ABSTRACT

@#AIM: To compare the efficacy of air versus silicone oil tamponade for management of rhegmatogenous retinal detachment(RRD)following 25G pars plana vitrectomy(PPV).<p>METHODS: A prospective, randomized comparative study. 146 eyes from 146 patients who underwent 25G transconjunctival sutureless vitrectomy to repair rhegmatogenous retinal detachment were performed. Totally 60 eyes used air tamponade but 86 eyes used silicone oil tamponade. The follow-up time ranged from 6-12mo. The best corrected visual acuity(LogMAR), intraocular pressure, retinal reattachment rate, intraoperative and postoperative complications were compared.<p>RESULTS: One month after surgery, the mean BCVA was 0.45±0.5 in the air tamponade group and 0.78±0.65 in the silicone oil tamponade group, it were both evidently improved in comparison with before surgery, what's more, air tamponade had significantly better BCVA than those in the silicone oil tamponade(both <i>P</i><0.05). 3mo after surgery, the reattachment rate of patients in air group was lower than that of silicone oil group(93.3% <i>vs</i> 97.7%), but had no significant differences between the two groups. 6mo after surgery, the anatomical success rate were 100% in both groups. The main intraoperative complication was iatrgenic retinal breaks in 10 eyes(6.8%). The main postoperative complication was high intraocular pressure, the intraocular pressure 1wk after surgery in the silicone oil tamponade group was evidently higher than that air tamponade group(<i>P</i><0.001). No serious complication such as endophthalmitis and choroidal hemorrhage were observed in both groups.<p>CONCLUSION: For the simple early RRD, air tamponade had equivalent reattachment rate to silicone oil tamponade after 25G PPV. In the early postoperative, the visual acuity of air group was better comparing with silicone oil group, and was lower incidence of high intraocular pressure.

ABSTRACT

To observe the incidence of high intraocular pressure ( lOP ) and its risk factors in the early stage (within 7d) after 23 - gauge sutureless microincisional vitrectomy. ●METHODS: Retrospective case series study. Totally 98 patients ( 98 eyes ) who had undergone 23 - gauge sutureless microincisional vitrectomy were enrolled. lOP was measured by non-contact tonometry. High lOP was defined as an lOP > 25mmHg at any time within 7d after surgery. The influence of age, sex, side of operation, course of disease, primary disease pre - operation, reoperation, surgical options, type of tamponade, status of lens, surgical time on postoperative high lOP were analyzed. ● RESULTS: High lOP was found in 33 eyes ( 33. 7%) within 7d after surgery. High lOP was found in 8 eyes on postoperative 1d, the incidence was 24. 2% (8 / 33), and 25 eyes was found on postoperative 3d, the incidence was 75. 8% (25 / 33). The incidence of high lOP of male and female were 32. 8% ( 20 / 61 ) and 35. 1% ( 13 / 37 ) respectively. The incidence of high lOP of right and left eye were 36. 8% (21 / 57) and 29. 3% (12 / 41) respectively. There were no significant differences comparing age (Z =-0. 22), sex( χ2 = 0. 057), side of operation( χ2 = 0. 612), course of disease (Z= -0.079) and surgical time (Z= -0. 553) between patients with high lOP and those without it (P>0. 05). Similarly, no statistical significance was found between eyes with gas tamponade and silicone oil tamponade (χ2 = 1. 04), traumatic eyes and non traumatic eyes(χ2 = 0. 044), and retinal detachment eyes and without retinal detachment eyes ( χ2 = 2. 282, P > 0. 05 ). The incidence of high lOP in eyes with several operations was higher than that in eyes with the first operation ( χ2 =5. 211,P ●CONCLUSlON:High lOP occurs commonly in 3d after 23- gauge sutureless microincisional vitrectomy. The risk factors of high lOP are reoperation, combined operations and aphakic eye.

