ABSTRACT

Objective@#To investigate the mortality trends and life loss due to pancreatic cancer death among residents in Suzhou City from 2006 to 2020, so as to provide insights into the management of pancreatic cancer.@*Methods @#The data pertaining to the death of pancreatic cancer in Suzhou City from 2006 to 2020 were collected from the Jiangsu provincial mortality surveillance system. The crude mortality, standardized mortality, years of potential life lost ( YPL L), rate of YPLL ( YPLLR ), average years of life lost ( AYLL ) and annual percent change (APC) were calculated to analyze the changing trend in mortality and life loss due to pancreatic cancer.@*Results@#The crude mortality of pancreatic cancer was 13.57/105 in Suzhou City from 2006 to 2020, appearing a tendency towards a rise ( P<0.05) with APC of 2.95%. The standardized mortality of pancreatic cancer was 6.39/105. The crude mortality of pancreatic cancer was 15.14/105 in men and 12.06/105 in women, which both showed a tendency towards a rise ( P<0.05 ) with APC of 3.18% and 2.73%. The crude mortality of pancreatic cancer appeared a tendency towards a decline in residents at ages of 30 to 45 years and 45 to 60 years ( P<0.05 ), with APC of -4.93% and -1.63%, and appeared a tendency towards a rise in residents at ages of 60 years and greater ( P<0.05 ), with APC of 0.53%. The YPLL and YPLLR of pancreatic cancer were 55 340 person-years and 0.06% in Suzhou City from 2006 to 2020, while the AYLL of pancreatic cancer was 9.81 years per person, which appeared a tendency towards a decline ( P<0.05 ), with APC of -2.35%.@*Conclusions@#The crude mortality of pancreatic cancer appeared a tendency towards a rise in Suzhou City from 2006 to 2020, and the AYLL showed a tendency towards a decline. Health education and screening of pancreatic cancer should be reinforced among men and the elderly.

ABSTRACT

Objective:To investigate the viral molecular mutations of 2019 novel coronavirus (2019-nCoV) and host adaptability in Suzhou City. Methods:The throat swab specimens from nine local cases and six imported cases with 2019-nCoV viral nucleic acid test positive in Suzhou City were sequenced for the whole genome of the virus, and the Wuhan-Hu-1 strain was used as the reference sequence for alignment and analysis. The phylogenetic tree of the viral whole genome sequence was constructed by MEGA 7.0 software. Results:According to the Chinese typing method, Nextstrain typing method, Pangolin classification method and Global Initiative on Sharing All Influenza Data (GISAID) typing method, the 15 2019-nCoV genome sequences could be divided into seven types, six types, eight types and five subtypes, respectively. Compared with Wuhan-Hu-1 strain, the median number of amino acid sequence mutation sites based on nucleotide translation was three (ranging from 0 to 12). D614G mutation of spike protein was identified from all six imported viral strains, which could enhance the transmissibility. No Alpha, Beta, or Gamma mutants, which also could enhance the transmissibility, was found in the genomic sequences of the imported cases. The median number of nucleotide mutation sites in 15 sequences was eight (ranging from three to 23).Conclusions:2019-nCoV is constantly mutating, and a variety of evolutionary lineages/genotypes have been derived. All imported viruses in Suzhou City carry mutations that can increase infectivity.

ABSTRACT

Objective To explore the effect of L-ascorbic acid(AA) for reducing the fibroblast cells induced by cobalt(co)cytotoxicity. Methods The experiment was divided into the blank control group,Co2+ group,Co2++AA group, cobalt nanoparticles(CoNPs) group,CoNPs+AA group and AA group. The AA pretreated for 1 h was performed in advance. The CCK8 assay was used to detect the CoNPs and Co2+ induced and AA-treated cytotoxicity. The fluorescence staining was used to measure the production of ROS in mitochondria. The real-time polymerase chain reaction(PCR) and Western blot were used to measure the expression of related protein and related molecular mRNA respectively. Moreover the cytochrome-C level in cytoplasml was detected. Results After treating the cells by CoNPs and Co2+,the cells appeared apoptosis.CoNPs could significantly induce ROS generation;the proapoptotic factors(caspases-3,and Bax) were significantly increased, while the anti-apoptotic factor Bcl-2 expression was decreased;cytochrome C and AIF expressions were up-regulated and released from mitochondria into the cytoplasm, after AA pretreatment, these changes caused by CoNPs were decreased. Conclusion AA can reduce the CoNPs caused cytotoxicity by reducing ROS generation and release, but can not weaken the Co2+ toxic effect.

