ABSTRACT

AIM: To compare the control effects between toric-designed orthokeratology and spherical orthokeratology on adolescents with moderate-to-low myopia.METHODS: The clinical data of 169 adolescents(290 eyes)with moderate-to-low myopia in Jiayuan Outpatient Department of Shanghai Demu Ophthalmology from July 2020 to June 2021 were analyzed retrospectively. The patients were divided into toric group and spherical group according to the type of orthokeratology, with 81 cases(135 eyes)and 88 cases(155 eyes)respectively. The changes of visual acuity and ocular axis before and after treatment were recorded to evaluate the therapeutic effect.RESULTS: The uncorrected visual acuity of both groups significantly improved at 1a after treatment(P<0.01), and the axial length increased compared to that before treatment(P<0.01). But there were no significant differences in uncorrected visual acuity(0.014±0.043, 0.017±0.047LogMAR)and axial growth(0.18±0.22, 0.19±0.22mm)between the two groups(P>0.05).CONCLUSION: Both toric-designed orthokeratology and spherical orthokeratology can improve the uncorrected visual acuity of adolescents with low-to-moderate myopia, and there is no significant difference in controlling effect on myopia.

ABSTRACT

Based on the systematic summary of the results of the fourth general survey of traditional Chinese medicine resources, the cultivation of large varieties of Chinese material medica and the latest research on health industrial development, the novel concepts and scientific connotations of generalized science of Chinese material medica are put forward, and the basic ideas and methods of a new Chinese medicine academic system, the cultivation system of large varieties of Chinese medicinal materials and the application system of the large health industry are constructed. This kind of generalized science of Chinese material medica, rooted in the traditional Chinese culture and the theory of "preventive treatment of disease", can avoid the narrow prospect induced by the increasing specialization and refinement of knowledge of science of Chinese material medica. It will play an important role in the modernization, industrialization, internationalization of traditional Chinese medicine.

ABSTRACT

An electrochemical analysis system for rapid determination of chemical oxygen demand ( COD) in flow state was established. A planar electrode modified with GO-NiNPs was matched with a 3D printed thin-layer cell. The sample was driven smoothly through the electrode surface by a micro peristaltic pump and then measured by chronoamperometry. The effect of modified materials, dielectric and electrochemical operating conditions were investigated. The whole response time of COD was 1. 5 min and the demand for the sample was about 2 mL. It turned out that the linear range of response in the low concentration region was 0. 15-100 mg/L, the linear equation was i(μA)=3. 974c (mg/L)+0. 2295 (r = 0. 9991) and the detection limit was 0. 04 mg/L. The linear response range in the high concentration region was 100-450 mg/L, and the linear equation was i(μA)=1. 938c (mg/L)+ 230. 9 (r = 0. 9877). Compared with the national standard method (GB11914-89) for measuring the actual water samples (Qinhuai River, Xuanwu Lake and Nanjing tap water), the correlation between them was quite good and the analysis time was dropped to 1/100. This new sensing system provided an environmentally friendly and portable method for detection of COD without using expensive, highly corrosive and toxic reagents.

