ABSTRACT

OBJECTIVE To construct the quantitative evaluation system of regional clinical pharmacists’ professional ability, and provide reference for the evaluation of regional clinical pharmacists’ professional ability. METHODS Twenty-one experts from 18 hospitals in Chongqing were consulted to construct a professional ability index system for clinical pharmacists. TOPSIS model was used to calculate and obtain the expert authority index (EI), and the weighted averaging method was used to construct the judgment matrix. Analytic hierarchy process (AHP) was used to calculate the weights of all indicators for establishing a quantitative evaluation system of regional clinical pharmacists’ professional ability according to the weights of each item. RESULTS The results of TOPSIS showed that the EI range was 0.010-0.100, and the relative authority of experts was distinguished and measured effectively. The results of AHP showed that the judgment matrix of the quantitative evaluation system met the requirements of consistency test (consistency test index CR<0.1). Finally, a quantitative evaluation system for regional clinical pharmacists’ professional ability was established, including 6 sub-objective items (basic ability, clinical practice ability, coordination and communication ability, publicity ability, scientific research and teaching ability, continuous improvement ability) and 25 index items (such as educational background, professional title, clinical pharmacy working years, daily theoretical skills assessment, information ability level, medication education, etc.). CONCLUSIONS A quantitative evaluation system of regional clinical pharmacists’ professional ability has been established. Our study provides a theoretical reference for the quantitative evaluation and optimal management of regional clinical pharmacists.

ABSTRACT

Objective To evaluate the effects of information feedback mode on auditing medical orders between pharmacy intravenous admixture services ( PIVAS ) and clinical departments. Methods Sorting and analyzing the work contents, satisfaction, and unreasonable medical order rates before and after auditing medical orders between PIVAS and clinical departments by using the information feedback mode. Results The information feedback mode of auditing medical orders has been well accepted. For example, the satisfaction between clinical departments and PIVAS was increased significantly (P < 0. 05), and unreasonable medical order rate was decreased significantly as well (P < 0. 05). There was a strong correlation between satisfaction and unreasonable medical order rate. Conclusions Establishment of comprehensive information feedback mode for auditing medical order between PIVAS and clinical departments has improved the PIVAS quality of clinical service, and strongly supported the introduction of PIVAS.

ABSTRACT

Objective To discuss the clinical application value of 256-slice helical CT 3D-imaging in guiding interventional occlusion therapy for patent ductus arteriosus (PDA).Methods A total of 40 patients with sonography-proved PDA were randomly divided into group A (angiography group) and group B (CT-guided group) with 20 patients in each group.For the patients of group A,occlusion of PDA was performed based on the intraoperative angiography findings;and for the patients of group B,occlusion of PDA was carried out according to CT examination results.Intraoperative cardiac ultrasound monitoring was adopted and the curative effect was evaluated.Results The morphology of PDA demonstrated on CT 3Dimaging in group B was highly consistent with the configuration of PDA displayed on intraoperative angiography in group A.The most narrow diameters of PDA in group B and group A were (3.88±1.59) mm and (3.63±1.41) mm respectively,and the lengths of PDA in group B and group A were (6.1±1.06) mm and (6.82±0.74) mm respectively;the differences between the two groups were not statistically significant (P>0.05).The time spent for surgery in group B and group A was (34.3±9.11) min and (17.33±5.81) min respectively,and the intraoperative X-ray radiation doses in group B and group A were (33.93±11.0) mGy and (66.48±9.77) mGy respectively;the differences between the two groups were statistically significant (P<0.001).In group B,the preoperative X-ray radiation dose from CT examination was (119.79±29.45) mGy,when it was added to the intraoperative X-ray radiation dose the total cumulative radiation dose of group B was strikingly higher than that of group A.Conclusion Contrast-enhanced 256-slice helical CT scan and 3D-imaging technique can replace intraoperative angiography to get accurate anatomical imaging information of PDA,which are very helpful for the performance of interventional occlusion of PDA,meanwhile,it can effectively reduce the damage to the punctured artery and shorten the operation time.However,the radiation dose is a factor that should be taken into consideration.(J Intervent Radiol,2017,26:206-209)

