ABSTRACT

Objective:To investigate clinicopathological features of swimming pool granuloma. Methods:From January 2018 to January 2021, 56 patients with swimming pool granuloma were collected from Tianjin Academy of Traditional Chinese Medicine Affiliated Hospital, and their clinical and pathological characteristics were retrospectively analyzed. Results:Among the 56 patients, 16 were males and 40 were females, with an average age of 60.84 years. The most common exposure type among these patients was handling of infected fishes or seafood by aquaculture workers and residents in daily cooking (31/56) , the average incubation period was 4.58 weeks, and the average time to diagnosis was 3.19 months. All skin lesions were located at the upper extremities, mainly manifesting as erythema and papular nodules, and sometimes as pustules, ulcers, granulomas or verrucous plaques. Eleven patients presented with solitary skin lesions, 36 with sporotrichoid skin lesions, and 6 with bilateral sporotrichoid lesions. Histopathologically, infectious granulomas were observed in all patients except 4 without specific changes, and 37 presented with characteristic exudative necrosis, with varying amounts of fibrinoid exudative or necrotic elements in the center, and a large number of neutrophils, histiocytes and multinucleated giant cells infiltrating inside or around it. Sequences of Mycobacterium marinum were identified in all 56 cases by metagenomic DNA sequencing of pathogenic microorganisms. Conclusion:In Tianjin area, swimming pool granuloma mostly affected elderly females, handling of infected fishes or seafood was the main type of exposure, and skin lesions were histopathologically characterized by exudative necrotic granulomas.

ABSTRACT

Objective To evaluate the effect of carbon monoxide (CO) postconditioning on pyroptosis induced by oxygen-glucose deprivation and restoration (OGD/R) in rat hippocampai neurons and the relationship with mitochondrial permeability transition pore (mPTP)/reactive oxygen species (ROS) signaling pathway. Methods Primary hippocampal neurons were cultured in vitro, seed in 6-well or 96-well plates, and divided into 5 groups (n =24 each) using a random number table method:control group (C group),OGD/R group,CO postconditioning group (CO group),specific mPTP opener atractyloside plus CO postconditioning group (ACO group),and specific ROS inducer antimycin A plus CO postconditioning group (KCO group).Neurons were subjected to O2-glucose deprivation (OGD) for 16 h followed by restoration of O2-glucose supply for 24 h to establish the model of OGD/R injury. In group CO,neurons were exposed to 2% CO-5% CO2 for 3 h at 37 ℃ starting from the end of OGD,followed by normal culture for 21 h.In ACO and KCO groups, atractyloside 20 μmol/L and antimycin A 50 μmol/L were added at the end of OGD,respectively, and the other treatments were similar to those previously described in group CO.Neuronal pyroptosis rate was determined using double immunofluorescent staining cleaved caspase-1-AlexaFluor 568/DAPI after the end of treatments in each group. The neuronal survival rate was determined by MTT,opening of mPTP by Calcein-AM fluorescence,ROS content by DCFH-DA,and expression of interleukin1beta (IL-1β) and IL-18 by Western blot. Results Compared with C group, neuronal pyroptosis rate,ROS content and opening of mPTP were significantly increased, the neuronal survival rate was decreased, and the expression of IL-1β and IL-18 was up-regulated in the other groups (P<0.05).Compared with OGD/R group, neuronal pyroptosis rate,ROS content and opening of mPTP were significantly decreased, the neuronal survival rate was increased, and the expression of IL-1β and IL-18 was down-regulated in CO,ACO and KCO groups (P<0.05).Compared with CO group, neuronal pyroptosis rate and ROS content were significantly increased, the neuronal survival rate was decreased, and the expression of IL-1β and IL-18 was up-regulated in ACO and KCO groups, and opening of mPTP was significantly inctreased in ACO group (P<0.05).Conclusion CO postconditioning can inhibit OGD/R-induced pyroptosis in rat hippocampal neurons, and the mechanism is related to inhibiting mPTP/ROS signaling pathway.

ABSTRACT

Objective To analyze and compare reflectance confocal microscopy (RCM) features between Paget's disease and eczema on the scrotum. Methods RCM was performed to image scrotal lesions of 5 male patients with suspected Paget's disease and 5 male patients with eczema. Then, the scrotal lesions were resected and subjected to histopathological examination, and a comparison was conducted between the confocal microscopic and pathological findings. Results RCM imaging for the 5 cases of Paget's disease showed disorganized epidermal structure, absence of normal honeycomb structures, single or clustered pagetoid cells in the prickle cell layer.RCM imaging for the 5 cases of eczema showed intercellular edema in the prickle cell layer, reticular degeneration of blisters in all the cases, and pustule formation in some cases. Conclusion RCM features are obviously different between Paget's disease of the scrotum and scrotal eczema,so RCM imaging can facilitate the early diagnosis of Paget's disease.

ABSTRACT

Objective To investigate reflectance confocal microscopy (RCM) characteristics of childhood hypopigmented lichen striatus. Methods RCM was performed to image skin lesions and perilesional normal skin in 11 children with hypopigmented lichen striatus. Then, confocal microscopic findings of the skin lesions were compared with pathological findings. Results Histopathological examination of the skin lesions showed mild intercellular and intracellular edema, thickening of prickle cell layer to different extent, local liquefaction degeneration of basal cells, and perivascular infiltration of plenty of lymphocytes and a few melanophages in the superficial dermis.RCM horizontal images showed multifocal liquefaction degeneration of basal cells, which caused indistinct dermo-epidermal junction, incomplete or unclear dermal papillary rings, infiltration of many highly refractive melanophages and slightly to moderately refractive inflammatory cells in papillary and superficial dermis. Conclusion RCM characteristics can serve as strong evidences for the diagnosis and differential diagnosis of childhood hypopigmented lichen striatus.

ABSTRACT

Objective To investigate the clinical and reflectance confocal microscopy (RCM) characteristics of hypopigmentation induced by local glucocorticoid injection. Methods A retrospective study was carried out on 26 cases of hypopigmentation induced by local glucocorticoid injection. The clinical manifestations,RCM characteristics and treatment outcomes of hypopigmentation induced by local glucocorticoid injection were analyzed in these cases. Results Of the 26 patients,22 were female and 4 were male.Lesions were round or oval, pale white or white patches with obscure boundaries.Different degrees of localized skin atrophy occurred in 15 patients, and subcutaneous venous networks were observed in 8 of the 15 patients.RCM imaging of lesions in the 26 cases showed different degrees of epidermal thinning in the areas of hypopigmentation, shortening or disappearance of trochanterellus, and decreased number or disappearance of chromocytes in the basal layer, which were totally different from RCM characteristics of vitiligo lesions. Conclusion Hypopigmentation induced by local glucocorticoid injection is a kind of depigmentation disease complicated by skin atrophy with definite inducements, and RCM is of great value to its diagnosis.

ABSTRACT

Protective effects of Ginkgo biloba extract (GBE) on ischemia in rat and mice brain were studided. The ischemia model in brains was prepared by occlusion of bilateral carotid arteries and vertebra arteries in rat. The results demonstrated that GBE had no effect on electroencephalogram (EEG) of 5 min of ischemia, but it promoted the recovery of EEG during reperfusion. When it was reperfushed for 10 min, amplitude of EEG in the control group only recovered from 17. 2% after ischemia to 46. 6% of normal level. While 0. 2g/kg GBE has a recovery of 93% from normal level. At 4. 0g/kg GBE the life-span of mice with bilateral ligated carotid arteries was significantly lengthened. The above results proved that GBE has some protective effect on anoxia caused by acute ischemia in rat and mouse brain.
