ABSTRACT

Teaching rounds is an important part of clinical teaching. It bright the theory guide to clinical practice. The classical Chinese medicine theory plays an essential role in teaching and clinical practice. During the CBL (case-based learning) teaching rounds of acupuncture and moxibustion, by selecting typical cases, in combination with directional problem settings, the classical Chinese medicine theory is run through the whole process to motivate students' initiative, improve teaching quality and promote inheritance and development of TCM.

ABSTRACT

Post-traumatic brain syndrome is a common disease in neurosurgery department, referring to a variety of neurological symptoms after brain traumas. Currently, research shows that this disease involves the fields of neurologic anatomy, pathology, trauma mechanics, neurobiochemistry, cerebral hemodynamics, etc, and the treatment majorly includes medication, hyperbaric oxygen, and psychotherapy, but they don't work so well. In primary hospitals, the convenient, low-cost and safe acupuncture-moxibustion therapy should be paid more attention. By retrieving literatures during the latest 10 years, this article has concluded that the acupuncture-moxibustion treatments for this disease covered acupuncture, auricular acupoint therapy, bloodletting therapy, and moxibustion, and the action mechanisms were mostly the improvement of cerebral circulation and cerebral metabolism, and the change of neurotransmitters and cytokines. The improvement of cerebral circulation and increase of cerebral blood flow can be taken as the future direction in the research on acupuncture-moxibustion for this disease. However, in the treatment of post-traumatic brain syndrome with acupuncture-moxibustion, it still requires the standardization of acupuncture and the objectification of evaluation indexes.

ABSTRACT

[Objective]Analyzing clinical experience and academic view of Pro. LIAN Jianwei for the treatment of eructation in order to completely learn and master his experience.[Method] Analyzing 4 belching cases of Pro. LIAN, and combining his therapeutic experience and view. [Result] According to the view of Pro. LIAN Jianwei, there are 4 common types in clinic, the type of accumulation in stomach and intestine, the type of spleen and stomach deficiency, the type of liver qi stagnation, the type of liver and spleen disharmony. Promoting digestion and harmonizing stomach to treat the type of food accumulation in stomach;benefiting qi and strengthening spleen to treat the type pf spleen and stomach qi deficiency;smoothing liver and regulating qi to treat type of liver and stomach disharmony; regulating liver and spleen to treat type of liver and spleen disharmony;eliminating damp to treat type of spleen deficiency with damp;activating blood to treat stasis caused by prolonged diseases. Prescriptions of Pro. LIANbased on methods which changed with syndromes. He insisted that treatments base on differentiation;prescriptions base on methods, which make effect well. [Conclusion] It is worthy for our students to learn and imitate clinical experience and academic view of Pro. LIAN Jianwei for the treatment of eructation.

[Objective]To improve the teaching quality ofMeridians and Acupoints. [Methods]Based on the ten-year experience of teaching the course ofMeridians and Acupoints, the authors suggest three issues on how to study well with this subject. [Results] The first, using the comparing method to remember meridians and acupoints, always do analysis and summary are all important ways to learn the basic knowledge.The second, understanding the property of acupoints, make prescription of acupoints like the way of making Chinese herbal formula according to syndrome differentiation, can cultivate students' dialectical thinking and improve the ability of diagnosis and treatment in acupuncture clinic.The third, introducing the origin,history and development of each meridian and some special acupoints. [Conclusion]The above three suggestions can help students to learn better about the science of acupuncture,and improve their professional thinking, and mine the potential ability of innovation.

