ABSTRACT

Objective:To investigate the clinical diagnostic value of MRI retention enema cannula enhanced scanning in the high complex anal fistula.Methods:The clinical data of 60 anal fistula patients underwent surgery treatment from May 2020 to May 2022 in Affiliated Hospital of Shanxi University of Traditional Chinese Medicine were retrospectively analyzed. All patients underwent MRI plain scanning and enhanced scanning before operation. Compared with the surgical results, the difference between MRI plain scanning and enhanced scanning in the diagnosis of high complex anal fistula was compared.Results:All of the 60 patients successfully completed surgical treatment, and 58 cases internal orifices, 55 cases complex anal fistulas and 53 cases high anal fistulas were found intraoperatively. MRI plain scanning results showed 32 cases internal orifices, 46 cases complex anal fistulas and 42 cases high anal fistulas were found. MRI enhanced scanning results showed 54 cases internal orifices, 53 cases complex anal fistulas and 50 cases high anal fistulas were found. Based on surgical results, the coincidence rates of internal orifice, complex anal fistula and high anal fistula in MRI enhanced scanning were significantly higher than those in MRI plain scanning: 93.10% (54/58) vs. 55.17% (32/58), 96.36% (53/55) vs. 83.64% (46/55) and 94.34% (50/53) vs. 79.25% (42/53), and there were statistical differences ( χ2 = 21.76, 4.95 and 5.27; P<0.01 or <0.05). Conclusions:The MRI retention enema cannula enhanced scanning has obvious advantages in the diagnosis of high complex anal fistula, which provides scientific reference value for the diagnosis and operation of high complex anal fistula in clinic.

ABSTRACT

Severe purulent meningitis ( PM) is urgent suddenly and progresses quickly with high mortality and disabled rate. The occurrence of severe PM is associated with young children,immature immune function and barrier function,immune deficiency,or anatomic defects. The virulence of pathogenic and resist-ance to antibiotics are also related to severe PM. It will be significant for improving prognosis to identify and control pathogenic factors,early diagnosis,to strengthen antibiotic treatment as early as possible,to carry out appropriate symptomatic and supportive treatment,to monitor and manage complications.

ABSTRACT

Objective To construct a collagen matrix-based skin model that can last a long time in vitro for potential application of subsequent tests and studies.Methods The porcine collagen and normal human skin fibroblasts were mixed and seeded into the culture dish,which were cultured for 3-4 days to obtain a dermal structure.Then,the third-to seventh-passage normal human keratinocytes were seeded onto the dermal surface and cultured for 14 days to obtain a double-layer skin model.Hematoxylin and eosin (HE)staining and Masson staining were performed to evaluate the morphology and structure of the skin model,and electron microscopy was conducted to observe the ultrastructure of the skin model.Immunohistochemical study and immunofluorescence staining were conducted to determine the expression of major markers in each layer of the skin model.Results HE and Masson staining showed that the skin model and normal human skin tissues showed very similar dermal and epidermal structures.After harvest of the skin model,it can be cultured in vitro for another 14 days with favorable and stable dermal and epidermal structures.Electron microscopy showed lipids in the stratum corneum,keratohyalin granules in the stratum granulosum,corneodesmosomes,desmosomes and basal membrane in the skin model.Immunohistochemical and immunofluorescence staining showed the consistent expression of transglutaminase,filaggrin,keratin 10 and Ki67 in the epidermis in both the skin model and normal skin.Moreover,the expression of type Ⅳ collagen and laminin-5 in the basal membrane,as well as that of type Ⅰ collagen,type Ⅲ collagen and fibrillin in the dermis were both consistent between the skin model and normal skin.Conclusion The constructed three-dimensional (3D) skin model is highly analogous to normal human skin in the aspect of tissue structure and various protein expression,and the skin model can be stably cultured in vitro for at least 14 days after harvest.

