ABSTRACT

@#Objective    To analyze the occurrence of postoperative pulmonary complications (PPC) and the risk factors in patients with spontaneous pneumothorax who underwent micro single-port video-assisted thoracoscopic surgery (VATS). Methods    A total of 158 patients with spontaneous pneumothorax who underwent micro single-port VATS in our hospital from April 2017 to December 2019 were retrospectively included, including 99 males and 59 females, with an average age of 40.53±9.97 years. The patients were divided into a PPC group (n=21) and a non-PPC group (n=137) according to whether PPC occurred after the operation, and the risk factors for the occurrence of PPC were analyzed. Results    All 158 patients successfully completed the micro single-port VATS, and there was no intraoperative death. The postoperative chest tightness, chest pain, and dyspnea symptoms basically disappeared. During the postoperative period, there were 3 patients of pulmonary infection, 7 patients of atelectasis, 4 patients of pulmonary leak, 6 patients of pleural effusion, 1 patient of atelectasis and pleural effusion, and the incidence of PPC was 13.29% (21/158). Multivariate logistic regression analysis showed that lung disease [OR=32.404, 95%CI (2.717, 386.452), P=0.006], preoperative albumin level≤35 g/L [OR=14.912, 95%CI (1.719, 129.353), P=0.014], severe pleural adhesions [OR=26.023, 95%CI (3.294, 205.557), P=0.002], pain grade Ⅱ-Ⅲ 24 hours after the surgery [OR=64.024, 95%CI (3.606, 1 136.677), P=0.005] , age [OR=1.195, 95%CI (1.065, 1.342), P=0.002], intraoperative blood loss [OR=1.087, 95%CI (1.018, 1.162), P=0.013] were the risk factors for PPC after micro single-port VATS. Conclusion    There is a close relationship between PPC after micro single-port VATS and perioperative indexes in patients with spontaneous pneumothorax. Clinically, targeted prevention and treatment can be implemented according to the age, pulmonary disease, preoperative albumin level, intraoperative blood loss, degree of pleural adhesion and pain grading 24 hours after surgery.

ABSTRACT

Objective To evaluate the surgical techniques and clinical outcomes of ankle distraction arthroplasty with platelet-rich plasma(PRP) injection for post-traumatic ankle arthritis.Methods From May 2012 to May 2017,32 patients with post-traumatic ankle arthritis were treated at Department of Orthopaedic Surgery,Shanghai Sixth Peopled Hospital.They were 22 males and 10 females,with an age of 32.6±5.8 years(from 18 to 40 years).Their course of disease was 14.0±2.8 months(from 6 to 24 months).After failure of the conservative management for at least 6 months,they received ankle distraction arthroplasty with PRP injection.American Orthopaedic Foot&Ankle Society(AOFAS) ankle-hindfoot score and visual analogue scale(VAS) were used to evaluate the final overall outcomes.Range of motion(ROM) of the ankle joint and complications were also recorded postoperatively.Results Pin tract infection occurred in 12 patients but responded to alcohol care.All the patients were followed up for an average of 36 months(from 18 to 60 months).The VAS score decreased significantly from preoperative 6.8±1.2 to postoperative 8±1.4;the AOFAS ankle-hindfoot score increased significantly from preoperative 38.5±6.2 to 80.3±8.1 at the final follow-up;dorsal expansion and plantar flexion of the ankle increased significantly from preoperative 8.3°±7.0° and 24.7°±6.4° to 12.8°±6.5° and 31.4°±5.3° at the final follow-up,respectively(all P<0.05).Two patients had to receive salvage arthrodesis after conservative management failed to relieve their symptoms or delay the progression of their post-traumatic arthritis.Conclusion Ankle distraction arthroplasty with PRP injection is an effective ankle preserving surgery which can relieve symptoms,improve functions and delay progression of post-traumatic ankle arthritis.

