ABSTRACT

Objective:Based on knowledge visualization analysis to explore the progress of diabetic cardiomyopathy from 2000 to 2020.Methods:Literatures related to diabetic cardiomyopathy indexed in Web of Sciences databases ranging from 2000 to 2020 were collected. Excel, VOSviewer, and CiteSpace 5.0.R1 software were used for visual analysis of countries, research institutions, authors, keywords and related citations.Results:A total of 2 536 articles were collected preliminarily. China has published the most articles in this field through visualization. A total of 909 related articles have been published in the past 20 years, and China has been closely communicated internationally. The United States and Canada ranked the second and the third respectively, totaling 723 and 201. For the distribution of research institutions, the University of Louisville ranked first with 86 articles. A few top high-impact authors, such as Lu Cai and Tan Yi, are from this institution and have published the most articles, indicating that the University of Louisville can be regarded as an international academic center in the field of diabetic cardiomyopathy. Domestic Jilin University and Shandong University followed, ranking the second and the third with 70 and 66 papers respectively. Keyword co-occurrence analysis shows that research hotspots in this field at home and abroad mainly focus on the investigation of the clinical manifestations and pathogenesis of diabetic cardiomyopathy. Finally, through the co-cited literature network, cluster #4 (SGLT-2 inhibitor) is still in evolvement. The development process shows that the treatment is a new trend in the field of diabetic cardiomyopathy research.Conclusions:Knowledge visualization analysis can visually exhibit the intellectual structure, evolution process, hotspots and trending in the field of diabetic cardiomyopathy research. The results showed that the main research power was located in the China, but the cooperation among personnel needs to be strengthened. The future research trend is mainly focused on the treatment of diabetic cardiomyopathy.

ABSTRACT

Sudden cardiac arrest survivors are a special group of patients, often with unique and complex needs that are not adequately addressed by current treatments. In May 2020, the American Heart Association (AHA) developed a scientific statement on sudden cardiac arrest survivorship based on relevant research developments, which emphasized the importance of a unified patient-centered assessment and resources. This article attempts to interpret the key content of the scientific statement, hoping to provide reference and help for domestic medical staff to understand and apply it.

ABSTRACT

To explore the effect of intensive insulin therapy (IIT) on high mobility group box-1/nuclear factor-κB (HMGB1/NF-κB) signaling pathway in severe traumatic brain injury (sTBI) patient with stress hyperglycemia. Methods Sixty-one sTBI patients with stress hyperglycemia [Glasgow coma scale (GCS) ≤ 8, three times of random blood glucose levels > 11.1 mmoL/L, glycosylated hemoglobin (HbA1c) < 0.065] admitted to the Affiliated Huaian No.1 People's Hospital of Nanjing Medical University from July 2015 to October 2017 were enrolled. Patients were divided into IIT group (29 cases, keeping blood glucose at 4.4-7.8 mmol/L) and conventional glycemic therapy (CGT) group (32 cases, keeping blood glucose at 7.8-12.2 mmo/L) according to the random number table method. Before treatment and 1, 7 and 14 days after treatment, the levels of plasma HMGB1 and tumor necrosis factor-α (TNF-α) were measured by enzyme linked immunosorbent assay (ELISA); C-reactive protein (CRP) was determined by automatic biochemical analyzer, and NF-κB p65 gene expression in peripheral blood mononuclear cells was detected by real-time quantitative polymerase chain reaction (PCR). Results Nine patients were withdrawn from the observation because the 4 consecutive blood glucose monitoring did not reach the target value, combined with severe infection, or abandoned the treatment with serious brain damage. Finally, 52 patients were enrolled in the analysis, including 28 in CGT group and 24 in IIT group. The levels of plasma HMGB1, TNF-α, CRP and the expression of NF-κB gene in monocytes of the two groups at 1 day after treatment were significantly higher than those before treatment, and reached the peak value, then gradually decreased. After 7 days of treatment, they were significantly lower than 1 day. The levels of plasma CRP and TNF-α in the IIT group were significantly lower than those in the CGT group [CRP (mg/L): 36.7±4.4 vs. 45.1±6.1, TNF-α (ng/L): 42.4±9.7 vs. 53.2±9.1, both P < 0.05], the level of HMGB1 in plasma and the expression of NF-κB p65 in monocytes were significantly lower than those in the CGT group after 14 days of treatment [HMGB1 (μg/L): 60.1±8.7 vs. 80.5±9.1, NF-κB p65 (ΔCt): 35.8±8.5 vs. 53.5±7.3, both P < 0.05]. Conclusion IIT inhibits the inflammatory response in sTBI patients with stress hyperglycemia through HMGB1/NF-κB pathway.

ABSTRACT

Objective Resarech on the correlation between serum tumor markers CA19-9,CEA,CAS0 and the early diagnosis and staging of pancreatic cancer.Methods To compare the serum levels of the CA19-9,CEA,CA50 in 51 patients with pancreatic cancer,10 benign tumor patients and 12 chronic pancreatitis patients admitted to the Department of general surgery,Anhui Provincial Hospital from January 2013 to October 2015.Results CA19-9 expression in different stages of pancreatic cancer and other pancreatic diseases were statistically significant (P < 0.05).The difference of CEA and CA50 expression in pancreatic cancer and other pancreatic diseases was statistically significant (P<0.05).But the CEA in stage Ⅰ and stage Ⅱ,stage Ⅰ and stage Ⅲ,stage Ⅱ and stage Ⅲ patients with clear expression of the difference was not statistically significant (P > 0.05).The was no significant difference in the expression of CA50 in pancreatic cancer stage Ⅰ and stage Ⅱ,Ⅲ and Ⅳ (P > 0.05).The positive rate of CA19-9 in the diagnosis of pancreatic cancer was higher than that of CEA and CA50.The positive rate of CA19-9,CEA,CAS0 and were the highest in the diagnosis of pancreatic cancer.Conclusions The expression levels of CA19-9,CEA and CA50 in serum have a certain relevance to the early diagnosis of pancreatic cancer,and the value of CA19-9 is the highest in the stage of pancreatic cancer.

ABSTRACT

NKG2D is an activating receptor expressed on various immune cells' surfaces.Currently,eight ligands have been found,they can be divided into two kinds:MHC Ⅰ related protein and ULBP molecules.NKG2D receptors and their respective ligands differentially exist in normal and cancerous tissues.So far,many researchers have found that a lot of immune cells' anti-tumor mechanism is mediated by NKG2D receptor and it can't be replaced in the resistance to the development of carcinoma.adoptive immunotherapy is a new method of treating cancer that propose to find tumor-specific cytotoxicity immune cells and enlarge the amount of it,then injected into human body to resist tumor.Recently,many researchers have found that DNT cells (CD4-CD8-T 细胞)have the function of anti-tumor;sadly its mechanisms are unclear.After the summary of NKG2D receptor-mediated pathway of destruction,this paper try to analyze its relevance to DNT cells' killing effect,on the purpose of finding the Killing mechanism of DNT cells that may existed.
