ABSTRACT

Objective:To summarize the clinical characteristics of neonatal brain abscess and improve the understanding of diagnosis and treatment of this disease.Methods:Clinical data of five cases of neonatal brain abscess admitted to the First Affiliated Hospital of Zhengzhou University from December 2018 to March 2021 were retrospectively analyzed and followed-up.Results:Among five cases, four cases were premature and one was term infant, three were girls and two were boys. The age of onset was 10, 5, 2, 28 and 11 days after birth, and all had fever as the first manifestation. Three cases had positive blood or cerebrospinal fluid cultures, and the diagnosis of brain abscess was confirmed by head imaging, with the most common lesion being in the frontal lobe. One case was treated conservatively, and four cases underwent abscess aspiration and drainage. After treatment, the range of lesions in five cases was reduced and the clinical symptoms were improved. The neurodevelopmental assessment after discharge did not reveal any intelligence or motor retardation in three cases, and were developing as the same age, while the other two cases had various degrees of neurological sequelae.Conclusion:The clinical characteristics of neonatal brain abscess are not specific, so it is necessary to conduct head imaging examination as early as possible for neonates with septicemia and meningitis with poor therapeutic effect or recurrent disease, so as to improve the early diagnosis rate and long-term prognosis.

ABSTRACT

Objective To explore the value of GALAD model, including gender, age, AFP, AFP-L3 and DCP in diagnosis of primary hepatocellular carcinoma and prediction of microvascular invasion (MVI). Methods Using retrospective study method, 5919 patients with primary hepatocellular carcinoma (HCC) who received radical operation from January 2015 to December 2018 in Eastern Hepatobiliary Surgery Hospital were enrolled into study group. At the same time, 1745 patients with benign liver diseases (BLDs) were enrolled into control group. The concentration of DCP was detected by Lumipulse G1200 automatic immune analyzer, and the concentration of AFP was detected by Cobas e601 automatic immune analyzer. AFP-L3 was detected by affinity adsorption centrifugation. The non-parametric Mann Whitney test was used to compare the difference between two groups. The chi square test was used to compare the rates. The diagnostic value of single serological marker and GALAD model for primary hepatocellular carcinoma was analyzed. The predictive effect of GALAD model on MVI of primary hepatocellular carcinoma was evaluated. Results Compared with single serum marker, the diagnostic value of GALAD model is higher. When the cutoff value is-0.33, the diagnostic sensitivity, specificity and accuracy reach to 91.9%(5440/5919), 86.8% (1515/1745) and 90.7% (6955/7664), respectively. The area under the curve can reach 0.960 [95%CI (0.955-0.964)]. Compared with no MVI (MO) group, the value of GALAD model in MVI low-risk group (M1), MVI high-risk group (M2) and MVI (M1+2) were significantly higher (Z values were-12.517,-22.883,-21.655, P<0.05), Galad model predicts MVI (M2) in high risk group, AUC was 0.717 [95%CI (0.701-0.733)] (M0 ratio M2). Conclusion GALAD model has better diagnostic performance in primary hepatocellular carcinoma and has certain predictive value for microvascular invasion.

ABSTRACT

Objective@#The present study was conducted to investigate the response of peroxiredoxin 3 (PRX3) to oxidative stress induced by high-risk human papillomavirus (HPV).@*Methods@#Sixty patients with cervical cancer were included and sixty patients with hysteromyoma were assigned as controls. Serum PRX3 was detected by enzyme-linked immunosorbent assay. The expression of PRX3 and oncoprotein E6 of HPV16 or HPV18 was examined in cervical cancer tissues by immunohistochemistry and in cervical cancer cells by Western blotting respectively.@*Results@#Patients with invasive squamous cervical cancer showed higher level of serum PRX3 than control subjects with hysteromyoma. PRX3 expression was up-regulated and was positively associated with that of E6 of HPV16 or HPV18 in cervical cancer tissues. The correlation was confirmed in HPV-containing cervical cancer cell lines including CaSki, and HeLa.@*Conclusions@#Our result indicated a positive response of PRX3 to HPV-induced oxidative stress. Serum PRX3 might be a potential indicator of active amplification of high-risk HPV in cervical cancer cells.