ABSTRACT

<p><b>OBJECTIVE</b>To explore the type and distribution of thalassemia gene mutation in Longyan area of Fujian province in China, so as to provide a evidence for prenatal diagnosis and to reduce birth defects.</p><p><b>METHODS</b>The mean corpuscular volume (MCV), mean corpuscular hemoglobin (MCH) and hemoglobin electrophoresis were used for screened the mutation types of thalassemia. Genotyping of the screened positive sample was performed by gap single polymerase chain reaction (gap-PCR) and reverse dot blot hybridization (RDB).</p><p><b>RESULTS</b>Out of 7823 cases of routine positive blood test, 2826 cases were positive (36.12%) by using hemoglobin electrophoresis; 1905 out of 2710 cases were diagnosed as Mediterranean anemia by genetic test, with 24.35% of carrying rate; 1225 cases were positive alpha thalassaemia and the carrying rate was 15.66%, their major genetic types were --/αα,-α/αα,-α/--and -α/αα, with carrying rate of 12.91%, 1.28%, 0.51% and 0.74%, respectively; 632 cases were positive β thalassaemia, with carrying rate of 8.08%, the major genotypes were 654M/N,41-42M/N,17M/N,-28M/N and 27-28M/N and with carrying rate of 3.66%, 2.22%, 0.78%, 0.66% and 0.45%, respectively; 48 cases were diagnosed as both α- and β-thalassemia, with the carrying rate of 0.61%.</p><p><b>CONCLUSION</b>The main gene mutation types of α- and β-thalassemia in Longyan area of Fujian Province in China were --/aa and 654 M/N. As thalassemia gene mutation prevalents in Fujian, the screening of thalassemia genotypes for childbearing age woman has great significance for raising population quality.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To retrospectively analyze the effect of restrictive fluid management strategy (RFMS) on the early pulmonary function and the prognosis of patients with extremely severe and extensive burn.</p><p><b>METHODS</b>Thirteen patients with extremely severe burn hospitalized from June 2010 to November 2011, being treated with RFMS in the fluid reabsorption stage, were enrolled as treatment group. Twenty-six patients with extremely severe burn hospitalized from March 2008 to November 2011, being treated with normal fluid therapy in the fluid reabsorption stage, were enrolled as control group. The match proportion between treatment group and control group was 1:2. Fluid intake, fluid output, fluid balance (the difference between fluid intake and output), and plasma albumin level from post burn day (PBD) 3 to 10, pulmonary oxygenation index on PBD 3, 5, 7, 10, and 14, occurrence of lung and blood stream infections from PBD 7 to 14, and occurrence of acute respiratory distress syndrome (ARDS), occurrence of other organ complications, and mortality within 2 weeks post burn (PBW) were recorded and compared. Measurement data were processed with t test and randomized blocks analysis of variance, enumeration data were processed with Fisher's exact test.</p><p><b>RESULTS</b>Daily fluid intake of patients showed a tendency of decrease in both groups from PBD 3 to 10. Except for that of PBD 4, there was no statistically significant difference between two groups in fluid intake (with F values from 0.072 to 1.939, P values all above 0.05). Daily fluid output of patients showed a tendency of increase in both groups from PBD 3 to 10. It peaked on PBD 10 in control group and PBD 6 in treatment group. The mean daily fluid output was higher in treatment group than in control group from PBD 4 to 9, but without statistically significant difference (with F values from 0.001 to 3.026, P values all above 0.05). Fluid balance lowered in both groups, and it was the lowest on PBD 10 in control group and PBD 6 in treatment group. Fluid balance was lower in treatment group than in control group from PBD 3 to 7, and it showed statistically significant differences on PBD 4, 5, and 6 (with F values from 4.799 to 8.031, P values below 0.05). Plasma albumin level was higher in treatment group than in control group from PBD 3 to 10, with statistically significant differences observed on PBD 4, 9, and 10 (with F values from 5.691 to 10.551, P < 0.05 or P < 0.01). Pulmonary oxygenation index was higher in treatment group than in control group from PBD 3 to 14, with statistically significant differences observed on PBD 7 (respectively 372 ± 78 in treatment group and 291 ± 92 in control group, F = 5.184, P < 0.05) and 14 (respectively 354 ± 39 in treatment group and 283 ± 72 in control group, F = 8.683, P < 0.05). Lung infection and blood stream infection were respectively observed in 1 and 4 patient (s) in treatment group, and 9 and 11 patients in control group from PBD 7 to 14. Occurrence of ARDS, occurrence of other organ complications, and mortality were fewer in treatment group than in control group within PBW 2, though the differences were not statistically significant (P values all above 0.05).</p><p><b>CONCLUSIONS</b>RFMS is a useful strategy in improving early pulmonary oxygenation of patients with extremely severe and extensive burn by promoting the process of fluid reabsorption and rebalance. This strategy may be also beneficial for the prevention of organ complications as well as a better prognosis in severely burned patients.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To observe the effects of autologous fat granules in mixed grafting microskin grafts on repair of extensive deep burn wounds in patients.</p><p><b>METHODS</b>Twenty patients hospitalized in our ward were enrolled for autogenous self-control test in wounds on both or symmetrical parts of wounds of the trunk, and they were randomly divided into experimental (E) trol (C) groups, the wounds in E group were repaired with autologous fat granules together with microskin in mixed grafting (volume ratio 1 : 1), and in C group only autologous microskin grafting was given. Wound healing rate was measured on 30th, 45th, and 60th day after operation. Wound specimens harvested for HE staining and PCNA immunohistochemistry examination on 7th, 14th, 21st, and after operation.