ABSTRACT

Objective To investigate clinical effect of minimally invasive knotless bridge-suture technique for acute achilles tendon rupture.Methods The clinial data of 23 patients with acute achilles tendon rupture treated in Department of Orthopaedics,Shanghai Jiao Tong University Affiliated Sixth People's Hospital from August 2017 to February 2018 were analyzed retrospectively.There were 21 males and 2 females,aged (37.1 ± 5.6) years,with an age range of 29-51 years.Minimally invasive knotless bridge-suture technique was performed in all cases until the soft tissue condition of the patients improved.Patients were followed up by outpatient review at 3 weeks,6 weeks,3rd,and 6th months postoperatively,followed by every 3 months,followed up 12 to 18 months,and the deadline was April 2019.Ratio of perimeter of the affected lower extremity to that of the uninjuried,number of repetitions for one leg calf rise in one minute,American Orthopaedic Foot and Ankle Society (AOFAS) anklehind foot score,the achilles tendon total rupture score(ATRS) and time to return to work were investigated at last follow-up,as well as complications during the follow-up period.Results Twenty-three cases were followed up.At last follow-up,ratio of perimeter of the affected lower extremity to that of the uninjuried was (92.7 ± 1.8) %,number of repetitions for one leg calf rise in one minute was (25 ± 2) times,AOFAS ankle-hind foot score was (91.5 ±2.2) scores,ATRS was (92.6 ±2.4) scores,time to return to work ranged from 6 to 9 weeks.All patients underwent first-stage healing.No skin necrosis,superficial infection and sural nerve injury occurred.No achilles tendon re-rupture was seen during the follow-up period.Conclusion Minimally invasive knotless bridgesuture technique for acute achilles tendon rupture is applied easily and less invasive,enabling patients to return to work earlier and proved to be a secure and effective treatment.

Objective To compare instrumentation with 3D-printed patient-specific guides versus conventional techniques in supramalleolar osteotomy for varus ankle osteoarthritis.Methods A retrospective analysis was done of the 21 patients with varus ankle osteoarthritis who had been treated at Department of Orthopaedics,Shanghai JiaoTong University Affiliated Sixth People's Hospital from January 2017 to December 2018.They were divided into 2 groups by their treatment methods.In the 9 patients treated by instrumentation with 3D-printed patient-specific guides,there were 6 males and 3 females with an age of 54.6 ±8.6 years,4 left and 5 right sides involved,and one case of Takakura stage 1,3 cases of Takakura stage 2 and 5 cases of Takakura stage 3a.In the 12 patients treated by conventional techniques,there were 7 males and 5 females with an age of 53.0 ± 6.5 years,7 left and 5 right sides involved,and one case of Takakura stage 1,5 cases of Takakura stage 2 and 6 cases of Takakura stage 3a.The 3D printed guide group and the conventional group were compared in terms of operation time,intraoperative blood loss and frequency of intraoperative fluoroscopy,tibial anterior surface angle (TAS),talar tilt angle (TT),and tibial lateral surface angle (TLS).The differences in TAS,TT and TLS between pre-and post-operation in the 3D printed guide group were also evaluated.Results There were no significant differences in the preoperative general data between the 2 groups (P > 0.05),indicating they were comparable.All the patients were available for follow-up for an average of 7.8 months (from 3 to 15 months).The 3D printed guide group incurred significantly shorter operation time (106.2 ± 10.6 min),less intraoperative blood loss (207.2 ± 16.0 mL) and lower fluoroscopy frequency (2 ± 0) than the conventional osteotomy group (all P < 0.01).The post-operative TAS (94.3° ± 3.2°) and TT (3.8° ± 0.8°) angles in the 3D guide group were significantly different from their preoperative values (84.6°±3.5° and 7.6°± 1.6°) (P < 0.01).The 3D printed guide group was not significantly different from the conventional group in postoperative TAS (94.3°± 3.2° versus 92.4°±5.9°),TT (3.8° ± 0.8° versus 4.2° ± 1.1°) or TLS (83.7° ± 3.4° versus 84.2° ± 2.2°) angles (P >0.05).Conclusions Compared with conventional techniques,instrumentation with 3D-printed patient-specific guides can shorten operation time and reduce intraoperative blood loss and fluoroscopy frequency.The 3D printed patient-specific guides in osteotomy can facilitate accurate correction of varus deformity,leading to similar efficacy compared with conventional osteotomy.

Objective@#To establish the method for determination of tungsten and insoluble compounds in the air of workplace by inductively coupled plasma optical emission spectrometry (ICP-OES) .@*Methods@#The tungsten and insoluble compounds were digested by microwave digestion apparatus using nitric acid and hydrofluoric acid, detected by ICP-OES.@*Results@#The linearity of tungsten and insoluble compounds (as tungsten) were good at the range of 0.16-100.0 μg/ml, the minimum quantitation concentration was 0.11 mg/m3, the recovery was ranged from 81.9%-97.1%, the RSD of intra-and inter-batch precision were 0.5%-2.5% and 1.2%-4.4%, respectively.@*Conclusion@#The determination method meet the requirement of analysis and apply to the determination of tungsten and insoluble compounds in the air of workplace.