ABSTRACT

Objective To investigate the effect of flash glucose monitoring (FGM) on ambulatory glucose profile of only oral antidiabetic drugs treated patients with type 2 diabetes mellitus. Methods Twenty-eight type 2 diabetic mellitus patients with only oral antidiabetic drugs treatment from August 2017 to January 2018 were enrolled. All the patients were exposed to FGM for 14 d without changing the original treatment and encouraged to manage self-behavior by adjusting diet and activity based on the blood glucose data obtained from the real-time scanning. The changes in glucose profile during the FGM period were observed, including estimated glycated hemoglobin (HbA1c), standard deviation of blood glucose, variable coefficient of blood glucose, mean amplitude of glycemic excursions, time in range (blood glucose 3.9 to 10.0 mmol/L), area under the curve hyperglycemia (blood glucose> 10.0 mmol/L) and area under the curve hypoglycemia (blood glucose<3.9 mmol/L). The blood glucose levels on second day and thirteenth day were used as baseline and end point respectively. Results All of the 28 patients did not change their anti-diabetic drug therapy and there were no adverse events occurred. The estimated HbA1c was significantly lower than the baseline HbA1c: (6.90 ± 1.48)% vs. (7.57 ± 1.35)%, and there was statistical difference (P = 0.004). The standard deviation of blood glucose, variable coefficient of blood glucose, mean amplitude of glycemic excursions, area under the curve hyperglycemia and area under the curve hypoglycemia at end were significantly lower than those at baseline: (2.07 ± 0.86) mmol/L vs. (2.44 ± 0.86) mmol/L, 0.26 ± 0.11 vs. 0.30 ± 0.11, (5.32 ± 2.75) mmol/L vs. (6.76 ± 3.06) mmol/L, 265 (0, 1 310) vs. 351 (107, 2 177) and 0 (0, 0) vs. 0 (0, 19), the time in range at end was significantly higher than that at baseline: (1 069 ± 386) min vs. (921 ± 449) min, and there were statistical differences (P<0.05 or<0.01). The rate of scanning was (12.92 ± 4.87) times/d. Conclusions FGM could be applied by type 2 diabetic mellitus patients to make self-glycemic management without changing therapy, reduce the estimated HbA1c,and hypoglycemia, and improve the glucose fluctuations, which may result from real-time scanning to find abnormal glycemia and adjust daily behavior.

ABSTRACT

Objective@#To evaluate the clinical effects of CGF membrane for repairing the oral mucosa defects.@*Methods@#39 patients were selected with oral mucosa defects after surgical resections. 19 defects were covered by autologous CGF membranes as the experimental group. 20 defects were not covered by membrane as the control group. Regular follow-up were performed at one week, two weeks, three weeks, one month, two months, three months after the operations. And wound healings were compared in the groups.@*Results@#CGF membranes of the 19 patients in the experimental group survived completely. And no infection occurred. The wound healing rate of the experimental group was higher than that of the control group at two weeks, three weeks and one month after the operations. And the difference was statistically significant(P<0.05). The VAS score of the experimental group was lower than that of the control group at one week, two weeks, three weeks and one month(P<0.05). The mean score of VSS score in the experimental group was lower than that in the control group (P<0.05).@*Conclusions@#Repair of the oral mucosa defects with CGF membrane can shorten the period of wound healing, relieve the postoperative discomfort and reduce the scar. It was a safe and effective method worthy to be popularized.