</p><p><b>RESULTS</b>(1) The mean wound healing rate on 30th, 45th, and 60th day after E group was (56.3 +/- 3.1)%, (76.4 +/-6.1)%, (96.2 +/- 1.5)%, which were respectively higher C group [(28.3 +/-2.0)%, (47.3 +/-4.8)%, (85.4 +/- 2.2)%, P < 0.01]. HE staining showed epithelization in E group was earlier than that in C group, with regular arrangement of collagen fibers. The quantity NA positive cells in E group were larger than that in C group, and PCNA was mainly expressed cells of basal layer .</p><p><b>CONCLUSION</b>Autologous fat granules in mixed grafting with autologous microskin promote wound healing.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the effect of recombinant adenovirus-mediated heat shock protein 70 (HSP70) on energy metabolism of mitochondria in intestinal epithelial cells (IEC-6) after hypoxia/reoxygenation injury .</p><p><b>METHODS</b>IEC-6 cells were transfected with HSP70 recombinant adenovirus vectors (Ad-HSP70) and empty adenovirus vectors. The expression of HSP70 protein was detected by Western blotting. Cultured IEC-6 cells were divided into: control group (without treatment), hypoxia/reoxygenation group (with challenge of hypoxia/reoxygenation) and Ad-HSP70 transfection group (with challenge of hypoxia/reoxygenation after Ad-HSP70 transfection). The activity of mitochondrial dehydrogenase was assessed by MTf method. The contents of cellular ATP, ADP , AMP and energy charge (EC)were determined by high-performance liquid chromatography (HPLC).</p><p><b>RESULTS</b>The expression of HSP70 protein in IEC-6 cells was significantly upregulated after Ad-HSP70 transfection compared with empty adenovirus vector transfection. Compared with that in control group, the activity of mitochondrial dehydrogenase was significantly lowered in IEC-6 cells in hypoxia/reoxygenation group (P < 0.01). The activity of mitochondrial dehydrogenase in Ad-HSP70 transfection group was significantly greater than that in hypoxia/reoxygenation group (P < 0.01). Compared with those in control group,the content of cellular ATP was significantly decreased in hypoxia/reoxygenation group, the contents of cellular ADP and AMP were significantly increased. The above cell energy indices in Ad-HSP70 transfection group was similar to those in control group (P > 0.05), which were ameliorated compared with those in hypoxia/reoxygenation group (P < 0.050 or P < 0.01). The cellular EC in hypoxia/reoxygenation group (0.615 +/- 0.060) was significantly lower than that in control group (0.748 +/- 0.012, P < 0.01) and Ad-HSP70 transfection group (0.736 +/- 0.028, P < 0.01).</p><p><b>CONCLUSION</b>Ad-HSP70 transfection in IEC-6 cells can upregulate the expression of HSP70, the content of cellular ATP and EC after hypoxia/reoxygenation, and protect mitochondrial function. Mitochondria may be one of main target organelles for HSP70 in protection of IEC against hypoxia/reoxygenation injury.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the clinical efficacy and safety of Acticoat (nanocrystalline silver dressing) for the treatment of residual burn wounds.</p><p><b>METHODS</b>Ninety-eight patients with 166 residual burn wounds were enrolled in the multi-center randomized clinical trials. In addition to the routine treatment, Acticoat was applied onto the wounds of the trial group once a day if there was much exudation from the wound, or the dressing change was made every other two days when the wounds were clean. Silver sulfadiazine (SD-Ag) was used in the control group of patients. The healing time was observed up to 20 days. The healing rate on the 15th day after treatment was taken as the percentage of healing.</p><p><b>RESULTS</b>The average healing time was (12 +/- 5) days after the application of Acticoat, which was significantly shorter than that in control wounds with SD-Ag (16 +/- 6) days, (P = 0.005 < 0.01). The total effective rate of the wounds for trial was 97.05%, which was higher than that in control (94.17%) group, but there was no statistically significant difference. The bacterial clearing rate of the Acticoat group on the 6th and 12th post treatment day was 21.7% and 43.5% respectively, which was significantly higher than that in control group. No side-effect was observed in the two groups during the study.</p><p><b>CONCLUSION</b>Acticoat with nanocrystalline silver can promote the healing of residual burn wounds effectively.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the prevalence of hearing impairment and ear diseases in old people and provide scientific data for drawing up the prevention and treatment strategies.</p><p><b>METHODS</b>Using the probability proportion to size (PPS) method, 1261 people over 60 years were investigated in 40 clusters in Jiangsu Province with the WHO protocol.</p><p><b>RESULTS</b>The prevalence of hearing impairment was 58.1% (the standardized rate: 59.5% in the whole country, 60.9% in Jiangsu province). Degrees of hearing impairment were mild (33.1%), moderate (17.8%), severe (5.9%) and profound (1.3%). The prevalence of hearing disability was 25.0% (the standardized rate: 26.6% in the whole country, 28.1% in Jiangsu province). There were significant difference of the prevalence between male and female, as well as urban and rural, and different ages. The prevalence of the ear diseases was auricle malformation (0.2%), wax (1.7%), otitis externa (0.1%), fungi (0.5%), serous otitis media (1.2%), chronic suppurative otitis media (1.6%), dry perforation of tympanic membrance (2.3%). The causes of hearing impairment were ear diseases (2.9%), non-infectious condition (92.6%), genetic condition (0.3%) and undetermined causes (4.2%). Of which, 31.1% of persons needed hearing aids while 2.3% of persons needed medicine treatment, but 0.9% of persons needed non-urgent surgery and 1.0% of persons needed other treatment.</p><p><b>CONCLUSIONS</b>The prevalence of hearing impairment and disability in the old rised obviously than the last investigation in 1987. It was a heavy burden for social development in China. The government and the whole society should take more concern about the problem. The scientific strategies of prevention and treatment were urgently needed and implemented.</p>