ABSTRACT

Objective To investigate the effect of the treatment for Maisonneuve fracture with arthroscopicassistant plate-screw fixation ofsyndesmosis.Methods From January 2015 to December 2016,totally 22 patients with Maisonneuve fracture were treated in Shanghai Jiao Tong University Affiliated Six People's Hospital.Eleven patients in minimal invasive surgery (MIS)group were treated by the arthroscopic-assistant plate-screw fixation ofthe syndesmotic injury.Another 11 patients in the control group were performed a classic syndesmosis screw fixation.Plain radiographic examination was carried out during the follow-up.Functional evaluation was measured according to the American Orthopedic Foot Ankle Society (AOFAS) ankle hindfoot score and the Visual analogue scale(VAS).A t test was applied for the statistic analysis of post-operative outcome between the two groups at 6th and 12th month.Results The patients were followed for an average 18 months (range from 12 to 24 months)except one of MIS group.The X-ray demonstrated the solid bone union occurred on the 12th week in MIS group and 12.5th week in the control group.The implants of syndesmosis were removed at 12th week post-operatively.No cases of re-diastasis occurred during the follow-up.There was significant difference with in the groups.In the MIS group,the AOFAS ankle hindfoot score was (85.9 ±4.1) points at 6th month post-operatively,and increased to (90.8 ± 3.7) at 12th month (P < 0.05).VAS score was (2.4 ± 1.2) at 6th month and (1.1 ± 0.7) at 12th month after the operation(P <0.05).In the control group,the AOFAS ankle hindfoot score was (81.1 ±4.7) points at 6th month after operation,and increased to (89.1 ± 3.4) at 12th month after operation (P < 0.05).VAS score was (3.2 ± 1.3) at the 6th month and (1.0 ± 1.0) at the 12th month after operation (P < 0.05).However,for the outcome comparison between the two groups,only the AOFAS ankle hindfoot score of 6th month post-operatively in MIS group was better than the control group (P < 0.05),other results,had no significant difference.Nocomplications of implant failure,nonunion,malunion or post-traumatic arthritis were occurred during the followup.Conclusion The treatment of Maisonneuve fracture by arthroscopic-assistant plate-screw fixation proved to have an advantage of minimal invasion and accurate reduction and fixation,which is a safe and effective surgical method.

ABSTRACT

Surgeries for chronic lateral ankle intability patients have been applied at home and abroad for many years,although operation techniques have been improved,there have been some lags between operation and postoperative rehabilitation.Insufficent rehabilitation after repair or reconstruction would limit ankle range of motion in patients,otherwise undue rehabilitation would impair the tendon-bone interfaces so as to render recurrence of ankle instability symptom.Systematic postoperative rehabilitation should be evidence-based,some articles about tissue healing and rehabilitation,together with latest international concepts on rehibilitation,can be used to instruct patients rehabilitation in different phrases.In this paper,we summarized the rehabilitation of chronic lateral ankle intability.

ABSTRACT

Objective To evaluate surgical treatment for Lisfranc injury with cuboid compressive fracture.Methods Totally 19 cases of Lisfranc injury with cuboid compressive fracture were treated at Department of Orthopaedic Surgery,Shanghai Sixth People's Hospital from June 2010 to June 2016.They were 12 men and 7 women,with an average age of 41.2 years (from 20 to 70 years).Selective open reduction and internal fixation was not applied for all the cases until their soft tissue condition was improved.American Orthopaedic Foot and Ankle Society (AOFAS) and visual analogue scale (VAS) were used to evaluate the outcomes.Results Of this cohort,18 cases were followed up for an average of 4 years (from 1 to 7 years).Their postoperative AOFAS midfoot scores ranged from 56 to 97 points,averaging 81.9 points;their VAS scores ranged from 0 to 7 points,averaging 1.7 points.Two patients reported persistent pain due to traumatic midfoot arthritis which was cured by arthrodesis after conservative therapy failed.No such complications as nonunion,malunion or implant failure occurred in the other patients.Conclusion Open reduction and internal fixation combined with bone graft and external fixation is an effective treatment for Lisfranc injury with cuboid compressive fracture,because it can restore the alignment of lateral column and achieve rigid stabilization.

ABSTRACT

Objective To analyze the epidemiology and clinical characteristics of the children with measles. Methods Retro-spectively analyzed methods were applied to analyze the timing, and season of the break, age distribution, clinical manifestations and major complications of 3431 children with measles (<12 years) in the Second Hospital of Nanjing from 2008 to 2015. Results It found that there was a peak incidence of measles in children each in 2009, 2013, and 2015 , peaking in March to May. Onset age of measles mainly within the first year of life, and incidence increased with age within the first 8 months. Typical clinical manifestations are mainly fever, rash, cough, and catarrhal symptoms. The main complications of measles were pneumonia, laryngitis and heart failure. Conclusions Early diagnosis and effective treatment of the children with measles needs to be strengthened.