ABSTRACT

Objective: To investigate the prevalence of hyponatremia and the relationship between hyponatremia and prognosis of dilated cardiomyopathy (DCM) for in-hospital patients. Methods: A total of 515 consecutive in-hospital DCM patients treated in HF center of Fu Wai Hospifal from 2008-10 to 2013-10 were retrospectively studied. Hyponatremia was deifned as the serum level of sodium 6 months vs≤ months ), NYHA functional classiifcation (Ⅱ-Ⅳ), SBP (per 10 mmHg elevation), total bilirubin level (per 1 mg/dl change) and LVEDD (per 5 mm change), the hyponatremia at admission is still one of the important independent predictors for all cause death (HR=1.836, 95% CI (1.248-2.702),P<0.01 and HF worsening death HR=2.139, 95% CI (1.406-3.253),P<0.01 in DCM patients after discharge. Conclusion: Hyponatremia is a common electrolyte disorder for in-hospital DCM patients, it is related to longer in-hospital time and higher mortality; higher rates of all cause death and HF worsening death after discharge in DCM patients.

ABSTRACT

Objective: To analyze the prevalence and predictor for left ventricular reverse remodeling (LVRR) in patients of primary hypertension combining left ventricular systolic dysfunction (LVSD) with tailored medication. Methods: A total of 118 consecutive patients admitted in our unit from 2010-08 to 2012-10 with the base line left ventricular ejection fraction (LVEF)≤40%were enrolled. The demographic and clinical information with the findings of echocardiography at admission were collected. The patients were followed-up until 2013-12 or until the all cause death/cardiac transplantation. According to echocardiography, LVRR was deifned by 2 criteria at the same time:①the absolute elevation of LVEF≥10%than base line and the follow-up LVEF≥50%,②the relative reduction of left ventricular end-diastolic diameter (LVEDD) index≥10%than base line and the follow-up LVEDD index≤27 mm/m2. LVRR prevalence with its base line predictor was investigated. Results: The overall mean follow-up time was (23 ± 15) months, and 39/118 (33.1%) patients acquired LVRR as LVEF from the base line level (30.6 ± 6.8)%increased to the follow-up level (57.0 ± 4.9)%;LVEDD index from the base line level (31.6 ± 3.9) mm/m2 decreased to the follow-up level (24.4 ± 1.9) mm/m2, all P6 months vs≤6 months (OR=0.244, P Conclusion:With tailored medication, about 1/3 of patients with hypertension combining LVSD could acquire LVRR, the patients with shorter duration of HF, shorter QRS interval and higher ratio of SBP/LVEDD index had more possibilities.

ABSTRACT

OBJECTIVETo observe the frequency and predictors of recovery of normal left ventricular ejection fraction (LVEF) and end-diastolic diameter (LVEDD) in patients with dilated cardiomyopathy (DCM).

METHODSA consecutive cohort of 296 patients with DCM were reviewed and followed up for at least 12 months or to death or cardiac transplantation, to identify those with recovery of normal LVEF, defined as LVEF ≥ 50%, or recovery of normal LVEDD, defined as LVEDD ≤ 55/50 mm for male/female, or both by follow up echocardiography.Variables regarded as potentially relevant to left ventricular function and dimension recovery were evaluated to identify predictors using multivariable logistic regression analysis.

RESULTSAfter a median follow-up of 28 months, normal LVEF was evidenced in 81 patients (27.4%), normal LVEDD was found in 63 patients (21.3%) and both parameters were recovered in 52 patients (17.6%), LVEF was increased from (31.7 ± 6.3)% to ( 57.5 ± 5.2)% (P < 0.01) and LVEDD decreased from (62.7 ± 4.3) mm to (50.2 ± 3.7) mm (P < 0.01) in these 52 patients. Multivariable logistic regression analysis showed that shorter symptom duration, higher systolic blood pressure at admission, smaller LVEDD and lower LVEF by echocardiography at baseline were independent predictors of subsequent recovery of normal LVEF and LVEDD.

CONCLUSIONCurrent therapy for heart failure could lead to recovery of normal LVEF and LVEDD in part of DCM patients, especially for DCM patients with short symptom duration, higher systolic blood pressure at admission, less enlarged LVEDD and less reduced LVEF at baseline echocardiography.