ABSTRACT

Objective:To study the expressions of serum levels of intedeukin-12 (IL-12),interferon-γ (IFN-γ),erythropoietin (EPO) and ferritin in patients with acute leukemia and its clinical significance.Methods:76 patients with acute leukemia who were treated in our hospital from July 2015 to July 2016 were selected as the observation group,including 31 cases of newly diagnosed group,25 cases of remission group and 20 cases of relapse group.And another 76 cases who had taken the physical examination in our hospital were selected as the control group.Then the levels of serum IL-12,IFN-γ,EPO and ferritin in patients were observed and compared between the two groups.Results:The levels of IL-12 and IFN-γ in the observation group were significantly lower than those of the control group,and the levels of EPO and ferritin were significantly higher than those of the control group (P <0.05).The levels of serum IL-12 and IFN-γ in the untreated group and the relapse group were significantly lower than those of the remission group [(84.21± 5.43)pg/mL,(98.7± 7.98)pg/mL VS(112.43± 10.21) pg/mL,(38.54± 3.56)pg/mL,(49.87± 4.02)pg/mL VS(108.32± 8.43)pg/mL](P <0.05),and the levels of EPO and ferritin were significantly higher than those of the remission group [(402.32± 42.31) mIU/mL (321.58± 30.21)mIU/mL VS (98.21 ± 9.45) mIU/mLM (653.21 ± 54.24) ng/mLM (512.87 ± 43.45)ng/mL VS (342.15 ± 25.12)ng/mL] (P<0.05).Conclusion:The serum levels of IL-12 and IFN-γ in patients with acute leukemia were lower,and the expression of EPO and ferritin was higher,and the disease and prognosis could be evaluated by monitoring the changes of these indexes.

ABSTRACT

Objective@#To investigate the effect of overexpression of wild-type phosphatase and tensin homolog (PTEN) deleted on chromosome 10 and its mutant G129E (exhibiting the activity of protein phosphatase and losing the activity of lipid phosphatase) on F-actin in activated hepatic stellate cells (HSCs) cultured in vitro.@*Methods@#The activated hepatic stellate cell-T6 (HSC-T6) cells were cultured in vitro, and activated HSCs were transfected with adenovirus that carried wild-type PTEN gene and G129E gene using transient transfection. The HSCs were divided into the following groups: control group, which was transfected with DMEM medium instead of virus solution; Ad-GFP group, which was transfected with the empty adenovirus vector with the expression of green fluorescent protein (GFP); Ad-PTEN group, which was transfected with the recombinant adenovirus with wild-type PTEN gene and GFP expression; Ad-G129E group, which was transfected with the recombinant adenovirus with G129E gene and GFP expression. Western blot and quantitative real-time PCR were used to measure the protein and mRNA expression of PTEN in activated HSCs; under a laser scanning confocal microscope (LSCM), phalloidine labeled with the fluorescein tetramethylrhodamine isothiocyanate (TRITC) was used to observe the morphology of HSCs, distribution and fluorescence intensity of F-actin, and changes in pseudopodia and stress fibers, and a calcium fluorescence probe (Rhod-2/AM) was used to measure the changes in Ca2+ concentration in HSCs. A one-way analysis of variance was used for comparison between multiple groups, and the least significant difference test was used for comparison between two groups.@*Results@#Wild-type PTEN and G129E genes were highly expressed in activated HSCs. In the control group and the Ad-GFP group, HSCs had a starlike or polygonal shape, F-actin was reconfigured and formed a large number of stress fibers which stretched across the whole cell, and layered pseudopodia were seen around the cell. In the Ad-PTEN group and the Ad-G129E group, the HSCs had a fusiform shape, F-actin was mainly seen around the cell, a small number of stress fibers were seen inside the cell, and layered pseudopodia around the cell disappeared. The Ad-PTEN group and the Ad-G129E group had significant reductions in the fluorescence intensity of F-actin compared with the control group and the Ad-GFP group (357.67±13.39/377.25±14.55 vs 961.87±27.33/954.68±20.71, F = 1783.486, P < 0.05), while there were no significant differences between the Ad-PTEN group and the Ad-G129E group, as well as between the control group and the Ad-GFP group (P > 0.05). The Ad-PTEN group and the Ad-G129E group had significant reductions in the relative concentration of Ca2+ compared with the control group and the Ad-GFP group (251.60±90.88/352.18±146.01 vs 1953.95±132.99/1937.57±115.17, F = 834.988, P < 0.05), while there were no significant differences between the Ad-PTEN group and the Ad-G129E group, as well as between the control group and the Ad-GFP group (P > 0.05).@*Conclusion@#The overexpressed wild-type PTEN and its mutant G129E can significantly inhibit the formation and reconfiguration of cytoskeletal protein F-actin and reduce the concentration of Ca2+ in activated HSCs in vitro. In addition, there are no significant differences in the above effects between wild-type PTEN and G129E.

ABSTRACT

Objective To investigate the value of chromosomal microarray technique in genetic analysis of patients with embryo development arrest. Methods A total number of 273 patients with embryo development arrest were recruited for the chromosomal microarray testing.Results 41.4% of the 273 patients were chromosomal abnormalities. Among which 61(22.34%)were numerical chromosomal abnormalities,43 were structural anoma-lies,including which 15.75% were terminal deletion or duplication and microdeletion or microduplication. And 9 (3.3%)were mosaicisms.Conclusions Chromosomal microarray technique is highly accurate and specific,which can offer more genetic information than conventional karyotyping. And chromosomal microarray technique can also facilitate estimation of recurrence risk of future pregnancies for patients with embryo development arrest.

