ABSTRACT

【Objective】 To explore the incidence rate and characteristics of hepatitis-C virus (HCV) infection in heroin addicts with pulmonary tuberculosis (TB) in Xi’an so as to provide a basis for pulmonary TB and HCV prevention and control measures. 【Methods】 We carried out behavior investigation and serological detection of 219 heroin addicts with pulmonary TB infection in Xi’an Eighth Hospital. We analyzed HCV infection risk factors by Chi-square test and multi-factor Logistic regression method. 【Results】 The prevalence of HCV infection in heroin addicts with pulmonary TB was 71.2% and 92.9% in injected drug users (IDUs). The HCV infection rate was higher than the infection rate of HBV (10.9%) and HIV (5.9%) in heroin addicts with pulmonary TB. The major infection risk factors were intravenous injection (OR=18.632, 95% CI: 8.146-41.284) and longer duration of drug use (OR=1.428, 95% CI: 1.086-1.876). 【Conclusion】 The study found high HCV infection rate in heroin addicts with pulmonary TB and that the independent risk factors are the drug injection and duration of drug use. In order to prevent further spread of HCV in heroin addicts with pulmonary TB, we should establish and perfect the management system of HCV infection.

ABSTRACT

Objective:To investigate the clinical features of children with kidney diseases who developed posterior reversible encephalopathy syndrome (PRES), explore the risk factors of PRES in these children, improve the understanding of the diseases, and help early diagnosis and effective treatment of the diseases.Methods:The clinical manifestations, laboratory inspection results, magnetic resonance imaging(MRI) material as well as the prognosis of 10 children with kidney diseases complicated by PRES who were admitted to the Department of Nephrology, Xi′an Children′s Hospital from November 2016 to August 2018 were analyzed retrospectively.Results:A total of 10 children were recruited, including 1 boy and 9 girls, with the onset age ranging from 4 years and 3 months to 13 years [(8.53±3.09) years]. The diagnosed kidney diseases in these patients were primary nephritic syndrome (6 cases), lupus nephritis (1 case), Hepatitis B-related nephritis (1 case), polyarteritis (1 case) and hemorrhagic fever with renal syndrome (1 case). Eight children received corticosteroids and 4 of them received other immunosuppressants simultaneously.Nine children suffered from the infections.All of them had acute onset, and the main symptoms were hypertension (10/10 cases, 100.0%), headache and dizziness (5/10 cases, 50.0%), nausea and vomiting (5/10 cases, 50.0%), visual disturbance (3/10 cases, 30.0%) and convulsions by the ways of seizures definitely (9/10 cases, 90.0%). There was nothing positive in the examinations of the nervous system and fundus.Computer tomography examinations of 9 cases showed nonspecific low-density foci.The cranial MRI scan showed abnormal signals on the cerebral cortex of frontal lobe, parietal lobe and occipital lobe in all these 10 cases.The hyperintensities were observed on the fluid-attenuated inversion recovery sequences of all the 10 cases.Slight hyperintensities on diffusion-weighted images of 4 cases indicated that PRES progressed from reversible angiogenic edema to irreversible cytotoxic edema, meaning a poor prognosis.After blood purification treatment and reducing intracranial pressure, these 4 cases recovered.After timely treatment upon PRES diagnosis, patients had no recurrence and showed a good outcome.Conclusions:Children with kidney diseases and PRES also suffer from hypertension, and the treatment with immunosuppressive agents may precede the occurrence of PRES.Cranial MRI is important for the diagnosis of PRES.

ABSTRACT

Objective To evaluate the aortomitral angle(AMA)changes in primary hypertension patients with left ventricular normal configuration (LVN) by echocardiography,and explore the correlation with left ventricular systolic function.Methods LVN group included 54 patients,control group included 79 healthy subjects.All subjects were examined by conventional echocardiography,the 2-dimensional gray-scale dynamic images of 5 consecutive cardiac cycles were collected by routine echocardiography;from parasternal left ventricular long axis,the related AMA at the starting isovolumic contraction time (S-IVCT),S-wave peak(SP),starting isovolumic relaxation time(S-IVRT),and starting-end diastole(SD) were measured respectively based on the tissue Doppler imaging,calculating the angle difference(⊿θ) as well as the rate of the angle change.The global systolic longitudinal strain(GLS) and circumferential strain (GCS) of left ventricle were measured by 2-dimensional speckle tracking and quantitative analysis software.Left ventricular ejection fraction (LVEF) was measured by Simpson′s biplane method.Results Compared with control group,the ⊿θ and AMA of each phases in cardiac cycles of LVN group were increased (P<0.05);Compared with control group,the GCS of the left ventricular was increased in LVN group (P<0.05).There was a correlation between AMA of the S-IVCT and that of GLS or GCS,and the LVEF of the left ventricular in the control group (r=-0.18,P=0.04;r=-0.17,P=0.04;r=-0.19,P=0.03).Conclusions AMA angle in patients with LVN is significantly greater than the normal population value,which may be to maintain effective left ventricular systolic function and could be used as an quantitative indicator to assess the left ventricular remodeling in patients with LVN.

