ABSTRACT

Ginsenoside Compound K (CK) has anti-cancer and anti-inflammatory pharmacological activities. It has not been isolated from natural ginseng and is mainly prepared by deglycosylation of protopanaxadiol. Compared with the traditional physicochemical preparation methods, the preparation of CK by hydrolysis with protopanaxadiol-type (PPD-type) ginsenoside hydrolases has the advantages of high specificity, environmental-friendliness, high efficiency and high stability. In this review, the PPD-type ginsenoside hydrolases were classified into three categories based on the differences in the glycosyl-linked carbon atoms of the hydrolase action. It was found that most of the hydrolases that could prepare CK were PPD-type ginsenoside hydrolase type Ⅲ. In addition, the applications of hydrolases in the preparation of CK were summarized and evaluated to facilitate large-scale preparation of CK and its development in the food and pharmaceutical industries.

ABSTRACT

ObjectiveTo observe the clinical efficacy of Qiju Dihuangtang combined with Chinese medicine fumigation in the treatment of dry eye and its effect on the levels of interleukin-6 (IL-6) and matrix metalloproteinase-9 (MMP-9) in tears. MethodA total of 120 patients with dry eye of liver-kidney Yin deficiency syndrome who were treated in the Department of Ophthalmology, The Second Affiliated Hospital of Liaoning University of Traditional Chinese Medicine(TCM) from october 2019 to october 2021 were randomized into the observation group and control group. The control group was given sodium hyaluronate eye drops, and the observation group was treated with sodium hyaluronate eye drops, Qiju Dihuangtang, and Chinese medicine fumigation. The treatment lasted 30 days for both groups. The changes of ocular surface disease index (OSDI), TCM syndrome score, tear secretion (SIT), tear film breaking up time (BUT), corneal fluorescein staining (FL), and tear interleukin-6 (IL-6) and matrix metalloproteinase-9 (MMP-9) were observed. ResultAfter the treatment, the total effective rate was 90.0% (54/60) in the observation group and 75.0% (45/60) in the control group (χ2=4.675, P<0.05). After treatment, the OSDI score and TCM syndrome score were lower than those before treatment in both groups (P<0.05), and lower in the observation group than in the control group (P<0.05). After treatment, the SIT and BUT were higher (P<0.05) and FL score was lower (P<0.05) than those before treatment in both groups. After treatment, the improvement of the above indicators in the observation group was better than that in the control group (P<0.05). After treatment, the levels of IL-6 and MMP-9 were lower than those before treatment in both groups (P<0.05), and lower in the observation group than in the control group (P<0.05). ConclusionQiju Dihuangtang combined with Chinese medicine fumigation can effectively improve subjective symptoms, promote tear secretion, prolong BUT, enhance corneal epithelial repair, and reduce the levels of tear IL-6 and MMP-9 in the treatment of dry eye.

ABSTRACT

Objective:To observe the drainage effects of the venous and arterial perivascular and adventitial connective tissues(PACT)on edema of lower limbs in mice model, and to provide a new understanding of the drainage for the elderly lower extremity edema. Methods:Different doses of fluorescent sodium(FluoNa)were injected into the venous and arterial perivascular connective tissues in the feet and ankles of the lower limbs to create a model of lower extremity edema in mice. Then, at the level of the femoral artery and vein sheath, a stereoscopic fluorescence microscope was used to observe and record the drainage effect of vascular PACT pathways on the edema. Results:By tracking the diffusion and flowing of FluoNa in the interstitial space of ankle and foot, we found that(1)in addition to the local diffusion at the injection site, FluoNa can also flow along the venous and arterial PACT and the interstitial spaces between the corresponding vein and artery.(2)The flowing process along the vascular adventitia included transmission along the vascular long axis, and the long axis-perpendicular diffusion into the vascular peripheral tissue.(3)When low-dose and high-dose FluoNa were administered into the venous adventitia of the lower extremities, the venous PACT imaging was observed.(4)When low-dose and high-dose FluoNa were administered into the arterial adventitia of the lower extremities, the arterial PACT imaging was observed; (5)When low-dose and high-dose of FluoNa were administered into the near the arteriovenous-accompanying blood vessels, not only the venous PACT pathway and the arterial PACT pathway are imaged, but also the interstitial space between the pathway of the accompanying vein and artery was also visualized at the same time; (6)Histological analysis showed that the tissues stained by FluoNa were the venous and arterial adventitia and perivascular connective tissues. Conclusions:The interstitial fluid in lower extremity tissue edema can not only be drained by the venous or arterial PACT pathway, but also be drained through the interstitial space between the accompanying vein and artery when a large amount of the interstitial fluid accumulates.