ABSTRACT

Objective To observe the clinical efficacy of acupuncture at the six mind-calming points plus the four gate points in treating migraine.Method Totally 128 eligible migraine patients were randomized into a treatment group and a control group, 64 cases in each group. The treatment group was intervened by acupuncture at the six mind-calming points plus the four gate points, while the control group was by ordinary acupuncture. The Numerical Rating Scale (NRS), headache attack frequency, headache duration, headache intensity, complicated symptoms, Self-rating Depression Scale (SDS), and Self-rating Anxiety Scale (SAS) were observed before and after treatment and in the follow-up study.Result The NRS scores and the headache symptoms and functions scores were significantly changed respectively after 2-week and 4-week treatment in both groups (P<0.01,P<0.05). The NRS score, headache duration, and headache intensity in the treatment group were significantly different from that in the control group after 4-week treatment and in the follow-up study (P<0.05,P<0.01). The SDS and SAS scores were significantly changed after 2-week and 4-week treatments as well as in the follow-up study in the treatment group (P<0.01). The SDS and SAS scores were significantly changed after 4-week treatment in the control group (P<0.01). There were significant differences in comparing the SDS and SAS scores between the two groups after 2-week and 4-week treatment as well as in the follow-up study (P<0.05,P<0.01). In the treatment group, the SDS and SAS scores in the follow-up study were significantly different from that after 2-week treatment (P<0.01).Conclusion Acupuncture at the six mind-calming points plus the four gate points is an effective method in treating migraine, and it can reduce headache intensity, and improve the anxiety and depression state.

ABSTRACT

Objective To observe blood glucose values and serum insulin levels at different times after acupoint injection of insulin with PVP as a sustained release agent and analyze the correlation between them in SD Rats.Methods One hundred and forty-four male SD rats were randomized into groups A (acupoint injection of insulin alone), B (acupoint injection of sustained-release insulin) and C (control). At one hour after every group received an oral gavage of 40% glucose solution (2.2 g/kg), groups A and B were given point Zusanli injection and group C was not treated. In the three groups of rats, blood glucose values and serum insulin levels were measured at six time points: 0.5, 3, 6, 8, 10 and 24 hours after acupoint injection (in group C at the corresponding time points). In every group, the reaction of Point Zusanli region to the stimulation was observed by optical microscopy in at three time points: 3, 10 and 24 hours after acupoint injection.Results The glucose-lowering rate reached its peak at 3 hours after acupoint injection and then declined in groups A and B. The hypoglycemic effect tended to be stable at 6 hours after acupoint injection in group A and at 10 hours after acupoint injection in group B. The hypoglycemic effect was better in group B than in group A at 6 and 8 hours after acupoint injection (P<0.01). In group A, serum insulin reached its peak at 3 hours and tended to be stable at 6 hours after acupoint injection (P<0.05). Serum insulin levels were significantly lower in group B than in group A at four time pints: 0.5, 3, 6 and 8 hours after acupoint injection (P<0.05). An analysis of the correlation between rat serum insulin levels and the glucose-lowering rate showed a significant positive correlation in group A (P<0.01) and a low positive correlation in group B (P<0.05). In groups A and B of rats, clear stripes and complete structure of muscle fibers were seen with no obvious degeneration, necrosis and inflammatory reaction around point Zusanli at three time points: 3, 8 and 24 hours after acupoint injection; there were no obvious pathological changes compared with group C.Conclusions PVP as a sustained release agent can slow down the absorption rate of insulin in rat point Zusanli region. Meanwhile, it prolongs the continuous time of the hypoglycemic effect of insulin. There is a low correlation between serum insulin levels and the hypoglycemic effect, suggesting that acupoint injection with a sustained release agent can prolong the effect of acupoint injection of medicine.

