05)mm] were greater than those of the interventricular septum mitral annulus[SEPT-MAPSE-A (3.41±0.63)mm, SEPT-MAPSE-B (3.07±0.50)mm, SEPT-MAPSE-C (2.82±0.51)mm]. LAT-MAPSE-C and SEPT-MAPSE-A were the largest longitudinal excursions of mitral annulus. The differences were statistically significant in points B and C (P<0.05). There was no significant difference in point A (P>0.05). LAT-MAPSE-C was less than FAM-MAPSE[(6.06±1.35)mm]. There was a significant difference between them(P<0.05). Strong correlation was found between them(r=0.896, P<0.05). There were no significant differences in the time to peak of interventricular septal mitral annulus [SEPT-TTP-A (0.210±0.008)s, SEPT-TTP-B (0.213±0.008)s, SEPT-TTP-C (0.210±0.005)s] in directions including points A, B, C(P>0.05). There were no significant differences in time to peak of lateral mitral annulus[LAT-TTP-A(0.210±0.008)s, LAT-TTP-B(0.213±0.006)s, LAT-TTP-C(0.210±0.007)s] in directions inclucling points A, B, C(P>0.05).@*Conclusions@#Longitudinal systolic motion of fetal left ventricular wall during mid-late pregnancy has good synchronization. Longitudinal motion of fetal mitral annulus is a comprehensive movement of multiple directions and different degrees of displacement, with the movement perpendicular to the annulus as the maximum displacement direction. The displacement parameters of mitral annulus measured by STI can reflect the left ventricular longitudinal systolic function and have clinical application value in evaluating the left ventricular longitudinal systolic function of fetuses.

ABSTRACT

Objective To establish z-score model for fetal tricuspid annular plane systolic excursion(FAM-TAPSE) and mitral annular plane systolic excursion(FAM-MAPSE) based on gestational age(GA),then to evaluate the ventricle systolic function of fetus with heart failure.Methods One thousand and twelve normal fetuses and 24 fetuses with heart failure were involved. FAM-TAPSE and FAM-MAPSE were measured by free angle M-mode echocardiography,and FAM-TAPSE and FAM-MAPSE z-score models of normal fetuses were constructed by using first standard regression analysis with GA as independent variable.The fetuses with heart failure were divided into left heart failure (LHF) group and right heart failure (RHF) group by Tei index.Subsequently,the two parameters between normal and fetuses with heart failure were compared. Results The models used to calculate z-score for FAM-TAPSE and FAM-MAPSE were constructed,and GA had close correlation with them. Compared with normal fetuses,the mean z-scores of FAM-TAPSE and FAM-MAPSE were statistically different in fetuses with heart failure(P<0.001). The FAM-MAPSE z-scores of LHF and the FAM-TAPSE z-scores of RHF were all less than-2 z-scores.Conclusions The FAM-TAPSE and FAM-MAPSE z-scores decline in fetuses with heart failure and they can provide quantitative evidence in evaluation of heart systolic function,FAM-TAPSE and FAM-MAPSE z-score would be markers for assessing heart systolic function in fetuses with heart failure.

ABSTRACT

<p><b>OBJECTIVE</b>To construct a universal, highly attenuated orf virus expression vector for exogenous genes using green fluorescent protein (GFP) as the reporter gene.</p><p><b>METHODS</b>The flanking regions of the ORFV132 of orf virus DNA were amplified by PCR to construct the shuttle plasmid pSPV-132LF-EGFP-132RF. The shuttle plasmid was transfected into OFTu cells and GFP was incorporated into orf virus IA82Delta 121 by homologous recombination. The recombinant IA82Delta121-V was selected by green fluorescent signal. The deletion gene was identified by PCR and sequencing. The effects of ORFV132 knockout were evaluated by virus titration and by observing the proliferation of the infected vascular endothelial cells in vitro.</p><p><b>RESULTS</b>The recombinant orf virus IA82Delta121-V was obtained successfully and quickly, and the deletion of ORFV132 did not affect the replication of the virus in vitro but reduced its virulence.</p><p><b>CONCLUSION</b>Green fluorescent protein is a selectable marker for rapid, convenient and stable selection of the recombinant viruses. Highly attenuated recombinant orf virus IA82Delta121-V can serve as a new expression vector for exogenous genes.</p>