ABSTRACT

BACKGROUND:The main clinical manifestation of senile arteriosclerosis obliterans is lower limb ischemia, which is currently difficult to treat. One method is by autologous stem cell transplantation into the muscles of ischemic limbs to improve the formation of new capillaries and restore lower limb blood flow. Endothelial progenitor cell marker CD34+ cell transplantation has been shown to promote angiogenesis in ischemic limbs. Therefore, we propose that peripheral blood autologous CD34+ cell transplantation in older adult patients with atherosclerotic ischemia could effectively promote angiogenesis.OBJECTIVE:To assume that peripheral blood autologous CD34+ cell transplantation in the elderly with atherosclerotic ischemia could effectively promote angiogenesis.METHODS:This is a prospective, single-center, open-label, randomized, and controlled clinical trial that will be completed at the Qingdao No. 9 People's Hospital, China. Twenty older adult patients with atherosclerotic lower limb ischemia will be randomized into two groups. In the cell transplantation group (n=10), peripheral blood CD34+ cells transfected with vascular endothelial growth factor 165 (VEGF165) gene will be intramuscularly transplanted into the ischemic limbs in older adult patients with atherosclerotic lower limb ischemia. In the control group (n=10), normal saline will be intramuscularly injected into the ischemic limbs. All patients will be followed up for 6 months. The primary outcome will be ankle-brachial indices before and 6 months after transplantation to assess lower limb ischemia in both groups.The secondary outcomes will be the number of microvessels in the lower limb muscles before and 6 months after transplantation, the morphology of new blood vessels revealed by CT angiography, the number of VEGF-immunoreactive cells 6 months after transplantation and the incidence of adverse reactions. The trial was registered at the ClinicalTrials.gov (identifier:NCT03098771), and the study protocol was approved by the Ethics Committee of Qingdao No. 9 People's Hospital of China. All protocols will be in accordance with Declaration of Helsinki,formulated by the World Medical Association. All patients will be informed of study protocols and provide a written informed consent prior to the beginning of the trial.DISCUSSION:This trial will begin in January 2018 and finish in December 2019. We aim to quantify the effects of VEGF165 gene-modified CD34+ cell transplantation in the treatment of older adult patients with atherosclerotic ischemia to develop a new effective treatment of lower limb ischemia.

ABSTRACT

Objective To explore the effect of the integration of human papillomavirus 16 on the cervical cancer patients.Methods Outpatient colposcopy cervical liquid-based cytology examined (TCT)information in Kailuan General Hospital and Linxi Hospital of Kailuan General Hospital from January 2014 to June 2015 were selected.The Results of TCT were positive in 180 patients with as case group,were negative in 128 patients as control group.The effect of the integration of human papillomavirus 16 was analyzed.Results In the case group,there were 86 cases with HPV-positive,37 cases with HPV16-positive,10 cases with HPV16-free,10 cases with HPV16-concomitant,17 cases with HPV16-integration,and the integrated rate was 46.0%.While in the control group,there were 20 cases with HPV-positive,3 cases with HPV16-positive and all of them were HPV16-free.The integration degree was significantly higher than the control group (P =0.000).Conclusion The integration of human papillomavirusl6 has much significant influence on the cervical cancer.

