ABSTRACT

Objective To investigate the predictive value of serum oncogene[proliferation-related gene(C-myc),transformation gene(N-ras),silk/threonine kinase 1(PLK1),fibroblast growth factor 2(FGF2)]protein levels in patients with hepatitis B associated hepatocellular carcinoma(HCC)after hepatic arterial chemoem-bolization(TACE).Methods A total of 127 patients with hepatitis B-associated hepatocellular carcinoma ad-mitted to a hospital from July 2016 to January 2021 were selected and divided into death group and survival group according to the follow-up results. The serum oncogene C-myc,N-ras,PLK1 and FGF2 protein levels were determined by double-antibody sandwich enzyme-linked immunosorbent assay.Univariate and multivari-ate Cox analysis were used to analyze the risk factors of serum oncogene C-myc,N-ras,PLK1 and FGF2 pro-tein levels in patients with hepatitis B-associated hepatocellular carcinoma after TACE.The receiver operating characteristic curve was used to evaluate the prognostic value of the serum oncogene C-myc,N-ras,PLK1 and FGF2 protein levels, and the patients were divided into high expression group and low expression group ac-cording to the corresponding cutoff value.Kaplan-Meier survival curve was used to evaluate the prognosis of different serum oncogene C-myc,N-ras,PLK1 and FGF2 protein level. Results Multivariate Cox regression a-nalysis indicated that TNM stage Ⅲ to Ⅳ(HR=2.998,95%CI:1.239-7.257),portal vein metastasis(HR=3.737,95%CI:1.941-7.193),abdominal metastasis(HR=3.482,95%CI:1.709-7.097),Child-Pugh grade B(HR=2.587,95%CI:1.045-6.406),high serum oncogene C-myc protein level(HR=1.224,95%CI:1.090-1.374),high serum oncogene N-ras protein level(HR=1.218,95%CI:1.097-1.353),high serum oncogene PLK1 protein level(HR=1.237,95%CI:1.110-1.379)and high serum oncogene FGF2 protein level(HR=1.141,95%CI:1.060-1.228)were independent risk factors for the prognosis of hepatitis B-asso-ciated hepatocellular carcinoma patients after TACE(all P<0.05).The overall survival rate of low expression group of serum oncogene C-myc,N-ras,PLK1,FGF2 protein level was significantly higher than that of high expression group of serum oncogene C-myc,N-ras,PLK1,FGF2 protein level, the difference was statistically significant(all P<0.001).Conclusion Serum oncogene C-myc,N-ras,PLK1,FGF2 protein levels have predic-tive value for the prognosis of patients with HBV-related liver cancer after TACE.

ABSTRACT

Objective To analyze the expression of Kruppel-like factor 4 (KLF4 ) and CD44 in esophageal squamous cell carcinoma (ESCC) tissues and adjacent non-cancerous tissues ,and to investigate their effects on the prognosis .Methods From June 2012 to September 2016 ,in The Second People′s Hospital of Nanyang ,a total of 100 patients with ESCC who receiving operation were selected .The ESCC tissues and the adjacent non-cancerous tissues (control) of the patients were collected .The expression levels of KLF4 and CD44 protein were detected by immunohistochemistry .The expressions of KLF4 and CD44 at mRNA and protein level of 50 paired fresh tissues were examined by quantitative real-time polymerase chain reaction (qRT-PCR) and Western blotting ,respectively . T-test ,chi-square ,Kaplan-Meier method and Pearson correlation analysis were performed for statistical analysis .Results The positive expression rate of KLF4 protein in ESCC tissues was 55% (55/100) ,which was lower than that in adjacent non-cancerous tissues (77% ,77/100) ,and the difference was statistically significant (χ2 =10 .778 ,P=0 .001) .The positive expression rate of CD44 protein in ESCC tissues was 81% (81/100) ,which was higher than that in adjacent non-cancerous tissues (11% ,11/100) ,and the difference was statistically significant (χ2=112 .600 ,P<0 .01) .The expression level of KLF4 mRNA in 43 cases was lower than that in adjacent non-cancerous tissues ,the expression level of CD44 mRNA in 46 cases was much higher than that of adjacent non-cancerous tissues ,and the differences were statistically significant (χ2 =51 .837 and 70 .563 ,both P< 0 .01) .There were statistically significant differences in positive expression rates of KLF4 in cancer tissues between different gender , differentiation degree , invasion depth ,TNM stage and lymph node metastasis (all P<0 .05) .Similarly there were statistically significant differences in positive expression rates of CD 44 in cancer tissues between different differentiation degree ,invasion depth ,TNM stage and lymph node metastasis (all P< 0 .05) .The expression of KLF4 was negatively correlated with CD44 expression ,either at protein level or mRNA level (r= -0 .284、-0 .518 ,both P< 0 .01) .The median survival time of patients with positive KLF4 expression in cancer tissues was 33 months ,which was longer than that of patients with negative KLF4 expression (20 months) ,and the difference was statistically significant (χ2 =4 .021 , P= 0 .019) .The median survival time of patients with positive CD44 expression in cancer tissues was 24 months ,which was shorter than that of patients with negative CD44 expression (37 months) , and the difference was statistically significant (χ2 = 4 .379 , P= 0 .016) .The results of univariate analysis showed that TNM stage ,KLF4 expression and CD44 expression were correlated with overall survival time (all P<0 .05) . The results of multivariate analysis indicated that TNM stage ,lower KLF4 expression and higher CD44 expression were the independent risk factors for survival (all P<0 .05) .Conclusion Lower expression of KLF4 and higher expression of CD44 in ESCC may be closely correlated with its occurrence ,development and prognosis .

ABSTRACT

Objective To evaluate the effect of concomitantdifferent regimens chemotherapy and radiotherapy for inoperable stage Ⅲ non small cell lung cancer (NSCLC). Methods From September 1995 to December 1998, 62 patients with inoperable stage Ⅲ NSCLC were randomized into two groups. Twenty nine patients received paclitaxel 30 mg and cisplatin 30 mg weekly for 5~6 weeks (paclitaxel group), and 33 patients received etoposide (Vp 16) 100 mg and cisplatin 30 mg weekly for 5~6 weeks (VP 16 group). All patients received concomitant radiotherapy as well. Radiotherapy was given with conventional fraction in 2Gy per fraction and five fractions per week. The total tumor doses were 60~70 Gy. Treatment fields covered clinical tumor and lymph node involved. Results The overall response (CR+PR) rate in paclitaxel group was 82.8% with a complete response (CR) rate of 10.3%. The overall response rate in the VP 16 group was 54.6% with a CR rate of 18%. The difference of overall response rate between the two groups was statistically significant (P0.05). The major toxic effects of chemotherapy were gastrointestinal tract reaction and myelosuppression. Conclusions Concomitant chemotherapy of paclitaxel plus cisplatin and radiotherapy for inoperable stage Ⅲ NSCLC is acceptable, and its efficacy is superior to cisplatin plus etoposide combined with radiotherapy.
