ABSTRACT

Background@#Fatty acid-binding protein 4 (FABP4) has been demonstrated to be a predictor of early diabetic nephropathy. However, little is known about the relationship between FABP4 and diabetic retinopathy (DR). This study explored the value of FABP4 as a biomarker of DR in patients with type 2 diabetes mellitus (T2DM). @*Methods@#A total of 238 subjects were enrolled, including 20 healthy controls and 218 T2DM patients. Serum FABP4 levels were measured using a sandwich enzyme-linked immunosorbent assay. The grade of DR was determined using fundus fluorescence angiography. Based on the international classification of DR, all T2DM patients were classified into the following three subgroups: non-DR group, non-proliferative diabetic retinopathy (NPDR) group, and proliferative diabetic retinopathy (PDR) group. Multivariate logistic regression analyses were employed to assess the correlation between FABP4 levels and DR severity. @*Results@#FABP4 correlated positively with DR severity (r=0.225, P=0.001). Receiver operating characteristic curve analysis was used to assess the diagnostic potential of FABP4 in identifying DR, with an area under the curve of 0.624 (37% sensitivity, 83.6% specificity) and an optimum cut-off value of 76.4 μg/L. Multivariate logistic regression model including FABP4 as a categorized binary variable using the cut-off value of 76.4 μg/L showed that the concentration of FABP4 above the cut-off value increased the risk of NPDR (odds ratio [OR], 3.231; 95% confidence interval [CI], 1.574 to 6.632; P=0.001) and PDR (OR, 3.689; 95% CI, 1.306 to 10.424; P=0.014). @*Conclusion@#FABP4 may be used as a serum biomarker for the diagnosis of DR.

Islet-specific autoantibodies (ISAs) have been used as diagnostic markers for different phenotypic classifications of diabetes. This study was aimed to explore the relationships between ISA titers and the clinical characteristics of diabetic patients.MethodsA total of 509 diabetic patients admitted to Department of Endocrinology and Metabolism at the Affiliated Hospital of Nantong University were recruited. Anthropometric parameters, serum biochemical index, glycosylated hemoglobin, urinary microalbumin/creatinine ratio, ISAs, fat mass, and islet β-cell function were measured. Multiple linear regression analysis was performed to identify relationships between ISA titers and clinical characteristics.ResultsCompared with autoantibody negative group, blood pressure, weight, total cholesterol (TC), low density lipoprotein cholesterol (LDL-C), visceral fat mass, fasting C-peptide (FCP), 120 minutes C-peptide (120minCP) and area under C-peptide curve (AUCCP) of patients in either autoantibody positive or glutamate decarboxylase antibody (GADA) positive group were lower. Body mass index (BMI), waist circumference, triglycerides (TGs), body fat mass of patients in either autoantibody positive group were lower than autoantibody negative group. GADA titer negatively correlated with TC, LDL-C, FCP, 120minCP, and AUCCP. The islet cell antibody and insulin autoantibody titers both negatively correlated with body weight, BMI, TC, TG, and LDL-C. After adjusting confounders, multiple linear regression analysis showed that LDL-C and FCP negatively correlated with GADA titer.ConclusionDiabetic patients with a high ISA titer, especially GADA titer, have worse islet β-cell function, but less abdominal obesity and fewer features of the metabolic syndrome.

ABSTRACT

ABSTRACT OBJECTIVE:To study the metabolic transformation of total glycosides of Cistanche deserticola in artificial gastric and intestinal juice,and to speculate its metabolic transformation pathway in vivo. METHODS:UPLC/Q-TOF-MS was adopted. The determination was performed on ACQUITY UPLC BEH column with mobile phase consisted of 0.2% formic acid water-acetonitrile(gradient elution)at the flow rate of 0.2 mL/min. The detection wavelength was set at 330 nm,and column temperature was 25 ℃. The ion source was electrospray ion source,and mass to charge ratio(m/z)was 50→1 000. In the positive and negative ion mode,the metabolic components of the total glycosides of C. deserticola in artificial gastric and intestinal juice were identified analysis,and combined with the literature,the metabolic pathway of total glycosides of C. deserticola in artificial gastric and intestinal juice was speculated. RESULTS:After the total glycosides of C. deserticola were metabolized by artificial gastric juice,and a total of 69 components were estimated,including 14 prototype components (such as Mustard aldehyde glucoside,daucosstorol) and 55 metabolic components (such as Methyl-O-Kankanoside J,Methyl-O-Kankanoside E),it is speculated that its metabolic pathways were methylation,demethylation,hydroxylation,methoxylation,acetylation,sulfation,and glucuronidation. After the total glycosides of C. deserticola were metabolized by artificial intestinal juice,a total of 90 components were estimated,including 4 prototype components(such as Kankanoside M,Kankanoside L)and 86 metabolic components(such as Methyl-O-Kankanoside, Methyl-O-Kankanoside E). It was speculated that its metabolic pathways were methylation, demethylation,hydroxylation,dehydroxylation,methoxylation,acetylation,sulfation and glucuronidation. CONCLUSIONS:This study preliminarily speculates that the total glycosides of C. deserticola may be metabolized by methylation,demethylation, hydroxylation and other metabolic pathway in artificial gastrointestinal juice,which may provide reference for the in vivo metabolic transformation of total glycosides of C. deserticola.