ABSTRACT

Background:Rectal cancer is a common malignant tumor of alimentary tract. It has been demonstrated that oxaliplatin-based neoadjuvant chemotherapy is effective for rectal cancer, however, its mechanism is not fully clarified.Aims:To explore the effect of neoadjuvant chemnotherapy with FOLFOX4 (folinic acid, fluorouracil, and oxaliplatin) on expressions of Ki-67,a proliferating cell-associated nuclear antigen, matrix metalloproteinase-2 (MMP-2),and Fas,a death receptor in cancerous tissue of patients with rectal cancer. Methods:A total of 104 cases of patients with histologically proven rectal cancer from Aug.2014 to Feb.2016 at Central Hospital of China National Petroleum Corporation were enrolled prospectively and randomly allocated into treatment group (n =58) and control group (n =46).Patients in treatment group finished 6 cycles of neoadjuvant chemotherapy with FOLFOX4 before surgery, and those in control group underwent surgery directly.Expressions of Ki-67,MMP-2 and Fas protein in cancerous tissue of surgical specimens were determined immunohistochemically. Results:Immunoreactivity of Ki-67 mainly located in the nucleus of rectal cancer cells, and those of MMP-2 and Fas mainly located in the cytoplasm.Expression rates of Ki-67 and MMP-2 were significantly lower in treatment group than in control group (41.4% vs.80.4%,P < 0.05;36.2% vs.73.9%,P < 0.05),while those of Fas was significantly higher in treatment group than in control group (62.1% vs.32.6%,P < 0.05).Conclusions:The therapeutic effect of neoadjuvant chemotherapy with FOLFOX4 on rectal cancer might be associated with the inhibition of proliferative, invasive and metastatic capacities and induction of apoptosis in cancer cells.

ABSTRACT

Objective To investigate the clinical efficacy of endovascular treatment for iliac vein compression syndrome (IVCS) in 112 cases. Methods From September 2011 to June 2015, 112 cases of IVCS (Cockett syndrome) were treated in our hospital with endovascular methods .The case numbers of different CEAP ( clinical etiological anatomical pathophysical ) classifications were C2 (45 cases), C3 (23 cases), C4 (19 cases), C5 (11 cases) and C6 (14 cases). All the cases were diagnosed by venography .The operations started from puncturing the ipsilateral veins .The stenotic degree and compressed intensity of iliac vein were evaluated by the minimum diameter with low pressure balloon .And then a stent was placed into after percutaneous transluminal angioplasty ( PTA) when residual stenosis was more than 50%.The occlusive segment was dilated by 8 mm diameter balloon firstly , and catheter directed thrombolysis treatment was applied if thrombus was detected .The PTA and stenting were performed when the thrombus was completely removed .The improvement of clinical symptoms was observed after operation and the patients were followed up by ultrasound and venograghy . Results Nine cases were given PTA only , and the other 103 cases underwent stent placement . Postoperative venography showed collateral vessels were disappeared in 22 cases, obviously decreased in 56 cases and mildly decreased in 34 cases. The average difference of swelling leg ’s circle was diminished from (2.56 ±0.88) cm pre-operation to (0.93 ±0.71) cm post-operation, while the average area of skin ulcer was diminished from (6.34 ±3.78) cm2 pre-operation to (2.13 ±1.88) cm2 post-operation, all of which were statistically significant (t =20.24 and 7.19,P=0.000).A total of 86 cases were successfully followed up during a period from 2 months to 45 months (average, 19.2 months). The patency rate of treated iliac vein 12 months after operation was 76.2%in the PTA group, and in the PTA with stenting group was 96.7%, 88.6%, and 86.1%, respectively, at 12, 24, and 36 months after operation, in which a statistical significance was detected (log-rank χ2 =30.32, P=0.000).The patency rate in diameter ≥16 mm group was 95.2%and 95.2%, respectively, at 12 and 24 months after operation, while in diameter <16 mm group was 97.0%and 93.3%, respectively, in which no statistical significance was detected (log-rankχ2 =0.39, P=0.532).Thrombotic complication was found in 2 cases during the follow-up period , including 1 case of iliac vein thrombosis at 1 month and 1 case of thrombus in contralateral leg 8 months later .According to the results of venography in 78 cases at 12 months after operation , restenosis was found in 5 out of 6 cases in the PTA group, and the patency rate in the stent placement group was 95.8%(69/72).No stent migration was found . Conclusion Comfortable clinical results were obtained after endovascular therapy for iliac vein compression syndrome , and the application of stenting could increase the middle and long term patency rate of treated vessels .

