ABSTRACT

Objective:To analyze the clinical characteristics, diagnosis and treatment process, and prognosis of human immunodeficiency virus (HIV) positive patients with systemic lupus erythematosus (SLE).Methods:A retrospective study was used to collect and analyze the clinical characteristics, treatment and prognosis of eight HIV-positive patients with SLE treated in Yunnan Provincial Infectious Diseases Hospital from August 2017 to January 2020.Results:All of the eight patients were diagnosed with SLE after HIV infection. All of the patients were female. CD4 + T lymphocyte counts were >500/μL in four cases, 350 to 499/μL in two cases, and 200 to 349/μL in the remaining two cases. Case 6 presented with butterfly erythema on the face. In Case 1, hemoglobin was 40 g/L and urine occult blood was (+ + ). The hemoglobin of Case 2 was 76 g/L, the platelet count was 2×10 9/L, and the granulocyte count was 0.6×10 9/L. The lung computed tomography (CT) examination of Case 3 showed diffuse exudative lesions in both lungs. The 24 h urinary protein levels of Case 4 and 5 were 2 231.6 mg and 2 761.0 mg, respectively, and urine occult blood were (+ + ). The total bilirubin of Case 4 was 70.0 μmol/L and alanine aminotransferase (ALT) was 49 U/L. The total bilirubin of Case 7 was 129.6 μmol/L and ALT was 56 U/L. The lung CT examination of Case 8 showed moderate to massive pericardial effusion in the pericardium. Seven patients received antiviral therapy and immunotherapy, and their conditions were stable without relapse. Case 1 was refractory SLE complicated with autoimmune hemolytic anemia. After treated with rituximab combined with cyclophosphamide the patient achieved clinical remission. Case 7 was injection drug user and died after giving up treatment. Conclusions:The clinical characteristics of HIV-positive patients with SLE are heterogeneous, and the prognosis is generally good after antiviral therapy and immunotherapy. For patients with refractory SLE complicated with autoimmune hemolytic anemia, clinical remission can also be achieved through active treatment.

ABSTRACT

This study aimed at preparing paeonol thermosensitive gel and preliminary exploring its properties in vitro.Tube inversion method was adopted to investigate the effects of concentrations of poloxamer 407 and poloxamer 188 on gelation temperature. Then, viscosity of the gel was detected by rotary viscometer, and in vitro erosion and drug release characteristics of the gel by no film stripping method. As a result, the gelation temperature of poloxamer 407 decreased with the increase of its concentration, while gelation temperature of poloxamer 407 increased with the accelerating concentration of poloxamer 188.The cumulative drug release of paeonol thermo sensitive gel was up to 70% in 320 rin.Gel dissolution and drug release were simultaneously performed without burst release phenomenon. It was concluded that the preparation process of paeonol thermo sensitive gel was simple and easy to use with the overt effect of sustained-release.

ABSTRACT

Objective To evaluate the effects of penehyclidine hydroehloride as an atropine alternative on angioearpy and glandular secretions when premedieated in ketamine complex total intravenous anesthesia(TIVA)in children. Methods Forty patients aged 3-10 years undergoing ketamine and propofol complex TIVA were randomly divided into two groups.Penehyclidine hydrochloride(group P,n=20)or atropine(group A,n=20)was premedicated intramuscularly 30 min before anesthesia.Heart rate(HR),mean arterial pressure(MAP),breath rate(R)and the amount of saliva secretion(SS)were recorded before premedication(0 min),10 min,20 min,30 min,60 min and 150 min after. Results (1)SS reduced significantly 20 min,30 min and 60 min after premedication in both groups(P<0.01),and in 150 min,it was still in a significantly reduced level in group P(P<0.01),which was significantly lower than that in group A(P<0.01).(2)MAP,HR and R in group P showed no significant differences before and after premedication(P>0.05).But in group A,HR increased significantly at 20 min,30 min and 60 min after premedication(P<0.05 or<0.01),MAP increased significantly at 30 min and 60 min after premedication(P<0.01),and meanwhile of them were also significantly higher than those in group P(P<0.05 or<0.01).Conclusions Penehychdine hydrochloride can effectively reduce respiratory glandular secretion with longer persistence, and nearly has no influence on HR and blood pressure, which suggests it could be a superior to atropine alternative as an anesthesia premedication in children.

ABSTRACT

Objective To investigate the influent factors on normal intraabdominal pressure (IAP) and establish the IAP regressive equation. Methods The IAPs of 106 hospitalized patients were determined through monitoring the bladder pressures. The relationship between IAP and 14 factors including age, gender, height, weight, body mass index (BMI), previous and recent abdominal surgical history, reasons for admission, complications and their quantities were analyzed, respectively Results The mean IAP of the hospitalized patients was 5.5 mm Hg with a range from 0.4 mm Hg to 12.8 mm Hg. The difference among IAPs of different grades of BMIs had statistical significance ( F =5.550, P 0.05). Conclusion Normal IAP is possibly influenced by gender and BMI,and it is individually different.

ABSTRACT

Objective To explore the effect of chitosan on the prevention of postoperative peritoneal adhesion. Methods Forty-five Wistar rats were randomly assigned into group A(control group);group B and group C;2 ml of 1% lactic acid, 32% Dextran-70 or 2% Chitosan-lactic acid (CLA) was admitted on the region of serosal injury of intestine respectively during operation. The adhesive degree was observed and biopsy was taken on the 14th postoperative day. Results There were statistically significant differences among the degrees of adhesions between the three groups (P