ABSTRACT

Objective To obtain the annular plane systolic excursion difference (APSED)of fetuses in second and late trimester by free angle M-mode (FAM)and tissue motion of annular displacement (TMAD)in order to assess the fetal ventricular function.Methods The mitral annular plane systolic excursion(MAPSE)and tricuspid annular plane systolic excursion(TAPSE)of four hundred and fifty five normal fetuses from 20 to 41 weeks were measured by FAM echocardiography,and the difference between them were calculated.Early diastolic velocities (Em)of the mitral annular and early diastolic velocities (Em')of the tricuspid annular were estimated by pulsed-wave tissue Doppler imaging (TDI),meanwhile systolic velocities (Sm)of the mitral annular and systolic velocities (Sm')of the tricuspid annular were also estimated by TDI.Fifty normal fetuses in the second and late trimester were choosed randomly,two-dimension imaging was obtained at the apical four-chamber view,then mitral annular plane and tricuspid annular plane displacement curve were acquired at the same time by using off-line QLab 8.1 software,and the differences in the peak time between them were compared.The trace of annular displacement were recorded by color tissue tracking technology.Results There was a significant difference between FAM-TAPSE and FAM-MAPSE in different gestational weeks (P 0.05).Conclusions The APSED existed in the second and late trimester continually.It was considering associated with fetal ventricular torsion.As the growth of gestational weeks,the increase of APSED reflected the enhanced reserve capacity of the fetal ventricular torsion.APSED can reflect the longitudinal motion and torsion movement of fetal ventricle quantitatively,and can be used as a new index to assess the fetal cardiac function.

ABSTRACT

Objective To evaluate the usefulness of free angle M-mode echocardiography (FAM) in obtaining tricuspid annular plane systolic excursion (TAPSE) for assessing the fetal right ventricular function,and to compare the results of measurements of TAPSE by conventional M-mode(CM) and FAM.Methods Two hundred and forty-three normal fetuses in second and late trimester were divided into 5 groups by gestational age(GA):20-24 weeks,24+1-28 weeks,28+1-32 weeks,32+1-36 weeks,36+1-40 weeks.The TAPSE were measured by CM and FAM echocardiography,meanwhile multiple parameters for evaluating right ventricular function were obtained by using myocardial tissue Doppler imaging (TDI),and the correlation between TAPSE and other parameters were analyzed by linear correlation.ANOVA was used to compare CM-TAPSE,FAM-TAPSE,Em,Sm and right ventricular fractional shortening(RVFS)with different GA.Regression equation estimate was used to compare the relationship of FAM-TAPSE with GA.Independent sample t test was used to compare CM TAPSE with FAM TAPSE.Results There were significant differences in CM-TAPSE,FAM-TAPSE,Em,and Sm among 5 groups (P =0.000),the measured value of FAM-TAPSE was higher than that of CM TAPSE (P =0.000).Both FAM-TPASE and CM-TAPSE showed significant positive correlations with GA,Em and Sm,but FAM TAPSE demonstrated better correlation than CM-TAPSE.Conclusions By adjusting sample line of FAM,measuring the maximum displacement of tricuspid annular plane is feasible,and FAM may evaluate fetal right ventricular function more accurately and effectively.