ABSTRACT

OBJECTIVE:To explore the present situation of scientific research of clinical pharmacists in domestic third-grade class A hospitals,and discuss its feasible research ideas. METHODS:Questionnaire was conducted for the scientific research situa-tion of clinical pharmacists who obtained the certification in domestic 63 third-grade class A hospitals,the questionnaire data was collected,and data was statistically analyzed by using IBM SPSS Statistics 22.0 software. RESULTS:Totally 315 questionnaires were sent out,282 were effectively received,with effective recovery of 89.52%. In terms of attitude to scientific research,propor-tion of the surveyed clinical pharmacists who thought it was quite necessary and necessary was more than 70%;except for academ-ic degree(P0.05). In terms of the present situation of scientific research,achievements of them were relatively simple,thinking the biggest difficulty was insufficient of experiment condition;there was significant difference in the numbers of authorized patents in recent 5 years by different gender (P0.05). In terms of ability enhancement,the surveyed pharmacists thought liter-ature review,evidence-based literature/experimental interpreta-tion and foreign language reading were the abilities most need to be enhanced;except for gender,academic degree,age andjob title showed similarity in enhancing the literature review,evidence-based literature/experimental interpretation ability (P>0.05),while there were significant differences in enhancing foreign language reading ability by different gender,academic degree, age and job title (P<0.05). CONCLUSIONS:Clinical pharmacists show certain differences in the attitude to scientific research, present situation and direction of scientific research,and ability enhancement because of different gender,academic degree,age and job title,but there are also common. In order to improve the competence of scientific research and promote the discipline devel-opment of clinical pharmacy,pharmacy administrator in hospital should improve the academic degree and job title on clinical phar-macists and provide necessary advanced scientific platform and pertinent training at the same time. Additionally,except for the ratio-nal drug use,evidence-based pharmacy and pharmacoeconomics,clinical pharmacists can find new breakthrough point from phar-maceutical care,drug treatment decisions,translational medicine and medicine large data.

ABSTRACT

Objective To evaluate the feasibility and safety of prostatic artery embolization (PAE)via transradial approach in treating prostatic hyperplasia.Methods The clinical data of 18 patients with prostatic hyperplasia,who received C-arm CT-guided PAE via left or right radial artery access,were retrospectively analyzed.The following indexes were recorded:arterial spasm and injury of upper limbs,incidence of puncture point bleeding,postoperative radial artery pulse and congestion,blood supply and nerve injury of fingers,the surgical success rate,incidence of perioperative cerebral vascular complications,operation time,radiation dose and clinical curative effect.Results Among the 18 patients,PAE via left radial artery access was employed in 14,and PAE via right radial artery access was performed in 4.Bilateral PAE was carried out in 16 patients,and only unilateral PAE was able to be successfully accomplished in 2 patients as the prostatic artery opening of the other side was tortuous with stenosis.After PAE,decreased radial pulse was observed in one patient and ultrasound examination revealed decreased blood flow.The operation time ranged from 96 min to 245 min.The radiation dose received by the patient varied from 2435 mGy to 4958 mGy with a mean of (3342±156) mGy,which was not significantly different from the radiation dose received by the patients who underwent PAE via femoral artery access during the same study period (P=0.1167).Conclusion In treating prostatic hyperplasia,PAE by using transradial approach is clinically safe and technically feasible.

ABSTRACT

<p><b>OBJECTIVE</b>To observe the effects of electroacupuncture(EA) at different acupoints for fasting blood glucose(FBG) and oral glucose tolerance in type 2 diabetes rats so as to verify the glucose-lowering effects of EA.</p><p><b>METHODS</b>Total 100 SD male rats were seen as experimental objects,among which 13 were randomly assigned into a blank group. Other rats were fed with high fat and high sugar diet combined with intraperitoneal injection of small dose streptozotocin (STZ) to establish type 2 diabetes model. Sixty diabetic rats were randomly assigned into a model group,a Weiwanxiashu group,a Xinshu group,a Shenshu group and a Housanli group,12 cases in each one. Combined with the improved diet habits and routines,EA (2 Hz and 2 mA) was used at "Weiwanxiashu" (EX-B 3),"Xinshu" (BL 15),"Shenshu" (BL 23) and "Housanli" (ST 36) in the corresponding groups,6 times a week for 4 weeks. There was no treatment in the blank group and in the model group. The observation indexes were the FBG on the 7th,14th,21st,and 28th day of intervention,the instant glucose-lowering effect on the 21st day during treatment,and the area under the curve of oral glucose tolerance test (OGTT) after intervention. Also,the glucose regulation condition was observed.</p><p><b>RESULTS</b>Type 2 diabetes model could be established by high fat and high sugar diet combined with intraperitoneal injection of small dose STZ. Glucose decreased apparently at the end of the 1st week or the 2nd week compared with that before treatment in the Weiwanxiashu,Xinshu and Shenshu groups(<0.05,<0.01). The instant FBG of the Weiwanxiashu and Xinshu groups was obviously lower than that of the model and Housanli groups at the end of the 3rd week(all<0.01). The area under the curve of OGTT of the Weiwanxiashu group was apparently smaller than that of the model group (<0.05),and the results of the index in the other groups were not significantly different from that of the model group(all>0.05).</p><p><b>CONCLUSIONS</b>Low frequency EA at "Weiwanxiashu"(EX-B 3),"Xinshu"(BL 15) and "Shenshu"(BL 23) can reduce glucose with different onset times,effects,and durations. And "Weiwanxiashu"(EX-B 3) is more effective.</p>

