ABSTRACT

Objective:To provide a normal reference range for anogenital distance (AGD) in full-term neonates and to investigate factors possibly affecting neonatal AGD.Methods:Neonates with gestational age ≥37 weeks who were delivered in the Ruijin Hospital Affiliated to Shanghai Jiaotong University School of Medicine from November 2017 to March 2019 were enrolled.General information on mothers and newborns were collected and neonatal AGD were measured. The distance from the male anus center to the base of the scrotum was determined to be the male AGD, and the distance from the female anus center to the posterior labia was recorded as the female AGD.The effects of maternal and neonatal factors on neonatal AGD were analyzed. Results:A total of 1 078 newborns were included, including 586 males and 492 females.Male AGD [(22.90±3.80) mm] was significantly greater than female AGD [(11.80±2.10) mm] ( t=22.316, P<0.05). The AGD of singleton neonates was significantly greater than that of neonatal twins [males: (23.01±3.82) mm vs.(21.18±1.88) mm, females: (11.89±2.08) mm vs.(10.98±1.75) mm, t=26.185, 18.326, all P<0.05]. Neonatal gestational age, birth weight, head circumference and length were significantly associated with AGD (all P<0.05). Maternal factors (including age, height, weight, body mass index, gravidity, parity, occupation, etc.) were not significantly associated with neonatal AGD (all P>0.05). The AGD of 10 children with genital malformation was significantly smaller than that of males with normal genital appearance [(22.89±1.99) mm vs.(23.55±3.78) mm]( t=15.362, P=0.006). Conclusions:The reference ranges of AGD in full-term males and females in Shanghai are(22.90±3.80) mm and (11.80±2.10) mm, respectively. The neonatal gestational age, birth weight, head circumference and length may be the intrinsic factors affecting neonatal AGD.

ABSTRACT

Staff volunteers play the roles of both hospital staff and volunteers, whose volunteer services bear special significance for hospital management. The authors reviewed voluntary services provided by staff of Beijing Children′s Hospital affiliated to Capital Medical University in recent years, and concluded that staff volunteers feature better medical service quality, optimized medical process, doctor-patient harmony and better soft power of the hospital. In view of existing problems, they came up with proposals in terms of innovation forms, institutional improvement and incentive mechanisms, for better performance of such volunteers in hospital management.

ABSTRACT

The German ( Kooperation für Transoarenz und Qualit?t im Gesundheitswesen, KTQ) certification system was officially launched in Germany in December 2001. Over ten years′ development proves that the KTQ certification system as a high standard medical management system. The authors introduced its background, the update of certification content and procedures, and explained its characteristics and advantages. These insights provide valuable references for establishing a professional and efficient quality certification system, suitable for China′s social and economic development level.

ABSTRACT

Objective To investigate the mechanism of action of toll-like receptor 2 (TLR2),myeloid differentiation factor 88 (MyD88),and matrix metalloproteinase-9 (MMP-9) in cerebral ischemia-reperfusion injury in rats. Methods One hundred twenty-fiwe adult male SD rats were randomly divided into sham operation group, cerebral ischemia group and T2.5 (TLR2 antagonist) treatment group.A model of middle cerebral artery occlusion was induced by suture method.T2.5 (0.121 2 μg/g) was injected into jugular vein at the beginning of reperfusion in the T2.5 treatment group.Cerebral infarction volume, brain edema, bloodbrain barrier permeability and neurological deficit score were measured at 24 h after reperfusion.Western blotting was used to determine the expression levels of TLR2,MyD88 and MMP-9 at different time points in the ischemic cortex. Results Western blot analysis showed that compared with the sham operation group, the expression levels of TLR2 and MyD88 in the ischemic group increased significantly from 6 h after ischemia-reperfusion and lasted for 24 h (all P < 0.05),while MMP-9 increased significantly at 24 h after ischemia-reperfusion (P < 0.05).At 24 h after ischemia-reperfusion,the blood-brain barrier permeability, brain edema degree, cerebral infarction volume, and neurological deficit score in the ischemic group were significantly higher than those in the sham operation group (all P <0.05);at this time, the expression levels of TLR2,MyD88 and MMP-9 in the T2.5 treatment group were significantly lower than those in the ischemic group (all P< 0.05),and the blood-brain barrier permeability, brain edema degree, cerebral infarction volume, and neurological deficit score were significantly reduced (all P< 0.05).Conclusion TLR2,MyD88 and MMP-9 might be involved in cerebral ischemia-reperfusion injury.TLR2 antagonist T2.5 might inhibit the expression of MMP-9 through TLR2-MyD88 signaling pathway, thus alleviating cerebral ischemia-reperfusion injury.

ABSTRACT

BACKGROUND:Angiotensin II receptor antagonists have been found to exerct a stronger protective effect on bone than angiotensin converting enzyme inhibitors. OBJECTIVE:To investigate the effect of different concentrations of irbesartan (angiotensin II receptor antagonist) on the differentiation and mineralization of mouse preosteoblasts. METHODS:Mouse preosteoblast cel lines MC3T3-E1 in logarithmic phase were selected and cultured in the osteogenic induction medium containing 0 (control group), 0.001, 0.01, 0.1 mmol/L irbesartan, respectively. Ten days later, the cel differentiation was observed by alkaline phosphatase staining. The mineralization was observed by alizarin red staining after 21 days of culture. mRNA expressions of osteocalcin, alkaline phosphatase and Runt-associated transcription factor 2 in osteoblasts were detected by real-time PCR at 1, 4, 7, 14 and 21 days of culture. RESULTS AND CONCLUSION:The activity of alkaline phosphatase in al the irbesartan groups (0, 0.001, 0.01, 0.1) was higher than that in the control group (P<0.05), which was the most obvious in 0.01 mmol/L. The number and area of calcium nodules in each irbesartan group were significantly higher than those in the control group (P<0.05), especial y in 0.01 mmol/L. Compared with the control group, 0.01 mmol/L irbesartan significantly upregulated the mRNA expressions of osteocalcin, alkaline phosphatase and Runt-associated transcription factor 2 (P<0.05). These results suggest that 0.01 mmol/L irbesartan significantly promotes the differentiation and mineralization of osteoblasts.
