ABSTRACT

Objective:To explore the safety and efficacy of ultrasound-assisted facial filler injection, based on the anatomy of facial vessels to prevent intravascular embolization.Methods:From Jan. 2019 to Sep. 2020, 142 patients were treated with facial soft-tissue filler injection (mean age, 39.7 years; 131 female and 11 male). According to the patients' own will, autologous fat or hyaluronic acid was applied respectively. When injecting, the assistant could press over the periorbital artery to temporarily occlude the artery, confirmed with Doppler ultrasound, thus reduced the risk of intravascular embolization, and carefully injected with minimal pressure and tiny amount.Results:A total of 142 patients were enrolled in the study, and 54 patients were treated with autologous fat grafting, while 88 patients were injected with hyaluronic acid. The injection sites included forehead, temple, glabella, nasal root, tear trough, nasolabial fold, cheek, chin, and lips. Facial rejuvenation improvement was satisfied with a smooth contour and proper augmentation. No vascular embolization occurred. 9 patients received a second or third round fat grafting to achieve better outcome. Follow-up duration ranged from 1 month to 6 months.Conclusions:With ultrasound assistant, digital pressure over the orbital artery could temporarily occlude the artery and may reduce the risk of intravascular embolization. The simple technique may add a significant benefit with no additional cost or risk to the patients.

ABSTRACT

Objective@#To explore the clinical effect of cervico-acromial fasciocutaneous flap based on the supraclavicular artery as a method for face and neck reconstruction.@*Methods@#From Oct 1990 to June 2014, 17 patients, with the age range of 7 to 51 years, suffering from cicatricle contractures of neck or facial defect, were treated with cervico-acromial fascicutaneous flaps. 29 flaps were used in 12 patients bilaterally or multiply. The donor sites of 2 patients were closed by skin graft; while in the rest 15 patients, the donor sites were closed by expanded flaps.@*Results@#All the 29 flaps were survived, with satisfactory color and texture. Hematoma occurred in 1 case after tissue expander implantation, but the flap was successfully expanded after evacuation of hematoma. The size of flaps ranged from 15 cm×7 cm to 35 cm×15 cm. 10 patients completed 6 months follow-up, and presented with satisfactory appearance and better functional results.@*Conclusions@#Cervico-acromial fascicutaneous flap based on the supraclavicular artery is an good choice for reconstruction of neck and facial defects.

ABSTRACT

Objective@#To observe the safety and efficiency of face-lift combined with fat grafting in facial rejuvenation.@*Methods@#We performed a retrospective study, which included 23 patients. SMAS suspension and multi-site suspension were combined to correct the nasolabial fold, mid-cheek aging and malar mounds. Structural fat grafting was performed to treat the volume loss in mid-face.@*Results@#All patients demonstrated a significant improvement in midfacial appearance. No infection or nerve injury were found in this study. Only three patients did not get primary healing in temple region, which led to temporal hair loss from secondary healing.@*Conclusions@#This study demonstrates that fat grafting and multiple layers face-lift are efficient method for facial rejuvenation. These approaches appear to be very promising for facial anti-aging techniques.

Intestinal microecology is an important and complex biological system necessary for human health.Its disorder is involved in the development of various diseases of human body.The technology of intestinal microbiota transplantation can effectively regulate the intestinal flora,repair the imbalance of the intestinal microecology,and bring a new breakthrough for the treatment of many diseases of gastrointestinal tract and outside gastrointestinal tract.However,there is still no systematic and complete management standard for intestinal microbiota transplantation technology.This paper discussed related content involved in standardized management of intestinal microbiota transplantation technology and reflected the ethical problems involved in standardized management from the perspective of medical ethics,in order to promote the clinical application of intestinal microbiota transplantation technology.

