ABSTRACT

Objective To analyze the status and trend for the mortality and DALY rates of child growth failure (CGF) in children aged < 5 years in China from 1990 to 2019, so as to provide a scientific basis for CGF prevention and control. Methods The mortality and DALY rates of CGF in children aged < 5 years from 1990 to 2019 were obtained from GBD 2019. The changes of these indicators with the years in China , the United States, Japan, Russia, India and the global were compared and analyzed. Results In 2019, the mortality of child wasting, child stunting and child underweight in children aged < 5 years in China were 9.62/100 000, 1.23/100 000, and 1.29/100 000 respectively, the mortality rates were 867.50/100 000 , 129.23/100 000 , and 112.87/100 000 rescpectively, higher than those of the United States, Japan, and Russia, and far lower than those of India and the global. The disease burden of three types of CGF were all higher in males than females, and higher in children aged < 1 years than children aged 1-4 years. From 1990 to 2019, the mortality and DALY rates of CGF in children aged < 5 years in China decreased from 300.41/100 000 and 26 445.38/100 000 to 10.49/100 000 and 943.57/100 000, respectively. China had the largest drop rate compared with all analyzed countries. As for children aged < 5 years in China, the DALY rate of lower respiratory infection ranked first in all the diseases caused by CGF. Conclusion From 1990 to 2019, the disease burden of CGF in children aged < 5 years has shown a significant decrease in China , but it is still far behind the developed countries. In the future, more attention should be paid to the problems of child growth in hope of reducing the mortality and DALY rates of CGF.

ABSTRACT

Objective:To compare the difference and effect of fat grafting assisted by adjustable external volume expansion (EVE) and fat grafting only in female patients who chose autologous fat grafting for breast reconstruction after a breast cancer operation.Methods:A retrospective analysis was carried out in 17 patients in the past four years. The patients in the experimental group wore EVE 10 hours daily for four weeks before surgery, and the negative pressure value was -60 mmHg. From the second week after the operation, they continued to wear EVE 10 hours every day, and the initial negative pressure value was -40 mmHg. After one week, the negative pressure was adjusted to -20 mmHg, and the EVE was worn for four weeks after surgery. Both the experimental group and the control group chose classical Coleman fat for breast reconstruction.Results:The number of operations in the experimental and control groups was 3.0±0.8 and 3.9±1.2, respectively ( t=2.193; P<0.05). The single fat injection volume of the experimental group and the control group was (228.60±15.34) ml and (198.20±12.01) ml, respectively ( t=4.861; P<0.01). The single fat volume preservation rate of the experimental group and the control group was (31.6±5.8)% and (25.8±6.2)%, respectively ( t=2.226; P<0.05). For postoperative complications, there were 3 cases in the experimental group (10 cases in total) and 3 cases in the control group (7 cases in total). Conclusions:For breast cancer patients who choose autologous fat grafting for breast reconstruction, wearing EVE can reduce the number of operations, improve the single fat injection volume and postoperative fat preservation rate, and reduce postoperative complications.

ABSTRACT

Objective To investigate the effectiveness of anterior fixation with an oblique-ilioischial plate for acetabular fractures of lower posterior column.Methods A consecutive series of 7 patients were retrospectively reviewed who had been treated operatively from August 2016 to July 2017 for acetabular fractures of lower posterior column.They were 4 men and 3 women,aged from 45 to 62 years (average,48.6 years).The injury was caused by a traffic accident in 4 cases and by falling from a height in 3.By the Letournel-Judet classification,there were one case of transverse fracture,3 cases of T-shaped fracture,2 cases of anterior + posterior hemitransverse fracture and one case of associated both column fracture.The intervals from injury to operation ranged from 6 to 19 days (average,7 days).All of them were treated by the modified Stoppa approach (or combined with an iliac fossa approach).The anterior column was stabilized with a reconstruction plate for the iliac wing along the iliopectineal line to the pubis in all cases.The lower posterior column was fixated with a newly developed oblique-ilioischial plate running from the ilium to the ischial ramus.Operative time,intraoperative blood loss,reduction quality and postoperative function of the affected hip were recorded.Results The operative time for this cohort ranged from 1.0 to 3.2 h (average,2.1 h) and the intraoperative blood loss from 200 to 500 mL (average,300 mL).The postoperative reduction was rated by the Matta's criteria as excellent in 5 cases,as good in one and as fair in one.This cohort was followed up for 7 months (from 6 to 9 months).At the final follow-up,the hip function was rated by the modified Merle d'Aubigne-Postel scale as excellent in 5 cases,as good in one and as fair in one.Fracture healing was achieved in all the patients after 3 to 6 months (average,4.5 months).No implant failure,infection,heterotopic ossification or neurovascular injury occurred during the follow-up.Conclusion Anterior fixation with an oblique-ilioischial plate via the modified Stoppa approach or combined with an iliac fossa approach is a good treatment for acetabular fractures involving the lower column,due to its advantages of reliable fixation,limited invasion,little intraoperative bleeding and few complications.