ABSTRACT

Objective To explore the feasibility of mechanical chest compression to establish a rat model of car?diopulmonary resuscitation ( CPR) . Methods 4?month old healthy male Sprague Dawley rats were randomly divided into control group ( n=6 ) and model group ( n=10 ) . After induction of anaesthesia with 10% chloraldurate ( 3 ml/kg, i. p. ) , tracheal intubation and left femoral artery cannulation were performed. Under electrocardiographic and artery blood pressure monitoring, tracheal obstruction ( TO) was performed to rats in model group. At 2 min after the cardiac arrest ( CA) occurred, CPRs were administered to the rats using a self?made animal chest compressor, which provided chest?com?pression at a rate of 200 bpm. Results Shortly after TO, rats in the model group had respiratory arrest, cyanosis and ar?rhythmia. Electrocardiography indicated that CA occurred within 4-5 min, with a decreased artery systolic blood pressure ( <40 mmHg) and a zero pulse pressure. Return of spontaneous circulation ( ROSC) after the CPR was successfully a?chieved in 8 rats (80%), with a transient reperfusion arrhythmia. Finally, 60% of the rats (n=6) recovered to con?sciousness and survived for 24 hrs. The serum biochemical analysis indicated that there were electrolyte disturbances, aci?dosis, impaired renal functions and increased myocardial enzyme spectrum. Pathological examination revealed cardiac rhab? domyolysis, no?reflow phenomenon in renal glomeruli, decrease of neurons and pulmonary congestion in the model group rats. Conclusions Mechanical chest compression can provide minimal cardiac output for the requirement of CPR incardiac arrestin rats. It is feasible to establish rat CPR model with the mechanical chest compression.

ABSTRACT

Objective To study the protective effect of hyperin( Hyp) on the acute liver injury in rats induced by CCl4 . Methods The acute liver injury model was induced by CCl4 . The effect of Hyp on acute liver injury of rat liver histopathology was observed;and the impact of changes of homogenates total superoxide dismutase ( T-SOD) ,glutathione ( GSH) activity and malondialdehyde ( MDA) in liver were de-tected. Results The acute liver injury of tissue induced by CCl4,apparent pathological inflammation and fibrous tissue degeneration and necrosis were observed by HE staining;At the high doses of 60 mg/kg and medium doses of 30 mg/kg treated by Hyp,liver pathology changes was significantly obvious. The T-SOD,GSH activity of liver tissue was significantly increased in the groups of Hyp treatment,MDA content was significantly decreased,which related to the dosages. Conclusion In the rat model of acute liver injury induced by CCl4 . A better therapeu-tic effect of Hyp was shown,the mechanism may be related to its antioxidant activity.

ABSTRACT

Objective: The content of hesperidin in Jinshuiliujun Decoction (JSLJD) was determined by RP HPLC. Methods: The chromatographic column was Hyperisl BDS C 8(10?). The mobile phase was acetonitrile water containing 8% acetic acid (17∶83). The flow rate was 1.0 mL/min. The column temperature was 40 ?C and the detection wavelength was 284nm. The external standard and peak area method were used to determine hesperidin content. Results: There was a very good linear relationship between hesperidin and peak area when the sample injection was from 0.02 to 2.00?g. The average recovery of adding sample was 100.5% and RSD was 1.85%. Conclusions: This method is simple and believable. It can be effectively used for the quality control of JSLJD.
