ABSTRACT

Objective:To study the clinical value of left ventricle Tei index in evaluating hemodynamics after interventional therapy of patent ductus arteriosus (PDA).Methods:From May 2017 to May 2019, 50 children with PDA who underwent interventional therapy (PDA group) and 27 healthy children (healthy control group) in Anhui Provincial Children’s Hospital were selected. The left ventricle Tei index, plasma brain natriuretic peptides (BNP), left ventricular end-diastolic dimension (LVDD), left ventricular ejection fraction (LVEF) were compared between 2 groups.Results:The left ventricle Tei index was not correlated with heart rate and age in 2 groups ( P>0.05). The left ventricle Tei index before operation in PDA group was significantly lower than that in healthy control group: 0.20(0.16, 0.25) vs. 0.27(0.20, 0.30), and there was statistical difference ( P<0.05). In PDA group, the left ventricle Tei index immediately, 3 d, 1 month and 3 months after operation was significantly higher than before operation: 0.38(0.29, 0.47), 0.32(0.26, 0.40), 0.30(0.27, 0.35) and 0.32(0.26, 0.37) vs. 0.20(0.16, 0.25), and there was statistical difference ( P<0.05); the plasma BNP immediately after operation was significantly lower than before operation: 288 (126, 433) ng/L vs. 582 (303, 1 675) ng/L, and there was statistical difference ( P<0.05); the LVDD 3 months after operation was significantly lower than before operation: (3.03 ± 0.54) cm vs. (3.38 ± 0.51) cm, and there was statistical difference ( P<0.05); the LVEF immediately after operation was significantly lower than before operation: (54.24 ± 6.09)% vs. (59.45 ± 5.93)%, the LVEF 1 and 3 months after operation was significantly higher than that immediately after operation: (63.18 ± 4.71)% and (65.46 ± 4.78)% vs. (54.24 ± 6.09)%, and there were statistical differences ( P<0.05). The left ventricle Tei index before operation was negatively correlated with inner diameter of PDA and plasma BNP ( r = -0.362 and -0.388, P = 0.013 and 0.009), and there was no correlation between LVDD and LVEF ( r = -0.192 and -0.283, P = 0.229 and 0.053); the differences of Tei index before operation and immediately after operation (ΔTei) was positively correlated with inner diameter of PDA ( r = 0.325, P = 0.030), and there was no correlation with BNP, LVDD and LVEF ( r = 0.234, 0.283 and -0.039, P = 0.126, 0.076 and 0.798). Conclusions:The left ventricle Tei index can quickly and accurately assess the change of hemodynamics after interventional therapy of PDA.

ABSTRACT

Objective:To explore the guiding role of transthoracic echocardiography (TTE) for interventional therapy of atrial septal defect (ASD) in children, and observe the effects of interventional therapy on left ventricular structure and function.Methods:One hundred and seven children with ASD aged (5.03 ± 2.86) years, who received interventional therapy in Anhui Provincial Children′s Hospital from January 2012 and October 2018 were retrospectively analyzed. Before operation, by the examine of TTE all children were diagnosed as ostium secundum atrial septal defects and no other cardiovascular malformations were found to combined with. For inclusion in the study, patients had to have a distance of over 5 mm rims of ASD to entrances of superior and inferior vena cava and coronary sinus, which was more than 7 mm(waist of optioned occluder ≥ 7 mm) and more than 6 mm (waist of optioned occluder ≤ 10 mm) to mitral valve, while the length of atrial septum was not less than the maximum diameter of the defect +14 mm. During the operation, TTE was used to observe ASD from multiple views for measuring out its maximum diameter and minimum diameter, then based on these measurements the occluder′s size was determined. Left ventricular end-diastolic diameter (LVDD), left ventricular end-systolic diameter (LVDS), left ventricular ejection fraction (LVEF) and fractional shortening (FS) were measured by TTE before operation and 3 days, 1, 3, 6, and 12 months after operation.Results:All the children were successfully treated with occluders. In only 4 cases, occluders of the first choice were improper and then were replaced. No complications such as occluder′s embolization or malposition occurred. Size of ASD was (8.60 ± 4.72) mm for maximum diameter and (7.00 ± 3.69) mm for minimum diameter, and size of occluders was (10.56 ± 5.01) mm. Both LVDD and LVDS respectively with values of (33.01 ± 4.63) mm and (20.72 ± 3.50) mm before operation increased after operation. Compared with those before operation, LVDD at 3, 6 and 12 after operation [(34.95 ± 4.02), (34.63 ± 4.17), (35.47 ± 4.50) mm vs. (33.01 ± 4.63) mm], and LVDS at 1, 3, 6 and 12 months after operation [(21.59 ± 2.74), (22.16 ± 2.91), (21.77 ± 2.87), (22.60 ± 3.51) mm vs. (20.72 ± 3.50) mm] had significant differences ( P<0.05 or <0.01). For LVEF and FS with respectively preoperative values of (68.12 ± 5.78)% and (37.47 ± 6.28)%, there was no significant difference ( P>0.05) among preoperative and each postoperative observation points. Conclusions:TTE can accurately guide the interventional treatment of ASD in children.And after the treatment, left ventricular diameter increases and systolic function remains normal.

