ABSTRACT

Objective:To investigate the protective effect of dapagliflozin on cell damage and in HK-2 cells induced by high concentration of glucose.Methods:HK-2 cells were divided into four groups: control group (NC) , high glucose model group (HG) , dapagliflozin group (CS-179) and metformin group (PC) . After treatment with different drugs for 24 h, CCK-8 assay was applied to determine HK-2 cells viability; ROS, SOD, CAT and MDA levels were measured by a multi-detection reader; The protein expression of Nrf2 was determined by Western blot.Results:The results of CCK8 showed that the cell survival rate of the high glucose model group was 58.0%±0.8%, and that of the dapagliflozin group was 87.0%±0.4%. Dapagliflozin significantly increased the survival rate of HK-2 cells, and the results were statistically significant ( P<0.01) . The microplate reader test found that compared with the high glucose model group (ROS: 3.46 ± 0.05, MDA: 25.37 ± 0.61, SOD: 55.89 ± 4.09, CAT: 10.22 ± 1.67) , dapagliflozin reduced the accumulation of ROS (1.97±0.04) and MDA (9.5±0.4) caused by high glucose in HK-2 cells (both P<0.01) , increasing the vigor of SOD (114.95±4.19) and CAT (32.83±2.01) (both P<0.01) . Compared with the expression of Nrf2 protein in the high glucose model group (0.26±0.03) , the expression of Nrf2 protein (0.48±0.03) in dapagliflozin group was significantly increased ( P<0.01) . Conclusion:Dapagliflozin can alleviate the HK-2 cells damage induced by high concentration of glucose via reducing oxidative damage, and acti-vating Nrf2 antioxidant transcription factor.

ABSTRACT

Objective To investigate the effect of intensive insulin therapy in parenteral nutrition for patients with abdominal operation.Methods 50 patients with 1 week or more complete intestinal nutrition after abdominal surgery and parenteral nutrition stress hyperglycemia were selected and randomly divided into control group (25 cases) and treatment group (25 cases).The control group adopted conventional insulin treatment(10.0 ~11.1 mmol/L blood glucose target value),the treatment group adopted intensive insulin treatment (7.8 ~10.0 mmol/L blood glucose target).The blood glucose time,hospitalization time and cost were compared between the two groups,and the occurrence of complications was recorded.Results In the treatment group,the blood sugar control standard time was (5.16 ±1.25)d,hospitalization time was (7.69 ±2.14)d,which were shorter than those in the control group,the hospitalization expense in the treatment group was (1 045.16 ±114.17)yuan,which was lower than (2 217.18 ± 242.18)yuan of the control group,the differences were statistically significant(t =6.460,6.270,14.499,all P <0.05).The incidence rate of complications of the treatment group was 8.00%,which was significantly lower than 28.00% of the control group,the difference was statistically significant (χ2 =13.550,P <0.05).The mortality rate of the treatment group was 4.00%,which of the control group was 8.00%,the difference was not statistically signifi-cant between the two groups(χ2 =1.418,P =0.233).Conclusion Intensive insulin therapy can effectively control the blood glucose level in patients with non diabetes after abdominal operation,and can reduce the incidence of complications,it is worthy of recommending.

ABSTRACT

Objective To evaluate the effect of epalrestat combined with methylcobalamin on quality of life of patients with diabetic peripheral neuropathy.Methods 124 patients with diabetic peripheral neuropathy were randomly divided into the control group and the observation group according to the random number table, each group had 62 cases.The two groups were treated on the basis of the effective control of blood glucose and medication to control peripheral neuropathy, the control group used mecobalamin, the observation group used epalrestat combined with methylcobalamin, the two groups were treated continuously for 60 days.The effects of two main physiological changes, clinical efficacy and quality of life were observed.Results The total effective rate of the observation group was 95.2%,which was higher than 80.6% of the control group, the difference was statistically significant(x2=3.072,P<0.05).The fasting plasma glucose(FPG),postprandial 2 h plasma glucose(2 h PPG), glycosylated hemoglobin(HbA1c) were improved compared with before treatment, the differences were statistically significant(all P<0.05),which of the observation group improved significantly better than the control group, the differences were statistically significant(all P<0.05).The evaluation indicators of body pain(BP),social function(SF), role physiological(RP) and mental health(MH) had statistically significant differences compared with the control group (t=16.964,44.583,30.204,44.114,all P<0.05).Conclusion Using epalrestat combined with methylcobalamin tablets in the treatment of patients with diabetic peripheral neuropathy can significantly improve blood glucose levels, clinical symptoms, the quality of life of patients and it has great clinical significance.