ABSTRACT

OBJECTIVE:To prepare Ferulic acid/K/β-CD/metal organic framework (FA/K/β-CD/MOF) inclusion,and to opti-mize its preparation technology. METHODS:K/β-CD/MOF was synthesized by solvothermal method as inclusion material. Using FA as main component,FA/K/β-CD/MOF was prepared by grinding method. The preparation technology was optimized by orthogo-nal test using mole ratio of main component-inclusion material,grinding time,dropping time and inclusion temperature as factors, inclusion rate as index. Prepared FA/K/β-CD/MOF was indentified by IR spectrum and DSC,and inclusion rate and dissolution rate were determined. RESULTS:Optimized preparation technology was as follows as mole ratio of main main component to inclusion material 3∶1,dropping time 60 min,inclusion temperature 40 ℃,inclusion time 60 min. Prepared FA/K/β-CD-MOF had already formed a new kind of phase,and its average inclusion rate was(18.0±1.6)%(RSD=0.9%,n=6);its solubility was 15 times as much as FA(9.582 mg/ml vs. 0.647 mg/ml). CONCLUSIONS:FA/K/β-CD/MOF is prepared successfully;and the preparation tech-nology is reasonable and feasible.

ABSTRACT

BACKGROUND:After co-culture with osteoblasts, bone marrow stem cells can be induced to differentiate into osteoblasts. Whether adipose-derived stem cells co-cultured with osteoblasts can differentiate into osteoblasts or not? OBJECTIVE:To observe whether adipose-derived stem cells co-cultured with osteoblasts can differentiate into osteoblasts. METHODS:Adipose-derived stem cells and osteoblasts were isolated from New Zealand white rabbits. Then, passage 3 adipose-derived stem cells were co-cultured with passage 2 osteoblasts in 10%or 5%fetal bovine serum for 14 days. RESULTS AND CONCLUSION:After 7 days of co-culture, some adipose-derived stem cells became round in the two groups. After 14 days of co-culture, adipose-derived stem cells highly differentiated and differentiated cells were similar to mature osteoblasts that were positive for alkaline phosphatase staining and alizarin red staining. The mRNA expression of type I col agen and osteocalcin increased in both two group, especial y in the 10%fetal bovine serum group. These findings indicate that adipose-derived stem cells co-cultured with osteoblasts can differentiate into osteoblasts induced by high-concentration serum culture.

ABSTRACT

Objective To observe the antidiarrheal and analgesic effects of Dingguier paste on the mouse. Methods Tho diarrhea mouse models were made by senna leaf or castor oil to detect the antidiarrheal effect of Dingguier paste combination with other drugs. The hot plate test and acetic acid test were used to observe the analgesic effect of Dingguier oral medicine and Dingguier paste in combined use with other drugs. Results Compared with senna leaf model group, Dingguier paste low-dose, middle-dose, high-dose combined with diphenoxylate, Bupi-Yichang pill and Dingguier oral medicine group could decrease the number of feces in 2 h [Number of each group feces was(0.50±1.27), (2.27±2.90), (1.18±1.83);(2.00±2.26), (3.40±2.17), (3.82±1.72);(0.73±1.56), (1.91±2.95), (2.55±2.11)]. Dingguier paste low-dose and high-dose combined with diphenoxylate could decrease the number of feces in 4h [Number of each group feces was (6.70±2.11), (6.27±3.20)]. Dingguier oral group decrease the number of feces in 6h [Number of feces was (6.91±2.77)]. Compared with castor oil model group, Dingguier paste low-dose combined with diphenoxylate could obviously decrease the number of feces in 2 h, 4 h and 6 h [Number of each group feces was(0.45±0.82), (0.45±0.82), (4.27±2.15)]. Dingguier paste middle-dose combined with diphenoxylate could decrease the number of feces in 6 h [Number of feces was (4.64±2.45)]. Dingguier oral group decrease the number of feces in 2 h, 4 h, 6 h [Number of each group feces was (0.45±0.82), (2.91±2.07), (4.27±2.15)].Compared with normal group, the threshold of pain stimulated by hot plate of the Dingguier paste each dose combined with rotundin could increased remarkable after 30 min, 60 min, 90 min and 120 min[The time of Dingguier paste low-dose was(52.32 ± 17.29)s, (56.24 ± 12.48)s, (57.28 ± 6.61)s, (58.58 ± 4.70)s; the time of Dingguier paste middle-dose was (48.27±19.35)s, (54.92±12.59)s, (55.91±11.48)s, (55.15±12.49)s;the time of Dingguier paste high-dose was (49.15±15.69)s, (56.24±11.89)s, 60 s is(59.08±2.93)s]. Compared with normal group, the eclipse time of pain irritated by acetic acid of the Dingguier paste middle-dose combined with indomethacin could increased remarkable(14.72±5.99)min, the writhing frequency could decrease of the Dingguier paste each dose combined with indomethacin[Number of each dose writhing frequency was (7.62±8.31), (3.62± 6.14), (11.25±9.46)]. Conclusion Dingguier paste in the combination use with other drugs and Dingguier oral medicine may have some antidiarrheal and analgesia effects.