ABSTRACT

Objective To develop Z-score reference ranges for tricuspid annular plane systolic excursion(TAPSE) in normal fetuses from the measurements of gestational age(GA),biparietal diameter (BPD) or femur length(FL) using fetal echocardiography.Methods A retrospective cross sectional study of 1012 singleton normal fetuses were performed.The gestation age ranged from 20 to 41 weeks.Non-cardiac fetal biometric parameters included GA and BPD and FL were measured and calculated GA based on menstrual age.TAPSE was measured in a standard apical four-chamber view by free angle M-mode echocardiography.Normal Z-score ranges were developed for TAPSE using GA,BPD and FL as independent variables.These were accomplished by using first standard regression analysis and then weighted regression of absolute residual values for each parameter in order to adjust for inconstant variance.Results Linear regression model was the best description of the data in each case and correlations between TASPE and independent variables (GA,BPD,FL) were excellent.Heteroscedasticity of standard deviation (SD) with increasing independent variables also could be modeled with a simple linear regression.According to these equations,TAPSE Z-score =(the actual measurement of TAPSE-predicted TAPSE)/prediction SD.Conclusions Normal reference ranges and Z-scores for TAPSE have been provided.These normative data may be useful tools for assessment of fetal TAPSE,to evaluate fetal right ventricular function more accurately and effectively.

@#ObjectiveTo assess different treatments on nonspecific low back pain.Methods246 subjects were assigned to 3 groups, a control group(n=82), a home exercise group(n=82) or a strengthening exercise group(n=82). Follow-up data were obtained at 1 months, 3 months and 6 months after the treatment. Primary outcome measures were modified Oswestry Low Back Pain Disability Questionnaire for disability and Numeric Pain Rating Scale for pain.ResultsThe home exercise and strengthening exercise groups experienced greater improvements in disability than the control group 6 months after the treatment. The strengthening exercise group demonstrated greater change than home exercise group.ConclusionActive rehabilitation therapy may benefit nonspecific low back pain patients.

@#ObjectiveTo explore the relationship between rotary strength of trunks and stability of the low back in patients with nonspecific low back pain. MethodsBefore sling exercise therapy (SET), 22 patients with chronic nonspecific low back pain were measured the rotary strength of core muscles of trunks with Proxomed Tergumed functional spinal evaluation and training system and the core stability of the low back with SET system. Their correlation was analyzed. ResultsThe core rotary strength was positively correlated with the local stability in patients with chronic nonspecific low back pain (r=0.876, P<0.001). ConclusionIncreasing the rotary strength of trunks is important to improve local stability in patients with chronic nonspecific low back pain.

@#Objective To explore effects of sling exercise therapy on back pain of adults with idiopathic scoliosis.Methods38 subjects were age-matched assigned to a control program (n=18) or a treatment program (n=20). Follow-up data were obtained 4 weeks after treatment. Primary outcome measures were disability (Oswestry Low Back Pain Disability Questionnaire) and pain (Numeric Pain Rating Scale).ResultsThe treatment group experienced greater improvements in disability and pain compared with the control program 4 weeks after treatment.ConclusionSling exercise therapy may benefit back pain of adults with idiopathic scoliosis.

@#Objective To explore the proper method of rehabilitation after operation of arthroscopic rotator cuff repair.Methods A total of 86 cases treated by arthroscopic rotator cuff repair in Beijing Jishuitan Hospital from March 2002 to December 2006 were included in our study and randomly divided into two groups which are normal group and intensive group. The two groups were treated with same rehabilitation protocol but had different frequency in hospital. American Shoulder& Elbow Surgeons′ Score (ASES score) and Guides to the Evaluation of Permanent Impairment (GEPI) were adopted before operation and one year follow-up. Results There were no significant difference between two groups in ASES and GEPI scores before the operation(P>0.05). Comparing with preoperation, there were significant difference between the two groups in ASES and GEPI scores 1 year after operation(P<0.05). Comparing with the normal group, the intensive group have higher ASES scores than that of normal group and lower GEPI scores than that of normal group 1 year after operation (P<0.05). The range of motion of active forward flexion and external rotation were tested at the 7th week, 12th week and 1 year after the operation, all the result of intensive group were better than that of normal group (P<0.05).Conclusion Customized and systematic shoulder rehabilitation training therapy for the patients with rotator cuff repaired is an effective method to restore the function of shoulder joint. Intensive treatment has better result.

