ABSTRACT

Objective To analyze the X-ray,CT and MRI findings of chordoma in the cervical vertebrae,so as to improve its diagnostic accu-racy.Methods X-ray,CT and MRI findings of 7 cases of chordoma proved pathologically were analyzed retrospectively.X-ray and CT were undergone in all 7 cases,MRI in 3 cases. Results X-ray demonstrated bony destruction in 7 cases, bony collapse in 1 case. Narrowing of inter-vertebral space was in 6 cases and soft tissue mass was in 3 cases.CT demonstrated bony destruction in 7 cases, calcification within the tumors in 2 cases, involvement of the accessory of the cervical vertebra and transverse foramen in 5 cases. Soft tissue mass was around the cervical vertebra in 4 cases.MRI was undergone in 3 cases. On T1 WI middle signal in 3 cases were showed,on T2 WI low and high mixed signal in 3 cases, and soft tissue masses around the lesion in 3 cases. Conclusion Chordoma of the cervical vertebrae is uncommon.Full understanding X-ray,CT,MRI characteristics is useful to improve the diagnosis, guide clinical surgery treatment and forecast prognosis.

ABSTRACT

Purpose:To evaluate the role of multi - detector row CT(MDCT) using reconstruction techniques in the assessment of patients with obstructed diseases of biliary tract.Materials and Methods: 47 Patients with obstructed diseases of biliary tract confirmed clinically underwent MDCT and their reconstructed images of biliary tract including multi-planar reconstruction (MPR) images and curved planar reconstruction(CPR) images were compared with those of 50 patients without obstruction and dilatation of biliary tract. The display effect of biliary duct structure and biliary duct wall and the display ability of biliary tract by MPR and CPR images between the 2 groups were compared and analysed. The reconstruction images of biliary tract were analysed retrospectively to evaluate the location and possible causes of biliary obstruction. Results: The display effect of biliary duct structure and biliary duct wall in MPR and CPR images of the group with biliary obstruction is better than that of control group, and the display ability of biliary tract in CPR images of the group with biliary obstruction is also better than that of control group. The accuracy of localization and cause evaluation of obstruction by MPR and CPR images is 100% and 89.4% respectively. Conclusions: The MPR and CPR images of MDCT provide a good display of biliary duct structure, biliary duct wall and an accurate evaluation of obstruction localization. The reconstruction technique of MDCT such as MPR and CPR should be widely applied in the evaluation of biliary obstruction.

ABSTRACT

Aim To study clinical efficacy of valsartan,in comparison with amlodipine, in hypertentive patients with left ventricular hypertrophy. Methods 65 hypertentive patients with left ventricular hypertrophy is divided into two groups, with 33 cases in valsartan group and 32 cases in amlodipine group Valsartan 80~160mg and amlodipine 5~ 10 mg were taken by the patients in the two groups for 6 months respectively. 24 h ambulatory blood pressure monitoring ( 24 h ABPM) and color echocardiography were performed in the two groups before and after treatment. Results The parameters of 24 h ABPM ( 24 h SBP? 24 h DBP?dSBP?dDBP?nSBP?nDBP) and color echocardiography (IVST?PWT?LVMI)after treatment in the two groups were significant decreased compared with those before treatment respectively (P0.05). Conclusion Valsartan can lower significantly the blood pressure level and make left ventricular hypertrophy remarkably dispelled in hypertensive patients with left ventricular hypertrophy and has the effect similar to that of amlodipine.

ABSTRACT

Objective To determine the clinical value of cytological diagnosis of the impressed specimens in the radical gastrectomy for patients with gastric cancer. Methods During radical gastrectomies for 52 patients with gastric cancer, 232 impressed specimens were taken from the tumor tissues, paratumor tissues, proximal and distal incision margin, and partial lymph nodes respectively, to detect if there was tumor cells by cytological examination and compared with the pathology. Results In the tumor tissues, there were no pseudo positive specimen, and the pseudo negative specimen occurred in 1.In the incision margin, there was no pseudo negative specimen, while 12 pseudo positive specimens were found. In the paratumor tissues, there were 3 pseudo negative and 10 pseudo positive specimens. In all specimens, the sensibility was 92.8%; specificity was 85.8%; accuracy rate was 87.9%. Conclusions Cytological examination of the impressed specimens in radical gastrectomy is helpful in detecting residue tumor tissue in the incision margin, especially in hospitals without rapid pathologic examination. If cytological examination is combined with rapid pathological examination, the results will be better.