ABSTRACT

BACKGROUND:Calcitonin gene-related peptide has been confirmed to induce osteogenic differentiation, but whether it can induce the osteogenic differentiation of adipose-derived stem cells under three-dimensional culture to construct tissue-engineered bone is rarely reported. OBJECTIVE:To investigate the feasibility of exogenous calcitonin gene-related peptide to induce osteogenic differentiation of adipose-derived stem cells combined with calcium alginate gel in three-dimensional condition. METHODS:Adipose-derived stem cells were gained by col agenase I digestion of the subcutaneous adipose tissue of both inguinal regions of New Zealand rabbits. Passage 3 cells were mixed with sodium alginate to prepare calcium alginate gel, and then the cells were assigned into two groups and cultured in 24-wel plates. Adipose-derived stem cells in the control group were cultured in Dulbecco’s modified Eagle’s medium/Ham’s nutrient mixture F-12 supplemented with 10-2 mol/Lβ-glycerophosphate sodium, 10-7 mol/L dexamethasone, 50 mg/L ascorbic acid, 10%fetal bovine serum. While, adipose-derived stem cells in the experimental group were incubated with the same medium as above, but 1.5 μg/L calcitonin gene-related peptide was added. The cells proliferation and the mRNA expressions of col agen I and osteocalcin were detected by 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyltetrazolium bromide and reverse transcription-PCR respectively, and alkaline phosphatase and calcium concentration were detected at different induction time. RESULTS AND CONCLUSION:The cellproliferation curves were S shaped. The absorbance values of the experimental group were higher than those of the control group at1, 3, 5, 7, 14, 21 days after osteogenic induction (P<0.05). Alkaline phosphatase and alizarin red staining of adipose-derived stem cells was al positive, but golden round nodes became more and bigger in the experimental group compared with the control group after 2 weeks. At 7 and 14 days, col agen I and osteocalcin mRNA expressions were higher in the experimental group than in the control group. Alkaline phosphatase activity and calcium concentration of the experimental group were higher than those of the control group at 1, 2, 3, 4 weeks after osteogenic induction (P<0.05). Results showed that the calcitonin gene-related peptide can induce the osteogenic differentiation of adipose-derived stem cells combined with calcium alginate gel.

ABSTRACT

Objective To explore the diagnostic values of vascular endothelial growth factor (VEGF) and p -human chorionic gonadotropin( β -hCG) for early ectopic pregnancy. Methods The serum levels of VEGF and β-hCG were measured in 109 patients with suspected ectopic pregnancy by ELISA and RIA respectively, receiver operating characteristic (ROC) curvers were established, the diagnostic values of VEGF and β -hCG for ectopic pregnancy were analyzed. Results One hundred and nine patients with suspected ectopic pregnancy were diagnosed in the end: 62 cases with ectopic pregnancy and 47 cases with intrauterine pregnancy. The levels of VEGF in ectopic pregnancy were significantly higher than those in intrauterine pregnancy (P < 0.05), but the levels of β -hCG were significantly lower than those in intrauterine pregnancy (P < 0.05). The areas under ROC curves of VEGF were 0.87, the areas under ROC curves of β -hCG were 0.71. In VEGF > 140 ng/L for diagnosis of ectopic pregnancy, the sensitivity and specificity were 75.8% (47/62) and 80.9% (38/47) respectively. In β-hCG < 1200 U/L for diagnosis of ectopic pregnancy, the sensitivity and specificity were 64.5%(40/62) and 61.7%(29/47) respectively. The sensitivity and specificity of combined detection of VEGF and β -hCG were 80.6%(50/62) and 57.4%( 27/47) respectively. Conclusions The diagnostic values of VEGF are better than those of β -hCG. The combined detection of VEGF and β -hCG can increase the rate of early diagnosis and shorten observation time.

