ABSTRACT

BACKGROUND@#Conventional treatment has limited efficacy in relapsed/refractory B-cell lymphoma. Since chimeric antigen receptor T-cell (CAR-T) technology has shown high safety and results in high remission rates, we investigated its efficacy and safety in B-cell lymphoma treatment and analyzed potential affecting factors to provide evidence for therapeutic strategies and applications.@*METHODS@#We searched databases including PubMed, Embase, and Cochrane up to July 2019. Meta-analysis 1 was conducted to study the efficacy of CAR-T cell for treating B-cell lymphoma, measuring the response rate and complete remission rate as outcomes. Sub-group analysis was performed for age, pathological type, target antigen, co-stimulatory molecule, and conditioning chemotherapy. Meta-analysis 2 was undertaken on the safety of the treatment with the incidence rate of toxicity (cytokine-releasing syndrome [CRS], neurotoxicity) as an outcome.@*RESULTS@#Seventeen studies were included in the systematic review and meta-analysis. It was found that CAR-T cells had good therapeutic effects in the following cases: B-cell lymphoma (patients ≥65 years old); diffuse large B-cell lymphoma pathological type; patients with treatment target antigen other than CD19; patients treated with co-stimulatory molecules other than CD28, including 4-1BB+CD28 or 4-1BB; and patients treated with cyclophosphamide/fludarabine pre-treatment protocol conditioning chemotherapy. Although the CRS and neurotoxicity incidences were high, most were reversible with minimal risk of death.@*CONCLUSION@#CAR-T cell treatment is safe for clinical application; however, toxicity effects should be monitored.

ABSTRACT

5-Hydroxytryptamine 2C (5-HT) receptor is one of the major targets of anti-obesity agents, due to its role in regulation of appetite. In the present study, the 70% EtOH extract of the roots of Bupleurum chinense was revealed to have agonistic activity on 5-HT receptor, and the subsequent bioassay-guided isolation led to identification of several saikosaponins as the active constituents with 5-HT receptor agonistic activity in vitro and anti-obesity activity in vivo. The new compound, 22-oxosaikosaponin d (1), was determined by extensive spectroscopic analyses (HR-ESI-MS, IR, and 1D and 2D NMR). The primary structure-activity relationship study suggested that the intramolecular ether bond between C-13 and C-28 and the number of sugars at C-3 position were closely related to the 5-HT receptor agonistic activity. Saikosaponin a (3), the main saponin in B. chinense, showed obviously agonistic activity on 5-HT receptor with an EC value of 21.08 ± 0.33 μmol·Lin vitro and could reduce food intake by 39.1% and 69.2%, and weight gain by 13.6% and 16.4%, respectively, at 3.0 and 6.0 mg·kgin vivo. This investigation provided valuable information for the potential use of B. chinense as anti-obesity agent.

