ABSTRACT

Objective:To study the clinical value of the ISBAR communication model based on nursing priorities in nursing ward rounds. Methods:Using a quasi-experiment method, 104 patients who received secondary and tertiary care from April to June 2020 were set as the control group, and 122 patients who received secondary and tertiary care from July to September 2020 were set up as the experimental group, with non-randomized method. The experimental group used the ISBAR communication model based on nursing priorities for nursing ward rounds, and the control group used the traditional method for nursing ward rounds. The abilities of bedside nurses collecting the patients′condition, the bedside nurses′ condition observation and the nurses′ critical thinking were compared between the two groups, and the patients' nursing quality and satisfaction were also compared. Results:The scores of the bedside nurses′ knowledge of the patients′ condition were 81.14±3.09 in the test group and 75.90±2.84 in the control group, with statistically significant difference ( t value was -12.88, P<0.01). The scores of the bedside nurses′ ability in observing patients′ condition were 140.50 (137.00, 144.50) in the experimental group and 135.50 (128.75, 139.00) in the control group, with statistically significant difference ( Z value was -3.399, P<0.01). The scores of the Critical Thinking Disposition Inventory Chinese-version (CTDI-CV) were 309.82±36.33 in the experimental group and 286.86±33.54 in the control group, with statistically significant difference ( t value was -2.177, P<0.05). The scores of patient care quality evaluation were 62.50 (60.00, 64.00) in the experimental group and 59.00 (55.00, 61.50) in the control group, with statistically significant difference ( Z value was -6.47, P<0.01). The patient discharge satisfaction score of the experimental group was 53.00 (48.75, 55.00), and that of the control group was 51.00 (44.00, 54.00), with statistically significant difference ( Z value was -3.56, P<0.01). Conclusions:The application of ISBAR communication model based on nursing priorities in nursing ward rounds had a significant promotion effect on the improvement of nurses' abilities of collecting the patients′ condition, observing patients′ condition, and critical thinking, and can effectively improve nursing care quality and patients′ satisfaction.

ABSTRACT

Objective@#To investigate nutritional quality of school lunch in some primary schools and middle schools in the Pearl River Delta, and to provide the scientific basis for improving the nutritional quality of students lunch and formulating scientific and effective interventions.@*Methods@#Five-day lunch meal survey by chemical analysis were conducted, and students lunch at school were recorded by meal review in three age groups from 8 primary and middle schools in the Pear River Delat area. The energy and nutrient content were obtained and compared with the reference intake of dietary nutrients of student.@*Results@#The average protein intake at lunch of all age groups had reached the recommended standard (80%-95%), the energy supply ratio of carbohydrate in the range of 38.3%-42.3%, the energy supply ratio of fat in 63% school meal exceeded the recommended standard. Vitamin A, vitamin B 1, vitamin B 2, calcium, iron and other nutrients were seriously inadequate; while sodium intake far exceeded the recommended standard.@*Conclusion@#The main nutrients of school lunch of primary and middle school in Pearl River Delta can basically meet the growth and development needs, but there are still some deficiency and unbalanced diet nutrient content which are lower than the recommended intake. It is recommended to strengthen nutrition education of catering enterprises and school to improve the scientific combination of diets.

ABSTRACT

Objective To evaluate the liver damage induced by chemotherapy in patients with cancer and positive for hepatitis B virus (HBV) markers. Methods From January 2005 to January 2012,913 cancer patients were treated by chemotherapy including HBV-positive patients (HBV-positive group,288 cases) and HBV-negative patients (HBV-negative group,625 cases). The changes of hepatic function after chemotherapy between two groups were compared. Results The incidence of hepatic toxicity in HBV-positive group was higher than that in HBV-negative group [24.0% (69/288) vs.11.4% (71/625)],and there was significant difference between two groups (P< 0.01).The incidence of degree 11Ⅲ-Ⅳ hepatic toxicity was 11.4% (14/123) in HBV-DNA-positive patients and 0.6% (4/625) in HBV-negative group. In a variety of chemotherapy, there was significant difference in the incidence of hepatic toxicity in TP(paclitaxel +cisplatin),CAF(cyclophosphamide + doxorubicin + fluorouracil),CHOP(cyclophosphamide + doxorubicin +vincristine + prednisone) between two groups (P < 0.05).The incidence of hepatic toxicity was highest in TP,which was 34.6% (18/52) in HBV-positive group and 16.5% (20/121) in HBV-negative group. Conclusion HBV infection is associated with higher risk of hepatic toxicity in patients with cancer during chemotherapy.

ABSTRACT

Objective To observe the effecacy of modified CHOP regimen in the initial treatment of non-Hodgkin' slymphoma (NHL) patients with low Karnofsky score. Methods Twenty initial treatment NHI.patients with low Karnofsky score were randomly allocated into treatment group and control group, respectively treated with modified and standard CHOP regimen. Results Of 13 cases in treatment group,12 pacients'Karnofsky score were over 60.1 was still below 60,overall response (OR) rate was 92.3 % (12/13).while in control group,3 patients' Karnofsky score were over 60,4 were still below 60.OR rate was 100 % (7/7),rate of marrow toxicity was 57.1 % (4/7).There were no significantly difference for OR rates (P =0.4515),but for improving Karnofsky score there were significantly difference (P =0.0149).Conclusion Modified CHOP regimen can improve Karnofsky score of NHL patients with low Karnofsky score, benefical for further treatment and reducing hospitalization cost.

ABSTRACT

Objective To explore the relations between physiological retrogressive auricular fibrillation and organical pathological auricular fibrillation in elderly patients. Methods 36 cases paroxysmal auricular fibrillation in elderly patients without organical pathogeny were monitored for 24 hours using dynamic electrocardiogram. The attack time and frequency of paroxysmal auricular fibrillation, the change of heart rate, and other type arrhythmia complexing were analyzed. Results Most cases of paroxysmal auricular fibrillation in elderly patients were companied with arrhythmia, supraventricular arrhythmia occupy in the first place especially. The ventricle rate was slow in most cases. The attack had different rhythm in day and night, there were two flows and one edd in one day. Conclusion The elderly auricular fibrillation is associated with the physiological anaplasia of auricular muscle and artro-ventricular junction area directly. It also relates to the heighten excitability of pneumogastric nerve and secretive conduction.
