ABSTRACT

Objectives:To investigate the correlation between the systolic blood pressure variability and glomerular filtration rate in the elderly. Methods:From retired employees who participated in the third time physical examination of Kailuan group to underwent 24-hour ambulatory blood pressure monitoring. A total of 3 064 subjects aged over 60 years were recruited by cluster sampling method. 2 464 participants who met the inclusion were included and tested the renal function, with estimated glomerular filtration rate (eGFR) as indicators of renal function evaluation. Finally, 1 382 cases up to the standard. Multiviate regression models were performed to analyze the correlataion beteen short-term MMD and eGFR. Results:The mean age of 1 382 participants was (67.16±5.86) years, and 905 individuals (65.5%) were male. Levels of eGFR decreased with the increased of MMD (P<0.05). Pearson correlation analysis indicated that eGFR was positively correlated with 24 hr-MMD、day-MMD and night-MMD(P<0.05). Multivarite linear regrsssion analysis indicated that 24 hr-MMD、day-MMD were correlated with eGFR. Conclusions:24 hr-MMD、day-MMD are correlated with eGFR.

ABSTRACT

Objective:To explore changes of serum levels of chitinase protein-40(YKL-40),P-selectin and high sensi-tive C reactive protein(hsCRP)in patients with acute coronary syndrome(ACS)and their correlation.Methods:A total of 160 patients with coronary heart disease(CHD),including 80 ACS cases and 80 cases with stable angina pec-toris(SAP),were selected.Another 80 non-CHD patients were enrolled as normal control group.Serum levels of YKL-40,P-selectin and hsCRP were measured by enzyme linked immunosorbent assay and compared among all groups.Results:Compared with normal control group and SAP group,there were significant rise in serum levels of YKL-40[(20.92 ± 3.19)ng/ml vs.(45.18 ± 3.96)ng/ml vs.(61.84 ± 3.51)ng/ml],P-selectin[(65.01 ± 16.18) μg/L vs.(97.58 ± 31.43)μg/L vs.(137.41 ± 48.26)μg/L]and hsCRP[(6.91 ± 3.68)mg/L vs.(8.32 ± 4.21) mg/L vs.(30.01 ± 11.05)mg/L]in ACS group,P=0.001 all.Linear correlation analysis indicated that serum P-selectin and hsCRP levels was significant positively correlated with serum YKL-40 level(r= 0.46,0.041, P=0.021,0.015).Conclusion:Serum levels of YKL-40,P-selectin and hsCRP significantly rise in ACS patients,sug-gesting that these indexes are related to atherosclerotic plaque instability,which can be used as serological indexes judging atherosclerotic plaque instability.

ABSTRACT

To explore anti-tumor active components of Chimonanthus salicifolius, the phytochemistry of the chloroform fraction from leaves extract was investigated by repeated silica gel column chromatography. Twelve compounds were isolated and their structures were identified by physicochemical properties and spectroscopic data analysis as 9-epi-blumenol C(1), blumenol C(2), (+)-dehydrovomifoliol (3), (+)-vomifoliol (4), robinlin (5), (-)-loliolide (6), isofraxidin (7), scopoletin (8), 6,7-dimethoxycoumarin (9), 6, 7, 8-trimethoxycoumarin (10), beta-sitostenone (11), and beta-stigmasterol(12). Compounds 1-6 belonging to nor-sesquiterpenoids were isolated from the family Calycanthaceae for the first time. Compound 1 was a new natural product. Compounds 7, 11 and 12 were obtained from this plant for the first time.

ABSTRACT

<p><b>BACKGROUND</b>Intensive blood glucose control is proven to be associated with the diabetic microvascular and macrovascular complications, which could affect quality of life (QOL). This study was performed to determine the effects of intensive glucose control therapy on QOL of elderly patients with type 2 diabetes in Anhui Province.</p><p><b>METHODS</b>Ninety-seven elderly patients with type 2 diabetes in Anhui were randomly assigned to standard treatment group and intensive therapy group. All patients were followed up for five years on average. Correlated information has been collected during the regular follow-up.</p><p><b>RESULTS</b>Patients with microvascular complications reported significantly lower European Quality of Life-5 Dimensions (EQ-5D) scores and had more problems with usual activities, pain and anxiety than those without complications (P < 0.05). Patients having experienced hypoglycemic episodes had significantly more problems with anxiety than those without hypoglycemic episodes (P < 0.05). No significant difference was detected in all dimensions in quality of life, as well as in Visual Analog Scale score between two groups (P > 0.05). There was no significant difference in quality of life at the fifth year compared with that of the first year in both groups. Women had more feelings of pain and anxiety than men (P < 0.05) and longer disease course was associated with increased levels of pain and anxiety (P < 0.05), as well as with lower QOL. In addition, patients with higher body mass index (BMI) had more problems with daily activities than patients with lower BMI (P < 0.05).</p><p><b>CONCLUSIONS</b>Anxiety is common in elderly diabetic patients and they experienced frequent hypoglycemic episodes. Diabetic vascular complications significantly affect QOL of the patients. Intensive glucose control has no significant effect on QOL of the diabetic patients. Female, older age, long disease course, less education and high BMI are all factors caused reduced QOL and patients with these factors should be given more psychological support. Frequent mild hypoglycemic episodes do not cause impaired function of the central nervous system.</p>