ABSTRACT

Objective To explore the clinical feature of Kimura disease complicated with nephrotic syndrome in children. Methods The clinical data from 4 children with Kimura disease complicated with nephrotic syndrome were retrospectively analyzed. Results In all of the 4 male children, level of serum IgE was increased but level of eosinophils was not increased in peripheral blood. The renal pathological manifestations were different among them. Two cases had acute tubular injury, one case had pathological changes in repeated renal biopsy, and one case had a little of eosinophils infiltration in renal interstitium. All of them were sensitive to oral hormone treatment. Except that one case had no relapse yet in short follow-up period, the other 3 cases had relapsed and all manifested as positive urinary protein without lymphadenectasis. The 3 cases with recurrence were treated by combined immunosuppressive agents, the palindromia of two cases were reduced which were combined with tacrolimus. Conclusion The Kimura disease complicated with nephrotic syndrome in children has a long course and different renal pathological manifestations. It is sensitive to hormone treatment but easy to relapse, and the treatment should be combined with immunosuppressive therapy. Tacrolimus may have the effect on reducing relapse and delaying the progress of renal pathology.

ABSTRACT

Objective:To observe the clinical efficacy of nitrous oxide inhalation in oral and maxillofacial surgery.Methods:1 80 pa-tients underwent maxillofacial surgery were randomly divided into 2 groups(n =90).The patients in group A were given nitrous oxide inhalation before surgery,those in group B were served as the controls.The blood pressure(BP),heart rate(HR)and oxygen saturation of blood(SpO2 )were monitored during surgery,the feelings of the patients for surgical procedures,pain VAS score and Houpt behavior rating scale were surveyed after surgery.Results:In the whole operation process,BP,HR and SpO2 of group A changed smoothly(P >0.05),BP and HR of group B increased during operation(P 0.05);Frankl treatment adher-ence score in group A and B was 3.66 ±0.05 and 2.91 ±0.05 respectively(P <0.05);Houpt behavior rating scale were 5.06 ±0.1 1 and 3.29 ±0.06 respectively(P <0.05).VAS score in group A was lower than that in group B(P <0.05).Mild adverse reaction was observed in 6 cases in group A.Conclusion:In the process of maxillofacial surgery,nitrous oxide inhalation has sedation,analgesia and anti-anxiety effects without severe adverse effect.

ABSTRACT

BACKGROUND:Implant stability is mainly influenced by peri-implant inflammatory stimulation. OBJECTIVE:To build a beagle model of peri-implantitis under orthodontic force and to observe the bone remodeling of the Beagle dog model. METHODS: Micro-implants were randomly implanted into the maxilary interradicular region at the center of the mesial and distal roots of bilateral P2, P3, P4 and M1 of Beagle dogs. One side served as a loaded micro-implant with peri-implantitis under 100 g of orthodontic force at 1, 2, 3, 4 weeks of peri-implantitis, and the force lasted for 1 month. After that, the animals were kiled to prepare specimens with micro-implants. Bone-to-implant contacts were calculated and histological changes of the bone interface under continuous orthodontic force at different stages of peri-implantitis were observed under light microscope. RESULTS AND CONCLUSION:There were a large number of inflammatory cels after micro-implants were implanted with silk thread ligation to the cervical part. Over time, inflammatory cels were gradualy diffused to the tip of micro-implant, and there were a great quantity of colagenous fibers, osteocytes and active bone remodeling. When the inflammation was diffused to the tip of micro-implant after 2 weeks of peri-implantitis, woven bones composed of newly formed trabeculae appeared, and imflammatory cels dispersed. The medulary cavity was irregular after colagen fibrils absorption, and there were 3-4 layers of osteoblasts in the bone lacunae, with active bone formation. These findings indicate that the Beagle model of pure titanium peri-implantitis under orthodontic force was successfuly built in the experiment, and bone formation became active at 2 weeks after modeling.

ABSTRACT

<p><b>OBJECTIVE</b>Osseointegration of orthodontic microscrew implant is influenced by tooth extraction. This study aims to evaluate the safety margin of the osseointegration of orthodontic implants by investigating the healing process of the implant-bone interface through histopathological studies and quantitative determination.</p><p><b>METHODS</b>Twelve male beagles were selected and randomly divided into four groups. An orthodontic microscrew was implanted beside the tooth extraction area. Animals were killed in 1, 3, 8, and, 12 weeks to investigate tissue response. Histomorphological observation and bone implant contact ratio (BIC) tests were performed at different healing time after implantation.</p><p><b>RESULTS</b>A new bone was formed on the implant-bone interface of the control group. Bone resorptions were also detected in the experimental group 3 weeks after implantation. The BIC level of the control groups increased during the first 8 weeks; no change was observed anymore after the 8th week. On the other hand, the BIC value in the experimental group decreased in the first 3 weeks. It then increased rapidly and reached its peak of 80.08% in the 8th week. No significant difference wa s found between the experimental and control groups in the first 3 weeks. After the 3rd week, the BIC value of the experimental group (44.35%) was lower than that of the control group (55.46%) (P < 0.01).</p><p><b>CONCLUSION</b>The healing process after implantation was influenced by tooth extraction. Bone resorption was detected at an early stage. However, vigorous bone remodeling was observed subsequently.</p>

ABSTRACT

Objective To study the expression of interleukin-13(IL-13) in renal tissues of several common pathologic types of primary glomerulonephritis(PGN).Methods Immunohistochemistry was used to examine the expression of IL-13 in renal tissues of 81 patients with PGN and 10 normal control cases.The correlation analysis was performed with biochemical and pathological indicators.Results Extensive expression of IL-13 was found in renal tissues of patients with PGN.The expression of IL-13 in renal tissues was positively correlated with the concentration of blood urea nitrogen,serum creatinine,the quantitation of urine protein and the crescent formation in glomerular(r=0.325,0.275,0.291,0.231,respectively).It was negatively correlated with glomerular filtration rate and serum albumin(r=-0.448,-0.296,respectively).Conclusion IL-13 participates in the process of inflammation reaction and crescent formation in the renal tissues in patients with PGN,and is correlated with the progression of renal function and the level of urea protein.