ABSTRACT

Objective To investigate the effects of gabapentin(GBP) on plasma β-endorphin(β-EP) level in the patients with painful diabetic neuropathy(PDN). Methods We detected the plasma β-EP level in 24 PDN patients in the treatment with GBP, 18 PDN patients without GBP treatment, 20 diabetic mellitus patients without PDN and 24 healthy control subjects. Results (1)The level of β-EP in diabetic mellitus patients was lower than in the healthy control(P<0.01). (2)The patients who received GBP had a significant change of β-EP level after treatment(P<0. 01).. Conclusions Gabapentin is effective for the treatment of PDN and its adverse effects are mild. It can lower the plasma β-EP level in the patients with PDN.

ABSTRACT

Objective To study the reasons and prophylaxes of conversion to laparotomy during gy-necological laparoscopic surgery. Method Clinical records of 36 cases of conversion to laparotomy out of 1143 cases of gynecological laparescopic surgery were analyzed retrospectively. Results The rate of conver-sion to laparotomy was 3.15%. The reasons of conversion to laparotomy were abdominopelvic cavity adhesion in 23 cases, technical difficulty and complication in 8 cases, malignant tumor in 3 cases, accounting for 63.89%, 22.22%, 8.33%, respectively. The rate of conversion to hparotomy in patients with a history of laparotomy was significantly higher than that without a history of laparotomy. The rate of conversion to laparoto-my in early stage of developing laparoscopic surgery was significantly higher than that in other stages. Con-clusions Abdominopelvic cavity adhesion is the primary reason of conversion to laparotomy during gyneco-logical laparoscopic surgery. Careful evaluation before surgery and improving ability of performance can de-crease the rate of conversion to laparotomy.

ABSTRACT

[Objective]To assess the early diagnosis and treatment and outcome of patients with crush syndrome in earthquake disaster.[Method]We conducted a retrospective analysis of 27 patients with crush syndrome and subsequent acute renal failure (ARF) and wound complication who were treated in our university hospital.All of patients had been extricated from buildings that collapsed in the Wenchuan earthquake.Crush injury involved the upper extremities and low extremities.Each patient received intravenous fluids and diuretic drugs.Twenty-one patients received hemodialysis.Emergency fasciotomy performed in 11 patients and amputation were performed in 16 patients,4 to 102 hours after extrication.[Result]No patient died.Twenty-one patients among the ARF patients were retracted from hemodialysis.Among the patients who were performed fasciotomy or amputation,13 patients had wound complication including infection,however,the wounds in 10 patients were healed by dressing change,debridement and coordinate treatment with ICU and related subjectives.[Conclusion]Crush syndrome requires urgent recognition and prompt surgical treatment with simultaneous measures to control hyperkalemia and ARF.Wound healing is a difficult task for the crush syndrome patients.The authors believe that immediate intensive care therapy and multisubjective coordination would improve the survival rate.