ABSTRACT

OBJECTIVE To analyze the clinical and genetic features of 10 unrelated patients with duplications of 15q11q13 region and autism features.METHODS Karyotyping,chromosomal microarray analysis (CMA) and fluorescence in situ hybridization (FISH) were carried out for the patients and their parents.RESULTS Eight patients presented with a supernumerary marker chromosome (SMC) of unknown origin by G-banding analysis and triplication of the 15q11q13 region by high-resolution CMA analysis. Two remaining patients had normal karyotypes but duplications of the 15q11q13 region. All duplications have encompassed the Prader Willi/Angelman syndrome critical region (PWACR). Similar gains in copy number were not detected among the parents of the patients,suggesting a de novo origin for them. Analysis of SNP-array data of the family trios using Chromosome Analysis Suite Software found that the copy number gains have originated from the mothers.The diagnosis of 15q11q13 duplication syndrome was ascertained. For patients with SMC detected by karyotyping analysis,a FISH assay using probes specific for the 15q11q13 region showed that such SMC also derived from chromosome 15q11q13 region and contained two copy numbers, which was consistent with the result of CMA.CONCLUSION Ten patients with autism and 15q11q13 duplications were identified with combined karyotyping, CMA and FISH analysis. A phenotype - genotype correlation was established.

ABSTRACT

Objective To investigate the effectiveness of ilioischial plating through modified Stoppa and iliac fossa approaches for complex acetabular fractures. Methods A consecutive series of 40 patients with complex acetabular fracture were treated operatively from January 2014 to February 2015. Of them, 20 were treated through modified Stoppa plus iliac fossa approaches as an experimental group ( including 12 males and 8 females with a mean age of 46. 8 ± 10. 3 years ) . The anterior column was stabilized with a recon-struction plate for the iliac wing along the iliopectineal line to the pubis in all cases. The posterior column was fixed with a newly developed ilioischial plate running from the ilium to the ischial ramus. The other 20 patients ( 10 males and 10 females with a mean age of 45. 6 ± 11. 7 years ) served as a control group, treated with a reconstruction plate for the conventional posterior column fixation through the Kocher-Langenbeck approach. The 2 groups were compared in terms of operative time, intraoperative blood loss, reduction and functional recovery of the hip. Results The 40 patients obtained an average follow-up of 18 months ( from 8 to 24 months ) . The experimental group reported significantly shorter operation time ( 2. 1 ± 0. 7 hours ) and signifi-cantly less intraoperative bleeding ( 320. 8 ± 100. 4 mL ) than the control group ( 2. 9 ± 0. 6 hours and 621. 6 ± 118. 7 mL, respectively ) ( P 0. 05 ) . There were no significant differences in the MOS item short form health survey score and postoperative complication rate between the experimental group and the control group. Conclu-sion Ilioischial plating through modified Stoppa and iliac fossa approaches has advantages of reliable fixa-tion, limited invasion, less intraoperative blood loss and fewer complications for complex acetabular fractures.

ABSTRACT

Objective To analyze the developments and frontiers of systemic lupus erythematosus (SLE) research in order to make possible breakthroughs in subsequent studies.Methods Information visualization software Citespace V was applied to draw the mapping knowledge domain based on the database of SLE literature in Science Citation Index Expanded of Web of Science from 1975 to 2016.In this way,it was possible to analyze the basis of knowledge,network relationships between countries or regions,hot spots and courses frontier evolution in the recent 40 years.Results Totally 39 847 articles of literature were collected.The USA,UK,Spain,Italy and Australia were known as the leading countries in the study of SLE.The relationships between SLE and other autoimmune diseases,autoantibody detection and significance,anticardiolipin antibody expression,gene expression,classification criteria,disease activity were hot spots of SLE.Depression of SLE patients,management of SLE and differentiation of SLE were eye-catching research frontiers in recent years.Conclusion The mapping knowledge domain can display the main hot spots in research of SLE,show the frontier spheres and evolution course of related branches of learning,reveal the potential information in complex giant data networks,and guide future studies.

ABSTRACT

BACKGROUND:Single scaffold materials have some shortcomings to some extent or in different ranges. Therefore, new composite materials are developed in recent year, which are compounded by two or more materials with complementary characteristics in a certain manner or ratio. OBJECTIVE:To investigate the histocompatibility and osteogenic induction of geneX artificial bone combined with bone marrow mesenchymal stem cel s and transforming growth factor beta 2 (TGF-β2). METHODS:Injectable geneX was co-cultured with bone marrow mesenchymal stem cel s for 5 days, and under the electron microscope, the histocompatibility of the artificial bone was observed. Passage 3 rabbit bone marrow mesenchymal stem cel s were divided into three groups:simple cel group cultured with osteogenic medium, cel scaffold group cultured with geneX+osteogenic medium, combined group cultured with geneX+TGF-β2+osteogenic medium. After 7, 14, 21 days, cel morphology, alkaline phosphatase activity detection, MTT detection, methyl thymol blue detection and alizarin red staining were performed. RESULTS AND CONCLUSION:After osteogenic induction, bone marrow mesenchymal stem cel s were fibroblast-like cel s and adhered to the surface of geneX bone with strong secretion of extracel ular matrix. Cel proliferation and osteogenic activity in the combined group were stronger than those in the simple cel and cel scaffold groups (P<0.05). The alkaline phosphatase activity was also higher in the combined group than the other two groups. These findings indicate that the geneX/bone marrow mesenchymal stem cel s/TGF-β2 composite has good histocompatibility and pro-osteogenic differentiation ability.