@#Objective To research the incidence rate,potential etiologic factors,treatment of complications in patients with acute spinal column and spinal cord injury.Methods The date of 543 patients with acute spinal column and spinal injury were analyzed retrospectively.Results Of 543 patients,242 cases(44.6%)had complications.The first to third complications were hyponatremia(202 cases,37.2%),urinary tract infections(150 cases,27.6%)and pulmonary infections(54 cases,9.9%)respectively.The incidence rate of hyponatremia and pulmonary infections in patients with complete cervical spinal cord injuries increased obviously.Conclusion Early rehabilitation and active prevention can prevent the complications in patients with acute spinal column and spinal cord injury.

ABSTRACT

Objective To explore the clinical value of measuring solubie serum CD44 variant 6sCD44v6 in patients with colorectal cancer. Methods Serum leves of sCD44v6 and CEA in96 patients with colorectal and in 24 healthy indivduals were detected by ELISA andchem-iluninesenle immunoassoly,respectively.Some ehnieal pathological indexes were analyzed.Results Mean sCD44v6 level in patients with colorectal cancer was 275.3+-115.8 ng/ml,which was significantly hirher than that in healthy controls(61.3+-7.8 ng/ml,P<0.05).When using the mean+2s sCD44v6 level of healthy controls as the cut-off value,sCD44v6 level were abnormally elevated in 71 of 96(73.96%)patients with coloreeatal cancer.Conclusion The sCD44v6 leveland positive rate also increased with Dukesstage and tumor metastasis Compared with serum CEA level,the sCD44v6 level might reflect the tumor invasion and metastasis more directly and sensitively.

@# ObjectiveTo define the occurrence rate,time course, potential etiologic factors, treatment and prognosis of hyponatremia in patients with actue spinal cord injury.Methods99 patients who were admitted with hyponatremia after acute spinal cord injury were analysed retrospectively. ResultsAll these patients could concluded into three types:type I were mostly the patients with thoracic and lumbar spinal cord injury,whose mean lowest serum natrium concentration (MLSNC) were (128.6±6.6)mmol/L,continued (8±5.3)days,and it could be recovered after two weeks by accurate supply natrium and/or fluid restriction.Type II included the patients with cervical spinal cord injury,whose MLSNC were(125.1±6.0)mmol/L, continued(26.7±17.0)days,mean uric natrium concentration were 200mmol/24h,and poorly responded to the therapy of supply natrium.Type III also included the patients with cervical spinal cord injury, whose MLSNC were (118.3±9.4)mmol/L, continued (36.8±5.4)days, uric natrium concentration were 200-400mmol/24h, urinary volume had been above to 4000ml for 4 weeks.The serum natrium concentration would decrease after supply natrium therapy,but would recover.after fluid restriction. ConclusionsThe hyponatremia in patients with actue spinal cord injury sould be treated according to different type.

@#ObjectiveTo study the mechanisms and related factors and preventive managements of the stress ulcer complicating with acute cervical spinal cord injury. MethodsThe clinical findings of eight cases of the acute cervical spinal cord injury with stress ulcer were reviewed retrospectively.ResultsFive cases were cured by conservative treatment, one case was cured after operations, two cases died of breath and circulation failure.ConclusionStress ulcer is a critical complication of acute cervical spinal cord injury. It should be prevented actively.

@#Objective To summarize the experience in the treatment of ankylosing spondylitis with spinal fracture and spinal cord injuries. Methods The treatment and results of 12 patients with ankylosing spondylitis with spinal fracture and spinal cord injuries were reviewed and compared. Results and Conclusions We emphasize that the treatment of complications, especially pneumonia, is very important. Conservative treatment may be used at first, but the patients with surgical indications should be operated early.