ABSTRACT

<p><b>OBJECTIVE</b>To evaluate the clinical efficacy of warming-needle moxibustion on cervical headache.</p><p><b>METHODS</b>Sixty-one patients of cervical headache were randomly divided into a warming-needle moxibustion group (30 cases) and an acupuncture group (31 cases). Tianzhu (BL 10), Fengchi (GB 20), Wangu (GB 12), Tianyou (TE 16), Hegu (LI 4), Taichong (LR 3) and Zulinqi (GB 41) were selected in the two groups. Patients in the acupuncture group were treated with acupuncture; patients in the warming-needle moxibustion group were treated with warming-needle moxibustion at Tianzhu (BL 10), Fengchi (GB 20) and Wangu (GB 12) and acupuncture at the remaining acupoints. The treatment was given once every other day, 3 times a week, and totally 4-week treatment was given. The duration and frequency of headache, numerical rating scale (NRS) of headache and cervical range of motion (ROM) were compared before and after treatment in the two groups; also the efficacy of the two groups was evaluated.</p><p><b>RESULTS</b>After treatment, the duration and frequency of headache, headache NRS.and cervical ROM score were improved in both groups (all P < 0.05), which were more significant in the warming-needle moxibustion group (all P < 0.05). The total effective rate was 84.3% (25/30) in the warming-needle moxibustion group, which was significantly superior to 61.3% (19/31) in the acupuncture group (P < 0.05).</p><p><b>CONCLUSION</b>The warming-needle moxibustion presents significant efficacy on cervical headache, which can obviously improve headache symptoms and cervical ROM.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To compare the difference in the short-term and long-term efficacy on cervical spondylosis of neck type between warm needling therapy in the regions of both neck and lumbus and that only in the region of neck.</p><p><b>METHODS</b>Eighty-one patients of cervical spondylosis of neck type were randomized into group A (41 cases) and group B (40 cases), in which 2 cases dropped out. Finally, 40 cases in the group A and 39 cases in the group B accomplished the trial. In the group A, the warm needling therapy was applied to the acupoints in the region of neck and the lumbus. Fengchi (GB 20), Tianzhu (BL 10), Neck-Bailao (EX-HN 15), Wangu (GB 12), Tianyou (TE 16) and(including the tender points and code-like masses on palpation) were selected in the region of neck. Dachangshu (BL 25), Qihaishu (BL 24) and Jiaji (EX-B 2) of L5 were selected in the region of lumbus. The warm needling was applied to Fengchi (GB 20), Tianzhu (BL 10), Dachangshu (BL 25). In the group B, the warm needling therapy was applied only to the acupoints in the neck, which were same as the group A. The treatment was given once every two days, three times a week in the two groups. Separately, before treatment, 1 week after treatment, at the end of 2-week treatment and at the end of 1 month follow-up, the score of neck pain questionnaire (NPQ), the score of range of motion (ROM) in the cervical region and the score of the cervical symptoms were recorded. The efficacy at the end of treatment and in the follow-up was evaluated.</p><p><b>RESULTS</b>Compared with those before treatment, the scores at all the observation time points were significantly improved in the two groups after treatment (all<0.05). In the follow-up, NPQ score, ROM score and the score of cervicalsymptoms were different significantly between the two groups (all<0.05). The results in the group A were better than those in the group B. At the end of 2-week treatment, the total effective rate was 92.5% (37/40) in the group A and was 87.2% (34/39) in the group B (>0.05). In the follow-up, the total effective rate was 87.5% (35/40) in the group A, better than 64.1% (25/39) in the group B (<0.05).</p><p><b>CONCLUSIONS</b>The treatment for both neck and lumbar regions with warm needling therapy and the treatment in the local area all achieve the short-term efficacy on cervical spondylosis of neck type. For the long-term efficacy, the treatment for both neck and lumbar regions achieves the better result as compared with the routine treatment in the region of neck.</p>

ABSTRACT

Objective] To analyze the effect of acupuncture treatment on regulating patients’spirits who accepted IVF-ET surgery, in order to provide a new train of thought for clinical treatment. [Methods] Based on emotional factors, analyze the condition of patients accepting IVF-ET surgery. Explaining that spirit is also an important factor, can not be ignored during pregnancy and expounding the theory and prescription of acupuncture treatment on regulating spirits in patients with infertility, and in the clinical cases, patients undergoing IVF-ET can get active treatment by regulating spirits.[Results] Acupuncture treatment on regulating spirits to remove patients’depression and anxiety by regulating qi and blood, also nourishing heart and tranquilizing mind, so, this treatment can create conditions for conception. The clinical case cited also achieved good results. [Conclusions] This treatment,as a natural non-drug therapy,can be considered as a new train of thought in clinical treatment of patients with infertility after IVF-ET surgery and is worthy of research.