ABSTRACT

<p><b>BACKGROUND</b>Bilirubin is the end product of heme catabolism and has strong antioxidant properties. Serum bilirubin levels are reported to be reduced in patients with multiple sclerosis (MS) and neuromyelitis optica (NMO). The pathophysiology of optic neuritis (ON) resembles that of MS; however, the role of endogenous bilirubin in ON is unclear. The aim of this study is to measure serum bilirubin levels in patients with ON, and to investigate the correlation between ON and serum antioxidant status of bilirubin.</p><p><b>METHODS</b>Serum levels of bilirubin were measured in 42 patients with ON, 50 patients with multiple sclerosis (MS), 48 patients with neuromyelitis optica (NMO) and 48 healthy control subjects.</p><p><b>RESULTS</b>Serum total bilirubin (Tbil), direct bilirubin (Dbil) and indirect bilirubin (Ibil) levels in patients with ON were significantly lower than those in the healthy controls. However, no statistical significance was found between levels in the ON and MS, ON and NMO, and MS and NMO groups. In patients with ON, serum Tbil, Dbil, and Ibil levels were lower in those with recurrence or those with ON for a longer duration (≥ 1 year). Moreover, Tbil, Dbil, and Ibil concentrations were lower in patients with papillitis than in those with retrobulbar type ON, but the differences were not statistically significant.</p><p><b>CONCLUSIONS</b>Low antioxidant status may exist in patients with ON. But serum levels of Tbil, Dbil, and Ibil did not correlate with clinical presentations, such as recurrence, duration of disease and subtypes of ON. Low antioxidant status already existed in MS or NMO patients before systemic symptoms appeared.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To study the effect of angiotensin II (Ang II) on pregnancy-associated plasma protein A (PAPP-A) and insulin-like growth factor 1 (IGF-1) mRNA expressions in human umbilical artery smooth muscle cells (HUVSMCs).</p><p><b>METHODS</b>In the presence or absence of Ox-LDL, HUVSMCs were cultured with Ang II of 10(-5) mol/L for 0, 6, 12, 24, 36, and 48 h, or with Ang II at the concentrations of 10(-7), 10(-6), 10(-5), and 10(-4) mol/L for 24 h, after which the cells were then collected to detect PAPP-A and IGF-1 mRNA expressions in the cells using RT-PCR.</p><p><b>RESULTS</b>At the concentration of 10(-5) mol/L, Ang II showed a time-dependent effect in inducing PAPP-A and IGF-1 mRNA expressions, which began to increase at 12 h of culture and reaching the highest level at 24 h. Ang II also dose-dependently induced PAPP-A and IGF-1 mRNA expressions, and 10(-5) mol/L Ang II induced the highest expression levels of the two genes. Ox-LDL exposure significantly further increased the expression levels of PAPP-A and IGF-1 mRNA in the cells regardless of the Ang II concentration or duration for cell treatment (P<0.05).</p><p><b>CONCLUSION</b>Ang II can time- and dose-dependently induces PAPP-A and IGF-1 mRNA expression in HUVSMCs and is responsible for inducing platelet activity and inflammatory reaction in acute coronary syndromes, and the effects of Ang II can be enhanced by Ox-LDL.</p>

ABSTRACT

@#ObjectiveTo observe the treatment possibility and effects on rats models with experimental femoral artery obliterans(EFAO) by transplantation of endothelial progenitor cells(EPC) from bone marrow. MethodsEFAO rat models were successfully made. Bone marrow mesenchymal stem cells(MSCs) were isolated from allogeneic male Wistar rat, cultured in DMEM in vitro. EGF,bFGF and IGF-1 were added into culture medium on the 10th day and the active EPC were labled with BrdU. 5×106 of the above cells were transplanted into the right hindlimbs i.m. as Group A in the 16 rats, and the same volume of normal saline into the opposite as Group B as control. Laser Doppler perfusion imaging was taken on the 0 day、the 14th day and the 28th day after transplantation. On the 14 day and the 28 day 8 rats were sacrificed respectively and muscles of all hindlimbs were extracted for immunohistochemical examinations (by FⅧ and BrdU).Results1. In group A the skin blood perfusion in hindlimb were significantly increased as compared with group B(P<0.05). 2. Some positive stained EPC by BrdU were found in Group A but not in Group B. 3. The numbers of blood vessels with positive staining of FⅧ in hindlimb on the 28 day in Group A were more obviously than that in Group B (P<0.01). ConclusionTransplantation of EPC from bone marrow can significantly increase the skin blood perfusion and capillary density in ischemic hindlimbs in EFAOrats.