ABSTRACT

Objective To investigate the effects of cell suspension including Matrigel, normal culture medium and phosphate buffer saline (PBS) on the xenograft model establishment of human osteosarcoma. The function of Matrigel on regulating human osteosarcoma cell differentiation and proliferation was analyzed. Methods Twenty-four BALB/c-nu/nu nude mice were randomly divided into three groups with 8 animals in every group: Matrigel and RPMI 1640 suspension (group M), RPMI 1640 culture medium (group R), PBS (group P). Human osteosarcoma cell-SaOS-2 was suspended in the three groups respectively. 1×106/ml equivalent cell counts were injected into the back of each anesthetized nude mouse (400 μl per mouse). Xenograft tumors were measured at regular intervals and the tumor volume was calculated. After 5 weeks of inoculation, the tumor parts were dissected. Paraffin-embedded sections from xenograft tumor tissues were fixed in 4 % paraformaldehyde and pathological study was made after paraffin embedding and cutting under microscope by HE stains. Results Eight nude mice formed tumor lumps at 0 day in group M and were gradually increased over time. Xenograft tumors of group R and group P disappeared in 2-4 days and some appeared again after 1 week with an increase of tumor size. After 5 weeks, the tumor volume in the group M was significantly larger than that in the group P and group R [(3 185 ± 488), (598 ± 189), (512 ± 109) mm3 respectively,F=85.7,P<0.001].After 5 weeks,tumor body was dissected.The tumor weight in the group M was significantly larger than that in the group P and group R[tumor weight:(2.22 ± 0.18),(1.48 ± 0.13),(1.47 ± 0.17) g respectively, F= 37.07, P< 0.001]. There was no difference between group R and group P in tumor volume and weight(P>0.05).Histopathological analysis showed that cells in the group M could keep original degrees of pathological differentiation in osteosarcoma cells. Besides, cells suspension of culture medium or PBS in the group P and group R were poorly differentiated. Conclusions Matrigel can promote high tumor growth rate and good uniformity of human osteosarcoma cells in experimental animals. The histological state is similar to original structure,which conforms to the occurrence and development of human osteosarcoma.

Objective:To establish a method for the determination of 9 residual solvents including methanol, ethanol, acetone, i-sopropyl alcohol, acetonitrile, dichloromethane, methyl-butyl ether, ethyl acetate and methylbenzene by capillary gas chromatography. Methods:The residual solvents were separated on a Kromat KB-624(30 m × 0. 53 mm,3. 0 μm) capillary chromatographic column with temperature programming. Nitrogen was used as the carrier gas with the flow rate of 5. 0 ml·min-1 . The temperature of injector was 200 ℃. A flame ionization detector was used with the temperature of 250℃. The containers of head-space injector were in equilib-rium at 80℃ for 20 min. Dimethyl sulfoxide was used as the solvent and an external standard method was used for the determination of the 9 residual solvents. The injection volume was 1 ml. Results:Under the above conditions, the 9 solvents were well separated. There was good linear relationship within the concentration range, and the correlation coefficients were no less than 0. 9991. The average re-coveries were 99. 08%-105. 14% with RSD of 1. 1%-3. 3%(n=9). The LOQ was 15. 68, 25. 45, 1. 09, 10. 61, 4. 54, 5. 73, 1. 04, 10. 58 and 1. 89 μg, respectively. Conclusion:The established method is simple, accurate and reproducible, and can be used in the determination of residual solvents in irbesartan API.

ABSTRACT

BACKGROUND:Signal transducer and activator of transcription 3 (STAT3) and Caspase-3 are the important factors in JAK2/STAT3 signaling pathway. However, there are few studies on their role in the exercise preconditioning for myocardial protection. OBJECTIVE: To analyze the effects of exercise preconditioning on the expression of STAT3 and Caspase-3 in the heart of rats and investigate the cardioprotective mechanism of action. METHODS: Eighty Sprague-Dawley rats were randomly divided into control, exhaustive, exercise preconditioning, and exercise preconditioning+AG490 groups. The exercise preconditioning rat models were established by subjecting to intermittent treadmil exercise for 3 successive days. Rats in the control group were fed routinely for 3 days. Rats in the exhaustive group underwent treadmil exercise at 30 m/min until exhaustion after 3 days of routine raise. Rats in the exercise preconditioning group underwent exercise preconditioning for 3 days, and 24 hours later, they underwent treadmil exercises at the speed of 30 m/min until exhaustion. BACKGROUND:Studies have shown that the number of osteoblasts is often decreased after osteoporosis, and osteoblast replacement therapy becomes a new target for the treatment of osteoporosis. OBJECTIVE:To observe the osteogenic differentiation of human bone marrow mesenchymal stem cels cultured in dexamethasone, vitamin C and beta-glycerophosphate. METHODS:Mesenchymal stem cels were isolated and purified from adult bone marrow using human lymphocyte separation medium. The expression of cel surface markers was detected by flow cytometry. Cel ultrastructure was observed by transmission electron microscope. Then, the bone marrow mesenchymal stem cels were cultured in osteogenic induction medium containing dexamethasone, vitamin C andβ-glycerophosphate, and RT-PCR was used to detect the bone morphogenetic protein-2 mRNA expression after osteogenic induction. RESULTS AND CONCLUSION:A large number of adherent cels were visible as fibrous growth at 2 weeks after culture and strongly expressed CD44, CD29, but did not express CD34, CD45. These cels could be induced to differentiate into osteoblasts, and express bone morphogenetic protein-2 mRNA. Alizarin red staining and alkaline phosphatase staining were positive for the cels. These findings suggest that human bone marrow mesenchymal stem cels cultured in dexamethasone, vitamin C and beta-glycerophosphate can differentiate into osteoblasts, and has a potential for the treatment of osteoporosis.