ABSTRACT

Objective To observe the effect of electroacupuncture (EA) pretreatment of different durations on the expressions of AQP4 and CLN5 in the blood-brain barrier after cerebral ischemia and reperfusion Methods Sixty-four male Sprague-Dawley (SD) rats were randomly divided into a model group,a sham operation group,a 7-day EA-pretreatment group (EAP-7 group) and a 15-day EA-pretreatment group (EAP-15 group),each of 16.After EA-pretreatment on the baihui and shuigou acupoints for 7 days or 15 days,a model of unilateral middle cerebral artery embolism (MCAO) model was induced in the rats of the model,EAP-7 and EAP-15 group using a modified Longa method.In the sham operation group the carotid artery was separated without middle cerebral artery embolism.The reperfusion was begun 90 min after the MCAO modeling.Immunohistochemical methods and fluorescence quantitative PCR were applied to detect the expression of AQP4,CLN5 and their mRNAs expression in the blood brain barrier (BBB) 24h after the operation.Results Compared with the sham operation group,the expression of AQP4 positive cells and AQP4 mRNA in the BBB in the other three groups had increased significantly,while the expression of CLN5 positive cells and CLN5 mRNA was significantly less.Compared with the model group,the expression of AQP4 positive cells and AQP4 mRNA in the EAP-7 and EAP-15 groups was significantly reduced,while the expression of CLN5 positive cells and CLN5 mRNA was significantly increased.Moreover,the expression of AQP4 positive cells and AQP4 mRNA in the EAP-15 group were significantly higher than in the EAP-7 group,while the expression of CLN5 positive cells and CLN5 mRNA were significantly lower.Conclusions EA-pretreatment on the baihui and shuigou acupoints can restrain the expression of AQP4-positive cells and AQP4 mRNA and promote that of CLN5-positive cells and CLN5 mRNA in the BBB after cerebral ischemia and reperfusion,at least in rats.The BBB protection effect is better when the EA-pretreatment lasts longer.The mechanisms of cerebral ischemia tolerance may be related to the regulation of AQP4,CLN5 and their mRNAs in the blood-brain barrier after cerebral injury.

ABSTRACT

The problem based teaching (PBT) has been the main approach to the training in the universities o the world. Combined with the team oriented learning method, PBT will become the method available to the education in medical universities. In the paper, based on the common questions in teaching Jingluo Shuxue Xue (Science of Meridian and Acupoint), the concepts and characters of PBT and the team oriented learning method were analyzed. The implementation steps of PBT were set up in reference to the team oriented learning method. By quoting the original text in Beiji Qianjin Yaofang (Essential recipes for emergent use worth a thousand gold), the case analysis on "the thirteen devil points" was established with PBT.

ABSTRACT

ObjectiveTo investigate the impactsof different time-course electroacupuncture pretreatments on brain water content and blood-brain barrier permeability and assess their protecting effects on blood-brain barrier functionin cerebral ischemia rats. MethodClean-grade healthy male SD rats were randomly allocated to five groups (n=48): model control group, and one-day, three-day, seven-day and fifteen-day electroacupuncture pretreatment groups. A rat model of unilateral middle cerebral artery occlusion (MCAO) was made after electroacupuncture pretreatment was finished in various electroacupuncture groups. The water content of the brain and the amount of EB through blood brain barrier were measured at 12, 24 and 48 hrs after cerebral ischemia. ResultBoth brain water content and EB permeation amount were lower at 12 and 48 hrs than at 24 hrs after cerebral ischemia (P<0.05). There was a significant reducing effect on brain water content and EB permeation amount in the fifteen-day electroacupuncture pretreatment group at various time points after cerebral ischemia (P<0.05,P<0.01,P<0.001).Conclusion The blood-brain barrier is impaired most severely at 24 hrs of the three time points 12, 24 and 48 hrs after cerebral ischemia. Electroacupuncture pretreatment can produce a protecting effect on blood-brain barrier function in cerebral ischemia rats by markedly reducing cerebral edema and blood-brain barrier permeability.

