ABSTRACT

Objective:To investigate the clinical efficacy of oblique intervertebral fusion in the treatment of failed internal fixation of thoracolumbar fractures. Methods:A retrospective study was conducted to analyze the clinical data of 14 patients who had undergone revision surgery for failed internal fixation of thoracolumbar fracture at Department of Orthopedics, The First Hospital Affiliated to Zhengzhou University from January 2014 to December 2021. There were 6 men and 8 women with a mean age of 47.5 (42.0, 54.3) years. Fracture segments: T 12 in 2 cases, L 1 in 3 cases, L 2 in 4 cases, L 3 in 3 cases, and L 4 in 2 cases; AO classification: type A in 1 case, type B in 7 cases, and type C in 6 cases. Their prior surgical method was posterior internal fixation with pedicle screws. The revision surgery consisted of subtotal vertebral resection through the oblique lateral approach, bone column reconstruction and lateral screw-rod internal fixation for intervertebral fusion after posterior internal fixation reset. The operation time, intraoperative bleeding, postoperative hospital stay, and incidence of complications were recorded. Compared were visual analogue scale (VAS) and Oswestry disability index (ODI) scores for low back pain at preoperation, 3 days and 3 months postoperation, and the last follow-up, fusion at the last follow-up, and Frankel grading for neurological function at preoperation and postoperation. Results:All the 14 patients underwent surgery successfully and were followed up for 23 (18, 24) months. The operation time was (175.1±28.2) min, the intraoperative bleeding (300.4±122.6) mL, and the postoperative hospital stay 6 (6, 7) d. One case developed postoperative transient hip flexion weakness but was discharged after restoration of normal muscle strength by conservative treatment. Both VAS and ODI scores for low back pain at all postoperative time points were significantly improved compared with the preoperative values ( P<0.05), with a significant trend of 3 days postoperation >3 months postoperation > the last follow-up ( P<0.05). In the 12 patients with preoperative neurological damage, the Frankel grading rose by at least 1 level postoperatively ( Z=-3.110, P=0.002). The last follow-up revealed no loosening or fracture of the internal fixation. Complete bony fusion was visible in all CT sagittal reconstructions. Conclusion:For patients with thoracolumbar fracture undergoing failed internal fixation, oblique intervertebral fusion is an alternative minimally invasive surgical treatment due to its satisfactory overall outcomes.

ABSTRACT

Purpose To investigate the consistency and clinicopathologic correlation of BRAFV600E protein expression and gene mutation in papillary thyroid carcinoma. Methods BRAFV600E protein expression and genn mutation was detected respectively by immunohistochemistry of SP and real time-PCR, then the consistency between the both methods was analyzed by Kappa-test, the correlation between BRAFV600E and clinicopatho-logic parameters was analyzed by Chi-square test in papillary thyroid carcinoma. Results The gene mutation and protein expression rates of BRAFV600E were 89.3% and 88.3%, respec-tively, the differences were not significant, the concordance rate of the both methods was 97.0%, Kappa value was 0.847, the consistence was higher, meanwhile the mutation rates between age <45 and ≥45 were respectively 96.8% and 85.9%, there were significant differences, the positive rates of the both detec-tion methods were higher in thyroid capsule invaded group than non-invaded group, the differences were significant. Conclusion The both methods have higher consistency, the immunohisto-chemistry can be used as an initial screening tool for detecting gene mutation, the gene mutation of BRAFV600E is significantly associated with age and capsule invasion, the relationship is not found between BRAFV600E mutation and the other clinicopatholog-ic parameters.

ABSTRACT

For the employment orientation of graduates in clinical medical major from three-year medical eduction,a curriculum system, which will be used to foster practical medical talented person for urban and rural communities, has been proposed in this paper. We will take measures, which not only further defines training target and requirement for the practical medical person from the short school system and guides students to alterate their ideological concept but also reconstructures new curriculum system and reforms teaching methods,to ensure the implementation of the training objectives

ABSTRACT

Objective To evaluate the clinical efficacy of Dan-Hong Injection and Methycobal combined therapy in treating diabetic peripheral neuropathy(DPN).Methods 72 cases of peripheral neuropathy and type 2 diabetes(conformed to ADA standard published in 1999)were randomly separated into an experimem group and a control group.Members in the experiment group were treated with Dan-Hong injection 20ml/day i.v.and Methycobal 500ug/day i.m.Members in the control group were treated with Methycobal only.At the end of 2 weeks'therapeutic courses, motor nerve conducdon velocity (MNCV)and sural nerve conduction velocity(SNCV)were evaluated. Results The overall effective rate was 92.1% in the experiment group and 67.6% in the control group, showing significant difference between the two groups(P<0.01).The post-therapeutic values of both SNCV and MNCV were significantly different from those of previous treatment.(P<0.01)The post-therapeutic values of SNCV and MNCV of the experiment group were also significantly different from those of the control group(P<0.05).No evidently relative adverse effects were observed in both groups. Conclusions Combined therapy of Dan-Hong injection and Methycobal was more effective in controlling the clinical activity of diabetic peripheral neuropathy.