BACKGROUND:Several studies have demonstrated that JAK2 kinase plays an important role in myocardial protection by ischemic preconditioning and medicine preconditioning. OBJECTIVE:To investigate the effects of exercise preconditioning on JAK2 mRNA and protein expression in the rat heart and the underlying mechanism of action. METHODS:SD rats were randomly divided into control, exhaustive, exercise preconditioning, and exercise preconditioning + AG490 groups. Rats in the control and exhaustive groups were fed routinely for 3 days. Rats in the exercise preconditioning and exercise preconditioning + AG490 groups were subjected to intermittent treadmil exercise for 3 successive days to establish exercise preconditioning animal models. Rats in the exercise preconditioning + AG490 group were intraperitonealy injected with JAK2 inhibitor AG490 at 10 minutes before exercise preconditioning. After 3 days, rats in the exhaustive, exercise preconditioning, and exercise preconditioning + AG490 groups were subjected to exhaustive treadmil exercise. Serum levels of lactate dehydrogenase and creatine kinase isoenzyme were determined. JAK2 mRNA and protein expression in the rat myocardial tissue was detected by real-time fluorescent quantitative PCR and immunohistochemistry.RESULTS AND CONCLUSION:Compared with the control group, serum levels of actate dehydrogenase and creatine kinase isoenzyme were increased, and JAK2 mRNA and protein expression in the myocardial tissue was significantly increased in the exhaustive group (P < 0.05). Compared with the exhaustive group, serum levels of actate dehydrogenase and creatine kinase isoenzyme were decreased (P < 0.05), while JAK2 mRNA and protein expression in the myocardial tissue was increased in the exercise preconditioning group (P < 0.05). Compared with the exercise preconditioning group, serum levels of actate dehydrogenase and creatine kinase isoenzyme were increased (P < 0.05), while JAK2 mRNA and protein expression in the myocardial tissue was decreased in the exercise preconditioning + AG490 group (P < 0.05). The results confirm that exercise preconditioning plays its cardioprotective role through activating JAK/STAT signaling pathway, up-regulating the expression of JAK2 mRNA and protein in the cardiac tissue and aleviating myocardial ischemia injury.