ABSTRACT

Objective To observe the effect of nerve growth factor(NGF)and electro-acupuncture(EA) on learning and memory in rats with cerebral ischemia.Methods An experimental model of cerebral ischemiareperfusion was established in 40 rats,who were then randomly divided into a model group,a sham operated group,a NGF group,an acupuncture group and a NGF plus acupuncture group.Three weeks were allowed for the recovery of the blood-brain barrier lesion caused by ischemia-reperfusion before intravenous injection of NGF(10μg/kg,once daily for 15 days)via the vena caudalis.In the two acupuncture groups,this was administered in combination with EA at the Baihui(DU20)and Yamen(DU15)acupoints(frequency 100 Hz,2 mA,20 min/time,once daily for 15 days).Morris' water maze test was used to assess any changes in spatial learning and memory abilities.Results The ischemic rats showed significant learning and memory disorders compared with the sham-operated group.In the NGF group,the rats' learning and memory abilities did not impmve significantly compared with the model group and the EA group.Significantly better learning was observed in the NGF+EA group compared with the model group,the EA group and the NGF group.Conclusion NGF combined with EA can improve the learning and memory abilities of rats with learning and memory dysfunction caused by cerebral ischemia-repedusion.

ABSTRACT

Aim To investigate glutamate-induced [ Ca2 + ] i changes in cultured rat neonatal cortical astrocytes after hypoxia/reoxygenation, H2O2 or high concentration of L-glutamate injury. In the meantime, the effects of Gingko biloba extract (GbE) were examined. Methods [ Ca2+ ]i changes in astrocytes were monitored by laser scanning confocal microscopy with the Ca2+ sensitive fluorescent probe [ Ca2 + ] i, but decrease by ( 3.3 ± 1.6) %, (81 ± 11 ) % and ( 81 ± 7 ) %, respectively. Pretreatment with H2O2 or high concentration of L-glutamate injury were ( 135 ± 98 ) %, ( 117 ± 93 ) % and ( 89 ± 36) %,different injury. Conclusion Hypoxia/reoxygenation, H2O2 and high concentration of L-glutamate impaired astrocytes' response to exogenous L-glutamate, and then bidirectional communication between astrocytes and neurons could not take place. GbE could improve the abnormal responses and maintain the normal function of astroglical network. These effects support that GbE has potential beneficial actions against brain injury.

ABSTRACT

The effects of Arecoline (Are) on calcium mobilization were investigated. In isolated single ventricular myocyte of guinea pig, patch clamp whole cell recording techniques were used to record the current of L-type calcium channel and cytosolic Ca2+ level ([Ca2+]i) labeled with fluorescence probe Fluo-3/AM was measured under a laser scanning confocal microscope. Results revealed that Are (3-100 mumol/L) could inhibit L-type calcium current in a concentration-dependent manner and the value of IC50 was 33.73 mumol/L (n = 5). In the absence of extracellular calcium, the resting levels of [Ca2+]i was not affected by Are (n = 6, P > 0.05), but pretreatment with Are (30 mumol/L) could significantly inhibit the [Ca2+]i elevation induced by caffeine (10 mmol/L, n = 6, P < 0.01). It was concluded that Are could inhibit not only calcium influx through L-type calcium channel but also calcium release from sarcoplasmic reticulum.

ABSTRACT

The use-dependent characteristics of daurisoline on monophasic action potentials (MAP) of the left ventricle of the rabbit heart in situ were recorded with MAP recording technique. The results showed that daurisoline decreased monophasic action potential amplitude (MAPA) and prolonged monophasic action potential duration at the 50% and 90% (MAPD50, MAPD90), effective refractory period (ERP), ERP/MAPD90 at the steady-state cycle lengths of 240, 210, 180 ms. The percent of prolongation of MAPD50 was 12.8±2.9, 9.5±2.6, 9.0±1.6, that of MAPD90 was 10.9±2.6, 7.5±2.4, 6.5±2.7, that of ERP was 25.7±4.3, 18.5±4.1, 24.2±7.2, and that of ERP/MAPD90 was 13.7±4.5, 10.2±2.2, 16.1±5.1, respectively. Its effect showed no relation to frequency (n=6, P>0.05). The effects of sotalol on MAP were similar to that of daurisoline, but it prolonged MAPD90 and ERP in a reverse use-dependent manner (P<0.05). It is concluded that daurisoline has no use-dependent effects on MAP of rabbit hearts.