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the clinical efficacy of cryotherapy ablation treatment for advanced hepatocellular carcinoma, and to analyse the predictive factors of cryotherapy ablation treatment.</p><p><b>METHODS</b>190 patients of hepatitis B-related advanced HCC from 2005 to 2008 in our hospital underwent curative cryoablation. We used clinical cohort method to analyze cryoablation group (147 cases) and control group (43 cases). The median OS (over survival time) and TTP (time to disease progression) were compared. We also evaluated the clinical significance of age, gender, location of portal vein tumor thrombus, HBeAg, tumor histological grade, Child-Pugh classification, end-stage liver disease (MELD) score, advanced liver cancer prediction system (ALCPS) score and the Eastern Cooperative Oncology Group performance status (ECOG PS) score for predicting the efficacy of cryoablation. Two Groups were compared with the x² test. Survival rates were estimated by the Kaplan-Meier method and compared by the log rank test. The Cox proportional hazards model was used to determine the independent factors on survival based on the variables selected in univariate analysis.</p><p><b>RESULTS</b>Median survival time of cryoablation group and Control group were 7.5 (4.2 to 14.6) months and 3.2 (1.2 to 8.6) months, median TTP were 3.5 (2.5 to 4.5) months and 1.5 (1.0 to 3.5 months), the differences between were statistically significant (P < 0.05). Median OS and TTP of advanced HCC patients who had Well-differentiated tumor, Child-pugh A-class and low score of MELD score, ALCPS score; ECOG PS score were significantly longer than that of the poorly differentiated tumor, Child-pugh B-class and the high those scores (P < 0.05). ECOG PS (P less than 0.05, 95% CI 1.074 - 2.143) and ALCPS (P < 0.05, 95% CI 1.005-2.121) were independent predictors for OS of advanced HCC.</p><p><b>CONCLUSIONS</b>Cryoablation treatment can prolong median OS and TTP of advanced HCC. ECOG PS and ALCPS are important predictors for survival time of advanced HCC.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To observe the effect of scalp-acupuncture drawing method (SADM) on recovering motor function in hemiplegic patients following cerebral thrombosis at convalescent period.</p><p><b>METHODS</b>Adopting randomized, single-blinded, controlled method, 93 patients suffering from hemiplegia following cerebral thrombosis were randomly assigned to 3 groups, 31 in each group. All patients were treated based on neurological routine therapy, those in the treated group and the control group I were treated respectively with SADM and scalp-acupuncture twirling method (SATM) additionally, but for those in the control group II, no additional treatment was given. The treatment course for all was 4 weeks.</p><p><b>RESULTS</b>Before treatment, there were insignificant difference among 3 groups in the score of neural motor function deficits (NFDS) of limbs and activity of daily life (ADL) score (P > 0.05). After being treated for 4 weeks, the NFDS was significantly lower and in the control group II (P < 0.01, P < 0.05) and the change of the scores in the treated group was more than that in the control group I (P < 0.05).</p><p><b>CONCLUSION</b>SADM was superior to scalp-acupuncture twisting prove the ADL score and motor function of hemiplegia patients following cerebral thrombosis in the convalescent stage.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To estimate the size of men who have sex with men (MSM) in Shanghai.</p><p><b>METHODS</b>Multiplier method was used in the study and two popular Shanghai-based MSM websites were selected as targeted institutions. The number of MSM (r) who visited website during a given period was investigated by a web-based questionnaire. Meanwhile, a survey was conducted among MSM living in Shanghai to obtain the proportion of MSM who had visited the websites during the given period, the reciprocal number of this proportion was the multiplier (m), therefore, population size can be estimated by r multiplied by m.</p><p><b>RESULTS</b>The MSM population was estimated as 398 433 when website A was selected as target institution and 370 755 was estimated when website B was selected. The estimated population of MSM accounted for 7.1% and 6.6% of male population aged 15 to 49 years old in Shanghai, respectively.</p><p><b>CONCLUSION</b>It was feasible to use multiplier method which selecting MSM website as target institution to estimate the size of MSM population, however, the representativeness of this study sample should be considered seriously.</p>

ABSTRACT

OBJECTIVE@#To discuss forensic identification of floating shoulder injury (FSI).@*METHODS@#To analyze fifteen cases of FSI which were accepted from Jan. 1993 to Jan. 2006, including 15 shoulder neck fracture, 13 clavide stem fracture and 2 distal end clavide fracture, the function of shoulder joint was evaluated six months after injure considering the following three aspects: result of forensic examination such as X-ray photograph, CT and MRI, the injurers' symptom, objective sign and joint function, shoulder joint territory, degree of pain and local muscle power.@*RESULTS@#Basing on the curative effect standard of Herscovic, all cases were divided into good. Modest, worst, which included 2, 4, 9 cases respectively; referring the standard of GA35-92, GB18667-2002, all cases were divided into six, seven, eight, nine and ten degree, which included 2,9,2,1,1 cases respectively.@*CONCLUSION@#As a special powerful injure, FSI always companied with concurrent and multiple injure, and characterized by missed, incorrect and delayed diagnosis and infelicitous treatment, which lead to the high frequency and degree of injure. To prevent missed and incorrect forensic identification, we should have a full realization of the particularity of FSI, and evaluate the function of shoulder all-sidely, objectively and synseticaly.