@#ObjectiveTo investigate the effect of varied hyperglycemia and durations on preventing peripheral neuropathy (PN) with methylcobal in diabetic rats.Methods80 Sprague Dawley rats were selected and randomly chosen 16 as normal control (NC) group, others 64 animals as diabetic model induced by alloxan. The model rats were evenly divided into relatively good control (GC) group and relatively poor control (PC) group based on levels of hyperglycemia regulated with exogenous insulin. 16 rats of each GC/PC group were intramuscularly injected with methylcobal (500 μg/kg). The sensory nerve conduction velocity (SNCV), motor nerve conduction velocity (MNCV) and evoked potential amplitute (EPA) of sciatic nerve of rats were detected by evoked electromyogram respectively at 2nd, 8th and 12th week after onset of diabetes. Fructosamine in serum was also detected.ResultsThe rats injected with methylcobal had a significantly delayed reduction of SNCV, MNCV and EP in GC group (fructosamine,1.0 mmol/L) than that in PC group (fructosamine, 1.2 mmol/L) ( P<0.05~0.01). Up to 8th week after onset of diabetes, there was no obvious difference in physioelectroparameters between the GC group and NC group. At 12th week after onset of diabetes, physioelectroparameters in the PC group fell to the level of non-methylcobal injected group. No effect of methylcobal on blood glucose revealed in any groups ( P>0.05).ConclusionMethylcobal has a beneficial effect on delaying the onset of diabetic PN, and is of high efficacy with a good control of diabetic state, especially at early stage.

ABSTRACT

<p><b>OBJECTIVE</b>To find a new and effective way for the transfection of adenovirus vectors encoding HSP70 cDNA, so as to provide another possible method in gene therapy against ischemia and cellular hypoxia after burn injury.</p><p><b>METHODS</b>The replicated defective adenovirus vectors encoding HSP70 cDNA were encapsulated. Its acid resistance and dissolution in intestinal fluid were tested in artificial gastric juice and intestinal fluid. The expression of HSP70 gene which was transfected by the microcapsules orally was detected by RT-PCR.</p><p><b>RESULTS</b>The encapsulated replicated defective adenovirus vectors were viable in vitro. They exhibited good resistant to acid (resolution ratio less than 10%) and dissolution in intestinal juice (resolution ratio higher than 50%). The HSP70 gene expression of the tested rats was significantly higher than control, but there was no difference in the quantity of HSP70 induced by sodium arsenite or adenovirus transfection through injection by vein.</p><p><b>CONCLUSION</b>The encapsulation of adenovirus vectors can successfully keep the viability of the virus in vitro and protect the virus from harmful effect of acid and enzyme in the gastric juice. Its nice dissolution in intestinal juice should ensure its absorption by oral transfection. The expression of the HSP70 gene after oral intake of this preparation is as high as that with other traditional transfection methods. It is possible that in the future the encapsulated replication of defective adenovirus vectors encoding HSP70 cDNA can provide a safer, convenient and effective way for gene therapy for burn patients.</p>
