ABSTRACT

Objective:To investigate the effects of propofol and sevoflurane on post-traumatic stress disorder (PTSD) after emergency surgery in trauma patients.Methods:A total of 160 trauma patients undergoing emergency surgery under general anesthesia were randomly divided into the propofol group and the sevoflurane group. The perioperative clinical data of the two groups were collected. The incidence of PTSD was evaluated by PCL-5 score one month after the operation in the two groups. The relevance of the injury time and PCL-5 score was assessed by Spearman correlation analysis. Logistic regression analysis was used to analyze the risk factors of PTSD.Results:The incidence of PTSD in the propofol group was significantly higher than that in the sevoflurane group at postoperative 1 month (24.0% vs 10.8%, P=0.034). The injury time was negatively correlated with PCL-5 score in the propofol group ( r=0.229, P<0.01). There was no correlation between the injury time and the PCL-5 score in the sevoflurane group ( r=0.001, P=0.804). Logistic regression analysis showed that the use of propofol was an independent risk factor for PTSD ( P=0.004). Conclusions:Sevoflurane anesthesia is more effective than propofol anesthesia in reducing the occurrence of PTSD in emergency surgery for trauma patients.

ABSTRACT

BACKGROUND:Metabonomics has been proved to analyze and observe the pathological process of rat myocardial infarction and the underlying mechanism. OBJECTIVE:To further analyze the metabolomic pathways of bioinformatics in rat models of myocardial infarction. METHODS:The experimental studies about rat myocardial infarction were retrieved from CNKI, WanFang, CqVip, PubMed and Embase databases. The metabolic products described in the literatures were col ected and summarized. Signaling pathways were analyzed using KEGG database molecular function annotation, the enzymes, translocators and their properties were analyzed by HMDB database. Metabolites pathway were visualized with MetPA. RESULTS AND CONSLUSION:A total of 26 metabolic products were identified in the included literatures and mainly participated in 29 metabolic pathways. Through topology analysis, 5 of the 10 metabolic pathways were selected and regarded as the metabolic pathways of myocardial infarction in rats, including aminoacyl-tRNA biosynthesis;glycine, serine and threonine metabolism;valine, leucine and isoleucine biosynthesis;biosynthesis of unsaturated fatty acids;phenylalanine, tyrosine and tryptophan biosynthesis. In conclusion, the bioinformatics analysis of metabolites in rats with myocardial infarction show that myocardial infarction is related to the metabolism and metabolic pathways of carbohydrates, proteins, fat and RNA.

ABSTRACT

<p><b>OBJECTIVE</b>To evaluate the antibacterial property of ultraviolet rays (UV)-treated titanium and its alloy by observing the attachment and colony-forming units (CFU) of Staphylococcus Aureus (Sa) and Fusobacterium Nucleatum (Fn).</p><p><b>METHODS</b>Both of the titanium and its alloy were divided into 2 groups according to the UV treatment protocol as the UV-treated group (UV-Ti, UV-A), and two control groups (C-Ti, C-A) were also prepared. Then the sand-blasted and acid etched titanium and its alloy disks were stored in the dark and sealed containers for 4 weeks and treated with or without UV light for 48 hours. The hydrophilicity of disks were measured using contact angle measuring device. The attachment of the two kinds of bacteria were observed using inverted fluorescence microscope after stained by the Hoechst 33342 which was part of blue fluorescent dyes used to stain DNA and the results were replenished and verified by the Safranine O staining. CFU were used to measure the antibacterial property of the materials.</p><p><b>RESULTS</b>The contact angles of UV-Ti (1.32 ± 0.29)° and UV-A (0.89 ± 0.07)° were smaller than those of C-Ti (81.18 ± 1.46)° and C-A (83.34 ± 0.06)° (P < 0.05). After being cultured on the surface of the two kinds of materials for 15 min, 30 min and 1 h, the attachment of Sa and Fn on the surface of titanium [(31 ± 13, 73 ± 13, 263 ± 49), (10 ± 7, 26 ± 19, 105 ± 37)] and its alloy disks [(64 ± 19, 57 ± 16, 304 ± 53), (11 ± 5, 59 ± 18, 124 ± 53)] which were treated with the UV light were much lower than those in control titanium groups[(139 ± 30, 271 ± 51, 749 ± 172), (164 ± 16, 227 ± 65, 483 ± 64)] and its control alloy group [(174 ± 11, 387 ± 39, 1409 ± 262), (265 ± 45, 374 ± 133, 910 ± 103)] (P < 0.05). What's more, the CFU number of the two kinds of bacteria decreased sharply after the UV treatment (P < 0.05).</p><p><b>CONCLUSIONS</b>UV treatment can reduce the attachment and viability of different kinds of bacteria and the antibacterial effect will at least last for 24 hours.</p>

ABSTRACT

Objective To analyze the efficacy and cosmetic results of intensity modulate radiation therapy (IMRT) for breast cancer after breast-conserving surgery.Methods From 2003 to 2006, 117 patients with breast cancer, after breast-conserving surgery followed by 4 - 6 cycles of chemotherapy,received intensity modulated radiotherapy (IMRT) or three-dimensional conformal radiotherapy (3DCRT).The radiation dose was 50 Gy in 25 fractions to the whole breast and 10 Gy boost to the tumor bed.Patients with positive hormone receptors then received endocrine treatment.Results The follow-up rate was 94.0% until September 2009.114 and 91 patients were followed up to 3 and 5 years, respectively.The 3-and 5-year overall survival rates were 99.1% and 96%.The 5-year disease free survival and local recurrence rates were 88% and 3.6%.Cosmetic results were satisfied.Severe radiation toxicities, such as radiation pneumonitis, pulmonary fibrosis and heart injury were not found.Conclusions Patients treated with IMRT after breast-conserving surgery have a satisfied prognosis as well as cosmetic results.

ABSTRACT

Objective To evaluate the influence of a clinical pathway on mean length of hospital stay of ischemic stroke patients. Methods The study performed retrospective cohort study of stroke patients which were hospitalized in the Guangdong provincial hospital of TCM corresponding to the periods immediately before and after the implementation of the clinical pathway. The outcome measure was mean length of hospital stay. Results Mean length of hospital stay was significantly shortened after implementing clinical pathway. Conclusions The implementation of the ischemic stroke clinical pathway can shorten mean length of hospital stay of ischemic stroke patients and improve the utilization of medical source.

ABSTRACT

Objective:To study the stress distribution of the mandibular premolar cervix during clenching and mastication. Methods:Models of anisotropic enamel of the mandibular first premolar and the opposite maxillary first premolar crown were establishied. The stress distributions in the cervical region of the mandibular premolar were investigated in different loading conditions: simulating working, nonworking and vertical micromotions. In each case, the stroke was applied to the maxillary first mandibular premolar in a stepping procedure using nonlinear contact analysis. Results:The stress in the cervical region under loading condition simulating vertical micromotion was lower than that under loading conditions simulating working and nonworking micromotions. The stress was mainly concentrated near the buccal or the lingual cervical enamelo-dentinal junction under loading conditions simulating lateral excursion. Conclusion:The stress distribution in the cervical region is closely related to the pattern of occlusal force including location and direction.