ABSTRACT

Objective To understand the changes of schistosomiasis epidemic situation,so as to provide the evidence for for?mulating schistosomiasis control strategy in the Hexi reservoir area. Methods From 2012 to 2015,Xinyuan Village,Meishan Town in the north entrance of Hexi reservoir was selected as a monitoring site. According to the requirements of the monitoring program of schistosomiasis surveillance in Zhejiang Province,the Schistosoma japonicum infection was investigated by using the serological screening(IHA),and the basic situation of the surveillance site was also investigated. Results From 2012 to 2015,167 environments(21.68 hm2)were surveyed,and 2 slices(0.1 hm2)were found with Oncomelania hupensis snails. The detection rate of frames with snails was 0.12%,and the living snail density was 0.0192 snails per 0.1 m2. Totally 374 snails were dissected and no schistosome infected snails were found. A total of 970 local residents and 8 748 mobile people were investigated with the serological tests,and no schistosome infected people were found. In addition,3 085 cattle were investigated and no in?fected ones were found. Conclusion The schistosomiasis epidemic situation is stable in the Hexi reservoir area,but we still should strengthen the monitoring of imported source of infection and snail status,and increase the efforts of environmental trans?formation.

ABSTRACT

Aim To investigate the effects of Guaveno-ic acid (GA)on proliferation,insulin synthesis and secretion in INS-1 cells and their possible mechanism. Methods INS-1 βcells were cultured in vitro.Control group,medium group,model group,drug groups and positive group were set.INS-1 cells were treated with GA (0.3,1 ,3,1 0,30 nmol·L -1 )for 48 h.The cell proliferation was tested by MTT assay.Acid-alco-hol was used to extract the insulin in the cells and the amount of insulin synthesis of INS-1 cells was tested by RIA.5.6,1 6.7 mmol·L -1 glucose was used to chal-lenge INS-1 cells for 1 h to the insulin secretion model (BIS and GSIS)was tested,and the insulin secretion of INS-1 cells was tested via RIA.The mRNA expres-sion of insulin gene,PDX-1 and MafA was tested by q-PCR.Results Compared with medium group,GA could promote the proliferation of INS-1 cells signifi-cantly (P <0.01 )and promote the synthesis of insulin in INS-1 cells significantly (P <0.01 ).GA(0.3 ~30 nmol· L -1 )could promote the BIS,GSIS of INS-1 cells significantly (P <0.05 or P <0.01 ).GA (3,30 nmol·L -1 )could up-regulate the mRNA expression of insulin gene,PDX-1 ,MafA in INS-1 cells signifi-cantly (P <0.01 ).Conclusions GA could signifi-cantly improve the proliferation of INS-1 cells and pro-mote the insulin synthesis and secretion of INS-1 cells, which may be associated with up-regulation of insulin gene,PDX-1 ,MafA mRNA expression.

ABSTRACT

Objective To investigate the expression and clinical significance of survivin and PTEN in bladder transitional cell carcinoma(TCC).Methods The expression of survivin and PTEN was studied by S-P immunohistochemisty in 10 cases of normal urinary bladder tissues and 62 cases of the bladder TCC. Results All the normal urinary bladder tissues showed positive staining pattern of PTEN protein, negative staining pattern of survivin protein, positive rate of PTEN in 62 cases of the bladder TCC is 46.8%,the difference of positive rate between grade Ⅰ and grade Ⅲ was significant(P<0.05),the difference of positive rate between Tis~T_1 and T_2~T_4 showed a trend of decreasing, but there was no statistically significant difference(P>0.05);positive rate of survivin in 62 cases of the bladder TCC is 56.5%,the difference of positive rate between grade Ⅰ and grade Ⅲ was significant(P<0.05),the difference of positive rate between Tis~T_1 and T_2~T_4 was significant too(P<0.05).The expression of PTEN protein and survivin protein was reversely correlated(P<0.05).Conclusion The abnormal expression of PTEN protein and survivin protein play an important role in the occurrence and progress of the bladder TCC.It can be regarded as a useful diagnostic marker in the bladder TCC.

ABSTRACT

Objective To establish an animal model to investigate the effects of prolonged vaginal distension on the leak-point pressure(LPP) and urethral anatomy.Methods Eighteen virgin female rats were randomly divided into control group(6 rats) and experimental group(12 rats).In the experimental group,the rats were underwent vaginal distension for 4 h. The rats in the control group were not received vaginal distension.The greatest bladder volume and LPP were measured,and sneeze test was performed for all rats in two groups 4 weeks after distension.All the rats were killed after the test and the bladder and urethra were harvested for histological study.Results The greatest bladder volume and LPP were significantly decreased than those of control group.Stress urinary incontinence(SUI) was occurred in 45.5% rats of the experiment group,but none in control group.Histological studies showed that the urethral striated and smooth muscle became thin,but the collagenic tissue was increased.There was no change in control group.Conclusion Prolonged vaginal distension can cause SUI and it is a desirable animal model for studying the mechanism and treatment of SUI.
