ABSTRACT

@#AIM: To explore the factors that influence the pupil dilated in patients with type 2 diabetes mellitus.
METHODS: Totally 85 patients with type 2 diabetes were collected from April 2016 to August 2016 in department of Endocrinology in our hospital. Compound tropicamide was instilled in the right eyes of the 85 patients, then pupil diameters were measured 30min after instillation. They were divided into different groups according to the duration of diabetes, glycosylated hemoglobin levels, the degree of retinopathy and peripheral neuropathy. Influencing factors of the dilated pupil were analyzed.
RESULTS: In the three groups as the duration of diabetes was 5a, 5-10a or 10a or more respectively, the pupil diameters were 6.98±0.78mm, 6.18±0.67mm and 6.00±0.73mm, and the difference between the three groups was statistically significant(P<0.01). In non-proliferative diabetic retinopathy group, the pupil diameters(6.05±0.92mm)was significantly lower than that in the pre-clinical group(6.75±0.76mm), the difference between the two groups was statistically significant(P=0.001). The normal group of the ankle-knee conduction velocity(6.70±0.84mm)was significantly higherthan that in the abnormal group(6.23±0.81mm), and the difference was statistically significant between the two groups(P<0.05). In the three groups as the level of glycosylated hemoglobin was less than 8.4%, 8.4%~13.4% or more than 13.4%, pupil diameters were 6.60±0.82mm, 6.53±0.86mm and 6.73±0.73mm, the difference between the three groups was not statistically significant(P=0.708).
CONCLUSION: The main factors influencing the pupil are duration of diabetes, diabetic retinopathy and peripheral nerve lesions. In order to improve the effect of diabetes mellitus on the pupils, it is necessary to discover and treat the diabetic patients as early as possible. Diabetes patients with small pupil should more pay attention to the fundus examination, in order to avoid missed diagnosis of diabetic retinopathy.

ABSTRACT

Objective: To study the relativity between color and chemical composition in fruits of Schisandra chinensis by fruits' surface color value (L*, a*, and b*) and contents of active ingredients of fructus core S. chinensis). Methods: The surface chromatic value (multivariate linear regression equation of L*, a*, b* values and deoxyschizandrin, schizandrin B, schizandrol A, schinsantherin A) was established by SPSS 20.0 with mathematical statistics method, then the influence degree of the active ingredients of. S. chinensis was detected by F test. Results: The value of L* were significant negative with deoxyschizandrin (r ≥ 0.5), and other chromaticity values were very low or low-grade correlated with lignans Conclusion: The effective ingredient content of fruit of S. chinensis can be estimated by its chromatic values, and also can be used in the hierarchies of the fruit of S. chinensis, in order to provide better scientific basis of the quality of S. chinensis.

ABSTRACT

?AIM:To investigate the causes and characteristics of retinal detachment after vitrectomy.?METHODS: Retrospective analysis of patients in our hospital in recent 4a who underwent primary, standard, three -port pars plana vitrectomy ( PPV ) . Retinal detachment occurred in 24 eyes ( 23 cases ) , 24-67 years old, average 49.0±12.0 years old. There were male in 17 cases, female in 6 cases, 6 right eyes, 18 left eyes.?RESULTS:Eighteen (75%) eyes without any symptoms were found during a routine check, 6 eyes ( 25%) with decreased visual acuity or in front of shadow. It took 1-223d from PPV to retinal detachment, average 40.7 ± 54.2d, of which retinal detachment occurred in 10 (42%) eyes within 1wk, 14 (58%) eyes within 1mo.The direct cause of retinal detachment was new retinal break ( a total of 27 holes ) in 19 eyes, proliferative vitreoretinopathy ( PVR) in 4 eyes, 1 eyes with primary hole not closed. All retinal detachment were ultimately completely reset.?CONCLUSION: The occurrence of new retinal breaks is the main cause of retinal detachment after vitrectomy, attention should be paid to the early postoperative peripheral retinal examination;after a timely surgery, the retina can be reset.

ABSTRACT

OBJECTIVETo investigate the efficiency of damage control surgery (DCS) and predictors of mortality in critically multiple trauma patients.

METHODSFrom May 1998 to February 2007, DCS were carried out in 27 patients with critically multiple trauma. Of the patients 15 cases survived (survival group) and 12 cases died (dead group). The surgical complications, causes of death, demographic, physiologic and medical parameters were collected and compared between the two groups. Multiple logistic regression analysis were performed to identify possible predictors of mortality.

RESULTSThe incidence of surgical complications was 37.0 percent, and the intra-abdominal infections was the most frequent (18.5%). The overall mortality rate was 44.4 percent. The most common causes of death was multiple organ dysfunction syndrome (50.0%). With respect to predicting mortality, statistically significant differences was found in parameters as age, injury severity score (ISS), initial temperature and base excess (BE), estimated blood loss, initial ICU temperature and length of hospital stay. Older age, increased absolute value of initial BE and lower initial ICU temperature were determined as independent predictors of mortality on multiple logistic regression analysis.

CONCLUSIONSThere is a comparable high morbidity and mortality rate in severely injured patients managed with DCS. Increased age, a larger absolute value of initial BE and lower initial ICU temperature could independently predict death of the patients.

ABSTRACT

OBJECTIVETo provide morphological basis for chyle leakage due to operation on upper abdomen or retroperitoneum region.

METHODSThe original part of thoracic duct, cisterna chyle, intestinal trunk, left and right lumbar trunks were examined in 32 adult cadavers.

RESULTS(1) The occurrence rate of cisterna chili was 22% (7 cases), among which 4 cases were oval, 3 cases were triangle. The cisterna chyle was (24 +/- 6) mm in length; the width of middle part was (4.1 +/- 0.9) mm. It was located to the right of midline at the level between the twelfth thoracic vertebral body and the second lumbar vertebral body anteriorly. (2) The original part of thoracic duct was (2.8 +/- 0.7) mm in diameter. The confluence form of thoracic duct included: left lumbar trunk and intestinal trunk united to form the common trunk first, right lumbar trunk then joined the common trunk (9 cases, 36%); right lumbar trunk and intestinal trunk united to form the common trunk first, left lumbar trunk then joined the common trunk (8 cases, 32%); left and right lumbar trunk united to form the common trunk first, intestinal trunk then joined the common trunk (4 cases, 16%); left, right lumbar trunk and intestinal trunk joined together (3 cases, 12%). (3) The intestinal trunk was (36 +/- 15) mm in length. It ascended on the left of descending aorta, superior to the left renal artery, crossed the second lumbar vertebra anteriorly, and joined left or right lumbar trunk to form common trunk, which extended to the cisterna chili or thoracic duct to the right of lumbar vertebra. (4) The lengths of left and right lumbar trunks were (107 +/- 24) mm and (111 +/- 18) mm, the external diameters of origins were (1.7 +/- 0.4) mm and (1.9 +/- 0.4) mm, and the external diameters of terminations were (2.2 +/- 0.6) mm and (2.2 +/- 0.5) mm, respectively.

CONCLUSIONThe larger lymph tubes should be protected emphatically in the relevant region when dissecting the root of celiac and superior mesenteric artery and the termination of inferior mesenteric vein during abdominal operation.