ABSTRACT

Objective To predict the Hypertention by pregnancy in counting nuclear red blood cells pe-ripheral blood. Methods Blood samples were obtained from 69 pregnant women with the gestational age from 12 to 18 weeks. Peripheral blood mononuclear cells were isolated by density gradient centrifugation and then analyzed the cells marked with PE-GPA/FITC-CD71 monoclonal antibodies on the flow cytometer. Correlations between them were analyzed after tracking of hypertention by pregnancy. Results Percentage of GPA+/CD71+ cells in healthy pregnant women was 0.89 ± 0.81%, while that in pregnant women with hypertention was 3.57 ± 3.48%. There was a significant difference between the two groups (P<0.05), and the percentage of GPA+/CD71+cells in the group of hypertention by pregnancy was much higher than that in the healthy group. Conclusions The increased nuclear red blood cells in maternal peripheral blood related to hypertention by pregnancy. The counting of nuclear red blood cells in maternal peripheral blood have potential value to predict hypertention by pregnancy.

ABSTRACT

As one of the most common hematologic malignancy,bone marrow involvement in lymphoma has diagnosis and therapeutic implications,especially in early chemotherapy curative effect evaluation.At present,the diagnosis of bone marrow involvement was mainly depend on bone marrow biopsy,but it has poor sensitivity suffered from sampling errors resulting in delayed treatment.Recently,with the development and perfect of imaging technology,the new means of detection with higher sensitivity and specificity are applied to diagnosis of bone marrow involvement.In this article,the application of imaging,magnetic resonance imaging (MRI) and 18F-FDG PET-CT in this area is reviewed.

ABSTRACT

Objective To study the application of percutaneous cannulated screw fixation in patients with unstable pelvic fractures.Methods According to the digital table,98 patients were randomly divided into the two groups,the control group were operated through plate fixation,and the observation group were operated through percutaneous cannulated screw fixation.Results After the surgery,the excellent rates were 89.8% in the control group and 98.0% in the observation group,which was significantly higher than that of the control group (x2 =4.68,P < 0.05).The operative time,blood loss,wound length were (121.6 ± 26.4) min,(416.3 ± 39.2) mL,(134.5 ± 21.3) cm in the control group and (81.3 ± 19.2)min,(63.5 ± 12.8)mL,(15.8 ± 2.1)cm in the observation group.The operative time,blood loss,wound length were significantly lower than those of the control group (t =2.961,6.312,8.061,P < 0.05,P < 0.01).Callus occurrence,fracture healing time were (14.2 ± 2.7) weeks,(10.1±2.9) weeks in the control group and (9.1 ± 1.4) weeks,(6.5 ± 1.5) weeks in the observation group.Callus occurrence,fracture healing time in the observation group were less than that of the control group (t =3.061,2.772,P < 0.05).The control group had postoperative complications occurred in 10 patients,with an incidence of 20.4%,and the observation group had three cases of complications,the rate was 6.1%,the complications incidence of the observation group was significantly less than the control group (x2 =6.14,P < 0.05).Conclusion Percutaneous cannulated screw fixation had significant effectiveness for patient with unstable pelvic fractures and fewer complications.

ABSTRACT

Objective To investigate the application of three stages enhanced scan of multi-slice spiral CT for the characteristic analysis on the wall and lumen of colorectal mucous adenocarcinoma.Methods 51 patients with colorectal mucinous adenocarcinoma who had complete pathologic diagnosis were studied.GE Discovery CT 750HD scanner was used for line scan,arterial and portal venous phase scan,and delayed scan.The lesions of intestinal wall,enteric cavity and proximal normal lumen were observed by line scan and enhanced scan,and the related quantitative value and CT value were observed.Results The intestinal wall was uneven annular thickening or hemispherical thickening.The thickening intestinal wall showed lobular changes around the inner margin,outer margin,or without lobular changes.The hierarchical sign and cystoids sign were observed with three layers or two layers structure after enhanced scan.Metastases of the larger lymph node,liver,ovaries were characterized similar to cyst.According to the form,the stenosis was divided into natural streamlined,irregular fixed,and irregular constrictive types.The proximal normal intestinal canal of the lesion showed occlusion,mild expansion or severe expansion.Conclusions Walls of colorectal mucous adenocarcinoma was uneven annular thickening or hemispherical thickening,in which calcification was found,and there was features of similar to layer and cyst after enhancement.The stiff intestinal wall appeared less,and the proximal bowel obstruction was rare.Therefore,depend on these characteristics,the diagnosis of colorectal mucous adenocarcinoma can be achieved.