BACKGROUND:Clinical y, bone marrow mesenchymal stem cel s combined with artificial bones for early osteonecrosis of the femoral head have a wonderful outcome. OBJECTIVE:To review the biological properties of bone marrow mesenchymal stem cel s and to summarize the application progress of bone marrow mesenchymal stem cel s combined with different artificial bones in the treatment of early osteonecrosis of the femoral head. METHODS:PubMed (2003-2013), FMJS (2003-2013), Wanfang (2005-2013), CNKI (2005-2013) and CBM (2005-2013) databases were retrieved by computer using the keywords of“bone marrow mesenchymal stem cel s;artificial bone;osteonecrosis of the femoral head”in Chinese and English. RESULTS AND CONCLUSION:It wil lower the pressure of the femoral head, accelerate repair of the blood capil ary, improve the blood supply, osteoblast proliferation and differentiation, and thus delay or even prevent artificial joint replacement after osteonecrosis of the femoral head by applying bone marrow mesenchymal stem cel s combined with different artificial bones, such as corl ine hydroxyapatite, calcium hydroxylaptite, biological ceramics and calcium sulfate bones. But now, there are stil a lot of problems which need to be solved, including pathological mechanism underlying bone marrow mesenchymal stem cel s for treatment of osteonecrosis of the femoral head, obvious difference between the quantity and quality of seed cel s because of individual difference, different sites and culture techniques. So, artificial bone materials are under review, and large-sample randomized control trials are required. Its long-term outcomes also lack for fol ow-up observation, as wel as there is no a unified quantitative standard for the appropriate selection of indication, curative effect evaluation and the awareness of the operation.

ABSTRACT

AIM: To investigate the effect of puerarin on expression of the glucogen synthase kinase-3 bate (GSK-3?) in adipocyte of rats with insulin resistance induced by diets. METHODS: 30 Wister rats were randomized into two groups: control group (n=10) and model group (n=20).Control group were fed with general diets, while model group were fed with the high fat diets to induce insulin resistance. After 4 weeks, the models have formed then were randomized into two groups: model group (n=10) and puerarin group (n=10). Puerarin group were treated with puerarin (100 mg?kg -1 ?d -1 , ip) for 6 weeks.GSK-3? protein was detected by Western-blot analysis. Body weight, serum triglyceride and cholesterol, fasting plasma glucose and serum insulin concentration were detected termly and the insulin sensitivity index was calculated. RESULTS: The model group showed hyperglycemia, hyperinsulinism, obviously obesity and the insulin resistance index (ISI) decrease by high fat diets, insulin resistance was induced by this way. ABSTRACT

Objective To observe the effects of exercise training on the expression of glycogen synthase ki-nase-3 (GSK-3) in the skeletal muscles of rats with insulin resistance induced by a high-fat and high sucrose diet. Methods Thirty Wistar rats were randomly divided into three groups: a control group, a model group and an exercise group. Rats in the model and exercise groups were fed with a high-fat and sucrose diet (20% sucrose, 10% lard, 2.5% cholesterol, 1% cholic acid) for 4 weeks, and then the rats in the exercise group were trained by swimming 5 d every week for 6 weeks. After treatment, the concentration of GSK-3 protein in their muscles was detected by Western blot analysis, while their weights, serum triglyceride and total cholesterol, and their concentrations of fasting plasma glucose and fasting plasma insulin were measured periodically and their insulin sensitivity index was calculated. ABSTRACT

AIM: To develop a rat model of insulin resistance and observe the effect of puerarin injection on expression of GSK-3 in insulin resistance rats.METHEDS: 30 Wistar rats were divided into three groups randomly: control group,model group and puerarin group.The model group and puerarin group were given high fat diet(20% surcose,10% ard,(2.5)% chlesterol,1% cholic acid) for 4 weeks.Then the puerarin group was given abdominal injection puerarin 100(mg?kg~(-1)?d~(-1)) for six weeks.At the end of this experiment,we detect GSK-3 protein by western-blot analysis.Then computer image pattern analysis system was used to analyze the expression of GSK-3 in the hepatacyte.RESULTS: Tthe model group showed a hyperglycemia,hyperinsulinism,the ISI decreased and HOMA-IR increased.GSK-3 expression enhanced in the model group,but the expression of GSK-3 in puerarin group decreased compared to the model group.CONCLUSION: Puerarin injection can significantly decreased the expression of GSK-3 and improve insulin resistance.