ABSTRACT

Objective To analyze postoperative complications of surgical therapy for deep vein thrombosis (DVT) of the lower extremity. Method From January 2001 to January 2008 vena cava filters were placed in 171 DVT cases before surgery. Fogarty catheter (73 cases), Amplatz thrombectomy device (ATD) (55 cases), Acolysis ultrasound ablation(43 cases) were used to extract the thrombi in iliac and proximal femoral vein. The full extraction of thrombi in distal end was facilitated by compressing and massaging the legs in all cases. Iliac venous stenosis or occlusion was managed by interventional therapy, and temporary femoral arteriovenous fistula were carried out routinly. Result Operations were successful in 157 cases(70 cases in Fogarty group, 52 in ATD group,35 in Acolysis ultrasound ablation group), and failed in 14 cases (3 cases in Fogarty group, 3 in ATD group, 8 in Acolysis ultrasound ablation group). Permanent, retrievable and temporary vena cava filters were placed in 51,32 and 88 cases respectively. Stenosis or occulsions of the iliac vein was found in 143 cases and stents were placed after balloon dilatation in 41 cases. During the operation, residual thrombi was found in 80 cases. Vessel perforation and rupture were complicated in 14 cases, thrombosis adhering in eava vein filters in 18 cases. There was no mortality and no pulmonary thromboembolism. During the follow-up, we found iliae vein restenosis in 21 cases, thrombosis recurrence in 36 cases, stents displacement in 6 cases, and stents fracture in 2 cases. Conclusion Surgical therapy is effective for DVT of the lower extremities.

ABSTRACT

Objective To evaluate the prevention and management for the complications in interventional therapy for the lower extremity arteriosclerosis obliterans (ASO). Methods In this study 207 ASO cases received interventional therapy. The relationship between complication rates and different TASC type and the influence of diabetes mellitus (DM) with coronary heart disease (CHD) on critical organs complication rates was analyzed. Results The intervention succeeded in 190 cases. There was 17 failures, including 13 procedure failures and 4 deaths due to postoperative critical organ complications. Among the complications there were puncture site hemorrhage in 12, pseudoaneurysm in 4, alimentary tract hemorrhage in 2, arteriorrhexis in 6, cerebral infarction in 8, acute heart failure in 9, respiratory failure in 13, renal failure in 6, thrombosis in 5 and blue-toe syndrome in 1. The intraoperative complication rate of femoral-political type (39.84%, 51/128) was high than that of aorta-iliac type (18.99%, 15/79) (P< 0.05). Those critical organ complication rates of patients with DM and patients with DM&CHD were 27.66% (13/47)and 24.49% (12/49)respectively, they were higher than that of patients without (5%, 2/40) (P<0.05). Conclusions The high complication rates during interventional therapy of ASO were because of misoperation and complicated pathological type and together with DM and CHD. These rates could be decreased when we treated DM and CHD before operation and chosen a reasonable method and shorten the duration of operation.

ABSTRACT

Objective To discuss a new method of interventional and surgical therapy for the lower extremity postthrombotic syndrome (PTS). Methods 35 PTS cases were enrolled in our group. Dilation and stent placement in stenosis of iliac veins were performed in all cases, then temporary femoral arteriovenous fistula was established. After two weeks, 15 cases with varicosity underwent high ligation of the great saphenous vein and stripping, 5 cases with leg ulcers (ulcers diameter>3 cm) underwent skingrafting. After operation, patients took warfarin orally for 6 months and wear elastic compression stockings. Results Procedures were successful in all cases. Thrombectomy extracted only a few old thrombus. Before discharge, limb swelling subsided in 26 cases while 4 remain light swelling and 5 had no obviously relief. Twenty-six cases were followed-up for 3~24 monthes. Limb swelling subsided in 22 cases but still exist in 4. Limb ulcers were healed in 11 cases and the area of pigmentation were diminished. Limb ulcer recurred 12 months after skingrafting and healed finaly after perforator veins clipping with second skingrafting. Color Doppler was used in follow-up to check the potency of affected deep veins, good outcome was found in 20 cases and rough intima with stenosis of iliac veins was found in 2, no flow signal in 4. Conclusions Stenosis of iliac veins in PTS are common, the pathological changes could be treated by interveional method combined with temporary arteriovenous fistula. The degree of recanalization of femoral-popliteal veins determined the results.

ABSTRACT

Objective To discuss the methods and the efficacy of interventional treatment of vascular injuries. Methods From January 2006 to March 2008, interventional therapy was performed in 13 cases of vascular injuries including injuries of internal jugular vein, subclavical artery, axillary artery, inferior vena cave, abdominal aorta, superior mesenteric artery, arteria iliaca, vena iliaca and femoral artery.Types of these injuries included arteriovenous fistula in 3, vascular rupture complicating haematoma in 4,pseudoaneurysm in 3 and arterial stenosis ensuing from injury repair in 3. Covered stent-grafts were used in 9 cases (10 stent-graft), mesh stem in 1, sealed with balloon in 2 and introcoil embolism in 1.Result Interventional procedure was successful in all these 13 cases, there was no mortality nor severe complications. Small amount of endoleak developed in thoracic aorta pseudoaneurysm and contrast leaked from internal jugular vein in left subclavical arteriovenous fistula after stent-graft insertion, these were healedconservatively. Twelve cases were followed up from 1 to 26 months. Hemoptysis occurred in patient with thoracic aorta pseudoaneurysm 12 months after intervention but without abnormality in CTA. There were no stem break, shift, deformation or stennsis and there were no recurrence of primary disease. Conclusion Interventional therapy is of less invasion, short performation duration, simple manipulation and quick postoperative recovery for vascular injuries.