BACKGROUND:Epidural analgesia has been considered a gold standard for postoperative analgesia in the lower limbs. Its outcomes are accurate and adverse reactions are few, so it can be used in the clinic. However, this method has adverse reactions such as hypotension and urine retention. Low molecular weight heparin should be used after operation, which can increase the possibility of epidural hematoma, and limits its application to epidural analgesia in the clinic. At present, few studies concerned ultrasound guided continuous fascia iliaca compartment block technology. OBJECTIVE:To evaluate the efficacy of postoperative pain relief and the joint rehabilitation between a continuous fascia iliaca compartment block and a continuous epidural analgesia for patients undergoing total hip arthroplasty. METHODS:A total of 60 patients undergoing a selective total hip arthroplasty were assigned to continuous fascia iliaca compartment block group and continuous epidural analgesia group (n=30). Al patients in both groups received a pre-fluence before general anesthesia. Continuous fascia iliaca compartment block group were injected with 0.25%ropivacaine 30 mL via iliac fascia gap. Continuous epidural analgesia group received 0.20%ropivacaine 10 mL via epidural catheter, indwel ing catheter. When the analgesic effect was identified, anesthesia intubation was carried out. After operation, medicine was given via iliac fascia and epidural analgesia pump in both groups respectively. Postoperative analgesia in single dose was not given. If pain could not be endured, analgesia would be rescued (parecoxib 20-40 mg/time) according to pain degree. Visual analogue scale scores, supplemental analgesia of parecoxib, complication of anesthesia, Harris hip joint scores, day of first walk, and duration of hospital stay were recorded. RESULTS AND CONCLUSION:No significant difference in visual analogue scale scores, supplemental analgesia, Harris hip joint scores and duration of hospital stay was detected. Day of first walk was earlier in the continuous fascia iliaca compartment block group than in the continuous epidural analgesia group. The complications were apparently lower in the continuous fascia iliaca compartment block group than in the continuous epidural analgesia group. These data indicated that after total hip arthroplasty, two kinds of analgesia methods could provide satisfactory postoperative outcomes. Hip joint was perfectly recovered. However, the complications of continuous fascia iliaca compartment block were less, and helpful to patients’ early off-bed activities, and could be considered as a good choice for analgesia after total hip arthroplasty.

Objective To investigate the effects of altitude,race,gender and age on blood glucose and glycosylated hemoglobin(HbA1 c) of type 2 diabetes.Methods A total of 410 patients with type 2 diabetes (T2DM) who resided at different altitudes in Jinghong,Kunming and Lijiang city for over 5 years and received antidiabetic treatment without adjustments for over 3 months were enrolled.To compare blood glucose,hemoglobin (Hb) and HbA1 c levels of T2DM among different altitudes,different races,different genders and different ages,The correlation between races and HbA1c was evaluated with Pearson's correlation analysis.Results No significant differences were observed in fasting blood glucose (FBG) (P =0.49,P =0.1,P =0.09,P =0.3),2 h postprandial blood glucose (2 h PBG) (P =0.33,P =0.4,P =0.7,P =0.1),and HbA1 c (P =0.47,P =0.28,P =0.2,P =0.07) levels between patients at different altitudes,different races,different genders and different ages.However,there were significant differences in Hb (P <0.01).There was no significant correlation between races and HbA1c levels(r =0.003,P =0.6).Conclusions There were effects of altitude,race,gender and age on hemoglobin of type 2 diabetes,but no effects on glycosylated hemoglobin.

ABSTRACT

Objective To evaluate the efficacy and safety of acarbose tablets combined with protamine zinc recombinant lai pulp mixing insulin injection in the treatment of elderly patients with type 2 diabetes.Methods 170 elderly patients with type 2 diabetes were randomly divided into the observation group and the control group.85 cases in the observation group were given acarbose tablets combined with protamine zinc recombinant lai pulp mixing insulin injection,85 cases in the control group received aspart 30.The patients were followed up for 12 weeks,the blood glu cose levels,blood glucose fluctuation,glycosylated hemoglobin (HbAlc),the incidence of hypoglycemia and insulin dosage were observed.Results' The level of fasting plasma glucose (FPG) (t =9.92,P < 0.05),2h postprandial blood glucose(2hPG) (t =18.18,P <0.05),the mean blood glucose within 4h(t =13.62,P <0.05),days of average blood glucose fluctuations (t =14.56,P < 0.05) of observation group dropped significantly compared with that before treatment;HbAlc achieved rate(6.5%)of the observation group and control group were 35.29%,28.23%,re spectively,HbAlc achieved rate (7.0%) of observation group and control group were 57.64%,52.94%,the differences were not statistically significant(all P > 0.05).The dose of insulin was significantly lower in the observation group(20.11 ±3.36) u/d than(35.78 ±4.68) u/d in the control group(t =25.08,P <0.01).There were 2 cases of hypoglycemic events and 3 cases in the control group.Conclusion Protamine zinc recombinant insulin lispro injection combined with acarbose has reliable clinical efficacy and safety.

