ABSTRACT

Objective:To explore the clinical characteristics and distribution of Tranditional Chinese Medicine (TCM) syndrome types of refractory tic disorders in children based on clustering analysis.Methods:A cross-sectional study. 183 children aged 3-18 years with refractory tic disorders from pediatric encephalopathy outpatient department of the First Teaching Hospital of Tianjin University of Traditional Chinese Medicine who met the inclusion criteria from October 2015 to January 2022 were recruited. Frequency descriptive analysis, systematic clustering analysis and principal component analysis were conducted by Microsoft Office Excel 2016 and IBM SPSS Statistics 26.0 to explore the clinical characteristics and syndrome distribution of the disease.Results:The TCM symptoms of 183 children with refractory tic disorders were clustered into 6 categories, and finally 5 types of syndrome were obtained, among which the syndrome of spleen deficiency and liver hyperactivity mixed with dampness accounted for the largest proportion (27.32%), followed by the syndrome of phlegm-fire disturbing spirit (21.31%), the syndrome of wind-heat invading lung (18.03%), the syndrome of phlegm-qi stagnation (17.49%), and the syndrome of dampness and yin deficiency (15.85%).Conclusion:Through the mining and analysis of the outpatient information of pediatric encephalopathy in our hospital, the common syndrome differentiation types of refractory tic disorder are obtained, which can provide a reference for the TCM syndrome differentiation types of refractory tic disorder in children.

ABSTRACT

OBJECTIVE:To evaluate the efficacy and safety of Bacterial lysates(hereinafter referred to as"Broncho-Vaxom") for recurrent respiratory tract infections (RRTIs) of children,and to provide evidence-based reference for clinic. METHODS:Retrieved from PubMed,EMBase,Cochrane Library,CBM,CNKI,Wanfang database and VIP database,domestic and foreign published randomized controlled trials (RCTs) about Broncho-Vaxom (trail group) vs. placebo (control group) for RRTIs of children were collected during database establishment to Jan. 2018. After literature scanning and data extraction,the risk of bias of included trials were evaluated by using Cochrane 5.1.0 risk bias evaluation tool. Meta-analysis was performed by using Rev Man 5.3 software. RESULTS:A total of 13 RCTs involving 1 228 children were included. The results showed that the trial group was superior to control group in frequency of respiratory infection [MD=-1.14,95%CI(-1.29,-0.99),P<0.001],total response rate [RR=9.47,95%CI(2.33,38.54),P=0.002],the time of antibiotics use [MD=-4.36,95%CI(-6.52,-2.21),P<0.001], infection duration [MD=-3.89,95%CI(-4.47,-3.04),P<0.001],febrile time [MD=-1.81,95%CI(-3.40,-0.22),P=0.03],serum immunoglobulin (Ig)G level [MD=1.25,95%CI(0.13,2.37)),P=0.03],IgA level [MD=0.77,95%CI(0.07, 1.46),P=0.03] and the level of T cell subgroup CD4+[MD=1.33,95%CI(0.90,1.76),P<0.001] and CD8+[MD=0.64,95%CI (0.24,1.04),P=0.002],there was statistical significance. Trail group was similar to control group in respect of cough time [MD=-6.00,95%CI(-13.86,1.86),P=0.13] and IgM level [MD=-0.10,95%CI(-0.32,0.12),P=0.39] and the incidence of ADR [RR=0.76,95%CI(0.43,1.35),P=0.35]. CONCLUSIONS:The current evidence shows that Broncho-Vaxom could effectively prevent the RRTIs of children with good safety.

ABSTRACT

[Objective]To explore the correlation of the expression of alpha-fetoprotein(AFP)mRNA in the peripheral blood and postoperative survival and metastasis of patients with liver cancer.[Methods] A total of 66 patients with liver cancer who received radical resection surgery from January 2005to December 2006 was enrolled in this study.The cell total RNA was extracted from peripheral blood and the expression of AFP mRNA was detected by nested PCR.All the patients were followed up for 60 months after surgery.[Results]The expression rate of AFP mRNA in the peripheral blood was 40.91%(27/66).The expression of AFP mRNA in the peripheral blood in patients with liver cancer was significantly related to microvascular invasion and metastasis(P < 0.05 or < 0.01),but the expression had no relationship with sex,age,HBV infection,cirrhosis,AFP concentration,tumour size and number,and Edmondson grading(P>0.05).The overall 1,2,and > 3 years survival rates of patients with positive AFP mRNA after surgery were 66.7%(18/27),38.9%(7/18),28.6%(2/7),respectively.The overall 1,2,and ≥3 years survival rates of patients with negative AFP mRNA after surgery were 84.6%(33/39),60.6%(20/33),45.0%(9/20).There was statistical significance between the survival rates of AFP mRNA-negative patients and AFP mRNApositive patients(P < 0.01).[Conclusions] The detection of AFP mRNA in the peripheral blood may provide clue for early microscopic metastasis.It can be a prediction index for postoperative recurrence.