ABSTRACT

OBJECTIVE: To establish UPLC-Q- TOF-MS/MS fingerprint of Fritillaria thunbergii , and to define its anti-inflammatory quality markers. METHODS :The determination was performed on Eclipse Plus C 18 column with mobile phase consisted of methanol- 0.1% formic acid solution (gradient elution )at the flow rate of 0.4 mL/min. The column temperature was set at 30 ℃,and injection volume was 2 µL. The electrospray ion source was used to scan in the range of m/z 50-1 200 with positive and negative ion detection mode. UPLC-Q-TOF-MS/MS fingerprints of 10 batches of F. thunbergii from different habitats were established by using the Similarity Evaluation System of TCM Chromatographic Fingerprints (2004A edition ). With ear swelling degree,the serum levels of MDA and NO as anti-inflammatory indexes ,the correlation between the relative area of common peaks in fingerprint and the anti-inflammatory indexes was analyzed by using bivariate correlation analysis and grey correlation analysis , and the quality markers were screened and identified. RESULTS :In positive and negative ion mode ,10 batches of F. thunbergii had 26 peaks and 10 peaks. Based on bivariate correlation analysis and grey correlation analysis ,nine quality markers related to anti-inflammatory effect were found ,which were identified as peiminine ,peimine,cyclobalamine,daucidin,polyphyllin Ⅴ, bitumen podophyllotoxin ,phytosterol,ent-kaur-15-en-17-ol,ent-17-norkauran-16-one. CONCLUSIONS :Established UPLC-Q- TOF-MS/MS fingerprint can be used to evaluate the quality of F. thunbergii ;peiminine,peimine and cyclobalamine and so on may be the quality markers of anti-inflammatory effect of F. thunbergii .

ABSTRACT

Objective To monitor the clinical distribution of Gram-positive pathogen infection and drug-resistance situation in hospitalized children with lower respiratory tract, and guide rational application of antibiotics. Methods The isolated cultures results and drug sensitivity test result of 1 219 sputum specimens in hospitalized children with lower respiratory tract were studied. Results The 1 249 strains were isolated from 1 219 sputum specimen samples. Among which, Gram-positive pathogen was 318 strains, accounting for 25.46% (318/1 249). In 318 strains Gram-positive pathogens, staphylococcus aureus was 127 strains (39.94% ), streptococcus pneumoniae was 92 strains (28.93% ), staphylococcus epidermidis was 76 strains (23.90%), and enterococcus was 23 strains (7.23%). Then, different strains of pathogens showed totally disparate drug-resistance situations, especially towards penicillin, and the drug resistant rate was highest (89.62% , 285/318), while the drug resistant rates were also high among erythromycin, cefazolin, oxacillin, azithromycin and clindamycin: 66.67% (212/318), 52.52% (167/318), 49.06% (156/318), 49.06% (156/318) and 43.08% (137/318); meanwhile, the isolated Gram-positive pathogens showed no drug-resistance to vancomycin and linezolid. Conclusions Only using antibiotics rationally according to pathogen identification and drug sensitivity test result, can effectively control the pathogen infections.

ABSTRACT

Objective To investigate the incidence of middle ear barotrauma due to hybaric oxygen therapy by using Health Education Atlas.Methods 100 patients were divided into two groups by random number table. The research group(49 patients)was educated by Health Education Atlas.The control group(51 patients)was educated by traditional education approach.During the first three days,we observed and recorded the eardrum injury and asked patients ear discomfort everyday.Results The incidence rate of middle ear barotrauma of the research group was 6.1%,which of the control group was 19.6%.The eardrum injury of the research group was milder than the control group(χ2 =4.02,P <0.05).Conclusion Education using Health Education Atlascan reduce the incidence of middle ear barotrauma due to hybaric oxygen therapy.

ABSTRACT

Objective To study the efficacy and safety of oral calcium carbonate suspension and proton pump inhibitors (PPI) in symptoms relief of acid related diseases.Methods The multi-center,randomized and controlled study was carried out.A total of 400 acid-related diseases patients from 14 to 70 years old with at least one of moderate or severe symptoms such as acid regurgitation,epigastric pain,epigastric discomfort,upper abdominal burning or distension were equally divided into two groups and treated with oral calcium carbonate suspension or omeprazole enteric-coated tablets.The onset of symptoms relief of all patients of the first time after taking medicine was observed.The relief of clinical symptoms at the third day and seventh day after taking medicine was also observed.The adverse events were recorded.Analysis of variance was performed for statistical analysis.Results The onset of symptoms relief of oral calcium carbonate suspension group in acid regurgitation,epigastric pain,epigastric discomfort,upper abdominal burning or distension of the first time after taking medicine were significantly faster than those of PPI group (F=4.866,8.142,41.366,6.955,35.252; all P<0.05).At the third day after taking medicine,the treatment efficiency of oral calcium carbonate suspension group and PPI group were 69.50% and 72.50%,and at the seventh day the treatment efficiency of oral calcium carbonate suspension group and PPI group were 92.00% and 96.50%.There was no significant difference in treatment efficiency and comprehensive efficacy between two groups at day three and seven after taking medicine (both P> 0.05).There were 14 cases of mild adverse events and two cases of moderate adverse events in oral calcium carbonate suspension group.There were 10 cases of mild adverse events in PPI group.There was no significant difference between two groups (P>0.05).Conclusion Oral calcium carbonate suspension provides faster symptom relief than PPI and can effectively improve the symptoms of acid-related diseases.