ABSTRACT

Objective To explore the best keV conditions of show hepatocellular carcinoma lesions with the best contrast-to-noise ratio (CNR)and image quality,and evaluate the effect of monochromatic energy imaging on the detection of hepatocellular carcinomas.Methods The dual-energy CT imaging of 30 patients with hepatocellular carcinoma were analyzed retrospectively.Monochromatic energy images (40 to 140 keV,interval 10 keV)were produced by the dual-engery software.CNR and image noise of monochromatic energy,100 keV and 140 keV images were measured respectively.The CNR and the imaging noise were optimized.The results between the monochromatic energy images and the mixed energy images were compared by using the two related samples of non-parameter rank test.Results In the arterial phase, the monochromatic energy level with the highest CNR was concentrated at 70 keV,and the lowest level of the image noises was at 70 and 80 keV.The images at 70 keV were chosen to be compared with the fusion images.The CNR values of the monochromatic images at 70 keV were similar as those at 100 keV (1 .68 ± 1 .04 vs 1 .88 ± 1 .59 ,P =0 .149 ),and significantly higher than those at 140 keV (1 .68 ± 1 .04 vs 0.62±0.92,P =0.000).The image noises at 70 keV were similar as those at 140 keV (6.52±1.53 vs 7.60±2.73,P =0.050), and significantly lower than those at 100 keV (6.52±1.53 vs 8.55±1.11,P =0.000).Conclusion The monochromatic energy imaging at 70 keV can improve the CNR of the hepatocellular carcinoma lesions without reducing the image quality,which will help the lesion detection.

ABSTRACT

Objective:To evaluate various types of samples from the different marine environments as sources of actinomycetes from the Yalujiang coastal wetland, North China, and to screen their antimicrobial properties. Further, the identified actinomycetes were characterized based on morphological, biochemical, and physiological characteristics. Methods:Eight different production media were used to isolate actinomycets from different stations of marine soil sediments in Yalujiang coastal wetland and the genotypic positions were established by 16S rDNA. Results:A total of 172 actinomycetal isolates were obtained from 13 samples using five media. The most effective culture media in the isolation of actinobacteria were Gause’s Synthetic agar and Starch-casein agar. Among 172 isolates, 46 isolates (26.74%) showed antibacterial activity, 70.93%belonged to the genus Streptomyces, others were Micromonospora spp. and Rhodococcus spp. Out of the 46 isolates, two cultures were further supported by morphological characterization analysis. Conclusions: This is the first report about actinomycetes isolated from Yalujiang coastal wetland and it seems that the promising isolates from the unusual/unexplored wetland may prove to be an important step in the development of microbial natural product research.