Mycoplasma pneumoniae is the common pathogen of community-acquired pneumonia in children. In recent years,the incidence of mycoplasma pneumoniae pneumonia(MPP) has increased year by year. And it is found that case reports about intractable MPP at home and abroad have also significantly increased. The treatments of MPP become the focus in clinical physicians. This paper summarized about progress of MPP in antibiotic therapy,immune therapy and other ancillary treatment.

ABSTRACT

Chinese medicine has certain advantages in treating the disease of children ADHD which caused by brain dysfunction. According to the physiological characteristics of "kidney is often insufficient" in children, and the close relations among kidney, marrow, essence and brain, we put forward the pathomechanism hypothesis that growth retardation of spinal marrow caused ADHD, and thought that the def iciency of kidney essence, growth retardation of spinal marrow, and imbalance of yin and yang (yang superabundant and yin insufficient), are the keys of pathomechanism of ADHD. Accordingly, the tonifying kidney and replenishing essence treatment principle was established, and YiZhiNingShen granule was made which had been conf irmed effectively by clinical and experimental research.

ABSTRACT

Epilepsia is a frequent neural system disease of pediatrics,treatment based on syndrome differentiation in Traditional Chinese Medicine is more efficacious to certain extent.Because symptoms are multiplex and patients often go to see a doctor in resting stage,it is difficult to differentiate sydromes.During years of clinical practice,professor Ma Rong used the knowledge of electroencephalogram,attack type of Western Medicine,symptoms,remote cause,et al to diagnose and treat pediatric epilepsia,and procured positive curative effect.It will not only raise the level of treatment based on syndrome differentiation,but also have significant meanings in the objectivization of treatment based on syndrome differentiation of Traditional Chinese Medicine.

ABSTRACT

This article introduces two medical records of children's influenza A (H1N1). The first case is a child of influenza A (H1N1) complicated with pneumonia and it is differentiated as warm-heat syndrome. The treatment methods are dispelling wind and relieving exterior syndrome, clearing heat in qi level and removing heat at first, and then followed by dispersing lung-qi and clearing away heat pathogen, relieving cough and dissipating phlegm. The second case is a child of influenza A (H1N1) complicated with pneumonia, hypokalemia and exacerbation of primary nephrotic syndrome. The syndrome belongs to wet-heat from the beginning, so the treatment is promoting urination and eliminating turbidity, clearing heat and detoxicating, and combined with antiviral oseltamivir. The syndrome is phlegm-heat obstructing in the lung in the second phase of the disease, and the treatment is removing heat-phlegm, dispersing lung-qi and relieving cough.

ABSTRACT

Objective To construct recombinant adenovirus vector containing Rattus norvegicus microtubule-associated protein 4 gene,and transfect it into the rat cardiac myocytes cultured in vitro.Methods A pair of primers were designed,and full length MAP4 DNA was cloned from rat total mRNA by PCR.The PCR product was double-digested with restriction endonucleases NheⅠ and NocⅠ,and inserted orientationally into pShuttle2.The plasmid of pShuttle2-MAP4 was double-digested with restriction endonucleases NheⅠ and NocⅠ,and inserted BD Adeno-X~(TM) Virul DNA,named pAd2-MAP4.The non-recombinant adenovirus was screened out with PacⅠ, pAd2-MAP4 was linerized with SwaⅠ,and the recombinant virus genome was transfected into HEK293 cell line for packaging and amplification of Ad-MAP4 virus.The recombinant adenovirus was transfected into rat cardiac myocytes and MAP4 was identified by immunohistochemistry.Results The recombinant adenovirus-MAP4 was constructed successfully and the titer was about 2.3?10~(8) pfu/ml.The expression of MAP4 was enhanced at 48 h after the transfection.Conclusion We have successfully constructed a recombinant adenovirus Ad-MAP4 that has enforced the expression of MAP4 in vivo.

ABSTRACT

Objective To investigate whether microtubule disassembly plays an important role in the pathogenesis of the opening of mitochondria permeability transition pore (MPTP) in hypoxic cardiomyocytes and the decrease of its activity, resulting in its hypoxic injury. Methods Neonatal rat cardiomyocytes in primary culture were randomized as normoxia group (A), hypoxic group (B), normoxia treated with microtubule destabilizing agent (Colchicine) group (C), hypoxia treated with microtubule stabilizing agent (Taxol) group (D). At 0.5, 1, 3, 6, 12 h after treatment, polymeric tubulin was detected by immunofluorescence and Western blotting, mitochondria permeability transition pore (MPTP) open by coloading with calcein AM and cobalt chloride, and the activity of cells by measuring the mitochondrial-dependent reduction of MTT to formazan. Results Early microtubule disassembly, MPTP open and activity decrease of cardiomyocytes in both groups B and C were observed at 0.5 h after treatment. These phenomena all became more and more significant with the prolongation of treatment. However, microtubule disassembly, MPTP open and activity decrease of cardiomyocytes of group D were significantly lower than those of group B. Conclusion Microtubule disassembly happened at 0.5 h after hypoxic treatment. Microtubule stabling agent Taxol and destabilizing agent Colchicine can regulate microtubule integrity efficiently. The microtubule damage plays an important role in the hypoxic injury of cardiomyocytes.