ABSTRACT

Objective To evaluate glycemic control,complication and influence factors of glycosylated hemoglobin(HbA1c) in type 2 diabetic(T2DM) patients with hypertension in retired and veteran of military communities in Kunming.Methods Cross-sectional study was conducted and 216 T2DM patients with hypertension were enrolled in current study.A self-made questionnaire was used to collected information including age,course of the disease,complication,hypoglycemic agents and status of glucose controlling.The levels of HbA1 c,fasting blood-glucose (FBG),2h postprandial blood glucose (2hPBG),total cholesterol (TC),triglyceride(TG),high density lipoprotein cholesterol(HDL-C),low density lipoprotein cholesterol (LDL-C),Blood urea nitrogen(BUN),creatinine(Cr) and uric acid(UA) were measured.Relative factors of HbA1c were analyzed.Patients were divided into the standard (HbA1c < 7%) and substandard groups (HbA1c ≥ 7%) according to the level of HbA1c,and divided into the oral-medication,insulin-treatment and oral medication combined insulin groups according to therapeutic regimens.Results The goal attainment rate of HbA1c(HbA1c <7%) in retired and veteran of military communities in Kunming was 44.9% (97/216).The levels of FBG,postprandial plasma glucose(PPBS) and TG in standard group were (6.1 ± 1.2) mmol/L,(9.4 ± 1.8) mmol/L and (1.9 ± 1.3) mmol/L,significant different from those in substandard groups [(7.8 ± 1.6) mmol/L,(11.5 ±2.2) mmol/L and (2.4 ± 1.3) mmol/L],and the differences were significant (t =11.6,9.1,2.2; P < 0.05 or P < 0.01).Meanwhile,the rate of cardiovascular disease,cerebrovascular disease,proteinuria,peripheral neuropathy and retinopathy were 69.8%,34.5 %,20.5%,20.5%,29.6%,respectively,significantly different between the standard and substandard groups (P < 0.05).HbA1 c level was linear correlated with FBG (r =0.76,P < 0.01),PBG (r =0.61,P < 0.01),TG (r =0.24,P < 0.05) and the frequency of glucose monitoring (r =-0.55,P < 0.01).As the increasing of the stage of diabetes,the selected therapeutic regimen were oral anti-diabetic agents,followed by insulin and insulin combined oral antidiabetic agents.The levels of PBG and serum creatinine in patients with insulin monotherapy were higher than those in other groups (P <0.05).Conclusion The level of glucose controlling of retired and veteran of military communities in Kunming is better.The prevalence of diabetic chronic complications is higher in patients with poor glucose controlling.HbA1c level is correlated with FBG,PBG,TG and the frequency of glucose monitoring.Oral antidiabetic agents is the primary therapeutic regimen.