ABSTRACT

[Objective]To retrospectively analyze the therapy comparison between the unilateral hinged external fixator assisting the internal fixation, soft tissue repair and reconstruction and routine surgery for elbow instability of fracture dislocation.[Method]A total of 117 cases of elbow fracture dislocation were collected from Jan. 2001 to Jun. 2005. Some patients were treated by unilateral hinged external fixator, and others were treated by routine surgery and casting external fixation.The JOA assessment was applied for the pain, function, arc of motion, articular instability and deformity of elbow joint before surgery and after operational follow-up. [Result]Eighty-six cases were followed up by telephone and mail appointment.The average follow-up time was 18.6 months (from the range of 8 months to 30 months).The function and arc of motion of two group patients were assessed by JOA in post-operational follow-up, showing statistical significant difference (P0.05). The total assessment score between two groups indicates statistical significant difference.[Conclusion]The operation in combination with unilateral hinged external fixator for treating elbow instability of fracture dislocation will enhance lateral stability of elbows, guaranteen early safe elbow function exercises, facilitate stable environment for ligaments healing,suggesting a good subsidiary operational method.

ABSTRACT

Objective To evaluate the clinical treatments and results of traumatic osteoarthritis of the hip joint after acetabular fracture. Methods One hundred and seven patients with acetabular fractures admitted from March 1995 to December 2003 were reviewed. Twenty-five cases of traumatic osteoarthritis were found during an average follow up of 2.5 years (8 month to 5.5 years). There were 18 male and 7 female with an average age of 42.7 years (28 to 71). According to Letournel classification, there were anterior-column fractures in 3 patients, anterior-wall in 2, posterior column in 3, posterior wall in 6, posterior wall and column in 3, transvers combined posterior wall in 5, and both-column in 1 patients. The operative related complication of heterotopic ossification was found in 6 , and bone defect in 3 patients. The pathological changes ofⅠ-stage was in 4,Ⅱ-stage in 5, Ⅲ-stage in 10,Ⅳ-stage in 5, and Ⅴ-stage in 1 patient. The mean Harris score of these patients was 52.6 points. Fourteen cases of traumatic osteoarthritis were treated with conservative treatment and 11 cases with total hip arthroplasty. Results Fourteen patients with conservative treatment were followed up 16 months in average (8 to 30 months) .The symptom of traumatic osteoarthritis in 12 cases had improved with the same initial radiographic findings and 2 cases deteriorated with narrowing of the joint space and hyperplasia. The latest mean Harris score for these patients was 61.8 points in average. Eleven patients with total hip arthroplasty were followed up for an average of 32 months(13 to 60 months) and the latest mean Harris score was 87.6 points in average. There were no complications related to the surgery occurred. Conclusion The traumatic osteoarthritis of acetabular fractures after operation is relative higher. The quality of reduction is an important factor influencing the long-term results of acetabular fractures and resulting traumatic osteoarthritis. For the patients with Ⅰ to Ⅲ-stage of pathological changes, conservertive treatment may obtain a relative good results. For the Ⅳ to Ⅴ-stage, total hip arthroplasty can gain an early satisfactory results.

Objective To observe effects of 17?-estradiol on phenylephrine-induced hypertrophy in cultured neonatal rat cardiomyocytes and to explore its mechanisms. Methods Using cultured neonatal rat cardiomyocytes treated with different factors as a model,surface area of cardiomyocytes was measured by computer photograph analysis software;the protein synthesis rate was assayed with leucine intake method;the proto-oncogene c-fos protein expression was assessed with immunocytochemistry technique;the markers of cardiomyocyte hypertrophy,embryonic genes such as ?-MHC\,?-skA and ANP mRNA expressions were assessed with semiquantitative reverse transcription-PCR. Results 17?-estradiol inhibited obviously phenylephrine-induced increase of surface area and the protein synthesis rate of cardiomyocytes,reduced c-fos protein expression of hypertrophic cardiomyocytes,and down-regulated ?-MHC、?-skA mRNA expressions and up-regulated ANP mRNA expression.Conclusion 17?-estradiol inhibits hypertrophy of cardiomyocytes and its mechanisms might be related with the suppression of c-fos protein expression,the reversion of the embryonic switching of contractile protein genes(?MHC and ?-skA),the increase of ANP mRNA expression.