ABSTRACT

ObjectiveTo evaluate the effects on serum homocysteine(Hcy) level,vascular function and carotid intima-media thickness(IMT) in metformin-treated diabetic patients with or without supplementation with folate and Vitamin B12.MethodsA total of 100 newly diagnosed diabetic type 2patients were divided into two groups by random digits table with 50 cases each,90 patients completed study.Forty-seven participants (control group) received a 6-month course of metformin treatment,43 patients (treatment group) received mefformin,folate and Vitamin B12 treatment.The levels of serum Hcy,endothelin-1 (ET-1),carotid IMT,large or small arterial elasticity index (C1,C2),flow-mediated dilatation (FMD) of the brachial artery were evaluated before and after treatment.ResultsThe level of serum Hcy in control group significantly increased compared with before treatment[ (13.4 ± 2.7)μ mol/L vs.(11.1 ± 1.9)μ mol/L],hut the level of serum Hcy in treatment group significantly decreased compared with before treatment [ (9.2 ± 1.8 ) μ mol/L vs. ( 11.3 ± 2.0) μ mol/L ],there was significant difference(P < 0.05 ).A beneficial effect was observed in the serum ET-1,FMD,carotid IMT and C2 in treatment group[ (20.0 ± 6.2)ng/L,( 15.8 ± 7.6)%,(0.8 ± 0.2) mm,(4.1 ± 2.1 ) ml/mm Hg ( 1 mm Hg =0.133 kPa) × 100 vs. (31.3 ±10.1 ) ng/L,(9.7 ± 4.5)%,( 1.1 ± 0.4) mm,(2.3 ± 1.0) ml/mm Hg × 100 ] (P < 0.05).The levels of ET-1,FMD,carotid IMT and C2 after treatment in control group [ (24.8 ± 6.8) ng/L,( 12.9 ± 6.3 )%,(0.9 ± 0.3)mm,(3.0 ± 1.4) ml/mm Hg × 100] had significant difference compared with before treatment [ (30.6 ± 8.7)ng/L,(9.8 ± 4.6)%,( 1.0 ± 0.3) mm,(2.2 ± 0.9) ml/mm Hg × 100](P< 0.05).However,the results were improved significantly in treatment group than those in control group (P <0.05).In treatment group,significant correlation were detected between changes of Hcy and ET- 1 (r =0.43,P < 0.05 ),carotid IMT(r =0.56,P < 0.05),FMD (r =-0.54,P < 0.05 ),C2 (r =-0.37,P < 0.05 ).ConclusionsAdministration of folate and Vitamin B12 can reduce the levels of serum Hcy and ET-1 in metformin-treated type 2 diabetic patients,which exert beneficial effect on carotid IMT,FMD and C2.

ABSTRACT

Objective The goal of this study was to explore the effect of exercise preconditioning on the expression of calcitonin gene-related peptide(CGRP)in the dorsal root ganglion of rats.Methods Fifty healthy male SD rats were randomly divided into control group (C) and exercise preconditioning group(EP).Group EP performed intervial treadmill exercise for 3 weeks for establishing exercise preconditioning animal model.The expression of CGRP mRNA in dorsal root ganglion was investigated by real-time fluorescence quantitative PCR.The immunoreaetion of CGRP in dorsal root ganglion was shown by immunohistochemistry.Results There was no significant difference in the expression of CGRP mR.NA in two groups(P>0.05).As compared with the group C,the immunoreaction of CGRP was increased in group EP,and the positive area and mean optical density of CGRP immunoreaction in group PE were significant higher than that in group C(P<0.05).Conclusion Exercise preconditioning does not change the expression of CGRP mRNA in dorsal root ganglion,but enhances the expression of CGRP in dorsal root ganglion to promote the reserve and release of CGRP in peripheral nerve endings and has the same endogenous protection as ischemie preconditioning.

ABSTRACT

Objective To evaluate the efficacy and feasibility of Flu/ivBu/Tl" conditioning regimen for the treatment of refractory or relapsed acute non-lymphocytic leukemia in patients receiving allogeneic hematopoietie stem cell transplantation. Methods Seven patients with refractory or relapsed acute non-lymphocytic leukemia received HLA identical peripheral blood hematopoietie stem cell transplantation (PBSCT) following Flu/ivBu/TY conditioning regimen, which consisted of fludarbine, busulfex and thiotepa. All patients received cyclos-porin A (CsA) and mycophenolet mofetil (MMF) for prophylaxis of graft - versus - host disease (GVHD). Results The Flu/IVBu/TT regimen was tolerated very well, without severe regimen related toxicity. In the 31-month median follow-up duration, 5 of 7 patients were a-live in disease-free situation. Conclusion The Flu/ivBu/TT conditioning regimen reduced transplantation-related toxicities and offered high long-term disease-free survival, and was tolerated very well. Allogeneie hematopoietie stem cell transplantation using Flu/ivBu/TT condition-ing regimen is a safe and effective option for the patients with refractory/relapsed acute non-lymphocytic leukemia.
